Cadillac Lyriq Brake System failed, two type of brakes failed. One pedal driving automatically turned off, stopped working and normal breaking system stopped working, once breaks pressed it felt super hard like a ton of pressure/air was going through and brakes didn't work. Dash Board showed signs of: small car swerving sign, ABS sign, Brake sign with two cars crashing and an alert saying "Brake System Failure" This almost caused a multiple car crash. Dealer has the car and no detail give as of now. No police, insurance involved No prior issues.

What component or system failed or malfunctioned, and is it available for inspection upon request? Multiple components of our driver assistance software have failed, beyond what we can check in the fields above. Most recently, we have experienced two major failures. The first failure occurred when the car triggered the emergency parking brake on its own while we were driving in traffic at 30 mph. We narrowly avoided being rear ended. This is the incident I've detailed in the fields below. The second failure occurred when the adaptive cruise control failed to brake as we approached gridlock traffic. We again narrowly avoided collision on multiple fronts. We were going approximately 45 mph with roughly 2000 miles on the car. We have the service records, but the selling dealer and Cadillac engineering claim these issues are now fixed. How was your safety or the safety of others put at risk? We were well within less than a car length of collision in both of the situations described above. This car is a large SUV, it would presumably cause significant damage to cars it hit. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. They verified failures and out-of-date software in our vehicle in this last service visit. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? This vehicle has been directly inspected by Cadillac engineering through our selling dealer. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? The warning lights we experienced included the warnings that the system had engaged and "service driver assist system." There was no warning when the adaptive cruise control failed.

The headlamps in the Cadillac Lyriq are supposed to be self leveling, maintaining a consistent throw throughout a drive. My left headlamp droops significantly after 10-15 minutes of driving, failing to throw its beam more than 15 feet ahead. This poses a a safety risk because it decreases my ability to see pedestrians or road hazards. This has not been reviewed by a Cadillac dealer yet because it is a known issue to which GM had no current solution. There are no warning signals that appears in the car’s screen. The two pictures below show the headlamps at start up and after 15 minutes of driving. The droop in the left lamp is noticeable. The pictures were taken from the same spot and at the same angle.

Manufacturer has replaced and recalibrated headlights on 2 occasions however they will not maintain aim on road. This is consistent and as of now besides the repairs performed they do not know how to rectify this. Additionally in the rain or moving from a shadow to sunlight the rear cross traffic alert will trigger if enabled on vehicle causing emergency braking for no reason. it will continue to happen until turned off and car restarted. The car has been towed on 3 occasions in the first 4000 miles for complete loss of power. Dealer solution to remove 12v battery and reinstall same battery to clear elcectrical codes however problem keeps occurring.

Repeat issue from dealer repair. When sometimes car slams on breaks at 1 mph with nothing behind it. Can happen once or several times during this one time backing up. I pull forward and then back up again no issue. Also get a service parking brake error for no reason and goes away. Rear braking happen clear days sunny day and rainy days randomly

Driver assist forward continues to have issues after being at dealer for issue for 4 weeks for repair After getting car back from dealer for the first three days the error did not come up. Now the error comes and goes daily. Issue happens when approaching stop lights stop signs where there is nothing in front of you starts vibrating seat and slams on breaks. Same stop sign as yesterday but today it vibrates warnings Makes not sense.

Headlights start dropping as car is driven. Every 10 to 15 miles must pull car over and restart as you can not see further than a car’s length in front of you. Cycle car off and then back on the headlights return to normal for another 10 miles. Also seems that some of the individual lamps start pointing upward blinding oncoming drives even with most of the lamps are pointing down in front of the less than a cars length. Seems the headlights are making adjustments be are not correct until you do a power cycle.

The contact owns a 2023 Cadillac Lyriq. The contact stated while driving 30 MPH, the surround vision feature failed to detect nearby objects. The contact stated that the Autonomous Self Driving warning and the Forward Collision Avoidance Adaptive Cruise Control warning lights were illuminated. The vehicle was taken to the dealer, who was unable to duplicate the failure. The vehicle was not repaired. After investigating the failure, the contact related it to NHTSA Campaign Number: 22V710000 (Electrical System), but the VIN was not included. The manufacturer was not notified of the failure. The failure mileage was 3,100.

