Was having issues with the Van starting got an alert on the dashboard to service shifter after reading what the issue may be it states that the BUS can connector is a manufacturer default we replaced the battery in the key fob, replaced the main battery and the auxillary battery and still having the same issue, Van is hesitating when starting and now nothing on the dashboard where the speedometer is will come on.

Seat back falls to complete recline position and base slides forward and back. I googled it and found this has been happening since 2017 it is a safety issue. My Granddaughter was riding in the seat and it collapsed backwards. I don’t think there are a lot of these seats out there because it only on limited cars.

We purchased a 2025 Chrysler Pacifica in September 2025. In October, 2025 we begin to receive Urgent Safety Recall notifications regaring Driver Side Curtain Air Bag. My husband went to the dealer where we purchased van and was told we would have to be on a waiting list and we were number 20 on her list. The dealer has never contacted us about scheduling a repair on the Driver Side Curtain Air Bag. We have made 3 trips to the dealer and have been told the same story...we are 20th on her list. The Recall clearly states that because of this defect, "occupant could be ejected during a crash." To us, that seems critical to have repaired. Chrysler evidently doesn't think so putting us at great risk in case we were in a accident. Stated in their Recall: "We are sincerely concerned about your safety and reiterating scheduling a repair". After two calls to the Customer Service at the company and 3 trips to the dealer, we are no closer to having this repair schedule. I thank you in advance for any assistance you can provide in helping us expedite this repair.

While stopped at a traffic light on a downhill slope, my 2023 Chrysler Pacifica suddenly applied the brakes by itself and shifted into Park as I began to accelerate through the intersection. I did not touch the shifter. The vehicle locked up abruptly in the middle of the intersection. There were no warning lights, alerts, or messages before or after the incident. I had no idea as to what was happening. I was scared the car behind me was going to run into me.I was fortunate that she didn't This behavior is extremely dangerous and could have caused a collision. I have since learned that the 2023 Pacifica has numerous electrical‑system Technical Service Bulletins, and this appears to be an unintended Auto‑Park activation caused by an electrical or sensor fault. Vehicle details: 2023 Chrysler Pacifica, VIN [XXX]. I am submitting this complaint so this issue can be investigated as a potential safety defect affecting other vehicles as well. This incident scared me badly, and I no longer feel confident that the vehicle will behave predictably in traffic. This vehicle has experienced three separate powertrain failures stemming from the same transmission defect (DTC P0A43 — Drive Motor Position Sensor, eFlite transmission). Incident 1 — April 19, 2026: Complete sudden loss of motive power on the highway with family aboard. Vehicle towed to Castle Rock Chrysler CDJR. Failed Repair — May 8, 2026: Chrysler corporate mandated a software flash rather than a hardware repair. Vehicle failed identically within 10 miles of pickup in live highway traffic. Towed back same day. Incident 3 — June 6, 2026: A replacement transmission was installed. Within one week of pickup, a progressive transmission whine developed followed by visible smoke from the engine compartment while driving. Vehicle was immediately pulled over and towed to avoid a third highway failure. Chrysler's own STAR engineering team (TSB 21-014-26) has documented this as a known defect requiring transmission rebuild or replacement. Chrysler blocked a full transmission replacement for over 5 weeks citing backorder, then located one within hours when a repurchase review was requested. This defect causes sudden loss of motive power at highway speeds with no warning, creating an immediate safety hazard. I am requesting NHTSA investigate this defect pattern on 2022 Chrysler Pacifica Hybrid vehicles and evaluate whether a safety recall is warranted.

I am a full-time power wheelchair user with Multiple Sclerosis and rely on this 2024 Chrysler Pacifica wheelchair-accessible van as my primary means of transportation. Since purchase, the vehicle has repeatedly experienced dangerous loss-of-power events while being driven. Immediately before the failure occurs, warning beeps sound and multiple warning lights illuminate, including Check Engine, ETC, Park Brake, FCW, ESC, and MIL. After these warnings appear, the vehicle loses acceleration and enters a reduced-power/limp-mode condition. Pressing the accelerator does not result in normal acceleration, creating a serious safety hazard in traffic. Most recently, on June 4, 2026, this occurred approximately six times during a single trip. During one incident, the warning lights illuminated, the vehicle lost acceleration, and we were forced to pull over with emergency flashers activated in a dangerous location. A police officer came upon us, stopped to assist, and advised that the location was extremely dangerous. Because I use a power wheelchair, I cannot safely transfer into a standard vehicle, taxi, rideshare, or other transportation. We explained to the officer that if the van were to be towed, I would effectively be stranded because there was no practical way to transport me in my power wheelchair. The condition later cleared and we were able to continue. The vehicle has been inspected multiple times by Chrysler service departments and DriveMaster, the local mobility provider working with Freedom Motors. The problem has been reported repeatedly but is intermittent and has not been reproduced during inspections. Despite multiple repair attempts, the problem persists. The vehicle is available for inspection. These recurring loss-of-power events create a serious safety risk for myself, my family, and other motorists.

