The pedestrian warning system and horn are speakers in newer Tesla's. The speaker is in the front right ahead of the wheel facing down towards the road. When it is raining or snowing, rain or snow get lodged into the speaker and it is completely inaudible. I stood 4 feet away from my vehicle and had someone honk the horn and couldn't hear anything at all. This is very dangerous. The pedestrian warning system is complexly inaudible also. Issue persists for approx. 2 days after rain. If it continues to rain for several days, the horn does not work the entire time. I have contacted Tesla support and they state there is nothing they can do to fix it since it is how it was designed.

I continue to get completely FALSE forward collision warnings. Part of this is because my front camera housing keeps fogging up and over the past 529 weeks I’ve gotten a half dozen false FCWs. When these faulty alerts occur, a loud audible beeping is heard and red alerts are displayed on the screen. These faulty alerts are dangerous distractions, causing panic, fear, and uncertainty in the driver, causing them to take their attention off of the road while they assess what just occurred. This is a critical flaw in the functionality of Tesla vehicles that the company has refused to address. This also makes me insurance rate spike way higher and I’m getting charged extra for this.

SUDDEN ACCELERATION, I WAS DRIVING IN MY OWN DRIVEWAY AND A MODEST SPEED T15 MPH I HAVE A CIRCULAR DRIVEWAY AND I WAS ASCENDING UP THE CIRCLE WHEN THE CAR SUDDENLY ACCELERATED WITHOUT MY ACTION. i TOOK MYI FOOT OFF THE ACCELERATOR AND HIT THE BREAK BUT THE CAR DID NOT STOP OR SLOW DOWN. IT CONTINUED UP MY DRIVEWAY ON MY LAWN AND ACCROSS MY LAWN THROUGH A FENCE AND I WOUND UP IN MY NEIGHBORS LOT WITH CONSIDERABLE DAMAGE. I WAS SHAKEN BUT DID NOT SEEK MEDICAL ATTENTION AT THE TIME. MY NEIGHBOR AND MY WIFE ASSISTED. THE CAR WAS TOWED TO TESLA CONTRACTOR BODY SHOP -RECON. WHERE IT IS NOW. I HAVE REQUESTED THAT TESLA EVALUATE AND REPAIR THE CAR BEFORE I DRIVE IT AGAIN AND PICK IT UP FROM THE BODY SHOP. I NOTIFIED THE BODY SHOPP(TONI MCCARTHY) AND TESLA SERVICE(RADCLIFFE SERVICE PERSON WORKS FOR DON HADDON SERVICE MANAGER) .SO FAR NOTHING HAS HAPPENED . TESLA ASKED ME TO SEND A PHOTO OF THE ACCELERATOR BUT SINCE THE CAR IS STILL IN THEIR AUTHORIZED BODY SHOP, I AM UNABLE TO DO IT. i AM AFRAID TO DRIVE THE CAR AND ASKED THEM TO TOW IT FROM THE BODY SHOP TO THE ROCKVILLE TESLA DEALER BUT THEY SEEM RELUCTANT TO DO THIS. A year ago I went on a road trip from Florida to Iowa in the winter, temperatures were freezing or below freezing. I got an alert to maintenance powertrain, after driving to Tesla they informed me the oil pump on the rear motor was falling apart. It was replaced. A year later on a second road trip to the same location also in sub freezing conditions the exact same error pops up, in service mode it says rear oil pump error

