Summary of Safety Issues with Chicco Car Seat 1.) Magnetic Chest Clip Disassembly (October 2023) • At [XXX] old, my daughter disassembled the magnetic chest clip while seated in the car. • She placed a small part in her mouth, creating a choking hazard. • I was stranded until a replacement car seat could be brought. • I replaced the clip with a plastic version Chicco provided, but the original design posed a serious and unacceptable safety risk. 2.Child Unlocking Base Mechanism (April 2025) • My daughter is able to unlatch the seat from its base while rear-facing. • She does this repeatedly, even after being re-secured. • Chicco only sent a “lever lock” cover despite my request for a full replacement, which was ignored. • This flaw allows a toddler to disengage the seat mid-drive, rendering the product unsafe and unusable. 3.) Cup Holder Suffocation Hazard (June 2025) • My daughter easily removes the silicone cup holders. • On one occasion, she placed one over her mouth and nose, creating suction and restricting her breathing. • I have since removed the cup holders entirely, though this alters the seat’s intended use. • This presents a clear suffocation hazard. 4.) Excessive Rattling When Not in Use (Ongoing) • When installed rear-facing and unoccupied, the seat rattles loudly during travel. • The noise is so disruptive I have rotated the seat forward-facing at times to reduce it. • While less severe than the other hazards, this reflects poor product integrity and contributes to ongoing safety concerns. - #11684774September 3, 2025SERVICE BRAKES
While driving at varying speeds the service brake warning light would come on and the vehicle would shudder while decelerating before coming to a full stop as the brakes were engaged resulting in a longer distance required for breaking. After initial occurrence the vehicle was inspected by the local dealership where they determined that "The brake lines passed inspection and there were no other observable issues." This happened roughly 20 times over a collective 2 hours of drive time throughout a 3 week period. Eventually the vehicle's computer system disabled the ABS and provided an error code regarding a faulty ABS. Upon a second inspection by the local dealership service department accompanied by video footage that I took of the incident the following report was made by the service department "Brake Booster Replacement - C0047:31 - Internal EBB failure"

The contact owns a 2021 Ford Expedition. The contact stated that while driving 60 MPH in the rain with the windshield wipers engaged, the driver's side windshield wiper arm was jerking. The contact stated that upon pulling over to the side of the road and inspecting the vehicle, the contact noticed that the driver's side windshield wiper arm was lying on the windshield. The contact removed and placed the part inside the vehicle. After the rain had stopped, the vehicle was taken to the dealer, where it was determined that the driver's side windshield wiper arm needed to be replaced. Additionally, the dealer determined that the passenger's side windshield wiper arm needed to be replaced because the wiper arm had worn out. The dealer ordered the parts. The vehicle was not yet repaired. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline for assistance. The failure mileage was approximately 19,000.

- #11683069August 26, 2025LANE DEPARTURE: LANE KEEP: AUTOMATIC STEERING, STEERING, ELECTRICAL SYSTEM
The contact owns a 2024 Ford Expedition. The contact stated that while driving at various speeds, the message "Drive Assist Unavailable" was displayed. The contact stated that when the vehicle was turned off and restarted, the system was functional. The contact had taken the vehicle to a local dealer, who checked the 12-Volt battery and found that the battery was operational and there were no other electrical issues. The contact stated that later, while starting the vehicle, the message "Loss of Power Steering Assist" was displayed, and the steering wheel was difficult to turn to the left or the right. The vehicle was turned off and restarted, and regained power steering functionality; however, the failure reoccurred. The contact attempted to reset the electrical system by disconnecting the battery terminals. The contact stated that loss of power steering reoccurred with the loss of the Lane Keep Assist feature. The contact had taken the vehicle back to the same dealer who diagnosed the vehicle and checked the wiring harness and coated the terminal connectors with a gel for better connectivity. The vehicle was repaired; however, the failures reoccurred. Additionally, the contact stated that the “Driving Modes” light was illuminated. The contact stated that while attempting to switch modes to Snow or Sport, or another option, the light illuminated with the message that each selected mode was unavailable. The contact stated that the vehicle would be taken back to the dealer for the failure. The vehicle was not repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 36,000.