Rear view camera system display (360 surround) was completely inoperative. Rear view camera display did not appear on IP display when vehicle was shifted into reverse, nor did it appear when the "camera" app was selected from the center display. No warning messages present; OnStar remote code read indicated no codes. Danger is to others behind the vehicle, particularly children in blind spot. Problem persisted over several trips until I disconnected and reconnected the vehicle's 12 volt battery, and now everything is working correctly. Did not take vehicle to dealer now that everything is working; suspect software issue cleared by "vehicle reboot"

Headlights on vehicle “droop” after driving 20 minutes or more. They start off okay and then become non existent l, pointing directly down toward the pavement in front of the vehicle and you can’t see more than 1 car length in front of you. You must turn the vehicle off and then back on to reset the headlights to drive. This is a huge safety issue for driving at night.

Low beam headlights have very poor throw which degrades while driving. After driving the vehicle for 30 minutes, both left and right headlights point downward and do not throw light down the road. Cutoff is steep, with light only going about one car length. The fault is variable and somewhat random, with sometimes just one headlight drooping. The car must be driven for this to happen - does not occur when sitting flat or idle. The amount of droop varies also (confirmed by placing various pieces of tape on garage wall, and checking headlight position before and after a trip). Although the magnitude of droop is variable, nearly any trip will cause at least some amount of misalignment. When the fault is severe, even the high beams do not function well - just illuminating a very bright spot directly in front of the vehicle - and not down the road. Vehicle has electronic motorized leveling system for the headlights. The headlights go through some sort of automatic adjustment process when the vehicle is turned on. When the fault has occurred, turning the vehicle off and then on temporarily corrects the issue. Of course, this is not possible to do while driving. Driving at night in this vehicle is hazardous with this behavior. Numerous other Debut Edition and 2023 model year owners are experiencing this issue - not an isolated occurrence.

When the vehicle first starts, the headlights go through some sort of leveling routine and the light spread seems to illuminate a reasonable distance ahead of the vehicle. However, over as short as a few minutes of driving, the headlight seems to droop significantly and can result in only a very short section of road being illuminated ahead of the vehicle. This makes it very dangerous to drive at night as not enough of the road is illuminated ahead of the vehicle. The headlight throw pattern is not always uniform and there may appear to be something like a notch in the light pattern. This does not seem related to adaptive forward lighting as I do not see an active change in the light pattern based on driving.

The aim of the headlight beams droops over time to dangerous low level where the headlings barely light beyond 30 feet. The highlights would re-level and re-adjust the aim at restart, but they would start drooping to dangerous low level within 10 mins of driving.

When in reverse abrupt automatic braking occurs. Safety hazard with abrupt stop around other vehicles, hard braking causes driver and passenger to pitch forward. No warning. Automatic Braking disabled, sensors checked, settings adjusted by dealer. Dealer says there is nothing they can do. General Motors contacted but no action taken to date.

Low beam headlights work appropriately for 5 or so minutes before "drooping" or "sagging" so that they only illuminate one car length or so in front of the vehicle. Upon shutting off and restarting the vehicle, the lights return to correct illumination. However, they somehow droop again and within 5-10 minutes, it is not safe to drive the vehicle on low-beam headlights. It is the equivalent of having only fog lights on. 1) The car locked owners out and disabled any authorization from any of the owners key fob. It is missing features. 2) It is missing the rearview mirror camera. 3) Also, it states there is supposed to be a key entry next to the rear camera on the liftgate door, it is not there, there is just a hole.

While driving in the driveway, the brakes applied aggressively. While approaching a stop light, the collision detection system engaged, and the front driver's side seat was shaking. The contact stated that the brakes then locked up. Additionally, there was water entering the vehicle. The vehicle was taken to the local dealer but was not diagnosed or repaired. The manufacturer was contacted but no assistance was provided. The failure mileage was approximately 99.

Electric emergency brake light engaged engaged parking brake. Parking brake is unable to be disengaged with the control making the vehicle unable to move. Car is able to be put in drive, reverse and neutral but cannot move as the brakes are locked. Car is on the way to a dealership for them to look at it but without knowing what causes this or when it could be a safety concern to anyone driving or who needs to use their vehicle to get away.
