Seat belt latch system failed on rear middle seat during multiple trips in the car. Could buckle the seat when we left the house but after unbuckling at first destination could no longer latch the buckle. Looked on Google and this is a known issue with Tesla Model Y cars. Had to wait weeks for a repair appointment to fix the buckle leaving us with an unsafe car. No explanation from Tesla why it failed unsure if other buckles will also fail mid trip. Very concerned about the safety of seat belt latches in the event of an accident.

I have a notice that a sensor went out in my passenger seat. I took it into Tesla to get a quote to repay rid and was quoted $2,000 to fix it. When I asked why it is so expensive I was told it is be cause Tesla has to retrofit my car to fit the new sensors because they stopped making my years sensors, my car is only a 2021. This is safety issue that should be a recall, I have several other friends that own teslas that have had the same issue. The sensor that is not working properly is the sensor that registers if a passenger is in the seat or not determining if the airbag needs to deploy when in an accident. so without a working sensor it puts the passenger at risk. This is something I believe Tesla should covering as it is a safety issue & as a consumer I shouldn't have to pay over the fact they discontinued the proper sensor for my vehicle putting anyone that sits in my passenger seat at risk for the airbag to not deploy in the case of an accident.

- #11627134November 24, 2024VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
I have reported this same issue twice before. When on cruise control, the car brakes unexpectedly. My 2nd and most recent report was on [XXX]: "... I had already disabled "Apply Brakes when Regenerative Braking is Limited" under Dynamics on the screen. (see photo) He showed on the screen, under Autopilot, that I could disable Automatic Emergency Braking, which I did. I was concerned that this warning pops up: "The vehicle will not automatically apply the brakes even when a collision is detected" (see photo) but he suggested I try it to see if it would solve the cruise-control braking. I was hopeful, and disabled it, knowing that it might take a while to know if that solved the problem. I drove the 30 miles home on the frontage road instead of the interstate, and when there were no other cars around, I set cruise control at 50 mph, the speed limit. I came to a series of small hills in the road, and was reminded of the first time the automatic braking happened, on a 60 mph road with longer amplitude hills of greater height. Amazingly, it happened again! Pretty significant braking and slowed me to 25. This time I waited it out to see if it would resume the cruise control at 50. It did not. I have since restored the Automatic Emergency Braking as that seems important for a crash situation. I have restored the "Apply Brakes When Regenerative Braking is Limited" since that obviously was not the problem either." I did not use cruise control again until [XXX], when I happened to be returning home from Silver City to Las Cruces and was on the same highway, [XXX], where the problem first occurred. It is a 2-lane road and often there is no traffic, so I felt safe trying cruise control again. I set it at 55, the speed limit, and after only a few miles the car braked again. I do not use cruise control anymore, and was dismayed to find that Tesla is again offering a free trial of autodrive! This is a huge safety issue! Harsh braking while using cruise control. Known as "PHANTOM BRAKING". This is causing extremely dangerous situations. The vehicle slams on the brakes while at high speeds for no apparent reason. Perfect weather and road identification markers. Happens with oncoming vehicles in normal path of travel as well as when there is a break in painted lines on the roadway. This creates a serious potential for an accident injury or death. Happens consistently.

I [XXX] was driving at around 50mph with 100% visibility and in few seconds I got in to fog ( see pic shows clear condition ). I took of the accelerator and tesla slow down but not like it normally does auto braking ( hold mode ). The car in front of me that I didn't see was in the dense fog section in full stopped state and I crash into her. With no visibility and driver in front in full stop condition I could not avoid accident. I had no time to react. At the time crash my speed might be around 10/15mph. The point I am trying to make is Tesla Y model which is using Camera based collision prevention didn't delect car in front of me maybe because of fogged camera. Tesla Y didn't warned me for collision to avoid car crash. Tesla Y also blanked out on screen, didn't show me lanes which in normal case does. On the screen usually it show all the car moving around plus the object around the car but it didn't display anything. Tesla Y completely fails in foggy conditions as it uses camera to detect and we crashed with no warning or auto braking. I have recordings from all four cameras recorded and also collision. Tesla Model Y 2023 running on Enhanced AutoPilot (software up to date as of Nov. 19, 2024) on a highway in the rush hour when traffic is moving at the speed of 5 - 30 mph, failed to identify the trailer in the front and sped up. I had to take over control, apply brakes to avoid the rear-ending the trailer in front of me. Tesla was speeding up as if the road was open. In the computer display it does not show any traffic in front of the car. The trailer did not have any reflectors in the backside, but had working tail lights that would light up only when the trailer applied brakes. In the attached picture, blue Tesla is my car. There were no warnings of any kind of malfunctioning in my car. Highway safety can probably improve regulations on making the trailers more visible. Tesla should improve its image recognition software. Not being able to recognize a dark trailer from the dark asphalt road is a dangerous situation for Tesla cars running on AutoPilot.

