Incident Date: February 12, 2024, at 1:00 PM Location: [XXX] Vehicle Model: Tesla Model 3 VIN: [Insert VIN of the Vehicle] License Plate: [Insert License Plate Number] Description of the Incident: On [XXX], while driving westbound on [XXX] at a speed of 35 MPH, [XXX] experienced a sudden and dangerous malfunction of the Tesla Model 3. Despite the driver's best efforts to maintain control, the vehicle autonomously shifted from the innermost lane to the outermost lane, jumped over the concrete sidewalk, and fell into a trench. Fortunately, no other vehicles were involved, preventing what could have been a fatal accident. The police report filed with the Suwanee, GA police department documents the incident, and the car was subsequently towed to a body shop, where it was declared totaled. It is essential to note that the auto-drive feature was disabled, and no subscription to the self-driving feature had been purchased or renewed. This raises serious concerns about a potential malfunction in Tesla’s system, and it is imperative that this issue is thoroughly investigated. Attempts to Resolve: Since the incident, numerous attempts have been made to contact Tesla for a root cause investigation and resolution, but Tesla has failed to provide any substantive response or investigation. Below is a history of our efforts: 1. March 3, 2024: Incident claim submitted during a call with Raquel. No claim number received. 2. March 14, 2024: Called Tesla Customer Service and spoke with Anthony. Case was assigned to Duluth, GA service center, but no case number was provided. 3. April 15, 2024: Spoke with Christian, and later with manager Andrew. An email was received with reference ID [XXX], but no further communication or action has been taken. The contact owns a 2026 Tesla Model Y. The contact stated that while driving at various speeds in Autopilot Mode, the vehicle misread nearby road signs. The contact stated that a road sign in Illinois marked 83 and 60 was misread by the software (V12 2025.22.6E575ED98D527), while in Autopilot, with 60 being the Speed Limit, and the vehicle unintendedly accelerated or decelerated to 60 MPH. While driving on a road with a slight ramp with the Speed Limit of 40 MPH or 45 MPH, once the vehicle was on the ramp, the vehicle unintendedly accelerated to approximately 50 - 55 MPH. There were no warning lights illuminated during the failures. The contact stated that the vehicle failed to exceed 40 - 45 MPH while driving on other roadways. The vehicle failed to maintain the desired speed during each failure without a warning light being illuminated. The dealer was contacted, and the vehicle was taken to the dealer, but the vehicle was not yet diagnosed or repaired. The manufacturer was informed of the failure. The failure mileage was approximately 400.

Rear cameras and side cameras suddenly failed resulting in low visibility and severe blind spots increasing the chances of collision with other vehicles, people, etc. It is reproducible at the dealer and they are trying to fix the problem by replacing the computer. It can be inspected until dealer repairs the issue. The vehicle has only been inspected by the dealer. There was a warning on the screen starting that cameras are temporarily unavailable but camera function did not return after a week and brought to the dealer for repair.

*Lower lateral link assembly, front lower compliance link assembly, front aero shield, and front halfshaft assembly were damaged. Unable to steer the car afterwards. *Sudden loss of steering, blocking lanes of traffic. *Problem is common on Teslas and a previous isolated recall was done. Tesla denies this being a widespread issue. *Car made some clunking sounds prior, no lights or any indication this was life threateningly serious. Car suddenly made a loud snap sound backing up, can see parts of the suspension dragging on the ground. Car needed to be towed by flat bed to a Tesla service center.

