This is an online complaint because the manufacturer has failed to remedy the safety recall for the Occupant Classification System (OCS) in a timely manner. The recall was announced in December 2023 and they still don’t have a remedy - this is unacceptable because in a crash, the airbag may not deploy and there is a risk of injury.

My Gas tank does not fill up properly. It seems to trigger the automatic shut off at gas stations when it is only 3/4 full. I then have to keep putting gas it with about 6 more shut offs to get the tank full.

The contact owns a 2021 Toyota Rav4. The contact received notification of NHTSA Campaign Number: 23V865000 (Air Bags); however, the part to do the recall repair was not yet available. The contact stated that while driving at approximately 60 MPH, the OCS warning light was illuminated. The vehicle was taken to an independent mechanic and the contact was informed to not have passengers occupy the front passenger’s seat and that the remedy had not yet been developed. The local dealer was contacted. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The failure mileage was 15,000.

The contact owned a 2021 Toyota Rav4 Hybrid. The contact received notification of NHTSA Campaign Number: 23V865000 (AIR BAGS). The contact stated that while her son was driving down a hill at 25 MPH, the vehicle was rear-ended when the other vehicle's brakes failed to stop as intended. As a result, the contact's vehicle was pushed into oncoming traffic. The contact's son veered to the left, then crashed into a plastic barrier, and from the impact the vehicle came to a stop. The driver-side and passenger-side air bags did not deploy. The air bag warning light illuminated. The contact and her son went to Urgent Care. The contact suffered injuries to her jaw and back and had a concussion. The contact's son suffered injuries to his neck and back. A police report was filed. The contact's husband drove the vehicle to the residence and later to the collision shop. The insurance company was contacted. The manufacturer was made aware of the failure. The failure mileage was approximately 42,900.

Recall issued December 2023 with an estimated remedy available by Toyota in October 2024. As of February 2025, still no remedy available. I'm quite concerned at the length of time its taking to correct a simple OCS sensor. should be as easy as just inspect and/or replace the sensor. My pregnant wife now has to sit in a seat where the airbag may or may not deploy in an accident for the unknown future since no remedy has been made.

SERVICE BRAKES
While trying to stop on red light, I noticed break did not have enough strength and it was skidding a for second and break suspended message appeared on dash board. I went to Toyota dealership and they said if they can't replicate the problem, fee of $225 need to be charged. And I need to drive with them and due to time constraint, I had to return. I also mentioned about Toyota's voluntary recall on break which I found on Google but as I have not received any notice from them, I am not qualified.

POWER TRAIN
When remote starting the vehicle. The vehicle allows you to put the vehicle in gear from inside and turn off the parking brake allowing the vehicle to roll freely. The vehicle does shut off 1-2 seconds after but the vehicle is already in gear. To reproduce the defect you must remote start the vehicle using either the Toyota app or smart key fob. You then must be sitting in the vehicle once the vehicle is started you quickly depress the brake pedal shift into gear and turn off the parking brake quickly. The vehicle then shuts off while in gear allowing the vehicle to roll freely. This increases the risk of accident as the driver may not be in the vehicle and a child or other person may accidentally put the vehicle in gear while remote started. I believe this issue affects many other Toyota and Lexus vehicles as many share the same platforms and systems. Car shut off while driving because warning light for the power steering came on p.451stating check power steering fluid level. Vehicle has either a sealed power steering system or its electric. It was raining at the time that this happened. This is a big risk factor that could have led to a wreck but thank god that it did not was able to get out of traffic and stop. It took an hour of waiting before the vehicle would start back up and run properly. Unable to reproduce the problem at this time.

Tailgate will not close when button on the back or by the driver's seat is pressed. After pressing the close button at tailgate or driver's seat with tailgate open, it will lower an inch then beep several times and lift open again. Toyota dealer claims this is a known issue and the solution is to "reset" the tailgate, which they would do for me. Resetting immediately failed, and the issue persisted. Toyota service department said, "there is a button on the hatch that will reset it. you will hold that button until it does a sequence of beeps (beeeeeeeeepp, beep beep beep) then let go an press it again like you would want to close it and you should be good to go! :)" (sic). This solution worked once and immediately failed the second time I attempted to close the tailgate. This is a daily issue that was replicated at the dealer but not fixed.

SEAT BELTS
When back seat is empty, car incorrectly detects a person sitting in the back and dings until the back seat belts are buckled. Toyota dealer claims this is a known issue and claims it will not affect which airbags deploy in a collision. This is a daily issue, and can only be addressed by buckling the back seat belts at this time, according to the Toyota service department.

APPEARS TO BE A MANUFACTURING DEFECT. SIDE DOORS BACK AND FRONT FILL WITH WATER. WINDOWS ROLLED UP AND DOWN HAVE STREAKS OF WATER(WET ALL THE TIME). WATER POURS OUT OF DOOR FRAMES. SLOSHING OF WATER IN DOORS. CAR HAS NO MORE THAN 15,000 MILES ON IT. ALWAYS KEEP COVERED IN GARAGE. CAR DOES NOT ACCUMULATE DEBRIS OR LEAVES. HAVE TAKEN INTO TOYOTA PALO ALTO TWICE NOW. NO REAL HELP. CANNOT SEE WELL FROM WINDOWS - SAFETY ISSUE AS SIDE WINDOWS STAY STREAKED OBSCURING VIEW AND MOLD CONCERNS WITH WET INTERIOR DOORS.

2023 Toyota Rav4 hybrid (XSE) is said to have fuel tank capacity of 14.5 Gal with range in miles (city/hwy) 594.5/551.0 mi. When I first got my car, I can fuel it up to 520 mile driving distance. After a while, I can only fuel up to 430 mile, with no other accident. The tank cannot be fully fueled. This incidence has been widely observed in owners with the same year model. The actual reason is unknown. Some went to the dealership and replaced the sensor, some just got diagnosed with no issue. None of these has actually fixed the issue. The real answer could be the issue of the sensor or tank, or something else. This malfunction with unknown reason is risky for the use of the vehicle.

It was raining and we drove about an hour, parked and shutoff car, went into the store, came back out and car would not turn on. Error message that came up was “Engine Stopped Sterring Power Low” This is a brand new car just bought last year. We kept trying to turn car on and error message kept coming up. Waited for tow truck and car ended up turning on. Looked online and it seems a lot of other people are having the same issue in the rain.
