Over the last few weeks I've been on several long road trips. During this time I've experienced "Phantom Breaking" (Breaking that I didn't cause) - abruptly. Very strong breaking at time. It happens around once every 40 to 100 miles of driving. It happens when using Cruise Control or when using Enhanced Autopilot. It seems extremely dangerous at times and has caused at least one care behind us to swerve into a passing lane.

Going 40 MPH on CA 210W in left lane. Full Self Driving available on vehicle but no autopilot/lane assist activated at time of incident. Steering wheel locked up completely; I was unable to turn the wheel at all so I ended up drifting across 4 lanes of highway (miraculously didn’t hit anyone) and ended up under a semi. Car airbags did not deploy and system did not detect a collision. Contacted Tesla to ask about the cause of the accident multiple times; never got an answer. They had us pull the USB in glovebox and see if anything is on there but there wasn’t even though dash cam was activated. They “escalated” the ticket to a supervisor and have been ghosted ever since asking for an answer. We scheduled a service appointment to have them check out the vehicle and they canceled it. It is a miracle that I did not die; I have severe PTSD from the incident which has affected my career negatively. I would like Tesla to take accountability for things like this; it’s scary that they don’t and threaten legal action instead.

The car had several hard braking incidents while using the adaptive cruise control/autopilot. The hard brake incidents seemed correlated to dark areas on the highway pavement. The incidents appeared correlated with actual darker pavement as well as shadows from overhead bridges.

While driving my Tesla Model Y sudden braking happened numerous times. During the last event a Large Semi-trailer Truck was extremely close to my rear bumper felt as if I was going to be rear ended and the Semi was beeping the horn thinking I was placing the brakes. This is very concerning to me. This is a safety concern due to system malfunction.

2022 Tesla was in auto-pilot mode, not FSD. Phantom breaking occured on July 6 in Sacramento on Business 80, two incidents about 20 minutes apart. First incident, vehicle suddently slowed from about 65 to 30. Second incident, vehicle suddently slowed from about 65 to 10 (almost stopped) where we almost rearended. The driver was extremely angry as evidenced when he changed lanes to pass us (can't blame him), as Tesla began to reaccelerate. I have heard of phantom breaking incidents, but twice in one short trip where the second had the potential for a serious accident would indicate to me that Tesla needs to take action. I hope you relay this to them.

On July 2nd, 2022 I was driving back from a family visit in Palo Pinto County, TX and had auto pilot engaged for part of the drive. In a span of 40 minutes of that drive on a continuous stretch of Interstate 20 headed east to Dallas the car softly automatically braked once, and twice more strongly braked. In one of those instances I was near another car (near is defined as within 1/8th to 1/4 of a mile) and in both of the other instances I had NO one else within close proximity that could've caused a "reaction". That stretch of i20 is 2 lanes each direction with a median in between, no oncoming vehicles are within at least 75 - 100 feet. In the instance with the car closer, had I not been more reactive and paying attention closely, this WOULD have lead to an accident with another vehicle. In the instances with no traffic IMMEDIATELY near me.. given that rate of speed, it could STILL lead to an accident by over-reaction from others not expecting to see someone slamming on their brakes in a 75MPH zone. Tesla has been extremely unhelpful, their remote diag team says the system is "operating normally" and will not deal with the situation further.

The contact owned a 2022 Tesla Model Y. The contact stated prior to driving the vehicle, he was charging the vehicle due to low electric power. The manufacturer informed the contact to take the vehicle in for service. The contact stated while driving approximately 65 MPH, the computer inadvertently shutdown and the steering wheel was difficult to maneuver. The vehicle then drove off the road, and the contact drove over a grassy area and the curb. The TPMS warning light illuminated. The front axle was completely fractured. The police were not initially on the scene; however, the police arrived the following day. A police report was not filed. There were no injuries sustained. The contact attempted to have the vehicle towed several times, but was unsuccessful; however, the vehicle was finally towed to the dealer. The vehicle was not diagnosed or repaired. After the contact consistently discussed the failure; he was informed by the insurance company that the vehicle was deemed a total loss. The contact received a notification for NHTSA Campaign Number: 22V296000 (Back Over Prevention, Equipment) and related the failure to the recall. The approximate failure mileage was 9,700.

Braking system applied brakes while on the interstate going 75+ MPH no reason to brake, happens quickly on most instances, I hurry to hit the accelorator to speed back up to speed as to not cause any incidents. First several times it is very uncomfortable to experience, causing concern to be easily hit from the rear in areas with more traffic. The issue is not reproduced as it seems to happen when the car "thinks" it has a reason to brake,different enviromental changes such as cars, in the other lane, or shadows seem to have an affect as well. NO inspections have been completed at this time. No warnings seem to be triggered when this takes place, just rapid unexpected deacceleration.