Heat pump tends to fail when temperatures reach out temperatures below 0ºF. This is a safety risk as at these temperatures, a failure to the vehicle heat pump could lead to death. There are many reports in forums and social media, such as here: [XXX] and [XXX] that shows this is a common issue. The system started to fail last time the temperatures reached around that point (around the beginning of Jan 2015), however the car quickly recovered from that. The error shows up as vcfront_a447. How did Tesla pass the Safety Standards for the position of rear door handles to manually open the vehicle? The vehicle gets into an accident OR the car plunges into water - and this almost always results in a non-responsive/deal battery. *** The rear passengers are hurt and you want them to find that little piece of rubber mat hidden in a cranny, lift it, and then use their nail to open the plastic cover to reach that piece of junk to manually open the door? ** NO WAY !! Manual door handle in the rear doors should be placed where it's easily accessible, just like you have on the front doors. This is truly a life threatening safety issue with how Tesla has hide the rear passenger manual door handles. Please force Tesla to issue a recall and fix the issue by providing an easy access to manually open the rear door in emergencies. The car gets on fire and rear passengers may burn alive because they won't be able to open the door due to the current system of a super hidden rear door handle. I am lodging a formal complaint about recurring safety issues with my Tesla Model Y Long Range Dual Motor AWD, purchased on June 8, 2023. These issues have led to multiple skidding incidents under normal conditions, vehicle damage, and concerns about its safety and reliability. Incidents: Sept. 23, 2023 (6,279 miles): Suspension noise reported to Tesla Service Center, Clarkston, MI. Service concluded suspension was operating as intended (Invoice No. 3000S0008134598). Dec. 4, 2024 (29,425 miles): Skidding on a highway caused minor contact with another vehicle. Repairs (two rear tires and parking lamp) cost $1,007.28 at Grand Rapids Service Center (Invoice No. 3000S0011836159). Dec. 24, 2024: At 9 mph near a stop sign, the vehicle skidded, colliding with a cement pole, damaging the bumper and parking lamp. Jan. 9, 2025: During a service appointment at Ann Arbor, technicians experienced skidding during a road test. Inspection revealed worn rear tires (recently replaced), damaged suspension, and subframe assembly. Repair costs totaled $10,022.69, but Tesla denied warranty coverage. Concerns: Safety and Reliability: The AWD system consistently fails to prevent skidding, unlike older vehicles from other manufacturers like Hyundai and Toyota. Service Negligence: Tesla Grand Rapids provided worn rear tires. Previous services overlooked suspension and alignment issues, contributing to recurring incidents. Warranty Denial: Despite being under warranty, Tesla refused coverage, advising an insurance claim, which is unjustified given the defects and poor maintenance. Request for NHTSA Action: Investigate potential defects in the braking, traction control, and suspension systems. Recommend a recall if defects are confirmed. Hold Tesla accountable for repairs under warranty and ensure improved service practices. Seek compensation for emotional distress and service deficiencies. I hope NHTSA will act promptly to ensure public safety and uphold Tesla’s commitment to safety measures.

I defrosted my windows prior to entering the car. As I got into the vehicle I noticed it was extremely hot in the car. Turned off the defroster. I backed up out of my driveway. Drove passing two houses, my windshield started to crack towards the middle of my windshield. There were no cracks or impact prior to defrosting.

Since March 2024, FSD has failed to function as designed. Tesla denied any problem with the vehicle until October 2024, when a manager told me it's a known issue with no available fix. The issue started with FSD not recognizing traffic signals. Meaning that regardless of whether a traffic light was green or red, the vehicle would treat it like an 4 way stop. It would come to a stop and then proceed through the intersection if it didn't detect immediate cars. This was dangerous and Tesla refused to acknowledge the problem for months. Since then the issue has evolved with FSD only engaging in certain situations (interstate driving). As of the most recent update, FSD won't engage at all.

I believe that all Tesla have an issue where the brake lights do not turn on soon enough when the vehicle is slowing down especially at slower speeds and I feel this makes them get in a lot of rear end accidents at lower speeds. I've seen many videos online of people getting hit and the car in front of them is braking and the Tesla slows down with no brake lights then the vehicle behind them hits them.