Vehicle began making loud creaking noise audible at all speeds, loudest at low speeds. Car was maintained by Tesla dealership. Failure occurred at approximately 52,000 miles, just after 50,000 warranty expired. Diagnosed as failure of both control arms which need to be replaced at significant expense. Failure should not happen on such a low mileage and newer vehicle.

Subject: Urgent Safety Concern and Request for Immediate Resolution – Cracked Sunroof on Tesla Model Y 2024 To Whom It May Concern, My name is [XXX] , a United States citizen and resident of Florida. I am writing to formally express my deep concern and dissatisfaction regarding a significant safety issue with my Tesla Model Y 2024, which I purchased directly through Tesla’s official website. To date, my vehicle has never been involved in any accident and has been consistently stored in a secure garage, ensuring that it remains free from external damage. Issue Description: Recently, I discovered that the glassroof of my Tesla Model Y has developed noticeable cracks. This issue is perplexing and unacceptable, especially considering the vehicle has not been subjected to any conditions that could cause such damage. The sudden appearance of these cracks suggests a potential manufacturing defect or poor assembly quality, both of which are serious concerns for vehicle safety and integrity. Previous Communication: Upon discovering the issue, I promptly contacted Tesla Service to report the problem and seek assistance. Unfortunately, my claim was denied without a satisfactory explanation or resolution. This denial not only exacerbates my frustration but also raises significant concerns about the quality control and customer support standards upheld by Tesla. Safety Concerns: A cracked roof compromises the structural integrity of the vehicle and poses substantial safety risks to me and my passengers. glassroof malfunctions can lead to unexpected failures, potentially resulting in accidents or injury. Additionally, the aesthetic damage detracts from the overall value and appeal of the vehicle. Request for Immediate Action: Given the severity of this issue, I respectfully request the following: Immediate Investigation: A thorough investigation into the cause of the glassroof cracks, including a detailed assessment of potential manufacturing defects or assembly INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORM

While driving in town and pulling into a parking spot, my Tesla slammed on the brakes so hard it was like we struck something. The car was not in FSD or any assistive driving mode unless Tesla changed it without our knowledge. There were no warning or anything before the car slammed on the brakes. I reported the incident to Tesla and they never responded.

Summoned my car in parking lot. Vehicle showed on app that it planned to go straight then turn left but instead cut to a left and thus colliding with the car that was parked next to me. I have the footage if needed. Car should have stopped instead, or followed planned route. I was staring at the car the whole time and it looked that it was going straight, even after colliding, it looked like it was aimed to go forward. The car created a log of what happened if needed.

I noticed that now Tesla uses windshield mounted cameras for everything in front of car including FSD and forward collision. My model y was made in fremont, California. Problem is windshield glass is not cleaned before assembly to car. Either not a prescribed step by Tesla during assembly or overlooked by assembly workers. I noticed after I picked up my new model y from Tesla that there is haze and layer of residue on windshield glass. As if glass are sitting in bulk in factory and assembled to car without cleaning. I was able to clean windshield inside with glass cleaner and microfiber cloth. Problem is windshield haze and Layer of residue cannot come out of front facing camera module. This is causing all functions to degrade that rely on these windshield mounted cameras. I get random "one of cameras occulted" error during Full self driving FSD. Car cuts out during summon. Haze can be seen on app cameras during summon, while rear camera and fender cameras show clear view. This will get worse with winter now as fog also sets on windshield. Cleaning windshield glass behind camera module requires professional disassembly. Maybe it only affects model y made in fremont, CA or more

Model: Telsa Y 2022 AWD Long range What component or system failed or malfunctioned, and is it available for inspection upon request? Control Arm bolt fell from Driver side front Wheel. Not available for inspection. Bolt got fixed by Tesla Service Center How was your safety or the safety of others put at risk? Yes I imagine. Since I was driving in community driveway with less < 10 MPH. I was safe. If this happens in Highway, it would be big risk since Steering gets locked and only one wheel rotates. Has the problem been reproduced or confirmed by a dealer or independent service center? This same issue has been reported mulitple times by other car owners over internet. As per dealer, this issue cannot be produced by him. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Component has been inspected only by the Tesla Service center. No one else looked into it. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No warnings or indications. Car still doesn't know that it has a problem with front wheel or steering.