While driving the frunk/hood opened while vehicle was in motion. Everything says your not able to drive or opening while in drive. My wife had just used it shopping. Drove 300 yards with No warnings. Then stopped at a light then when she was accelerating it opened and broke windshield, hood and hinges, scratched left fender inside. She was able to pull over without cause anymore damage to her or anyone else. I then meet her and I was able to drive the car with the hood open and damaged. I then removed the hood completely in order to drive it home all while no safety warnings going off. Tesla has had it since 6/3 and no determination of why or fault. $4600 estimate for repairs Still not fixed

The car suddenly stops responding with the entire system seems to be shut down. I cannot access any controls including lights, brakes and any other safety features. I reached out to tesla regarding this and even sent them a video showing the same. They are not responsive at all and seem to not care about this. The car had sidetracked due to no visibility (Wouldn't consider a crash but almost one)This happened multiple times and is really a big safety concern. We were in the middle of woods when this happened and my wife was alone the other time. I would really appreciate if you expedite this issue and get us a solution. The image below is taken when the lights were on but eventually they were gone.

While driving on the highway with no other cars around while using cruise control, the car would slam on the brakes on its own slowing down by 20mph or more. At times there were other cars around and if there had been a car behind us we would have been rear-ended. Reading it appears this has been reported 1000's of times to nhtsa, but also on numerous forums. The Tesla dealership said " Our diagnostics are not showing any hardware faults, and there would be no further action that can be taken by Tesla Service at an in-person appt. At this time these behaviors are a limitation of software in the system." No warnings, msgs or other symptoms prior to the sudden breaking

When using cruise control (with or without auto steering to stay within a lane), the car randomly breaks. This does not appear to be associate with signage along the road. It appears to happen more frequently in the right most lane. There are certain locations where this problem always occurs, but we cannot determine what is different about that particular exit than others. The most dangerous situations have been when the breaking occurs with a car closely following while driving on an interstate in the mountains (I-70). A relative driving my car was almost rear ended at those speeds while driving on I-70. The breaking is random and unexpected, which makes it more dangerous because as driver you are not prepared for this to occur. At a speed limit of 65mph, when you are driving in a curve and suddenly the car brakes, especially when using regenerative breaking, it is the same as slamming on the brakes. This could cause you to loose control over your car. The issue has been known to Tesla but has not been fixed and new cars are advertised and sold with adaptive cruise control when in fact they are faulty, rendering the feature useless at best, dangerous in most cases.

Several times, after over the air software updates, several components of the electrical systems would stop working, including GPS & cameras, resulting in no cruise control, no back up camera, no navigation, no blind spot warning/heads up collision warning/lane departure warning. Essentially, all safety features, cruise control and navigation fail when GPS and cameras fail. In all cases, the reset options provided such as powering off the carf, powering off the screen etc did not help and instead the 12 V battery had to be disconnected, sometimes more than once, before car would reset to a safe state. this requires removal of a panel, removal of cabin filter, removal of cable to battery. This is a time consuming process and could theoretically void warranties. We are waiting for the next software update to see if replacement of a control board by Tesla this week fixed the issue.

I was traveling on the highway in Indiana between Bloomington and Indianapolis. I started to experience the car braking for no apparent reason. As I continued on my trip and reached a top speed of 75 mph, the car suddenly and without warning slammed on the brakes! It took me a minute to react and regain control of the car. I absolutely would have been rear-ended if anyone had been nearby or possibly caused a chain reaction accident. I am extremely lucky and now very remiss about purchasing this car. I luckily had a passenger who looked up what was happening. We turned off the emergency braking for the remainder of the trip but it is not something you can turn off permanently. You have to turn it off every single time you start a new trip. I wanted this incident reported so Tesla is forced to deal with this issue immediately.

On Saturday, 25 June, while driving our 2022 Tesla Model Y north on highway 395 between Lone Pine and Bishop, California, and while cruise control (but not lane assist) was active, I experienced three instances of so-called phantom braking. More precisely, on three occasions within a 10 to 20 minute window, the car automatically braked sharply despite there being no vehicle or other obstacle ahead of me. Each time, the braking was sudden, forceful, and without justification. Had a car been following closely behind me, I could well have been involved in a collision. I stress that my foot was not on the brake pedal when the phantom braking took place. After the 3rd episode, I turned cruise control off and drove another couple of hours (to Mammoth Lakes) without incident. The following day, my wife and I drove the Tesla to our home in the Bay Area. We did not activate cruise control at any point during this approximately 6 hour drive, and experienced no additional incidents. Understandably, we are reluctant to re-attempt the use of cruise control any time soon. I have owned this Tesla Model Y since January of 2022, and these were the first instances of phantom braking.

I own a 2021Tesla Model Y. I took possession of it on August 7, 2021, in Santa Barbara, Ca. Ever since, I have had thousands of instances of automatic emergency braking (phantom braking) episodes while using cruise control or autopilot. On January 26, 2022, I had 47 episodes, in just a couple of hours. This is typical. My latest incident of phantom braking occurred on June 27, 2022. In August 2021, I was promised by Tesla that a software update would be issued soon to remedy the problem. It still hasn't. I'm very exasperated. I understand Tesla was supposed to give you information about this issue on June 20, 2022. I am anxious to hear about Tesla's response. Furthermore, I sincerely hope the Tesla has told you they are fixing the problem. Has it? Please let me know of updates and proposed fixes to Tesla's phantom braking.
