The vehicle's infotainment touchscreen intermittently registers phantom/ghost touches without anyone touching the screen. It randomly changes radio stations, screens and functions. During these episodes the touchscreen becomes unresponsive to the driver's actual inputs. The problem is significantly worse when the vehicle interior/display is hot. This creates a significant driver distraction and prevents the driver from reliably controlling the infotainment system. The condition has been occurring repeatedly over approximately two years

SUBJECT: A REQUEST FOR HONDA CORPORATE INVESTIGATION AND GOODWILL ASSISTANCE - 2021 HONDA ACCORD I am submitting a formal complaint regarding serious and recurring electrical and safety-related problems with my 2021 Honda Accord. 1. Numerous dashboard warning lights and safety-system warnings illuminate simultaneously. 2. The vehicle repeatedly displays messages instructing me to "See Dealer." 3. The power door locks repeatedly click and cycle as though they are attempting to lock or unlock on their own. 4. The stereo, on its own, scrolls through buttons and bluetooth blinking through all the stereo options on its own. 5.An OBD diagnostic code reader is unable to properly communicate with the vehicle and reports "Unable to Connect/Vehicle Not Found." 6. Sudden brake activated without any object being in front of the vehicle. 7. Factory Rims Extensive finish failure The simultaneous occurrence of these problems strongly suggest that this may involve an electrical, communication-network, body control module, wiring, power-supply, or other vehicle system defect rather than ordinary wear and tear. I am particularly concerned because the dashboard warnings involve vehicle systems that may affect braking, steering, stability control, driver-assistance features, and other safety functions. When numerous safety warnings illuminate at the same time, I have no reliable way of knowing which vehicle systems are actually functioning correctly. The repeated activation of the door-lock system is also concerning. The locks should not continuously activate on their own during normal vehicle operation. Combined with the diagnostic communication failure and multiple warning messages, this raises concern regarding a larger electrical or vehicle-network malfunction. Although the vehicle is currently outside the factory warranty period, this type of widespread electrical and safety malfunction should not reasonably occur in a 2021 vehicle as the result of normal use.

The infotainment touchscreen in my 2022 Honda Accord is malfunctioning and repeatedly registering phantom or “ghost” touches without any input from me. While I am driving, the system unpredictably changes screens, changes radio stations and audio settings, interferes with phone/CarPlay functions, and makes selections on its own. The malfunction can happen suddenly and repeatedly while the vehicle is in motion. This is frightening and creates a serious driving distraction because the unexpected activity draws my attention away from the road and causes me to try to determine what the system is doing or correct unwanted selections. I am concerned that this unpredictable malfunction could contribute to a crash. There is no warning before it occurs, and I cannot reliably predict or control when it will happen. The vehicle is available for inspection, and I am seeking evaluation by an authorized Honda dealership. I am requesting that NHTSA investigate whether this is a widespread safety-related defect because other owners have reported similar phantom-touch behavior.

The head gasket failed in the engine. Coolant leaked into the cylinder head causing the engine to overheat and put the driver at danger to immediate vehicle failure while driving. The problem was confirmed by the dealer. The vehicle has only been inspected by the dealership. There were warning lamps that indicated engine overheating.

The vehicle was taken to the dealer, where it was diagnosed and determined that the turbocharger had failed and needed to be replaced. The contact was informed that the failure was a common issue. The vehicle was repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 108,579.

1) incident 1: While changing lanes on an empty street emergency brake kicked in although there is vehicle in front or rear in adjacent lane. Incident date: 08/6/2026 2) incident 2: during snow time on a free way I75 towards airport. AEB kicked in multiple times. Environment Conditions: Snowing, Road is icy and vehicle was moving at 30mph. This situation was scary as we had vehicle in front and rear. If wouldn’t have intervened my car would be sandwiched between front and rear traffic cars. Incident date: Feb 2025

The contact owns a 2023 Honda Accord. The contact stated that while driving at an undisclosed speed, smoke started coming from underneath the front driver’s side seat. The contact looked down and became aware that the front driver’s side seat belt anchor had caught fire. No warning lights were illuminated. The contact stated that the vehicle had been involved in a crash prior to the fire, and the front passenger’s side of the vehicle had crashed into rear left door of another vehicle. No injuries were sustained, and no medical attention was needed. The contact exited the vehicle and contacted the local Fire Department. The Fire Department arrived on the scene and extinguished the fire. A Police report was filed. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The failure mileage was approximately 50,000.