Yesterday on [XXX], my husband was driving our 2021 Tesla Y on a residential road (my son was on board at the time.) Suddenly they heard a loud noise, and the car rattled and went a loud noise. The next thing the driver felt like the car wasn't able to move and he stopped. One side of the car was dropped to the ground. When he got out, he saw the front of the car's bottom was scrapping on the ground. He realized it was a critical error. The suspension on the right side fell to the ground, and two brand new bolts fell on the ground a little behind the car. Right front tire was twisted and pointed to the opposite direction of the other wheel. Neither of the wheels were parallel. The front tire was scratched with some deep marks because it's been scrapping and dragging on the ground. The twisted tire damaged the side of the front right fender because it was out of place. The scariest thing was that the accident happened out of nowhere and it fell apart while the vehicle is moving. Very dangerous. Our Tesla has 53,000 miles on it. And got new tires and rotations about 2 years ago. It was done by Tesla Service. We never had anyone else touch the car but the dealership. We never had any collision. We had the car towed to the Service Center yesterday (Sunday and they were closed.) I contacted Service Center this morning and asked whether this error is related to the similar recall on Tesla Model 3. Suspension fell apart. The staff membered claimed that it had nothing to do with it. He also claimed that he's very knowledgeable about recalls, "If there is a specific recall relating this problem, I'll be the first one to know." I guess he might be right, but what if our car is the first one which experienced this deadliest problem. Here is the evidence of the current recalls relating to: [XXX] I'm very shocked to find out that this had happened, and there was no previous safety recall. I have been advised by Tesla my tires on the Tesla Model Y Performance 2023 vehicle with 23,506 miles are wearing out on the inner half of the tire due to a camber issue. They acknowledge the car’s weight causes the issue which can only be rectified with an adjustable camber. The camber installed on the 2023 Tesla Model Y Performance is not adjustable. They recommended I go to a third party to get an after market adjustable camber installed to avoid the inordinate wear and tear on the inner half of the stock Michelin Tires. They even gave me a name and address for a shop they normally recommend Tesla customers go to in order to get the work done. Tesla admits knowing of the engineering mistake and refuses to replace the camber part or the tires. I asked the service department why this issue was not addressed at the time of sale, they claim they can’t control what the sales people do are refrain from doing. This should be a recall on all Tesla Model Y Performance models. As a consumer I should not have to pay $1,550 to rectify their engineering mistake.

To Whom It May Concern: I am writing to report yet another dangerous incident of phantom braking in my Tesla vehicle. As a Tesla owner who has previously filed multiple complaints about this issue, I am deeply concerned and frustrated by the lack of resolution to this potentially life-threatening problem. Today, I experienced a terrifying phantom braking event that nearly resulted in a catastrophic accident. I was driving on a completely empty road with no traffic in sight - no vehicles behind me, none in front, and none approaching from any direction. Suddenly, without any apparent trigger, my Tesla aggressively applied the brakes, violently decelerating from approximately 25 mph to just 3 mph in a matter of seconds. This abrupt and unnecessary braking action caught me entirely off guard. The force of the deceleration threw me forward, and I narrowly avoided losing control of the vehicle. Had there been any traffic behind me, I have no doubt this incident would have resulted in a severe rear-end collision. The frequency and unpredictability of these phantom braking events make me feel unsafe every time I get behind the wheel. It’s only a matter of time before one of these incidents leads to a serious accident or fatality. Despite multiple reports to both Tesla and NHTSA, this critical safety issue remains unresolved. I implore NHTSA to take immediate and decisive action to address this problem. The safety of Tesla drivers, their passengers, and other road users is at stake. Please investigate this issue thoroughly and compel Tesla to implement a permanent fix for the phantom braking problem. Your swift action on this matter is crucial to prevent potential tragedies on our roads. Sincerely, [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Model Y s/w version 12.5.4.1 The model Y can be made to oscillate in the travel lane at highway speeds I think I experienced this at 70-75, it would seem to apply at any capable speed. Proper behavior: 1) driving on the highway in FSD 2) trigger the left or right turn signal to initiate a lane change by automation. What actually occurs: 1) driving on the highway in FSD 2) hold the turn signal in the desired up or down position to initiate a lane change. 3) the Tesla begins to switch lanes, and will continue to further its lane change provided you continue to hold the turn signal (up/down). 4) If you release the turn signal input "early", that is prior to the Tesla determining that the new lane is the current lane, it will immediately begin reverting to the original lane that you attempted to leave. 5) It begins to over correct, requiring maybe 5 damped oscillations. Its very uncomfortable, and exposes a vehicle on the edge of control - the vehicle is not traveling neutrally on its suspension. To be clear, it is damped, but I don't see how it could be described as stable at highway speeds.
