The two bolts securing the lower lateral link of the front passenger side loosened and dropped out in 14 months of the car ownership, causing the lateral link detached from the subframe of the car. The car makes loud noise when turning, poses danger of suspension failure and possibly loss of control on freeway driving. I have scheduled a service appointment with Tesla service center and see if they can fix it.

I was driving the car on supervised self drive and going home around 9 pm. When I reached my house I tapped the break to disengage the FSD and take over so that I can pull into our driveway. After coming to a stop the car all of a sudden just took off on its own at almost 60 mph on a residential street. I was repeatedly pressing on the brake to try to control the car. But it seemed like the car had a mind of its own and was not responding to me at all. approximately 8 seconds (based on Tesla dashcam recordings) later after travelling a distance of approximately 700 feet the car hit 2 parked cars on the street, which eventually activated the brake and brought the car to a stop at the front yard of a house across the street from where the parked cars were hit.

Incident Details: Date & Time: [XXX] Driver: My spouse, Purpose of Trip: Personal (heading to a nearby soccer park) Location: [XXX] Weather & Road Conditions: Clear weather, dry roads, normal traffic conditions Driver Safety: The driver was wearing a seatbelt at the time of the incident Incident Description: My spouse was driving to the nearby soccer park with the vehicle in FSD mode. Upon approaching a railway crossing, the signal was green, indicating it was safe to proceed. However, instead of maintaining a controlled speed, the car suddenly accelerated while attempting to cross. Before my spouse could take control, the vehicle struck the boom barrier, which was either lowering or had already lowered at that moment. The incident happened so suddenly that she did not have enough time to react and override the system. Fortunately, no injuries occurred, but the vehicle sustained damage to the front bumper, windshield, and frame. The unexpected behavior of FSD at a railway crossing raises serious safety concerns regarding its decision-making. Safety Concerns: I have been a long-time appreciative user of FSD, but this incident has shaken my confidence in the system. I now fear using FSD. Given the serious nature of this incident, I request an urgent review and response regarding my concerns, as well as coverage for the necessary repairs. A police report was filed. The vehicle was not diagnosed or repaired however it was pending diagnosis at a Tesla Service Center. The manufacturer was notified of the failure. I am writing to formally file a complaint against Tesla Motors regarding a serious safety issue involving defective seat belts in my vehicle. Despite my efforts to address this matter directly with Tesla, their response has been dismissive, prioritizing an out-of-pocket repair fee over a fundamental safety concern. Recently, my [XXX] son discovered that rear driver side seat belt does not latch onto the buckle. When I contacted Tesla to seek assistance in resolving this issue, I was informed I would need to pay out of pocket to replace the buckle because my warranty had expired as of January this year. During the replacement, the Tesla technician pointed out that the other 2 seat belt buckles in the back seat were also defective and would soon not function properly. The Tesla service center in Alhambra, CA was notified but they categorized it as a customer-pay repair. An expired warranty should not absolve Tesla of responsibility for a defect in a critical safety component. Seat belts are essential for driver and passenger protection, and any failure in their function could lead to serious injury or fatality. I am deeply concerned that Tesla does not seem to be taking this issue seriously. Instead of addressing the safety risk posed by the defective seat belts, their response has been to demand payment for a repair that should not be the driver's financial burden. Safety should always be the top priority, and I believe Tesla’s handling of this matter is unacceptable. I urge your office to investigate this issue further to ensure that Tesla is held accountable for addressing potential defects in their seat belts. If Tesla is unwilling to resolve this matter appropriately, I believe regulatory intervention is necessary to protect consumers from similar safety risks. The contact owns a 2024 Tesla Model Y. The contact stated that as he was driving forward at approximately 5 MPH after having stopped for a traffic signal his vehicle was crashed into the driver's right front fender by another vehicle. The contact stated the vehicle's front collision sensor did not detect the other vehicle. The contact stated that the driver's side front fender and the hood on his vehicle had to be replaced. The contact was not sure about the damage to the other vehicle, but there were no injuries. The police arrived on scene and wrote a report but he was not given the report number. The vehicle was towed to a local dealer and the body damage was repaired, however, the failure of the front collision avoidance sensor was not diagnosed. The contact stated that the failure reoccurred. The contact was driving at approximately 35 MPH at night when he crashed into the rear of a vehicle that was double parked on the street. The contact stated that the parked vehicle was in a poorly illuminated section of the street. The contact stated that his forward collision avoidance sensor had failed to engage and had not stopped his vehicle. The contact stated that the police were on the scene and had not issued a report. The contact had the vehicle towed to a local body shop, where the damage to the front bumper, grill, and headlights was repaired. The manufacturer was not informed of the failure. The failure mileage was approximately 45,000.