While driving the vehicle, the accelerator will become inactive and result in the vehicle slowing to a stop. I will shut off the ignition and let vehicle rest for 10-20 mins before restarting. This does not resolve the issue and I end up towing the vehicle into the manufacturer’s service department. This often results in 100s of dollars in “diagnostic” fees. The service dept claims it’s bad battery, changes and issue arises again. Next they say it needed a software update, and a few days later it fails again. Next they said it was PCM module and replaced it, and finally they replaced the transmission. The 4th visit, change of transmission, lasted a little over a year and then the issue arose once again! After paying $259 in diagnostic fees, they said I used a bad “charger”

Purchased vehicle through Carvana with approx. 21,504 miles on 4/1/2026. On 5/5/2026, with only 22,269 miles, the dashboard lights came on and informed us that vehicle will shut off soon and to stop safely. We were traveling 75 mph down the highway. We had it towed to a Chrysler dealer. They couldn’t find a problem after a week. The second time we experienced the same issue was on 5/19/2026 at 21,493 miles. Again, we had it towed to the Chrysler dealer. They told us the same thing and even implied it was the way we were driving that was the issue. The third time we experienced this problem was on 5/30/2026 with 22,636 miles on the odometer. We will be towing it back to a Chrysler dealer.

High voltage battery pack, Power Inverter Module (PIM), and HV charging system. PIM replaced twice (p/n 68381781AD, 8/4/25 and 8/18/25) after being found melted with arcing damage at HV connections. Battery pack has active BPCM fault codes and a recorded cell voltage excursion. Vehicle available for inspection. Vehicle lost all motive power without warning at highway speed on multiple occasions, requiring the driver to pull over. This occurred on a platform subject to recall 24V-536 (HV battery fire risk). A melted inverter and arcing at HV connections were found on a vehicle still being driven daily. BPCM data shows Cell 79 recorded a voltage excursion to 1,526 mV — less than half minimum safe operating voltage — stored in permanent memory. Yes. Lawless Inc. (Woburn, MA), authorized Chrysler dealer, confirmed fault codes PA121-00 and P0A43-00 on 8/4/25 and replaced the PIM. Vehicle returned 8/18/25 with same failure — dealer confirmed arcing damage and melted inverter. Three visits total; defect unresolved after two repair attempts on the same component. All three visits performed by authorized Chrysler dealer under 10-year/100,000-mile HV battery warranty. Stellantis warranty claim opened each visit. No police or insurance involvement. Red warning "Will Lose All Power" appeared on instrument cluster before each shutdown. Battery range has displayed erratically since 2/28/25 (~17,788 miles), when nonlinear discharge was first reported and dismissed. Active BPCM Level 2 safety fault (L2/L1 CELL MAX V CMP_FLT) remains present today.

Side curtain airbags (CAB) for both sides are under recall dated Mar 2026, Recall 06D (NHTSA 25V189) and prior to that Recall 06C (NHTSA 25V302) dated May 2025, Dealer says that parts are on back order and I am on a "list" - It has been two months for 06D and over 6 months for 06D and I am still waiting. I have called FCA Recall Assistance, Chrysler Cares and the Dealer every 3 weeks and still no word on when parts will be available. Dealer is not able to "schedule" the repair nor estimate approximate date the parts will be available for my repair. Due to safety concerns of this Urgent Safety Recall, I do not feel comfortable using the vehicle to transport myself or my family and the manufacturer has sent me notices regarding the recall and "claims" the parts are available on their recall information website - I beg to differ since I have not been able to get anyone give me a time frame for repair of this safety recall... Action (lack of) from the manufacturer is not acceptable. I have notified my Family Attorney of all the contact notes (dealer, manufacturer, NTHSA) and mailings from the manufacturer, regarding this recall in the event something occurs related to the CAB's before the repair can be completed.