- #11683059August 26, 2025FUEL/PROPULSION SYSTEM, ELECTRICAL SYSTEM, ENGINE
The fuel line/fuel pump was leaking. Smelled fumes from the gasoline. Fuel was leaking over the fuel tank from the top of the tank. The vehicle was drivable when fuel was leaking. Very concerned that their could have been a fire due to the fuel leak. Took the vehicle to the Ford service department where it was purchased. Service shop stated that during inspection they noticed that something chewed through the fuel line/fuel pump. Comments made by the service advisors stated that they have seen squirrels/rodents attracted to the soy based ingredients of the parts/wiring. The repair would not be covered under warranty due to "outside influence". The manufacturer is installing a similar part and not covering under warranty. No warning lights; no warning messages. If the same type of soy based part is being installed, what is going to prevent the same thing from happening. Ford is not offering different types of parts and they do not have a way to prevent additional damage. No shield/tape/covering is being offered. Numerous instances of this referenced online due to soy based parts and Ford knowingly installing these parts without a fix or prevention. Consumer is responsible for all repairs. No warranty.

The vehicle stopped running while driving, my wife stopped at a stop sign after vehicle had run ~ 10 minutes. Upon entering the inter-section the vehicle started to run rough, and ultimately stalled and would not restart. She was able to coast into a parking lot entrance within On Top of World subdivision where we live and get off of main road. After multiple AAA attempts to jump start, AAA confirmed the vehicle was unable to be jump started, therefore vehicle was towed to local ford dealership for diagnostics. They tried to jump start multiple times and after 2 days, pushed it into garage. They confirmed fuel pump failure and replaced with current design. We were charged for towing, new battery, and fuel pump system replacement with current design and advised we would have to bring it back in when Ford identified remedy for current recall. No loaner vehicle provided. We are very concerned as if this event had occurred on an interstate, there is a high risk of accident/injury. After repair with existing part designs we are worried about another failure on busy road/interstate.

- #11680963August 15, 2025FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The contact owns a 2023 Ford Expedition. The contact stated that while driving at various speeds with the Adaptative Cruise Control activated, the vehicle failed to decelerate automatically upon approaching another vehicle, requiring the contact to depress the brake pedal. The Pre-collision warning light was illuminated, and the vehicle made an abnormal beeping sound. The vehicle was taken to the dealer, and the contact was informed that the vehicle was operating as designed; however, the failure persisted. The vehicle was taken back to the dealer, where the contact was informed that a software update would become available in Spring of 2025 to correct the failure. The vehicle was later taken to Sarasota Ford (707 S Washington Blvd, Sarasota, FL 34236), where the contact was informed that the software update was not yet available. The vehicle was not diagnosed or repaired. The manufacturer was notified of the failure and informed the contact that a software update would become available to correct the failure. The failure mileage was approximately 9,000.

- #11680615August 14, 2025SERVICE BRAKES
I am the owner of a 2024 Ford Expedition, purchased new in March 2024. I am writing to formally report multiple instances of complete brake failure, which pose a significant safety risk. The most recent incident occurred on [XXX], at approximately [XXX], while I was driving northbound on [XXX] in Cupertino, California, with my wife and two grandchildren (ages [XXX] and [XXX]) in the vehicle. As I approached the [XXX] exit, I began decelerating. While applying continuous pressure on the brake pedal during the descent of the off-ramp, the brake pedal suddenly went soft and sank to the floor. The vehicle failed to slow down and began accelerating downhill, due to gravity, toward the intersection at [XXX] , where the light was red. This total loss of braking control lasted for approximately 5 to 10 seconds and nearly resulted in a serious collision. I was only able to regain control by releasing and reapplying the brake pedal and shifting the transmission into Park, at which point the brakes resumed normal function. A similar brake failure occurred earlier in 2024 under comparable driving conditions. At that time, I believed it to be an isolated mechanical glitch, and therefor did not take it to the dealer. I brought the vehicle to Frontier Ford in Santa Clara the following day ([XXX]) for immediate inspection. Despite ongoing efforts by the dealership and Ford engineering, the issue has not been duplicated or resolved. As of this writing, the vehicle remains at the dealership. I am filing this complaint so that the matter may be officially documented and thoroughly investigated as a potential systemic safety defect affecting this model. I urge you to investigate this issue promptly to help prevent similar incidents and ensure the safety of other drivers and passengers. Front seats have metal "Platinum" badges at about shoulder level. Badges have gap between badge and seat surface. Longer hair (below neck/ shoulder length or longer or hair in pony tail) gets trapped between badge and seat. This condition causes hair to be pulled and/or cut off. It's a painful distraction while driving and also when trapped, impairs head movement while vehicle is in motion. Tried to reach out to Ford, but being ignored. May cause a collision and it causing personal injury repeatedly. Date of incident recorded in this form is inaccurate in that it has not happened just once - rather it happens every day the vehicle is in use, repeatedly. We put this issue out on Facebook and it appears many consumers are having the same problem. Its not isolated. It's a design defect affecting all vehicles with this badge. Very poor design to incorporate a pinch point and cutting edge into a seating surface. Drivers or front seat passengers with short hair may not be affected. Everyone else is.