Occurred on [XXX] at [XXX]. Las Vegas. [XXX] and [XXX] are the closest intersections. My lane had construction cones and I had to merge. There was one Zoox ahead of me and one behind in the left lane and I tried to merge in between with blinkers on. The Zoox car behind sped up to close the gap to block me out. After the stop light turned green zi tried to merge again before my lane ended and the driver did the same thing by closing the cap and almost hitting the Zoox ahead of him. It also looks like that driver intentionally partially blocked the first left turn lane so I couldn't use it. I have everything recorded on my Tesla Front, side and back cameras. I attached the footage in the email for review. We should all be concerned if the autonomous vehicle drives this way or if it's learning from this driver's driving behavior. I wanted to add that I had my wife and infant in the car. There's a baby on board sticker on the back, driver side so the Zoox driver had to have seen it and still decided to run me into the cones unless I stopped despite I was already on the dashed lines merging in. I took Tesla to arbitration over defects and deceptive practices and their legal representative committed fraud by playing my own dashcam video and muting it so you couldn’t hear the auditory warnings the car makes when it makes errors. In this case the car was driving itself into oncoming traffic and stopping on the highway for non existent stop signs. I have reported this to the Florida Bar and Florida State Attorney Office Ashley Moody who is now investigating.

The rear view camera didn't work, neither did the windshield wiper. When turning left or right, the sensors did not display anything, the display no longer displayed any distances to other cars, proximity lines, nor speed limits. GPS also did not work. The problems were confirmed by the dealer (and fixed), but they charged us even though the incident involved a software update on their part that made most advanced safety features unavailable available. When we tried to reverse, we could not see anything. When we tried to make a left or right turn, there were no sensors available to sense blind spots. When we tried to understand the speed limit or change lanes, the information was not available. Wipers were patchy in use Auto pilot also did not work. Warnings kept beeping while driving, but we could not turn it off, serving as a distraction to driver, but Tesla was unable to address the repair due to their mass recall of other cars (with similar issues).

The horn becomes very quite when snow builds up in front of the car. I drove my car on a snowy day, and the weather was freezing for a week. Until the ice melted, the horn didn't work. I had two risky moments. Once, someone was backing up, and I couldn't get their attention, and the other one was when I was passing a truck, and they were drifting off their lane, and I couldn't get their attention. I booked a service appointment with Tesla, and the service technician told me that new models have the horn on a different side of the car, and this is normal. They didn't take any action.

Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start on 1/12, and I scheduled the earliest appointment available for 1/30. They did not offer me a rental or any assistance during this time, and I need my vehicle to commute to work and conduct other essential activities. It has just over 1,000 miles on it and is ~3 months old now. This issue impacts: backup camera, emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously, and i have received no reparations for it.

Description for NHTSA Report: I am reporting a significant safety concern with the new Teslas (starting with the 2024 models) that do not have a traditional physical horn. Instead, the horn sound is generated through the car’s external speakers. This design poses a severe safety risk, especially in rainy conditions or when the speakers are wet, as the horn becomes inaudible or significantly less effective. Additionally, the volume of the speaker-generated horn is not comparable to the loudness of traditional horns, making it insufficient for alerting other drivers and pedestrians in emergencies. This lack of an actual horn is extremely dangerous and has already led to situations where the horn was not effective. I urge the NHTSA to investigate this issue as it compromises vehicle safety.

After leaving the supercharger (car charged to 92%) I came to a stop light. The car was NOT in autopilot. I took my foot off the accelerator pedal. The car had nearly come to a complete stop when it starting accelerating again. My foot was NOT on the pedal. I tried to brake and the brakes would NOT work. I turned the car to try to avoid hitting the car in front of me while pressing on the brake. Brakes still would not work and I slammed into the car in front of me. I believe that the car was not able to put the energy from the acceleration back into the battery (regenerative braking technology) and therefore pushed it back into acceleration mode. I also believe this caused the brakes to not work. I know there have been many recalls in China and other complaints of the same issue. I've had the car for 2 years and had never had this problem before. Another note when the accident happened the screen was showing a software update that wanted to be installed. This was a very disturbing and rattling happening and I'm not worried to drive the car after a super charge.
