While turning slowly, right tun, into a parking space, the vehicle lunged forward at a much higher speed than anticipated. The vehicle jumped the sidewalk, went up an embankment, about 20 degrees, and about 25 feet until it stopped on its own. Happened on April 14th, 2025, about 11:30 am. Driver was is good health, no medical conditions. Driver also has a clean driving record, no infarctions. The vehicle jumped a curb 4 inches high, ran over some light brush, but luckily no damage to vehicle. There are train tracks about 15 feet away, which could have been another incident. Driver says when she slowly pulled into the spot, the car lunged forward, with good thrust, and I am a witness to this incident.

The contact owns a 2022 Toyota RAV4. The contact stated that a sudden complete stop on a major roadway was necessary due to a turtle crossing, and the vehicle in front was at a complete stop and exited the vehicle in their attempt to remove the turtle from the road. A third vehicle coming from the rear failed to slow down and was driving at 55 MPH, crashed into the rear of the contact. As a result, the contact rear-ended the vehicle in front, which was unoccupied due to the attempt of a turtle removal from the roadway. The contact mentioned that the seat belt did not retract during the impact. No air bags deployed. The vehicle was towed to Copart Collision Center pending the insurance company's inspection of the vehicle. The contact stated that she sustained a head injury that required medical attention. A police report was filed, but the police report number was unknown. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 36,098.

Sudden surge and acceleration upon parking. I had brakes depressed pulling into a parking space when the vehicle felt like it jumped and quickly accelerated. It veered left missing the parked van in front of me but crashing into the parked Honda Accord two spots over and one in front. It crashed into the back tire and fender area of the passenger side of the Honda pushing it completely out of its spot and sideways. My vehicle kept going even though pushing the brakes to stop. I had to force it into park to stop. The car’s air bags deployed but my Toyota’s airbags nor crash warning system worked.

The steering warning light comes on in stopped traffic. It goes back off after starting to drive again. I have had the car checked 2 times and everything checks out fine with no error codes. It also says voltage abnormally steering power low when the warning light comes on. The battery power was also checked at the auto shop and is fine. It has happened multiple times over the past couple months.

AIR BAGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
My pregnant wife is parking her car at her work when she accidentally step on the gas pedal instead of the brakes. She panicked and she drove all the way to the wall. Airbags were not activated. 911 was dispatched. Police report made. Only minor injury finger fracture and chest bruise. There was a severe frontal damage to the car. Insurance deemed it totaled due to frame damage. I also complained to Toyota. They hired a software engineer to investigate. Just received official letter from Toyota there weee no defects in their safety system. The issues are the airbags not being deployed at the time of impact and pre collision auto braking non existent. The impact on the wall according to the people inside the nursing she works felt like an earthquake inside. But our main concern is the auto brake feature did not function. This accident could have been worse. Good thing seatbelt was not an inch lower from her chest preventing injury to our unborn child. The impact could have been worse if it was made of wood and could have injured other people in the nursing home. The auto brake feature should have worked because there were many obstacles at her front including first the bushes and eventually the wall. Their response letter is frustrating for us because they are claiming there were no problems in the car. But our concern is their safety system is not reliable compare to other car companies. Kia and Honda safety system is more reactive based from my driving experience. I think there are lapses in their system.

STEERING, LANE DEPARTURE: ASSIST, ELECTRICAL SYSTEM
This vehicle is going to kill someone. Not only does it have this unintentional steering issue, but the electrical system keeps going out. The dash has been replaced and there are still issues. The adaptive cruise control used to hard brake now it pulsates when lane changing which isn’t cool if you’re passing people because then you’re turn in front of them and braking right away. The radio requires the vehicle to be turned completely off, wait, and then turn back on. The unintended steering will take you into incoming traffic, in the opposite lane, or even off-roading even when there is nothing in front of you. The compass randomly disappears. The Tim roof literally exploded while we were doing 80mph down the interstate. The curtain was closed or we would have had glass all over. The window blew out and up. If something hit the window it would have blown in and down. I have reported these safety issues to Toyota and they keep telling me that nothing is wrong. The steering issue has happened at least 7 times since I owned it. I have provided Toyota with pictures and videos showing these things and they’re still telling me that nothing is wrong but they even have a form that talks about the “unintended steering” issue.

Hybrid system failure resulting in vehicle not being drivable at all. Failure is due to corrosion from connector between high voltage harness into rear traction motor. Failure occurred 8,000 miles past warrantee cut-off (100,000) and will require weeks to months to fix 2/2 parts on backorder.

I was driving over 120 MPH and my car slid and hit the front car. My car later caught fire and ruptured shortly after. The FD extinguished it and it was a total loss. I have no clue as to where it was taken after the accident. Sorry :(

I was in a parking space at the local library, waiting on my children. It was raining, and had been raining most of the day. I had driven about 60 miles in the rain before going to the library. The car was on, but in park. The car started slightly shaking (like a big truck was going by), and it looked like some smoke was coming out of the engine. The car then flashed some lights on the dash and gave the following message: Engine Stopped. Steering Power Low. It then turned off, and I tried to turn it on multiple times to no avail. After researching online while waiting for a tow truck, I saw multiple references on Reddit to this problem. I waited about 20 minutes and was able to turn the car on, though it was hesitant. I was able to cancel the tow and drive the short distance home. It was scary because it came out of nowhere and this is a BRAND NEW CAR with only about 27,000 miles on it. I am not happy that this would be something I will have to worry about in the future. I have not reported it to the dealership, but saw online most people had no luck at the dealership and were told to report it here.