1. Most emergency lights come on while the vehicle's brake engages automatically. The brake then pumps up and down on its own if any pressure is applied. Shortly after, the vehicle goes into "limp mode". The vehicle is currently at a Chrysler Dealership service department and available for inspection. 2. This has occurred 8 times since the vehicle was purchased in October 2024. All but one time, it has happened while driving. Once it happened while on a major highway. I had to quickly veer into a turn lane so our vehicle was not rearended by another car. Another time, I was driving my children to school and it happened about 10 seconds before crossing the worst intersection in our city. That could have resulted in a major crash based on the history of that intersection. A third time, I was traveling at about 40 miles an hour when warnings came on and the brake started to engage. Luckily, I had just gotten off a major highway and I was able to get to the side of the road. 3. The problem has been confirmed by a dealer, since it happened while I was in their service parking lot. However, they have been unable to recreate, find the source of, or fix the issue after 7 times of servicing it. 4. I am unsure whether the manufacturer has sent anyone outside of the dealer's service department to inspect the vehicle. We have been in contact with the manufacturer directly since December 26, 2025 in relation to this issue. Our extended warranty company may have had an inspector out on the first visit to the dealership for the issue, but no one since that I am aware of. 5. Emergency messages have included: Service Electronic Stability Control, Service Charging System, Service Hybrid Electronic Vehicle System, Auto Emergency Braking Unavailable Service Required. Multiple emergency lights come on including: Check Engine, Electronic Throttle Control, Electronic Stability Control Active, Service Forward Collision Warning. The first incident was January 22, 2025.

While driving on the highway, the dashboard alerted that the engine would lose power, then the engine cut off and I had to coast the van to the shoulder of the highway. I was able to avoid hitting other cars, but there was no power steering or engine during this time. The vehicle has since been towed to our driveway, but it does not run. There was no warning lamp or messaging more than a few seconds before the engine quit.

Driving down a busy section of highway on I-25 in Colorado, without warning car lost all power and shutoff. Was able to veer off highway to shoulder without getting hit. Car will not restart. This was incredibly unsettling as my family and kids were all in the car on the way home and was a very dangerous situation. For a brand new vehicle to shut off on the highway without warning is unacceptable.

While driving at 75 mph, my vehicle displayed warning to pull over because engine/vehicle would shut off. I was on the outside lane and had to pull over all the way to the right immediately. I had recently had my car inspected and no warning lights were on at the time of the incident, which put all 4 passengers in the vehicle at risk. This is the second Chrysler Pacifica Hybrid to display the same transmission failure. We took the van in for service and the issue was coded: POA43-00

I own a 2024 Chrysler Pacifica with open safety recall FCA 06D / NHTSA 26V-189 for side curtain airbags/ejection mitigation. I contacted the dealer approximately two months ago to schedule the free recall repair. The dealer advised there is a parts delay and the repair is still approximately two more months out. I checked for earlier repair options and was told no dealer can complete it sooner due to parts availability. This is a family vehicle used to transport children, and the recall involves side curtain airbags that may not retain sufficient pressure during deployment, increasing risk of injury or ejection. I am requesting NHTSA review because the manufacturer appears unable to provide the safety recall remedy in a timely manner. I am also seeking guidance on whether the vehicle is safe to continue driving and whether alternate transportation should be provided while waiting.

My 2022 Chrysler Pacifica Hybrid has developed a dangerous rear brake system malfunction. During a dealer inspection on 4/27/26, the certified technician confirmed that the rear brake pads are only contacting approximately 50% of the rotor surface, leaving the remaining half completely unused and heavily corroded. This is not normal corrosion or wear. It is a mechanical failure in which the pads are not engaging the rotor correctly. The technician also stated that the pads were sticking in the caliper slides, which prevented full engagement. This is a functional defect in the brake hardware, not environmental corrosion. Corrosion does not selectively prevent half of a brake pad from engaging the rotor unless the caliper mechanism is not operating properly. This defect results in: Reduced rear braking force Increased stopping distance Excessive load on the front brakes Potential loss of stability during emergency braking Possible degradation of the parking brake The vehicle was serviced on 9/23/25 (tire rotation + NYS inspection) and 12/10/24 (four new tires + NYS inspection). No rear brake issues were identified at either visit. If this were normal corrosion, it would have been visible during those inspections. The sudden onset of severe partial engagement by 4/27/26 indicates a latent mechanical defect, not gradual wear. Chrysler/Stellantis has refused to classify this as a defect and instead labels it “corrosion,” despite the technician’s findings of pad binding in the slides and improper brake engagement. The manufacturer’s internal system blocks any warranty or engineering review because it incorrectly categorizes the failure as routine maintenance. This is a premature and unsafe brake system failure at only 33,000 miles. The braking system is not operating as designed, and the defect could lead to reduced braking performance or loss of control. I am requesting NHTSA’s review of this issue as a potential safety defect affecting the rear brake system
