What component or system failed or malfunctioned, and is it available for inspection upon request: Auto high beams; car is available for inspection How was your safety or the safety of others put at risk: In Arlington Heights, IL the car started automatically switching the high beams on and off. At first the auto high beams would engage for 7-10 seconds, then disengage for 7-10 seconds. Then the auto high beams engaged for 2-4 seconds and disengage for 2-4 seconds, as if flashing them. This is unsafe as it is distracting to the driver (me), it can obstruct the view of oncoming traffic, and it makes it appear as if I am driving erratically which can result in a traffic stop / unnecessary police intervention. Has the problem been reproduced or confirmed by a dealer or independent service center - no, they will not take the vehicle in to their service center and only provided mobile service appointments during the work day. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others - no Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear - the car was recently serviced (October 5, 2022) for a nonfunctioning A/C compressor and heating pump. I am not aware of what other service / software changes+resets were made to the car while out of my possession.

I started a drive from my house. I entered a destination into the NAV system, it plotted a route. I then proceeded to start my drive. When I tried to backup, the backup cameras did not display any video, I waited over 1 minute and there was still no video. I then proceeded with my drive. The car would not start the cruise control, autopilot or FSD functions. Whenever I tried to engage the systems the car would state that they are unavailable. At this time, I also noticed that the NAV system was not updating the display, it continued to display the same image that showed that I was at home in the driveway. After 15 minutes, I pulled into a parking lot to video the problem. First, I put the car in reverse and recorded that the cameras did not display video for over 30 seconds, I then started to drive and recorded that the navigation display was not updating. The display that usually has the lane markings and displays the other vehicles was also not working, there was no information other than a picture of my car with a totally blank gray area around it. I have contacted Tesla about the problem, but they keep saying that they don't detect any problem with the car. I sent them the video and am waiting to hear back from them.

While driving on the interstate with the cruise control on the car suddenly applied the brakes. There were no other cars around and no objects on the roadway. This happened twice without warning on 10/09/2022. I have not reported this issue to Tesla yet but plan to.

My 2022 Model Y has severe phantom braking problems when cruise control or auto steer is activated. While driving at highway speeds the car slams on the brakes for no reason. This happened several times in 50 miles. I spoke to many other Tesla owners with the same problem. While driving at 70 mph on the interstate the car will suddenly and instantly apply the brakes hard. This is a severe safety issue. I am so scared that this will cause an accident that I have stopped using the cars adaptive cruise control and auto steer functions. I have contacted Tesla like everyone else, but have not gotten a response or fix. Please add this note to your pile of Tesla owners that are very pissed off that nothing is being done to resolve this safety issue. Vehicle was driving with auto steer enabled at 65 mph on freeway and braked unexpectedly and unnecessarily 3 times within a few minutes. Each time it occurred, I canceled auto steer and re-engaged. At no time were any obstructions or objects visible in front of the vehicle, and at no time did any warning lights or messages appear, the car simply braked without notice. Safety was at risk as cars behind me were traveling closely and were not expecting my car to brake suddenly, although I responded quickly enough to prevent further braking and accelerated out of danger. I have researched "phantom braking" and it seems this incident is exactly what was described in those reports. The car has not been inspected since the incident. I am hesitant to use auto steer since I do not trust that the cameras are adequately detecting what's in front of the vehicle.

With climate control limited was extremely hot in vehicle and resulted in physical discomfort / distractions while driving Problem Confirmation Yes, was confirmed. Dealer allegedly replaced "A/C Compressor (Heat Pump)" Component Inspection Unable to personally inspect component Warning Lamps / Messages 0/1/22 ~12PM – driving and received notification that “Cabin climate control system requires service – Cabin heating / cooling limited or unavailable” VCFRONT_a447 error Issue did not correct itself App showed car climate control as “LO”; car would only start with climate control set to “LO” Defogger did not work; windshield visibility limited, had to wipe inside of windshield with towel which created streaks and decreased visibility. Wipers to remove condensation from outside Heated seats and heated steering wheel would automatically be turned on when car was started

Intermittently the ability to interact with my touch screen has stopped. This means I cannot access crucial vehicle controls while moving. After trying to get a service appointment to address, Tesla claims that this should be resolved in a future software update but refuses to give a timeline for the release while I continue to have to drive my vehicle in an unsafe manner.