While driving towards our friends place, an alert came up saying "Steering Assist reduced" with error code GTW_w075. The car was running at about 60 to 65 mph and at that speed l, we didn't notice much issue in handling the car. When we were leaving for home, the alert was still active and at that time, we realized that the steering was extremely difficult to use. Herculean effort was required to turn the car left or right and we decided it was unsafe to drive. I then researched this alert and lots of other drivers had observed this error on their 2023 or older model 3 or Yes. Mine is 2025 and not even 20 days old since I took delivery. The car had to be towed to the nearest service center.

The contact stated that the garage door was damaged. The contact called the Insurance company to report the failure and emailed pictures of the crash. No warning light was illuminated. A dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was not notified of the failure. The manufacturer referred the contact to the NHTSA Hotline to report the failure. The approximate failure mileage was 13,133.

The heat pump stopped working twice, which requires a trip to the service center and the repair to restore function is a minimum of $3,000. The vehical has no heat at all, no defrost, no cabin heat nothing! This defect happens in the coldest parts of winter and leaves the owner stranded wherever this occurs. The first time was at 52,000 miles. It wouldn't turn on. The heat pump seized. The second time was on a trip, it blow out all the freon and stopped working in Waterloo, Iowa. I was 4 hours from home and it was -4F outside. The car had 136,000 miles. Many others have had heat pumps go out in the same situation...dead of winter. The repair is $3,000 to get heat back into the car. The heat pumpscare clearly defective or not big enough and are destroying themselves every other winter in Minnesota. This is 1. Dangerous because you can't defrost your windshield. 2. You cold freeze to death in very cold conditions. 3. A very expensive repair pn a part that should last mire than a 1 or 2 years. Home heat pumpsvlast 10- 24 years!!

The windshield has a huge crack that just appeared out of nowhere. We took it to the shop and they said it wasn’t from a rock. There is no chip in the glass. Apparently a stress fracture. The crack is getting bigger every day and becoming a distraction while driving. Tesla said we are 10,000 miles over our warranty even though the vehicle is only 2 years old. It seems like they used cheap glass as this is common in the Tesla forums and chats.

The following alerts were displayed on the screen: •VCFRONT_a191: Electrical system power reduced – Vehicle shutting down •VCFRONT_a192: Electrical system unable to support all features – Switching off features to conserve energy •BMS_a151: Vehicle may not restart – Service is required •DI_a227: Please wait while high voltage system powers up •Safety Risk: The unexpected shutdown posed a serious safety risk. If this had occurred in high-speed traffic, it could have led to a crash, endangering my safety and that of others on the road. •Reproduction of Issue: The issue has not been reproduced yet, but it is an ongoing concern. I have not received a resolution from Tesla at this time. •Inspection by Authorities: The vehicle has not yet been inspected by Tesla, law enforcement, or insurance representatives. However, I am willing to make it available for investigation. •Warning Signs: There were no prior warning signs before the failure. The alerts appeared shortly before the vehicle lost power completely. This issue raises concerns about the reliability and safety of Tesla’s electrical system. I urge the NHTSA to investigate this matter to prevent potential accidents caused by similar failures in other vehicles.