This ongoing problem, not related to an accident or single incident. The center console screen will jump around / glitch / freeze without being touched. These “ghost touches” go in and out of apps, make calls, switch between audio sources , many other unprompted things. This issue renders the center console screen almost unusable and downright unsafe to use. This is now the second screen in the vehicle with this problem. Honda was aware a replacement may not fix the problem but did it anyway. They are claiming there will be a total fix by March of 2027.

On XXX, about 8:30 a.m., in clear, sunny weather, I was turning my 2024 Honda Accord Sport Hybrid into a marked parking space. My foot was off the accelerator; I applied light brake pressure while the vehicle moved under 5 mph. Without warning, it suddenly and spontaneously accelerated at high speed. Within seconds it drove over a concrete parking stop, traveled through dense landscaping, and collided into a palm tree that prevented it moving forward. I immediately powered it off. The airbag did not deploy; no warning message preceded the acceleration. Before repair, Germain Honda scanned the vehicle; May 28 invoice the Shop Foreman and a Honda District Parts & Service Manager inspected it. The report’s Page 1 checklist all items marked green "Satisfactory" and no damage noted in vehicle diagrams. This is not reconciled with the packet's Lighting "NG" and B1036 results or estimates documenting a shattered right headlamp. The invoice attributes many codes to headlight/bumper impact but identifies neither the codes nor the basis. Native HDS data was not provided. Pre-repair records from Germain Honda (May 28), Caliber/ProTech (XXX), and Crash Champions (XXX) record DTCs/faults involving propulsion/powertrain, braking, network, steering, driver assistance, restraint, and body controls. They do not establish when findings arose or whether any caused or contributed to the event. After repair, on XXX, Honda of [XXX] reported zero DTCs. The precise cause remains unknown. I believe Speed Control, Brakes, and Fuel/Propulsion System most likely caused or contributed to the sudden spontaneous acceleration, complete loss of control, crash, and vehicle damage. If this same event occurred at a stop sign or stoplight, significant injuries and potential fatalities would have occurred to myself, or any pedestrians, animals, and vehicles in its path. Odometer Fraud. The contact purchased a 2022 Honda Accord. It was discovered that there was a mileage discrepancy. The vehicle was a private sale. At the time of purchase, the vehicle mileage was 26,000; however, three days after retrieving a Carfax report, it was discovered that the mileage was 217,000.

Infotainment system recieving phantom / ghost touches. Screen selects menu items on its own, plays, pauses music, causes major distraction and disruption. Screen flashes when changing menus and can pull attention from the road. Safety issue!

The vehicle's infotainment system displays an OTA software update error mmessage on every startup. Honda dealerships are unable to address this issue without further intervention and software updates from Honda corporate. Dealer has stated that this issue may interfere with reliable over-the-air software updates, including any potential safety-related updates delivered through this OTA system.

Different codes started blanking after I got gas June 11, 2026. Soon as I cranked up it started flashing warnings. Emission system, brake system, sport mode doesn’t work, cruise control doesn’t work, check engine light on, tire pressure light on. I took it to midlands Honda on June 15, 2026. They told me turbocharger and need spark plugs based off mileage. I took it to oreily, they say it only scan the engine light code which is turbocharger. Started a claim with Honda support and they denied me based off my mileage for goodwill credit. Said they contacted dealership and it’s not a manufacturer issue so I have to fix it out of pocket. Warranty expired. They kept calling me weekly until August 4, 2026 (when they told me they denied me). On August 12, 2026 my trunk won’t open. I contacted them back and they told me I have to pay for the diagnostic again (almost 200.00) since it’s a separate issue. That’s crazy. It’s a whole group on social media where everybody having the same issue. Even some fixed the turbo and lights on dashboard still on. My turbo charger code can’t be the reason my trunk won’t open.

40,000 miles and 5 years after original in service date - failure of 2021 ACCORD 1.5T headgasket system between cylinder s #1-2. Required full head disassembly, removal, rebuild of head, and replacement of head gasket and associated components. $6000 quote from Honda Corporate as I was 55 DAYS outside of warranty. Absolutely ridiculous for a 5 year old car with low miles.