Adaptive cruise control has failed many times. Recently had adaptive cruise control set at 75 mph on interstate. With no warning, the car instantly slowed to 55 mph. If a car would have been behind me I would have been rear-ended. Or worse, if someone behind me thought I was intentionally braking hard or "squating" they could become angry and instigate a road rage incident. The car provided zero warning before it braked. The car has not been inspected yet. Yes, the car is available for inspection.

Traveling on a three lane highway at normal traffic speeds, at night with little to no traffic, the Traffic Aware Cruise Control (TACC) would activate braking with no identifiable reason. The braking is significant and would pose a danger to those vehicles that may be directly behind. For me this occurs mostly during night time driving because I tend to use the TACC when there is little traffic on the highway. I fear using the TACC during daytime due to the amount of traffic on New Jersey highways. The car is 100% drivable when TACC is not engaged.

When in autopilor mode (cruise controll) the vehicle ocassionally brakes for no known reason (phantom braking). This usually happens at the crest of hill if there is a guard rail. or if there is a low overpass overhead. I have reported this before (it is an ongoing problem) and discuss with Tesla representatives who reply that the car will "learn" to avoid this behavior. It is an obvious collision danger if someone is following closely. On 09/27/2022 this occured 8-10 times during a 600 mile trip

The contact stated that while driving approximately 55 - 80 MPH with the Adaptive Cruise Control engaged, the Forward Collision Avoidance; Automatic Emergency Braking was activated with no traffic within a ¼ mile. There was no warning light illuminated. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not contacted or notified of the failure. The approximate failure mileage was 3,500.

5YJYGDEE2MF161659 VIN 2021 Tesla Model Y AWD with basis cruise control This last weekend we drove from our home in Arroyo Grande CA to the Mammoth Lakes area. Enroute, we experienced a number of unneeded emergency braking events while on the highways at speed (65-72 mph). While this happened about a dozen time, 5-6 of them were significant slow downs and could have caused us to be rear ended. In none of these events was there any reason for the emergency braking. One happened when another car was approaching from the rear quickly to pass. These events led us to stop using the cruise control entirely for the last 2 hours of our trip home. I have not yet contacted Tesla about this which I will be doing right after I submit this report. Prior to this, the adaptive cruise control had functioned normally with very few unexpected emergency braking events in the last year or so. We did disable the feature of “automatic emergency breaking” but that had no effect at all. This is a serious issue that need to be addressed immediately.

On a 350 mile trip between Boise, ID and Salt Lake City, UT, we experiences four incidents of braking for no reason while using the adaptive cruise. No autopilot or full self driving functionality was engaged. The vehicle applied the brakes while at speeds of 80mph+ and it causes a violet slow down sometimes dropping 20mph very quickly requiring the driver to apply the gas to increase speed to meet the speed or traffic. This is a safety concern due to potential for being rear-ended, the violent nature could cause the driver to lose control, or cars following behind may think we are trying to 'brake-check' them in an aggressive manner. The problem has been reported previously, our vehicle has not been inspected. No warnings appeared.

The turn signals do not work correctly with Teslas. You have to hit the switch multiple times in order to register your intent. For example, when I plan to turn left, I hit the turn signal switch, nothing happens. Then I try it again and it works. There are times it takes three switches in order to get the signal to register correctly. This is a known issue and complaints in forums that Tesla turn signal don’t register the first time. This is dangerous if I have to quickly switch lanes or exit and having to hit the turn signal switch multiple times in order for it to work. Very dangerous. This should work every single time, the first time.