Several malfunction warnings (pre-collision system malfunction, AWD malfunction, engine malfunction, parking brake malfunction) as well as several warning lights appeared on my dash. Took vehicle to a Toyota dealership for repairs. Fuel pump was replaced. Drove car a few days later and all the same warnings and lights came back on and this time the vehicle stalled in the middle of the road while driving. Was able to restart but had as soon as vehicle was stopped it would stall. Had to restart the vehicle several times while driving 2 miles just to get it home.

TIRES:PRESSURE MONITORING AND REGULATING SYSTEMS
I am customer at Mavis Salem, NH and purchased a Cooper Tire Starfire Solarus AS 225/65r17 tire on [XXX]. Undetected at the time of purchase due to lack of inspection by service manager Scott who sold me the tire. Once I drove on the highway I start to experience the vehicle wobbling due to consistent low tire pressure, I found distortion in the tread area and separation of tire tread depth that does not the standards. Mavis would not inspect the tire when i discussed the concerns and denied refunding the purchase because the specific size was not listed on the Cooper Tire recall sheet. I di explain that the tire is part of the series of defective tire: Starfire Solarus AS. I even asked Mavis manager to call Cooper Tire to explain what I as the customer was experiencing. This is a follow up of an incident reported on 3/30/2025, after the vehicle was driven approximately 30 miles in the rain, then stopped for 5 to 10 minutes idling with windshield wipers on intermittent setting. Engine began to shudder, engine rpms increased/decreased erratically as the engine shook. The vehicle was turned off. After approximately 5 minutes, restarted vehicle and engine again ran erratically and then stalled, displaying “Engine Stopped, Steering Power Low” on the message center in the driver’s display. After the engine stalled, the vehicle would not restart and had to be towed. After being towed, the vehicle started and ran smooth. A short while later, the vehicle was started but again ran erratically, but did not stall right away and was turned off. The next day, 3/31/2025, the vehicle was taken to the dealership service department. The dealer stated that there were no diagnostic trouble codes stored in the vehicle computer, the battery was good condition, and the charging system was operating normally. They had no explanation for what happened. The service advisor suggested that when the gas tank was filled, it was overfilled after the gas pump clicked off. This, in fact is not the case. The gas tank was not overfilled. The service advisor was informed that there are other complaints online by other owners of the same symptoms under similar weather conditions while the vehicle is idling. The advisor stated that if it happens again, have it towed to the dealership service department

Today on 4/1/2025 while driving home, the roads were wet/slushy, it was actively rain/sleeting outside. During the drive, my lane assist and front impact assist was disabled due what the dash read of "sensor blocked" or somthing similar. I got home minutes later and pulled into my garage, put my car in park and sat with it idling in park for about 2-3 minutes, at that time, is when I felt it start to shake a little bit and then it shut off by itself. A warning popped up on my dash that read "engine stopped, steering power low".

- While driving the vehicle engaged in the automatic emergency breaking system due to the car in front of me, the car was awaiting to turn left, but my lane was clear and I actually did not have the car in front of my vehicle. - While driving to work, at about 40 miles per hour speed, during a not so windy day, in an non-graveled road, my sunroof glass shattered out of nowhere. the dealership checked the affected area and confirmed there was no foreign objects that might have caused the incident. (not covered under warranty)

The contact owns a 2021 Toyota Rav4. The contact stated while driving 55 MPH, the screen displayed the steering wheel with an exclamation mark. The warning light was blinking and then turned off. The vehicle was taken to the local dealer, where it was diagnosed that the steering angle sensor was faulty. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was approximately 27,000.

On 3/30/2025, I drove my 2024 Toyota RAV 4 Adventure about 30 miles in the rain with the wipers on. I then put the car in park, with the wipers still on intermittent setting and the engine still running. After sitting in park like this for approximately 5 minutes, the engine began to shudder, with engine RPMs decreasing and increasing erratically, and the engine was also shaking as the RPMs increased and decreased. I then turned the car off for approximately 5 minutes. When I restarted the car after 5 minutes, the erratic RPM resumed, and then the engine stopped running, and my display screen showed the following message: “Engine Stopped, Steering Power Low”. The check engine, check steering, and check battery lights were also illuminated on the dashboard. The vehicle would not restart after several tries and ultimately ended up having the vehicle towed 30 minutes. Once the tow truck dropped it off, the engine started right up and the error messages were no longer displayed.

Car was stationary in a fast food drive-thru. The car started shuddering/idling funny and rpm needle was jumping a bit. Turned the car off and tried to turn it back on and the car would not crank. (I then had to put the car in Neutral and have some workers at the restaurant push my car out of the drive-thru). After about 5 minutes of sitting in the parking lot, the car finally cranked and I was able to drive it home normally. This happened after a heavy rain. Message on the dash said something like "Engine off Power Steering Low."

The local dealer was contacted, and it was confirmed that the part was not available. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the issue. The contact stated that the air bag warning light remained illuminated while operating the vehicle. The cause of the failure was not yet determined. The local dealer was contacted. The failure mileage was 40,000. VIN tool confirms parts not available.