This is my 4th safety report about the same issue: unexpected braking while on cruise control. Since my last report, Tesla offered another free month of auto pilot but I did not take it for fear of this braking issue. On Tuesday [XXX] I was returning to my home in Las Cruces NM from Colorado. I was on [XXX] toward Albuquerque, and there was very little traffic. I decided to see if I could get away with the cruise control. As I was pulling into Bernalillo, there were some oncoming cars in a left-turn lane at a signal, and my car braked automatically...enough to lurch me forward. I decided to stop at the "new" Tesla dealer in Bernalillo (I got my car in Santa Fe) and ask about the automatic breaking and whether they'd fixed it. The 2 service employees looked at each other and one said "No". I then asked about whether I needed to re-boot my key card since my screen went blank while I was in CO, during a snow storm, parked. I luckily had my phone and on the Tesla App, under Roadside Service, it gave me the option to re-boot the screen! I did so. The serviceman said he didn't think the blank screen had anything to do with my key card, but I could re-boot it if I want to. (A prior complaint also reported the failed key card in less than a year and Tesla's unwillingness to replace it). Tesla offers a "Smart Summon" as part of a PAID feature of Full Self Driving. I have used this feature many times without issue. In this incident, it failed to detect a support column next to a parking spot and while pulling out, it turned right and collided with the pole causing damage to the vehicle. Tesla refused to take responsibility for their feature failing.

Alerts regarding vehicle steering, stability, emergency braking, lane control traction, received all at once and the Tesla AI advisor indicated others were reporting the same. No other vehicles in lanes around us showed up on the vehicle LCD screen as normally they do too. Car did not brake normally (foot off accelerator and no braking) and handling seemed different although we were very cautious. Tried to resolve with screen reboot and multiple drives as AI Advisor suggested and it did not self resolve. Contacted Tesla service and they suggested HCU was faulty (hydronic control unit) and ABS modulator. Also wanted to look at vehicle low voltage circuit. They scheduled me for a first available $1600 service on Feb 26th (in 9 days). See attached. Seems like a significant safety issue and defect and should be recalled before some gets hurt, especially if others are seeing this too.

My Tesla model Y rear passenger window on the right side exploded, and glasses were shattered on February 14, 2025. I was driving on [XXX] and near the exit of [XXX] at approximately 11:40AM.,I was driving at approximately 65 mph. I heard an explosion sound at the back. The noise was very loud and sudden, and I was startled and distressed by the sound, and my eardrum was in pain due by the loud sound and the rush of the wind to inside my vehicle. I took my car to a Tesla svc center at Santa Clara . Tesla service center made me paid for the repair for $339.06. I am very concerned about the safety issue, fortunately there was no passenger at the back, otherwise the passenger could be severely injured. The weather was cloudy and the traffic was light. I found an article on the Internet - There were another similar incident about the rear right window explosion. There were no vehicles adjacent to my car while I was driving. I would like to express my concern regarding the potential safety implication of this incident. I urge your organization, investigate whether this issue is isolated or a part of broad pattern that may affect other vehicles. This left me a dark shadow while my driving now. Rear lift gate misaligned and having issue closing the trunk after 2 years of owning. Im concern since I have to push down on the trunk to latch, that it may unlatch while driving and lift gate will open. If the trunk doesnt latch, vehicle will not shift to drive. I took the vehicle to a Tesla service center where they told me that my left hinge of the liftgate needs to be replace and to take it to a body shop. How can a metal hinge be a problem on a 2 years vehicle. I believe that the material are poorly made if this happens in just 2 year old vehicle, I wonder if this vehicle can hold for another 3.

While in an automatic car wash with the vehicle in "car wash mode" the vehicle suddenly stopped free rolling and the vehicle behind the Tesla Model Y crashed into the Tesla. The Brown Bear car wash reported that these types of incidents are common at Brown Bear car washes and that they ocurr when the drivers of Tesla cars remove their seat belts while the car is in the car wash.

There was no property damage. The vehicle came to a stop upon making contact with the other vehicle's front bumper. The contact stated that the vehicle was traveling at approximately 5-8 MPH at the time of impact. The airbags did not deploy. The vehicle was not required to be towed.