After Tesla Berkeley performed an alignment on my 2024 Model Y on July 22, 2025, the car suffered a complete systems failure while driving on the freeway. Within minutes of leaving the service center, multiple safety systems shut down at once — steering assist, emergency braking, lane keeping, stability control, etc... all turned off, and the car went into manual mode. The vehicle became extremely difficult to control at highway speeds, forcing me to steer and brake manually without assist. Because the Service Center was closed that evening, I couldn’t contact anyone, so I drove back first thing the next morning (July 23) to report the issue. I was initially told the next available appointment was two weeks later, but I had to beg to be seen the same day because the car was unsafe to drive. Tesla redid the alignment and told me the car was fine. After reviewing the alignment reports, both the July 22 and July 23 data are mechanically impossible. The reports show rear camber changes from −1.8° / −2.3° to −1.5° / −1.6°, then to −2.0° / −1.8°, even though rear camber on a stock Model Y is not adjustable. This means the alignment equipment or calibration was inaccurate, and therefore the toe settings were also unreliable. The car was almost certainly released with incorrect rear toe, which explains the immediate handling instability and safety system shutdowns. A later independent alignment on October 13, 2025 (America’s Tire, Roseville) confirmed the correct geometry (−1.9° / −2.0°) and proved Tesla’s readings were wrong. Since the faulty alignments, my car developed severe inner-edge tire wear, and I had to replace all four tires prematurely. This incident involved a total system and steering-assist failure immediately after service, making it a clear safety hazard. Reporting FSD incident on 10/08/25 approx around 8.15 PM I was going on I-90 towards western mass / u mass Amherst using FSD on 10/08/25 On I-90 road work was ongoing and it was 2 lanes. All the vehicles started going in the right lane as left lane had cones . Left lanes cones were tapering on its way ahead. My Tesla FSD chose the left lane which I would not choose as I saw at the distance cones were tapering to form a single lane . Tesla FSD drove in left lane and as it noticed cones tapering into single lane FSD acutely cut into the right lane in front of the of the truck at a very very narrow margin . Once the right lane , FSD asked me to take control of the car. From there till U mass Amherst I could not use FSD . This was scary experience. I wanted to report using voice command but I got busy . I was driving along one of the main highways in Orlando, Florida called [XXX], on Wednesday [XXX], in my Model Y Tesla. I was between the exits of [XXX], and [XXX]. I was in the middle lane. There was a software update issued the previous Sunday. The warning came on the display that the lane changing was not valid. At that point the car took over and accelerated. The brakes would not work, until I slammed on it after 3 times, and the car began to decelerate. The air conditioner went off, along with the electrical turn signals. I as able to get off at the [XXX] exit for it is a bit of a "hill" up in Florida, but knew I could coast into a lane hopefully no one would be in. I was able to at least coast into a parking lot, and bump the car into a concrete barrier, and a tree infront of the barrier. The car would not respond to park, neutral or the emergency brake. I tried re-booting the car 3 times and nothing. I had to call a tow truck to tow to the Tesla Service Center in Clermont, Florida. They could not get it in park either and had to drag the Tesla onto the flatbed. After 5 days of not responding to my text, I walked into another Tesla service center to plead my problem. Long story short, there is an unexplained anomaly that is in the CANBUS and cannot duplicate the error. More $ to have to tested for another week. They gave me a loaner to use, a Model S. After a few days driving it the loaner screen went blank and I was stranded. I had their loaner towed back to the service center. They will not 100% address the problem. They know it is in the CANBUS system, but now I definitely am scared to drive the car or get it back. How do I know if they will really tell me it is fixed? I fear if I get back in the car. Using FSD, my Tesla was at a stoplight (first car) in a dedicated left turn lane with a red left-turn arrow. After cross traffic went by, the Tesla began to proceed through the intersection even though the red arrow was still on. I slammed on the brakes. Watching the dashcam video later, I counted it was about 10 seconds after the Tesla began to run the red light before the light actually changed to a green arrow.

I took delivery of a 2026 Tesla Model Y on Sept. 20, 2025. It came with a 30 day trial of FSD. I tried it out for less than a week, and decided that I don't have a death wish. The ifrst failure was when it stoppped for a red light, and there was a not right turn on red sign. The car patiently waited for a little while and then suddenly started to begin turning on the red light - like it ran out of patience.I slammed the brake which triggered an insurance ding (Telsa rate is increased) for hitting the brake. The the next day the car was poised to turn left. There were 2 lanes of opposition traffic and 1 lane for my direction. Luckily traffic was low, because it turned into the lane for incoming traffic and I was able to steer the car into the right lane, in between some traffic cones. After that event, which really shook me up, I stopped using it, even though it had been nice to have the car do the driving. Incidents like that are just too scary and dangerous. And it is an insult that Tesla penalizes me for driving safely - like hitting my brake as a caution, if I see a person who might bolt out in front of me - or does. Braking is discouraged. Also , dot penalized when I pulled down my sun visor when I turned into blinding sunlight. The car alarms blasted and the screen filled with 'caution' triangles. When I started with the FSD, I had to find controls because it sped 40 in a 30 zone and followed too closely to the car in front of me. I'm a senior and I use a lot of caution driving, in anticipation of other drivers doing bad things. It seemed like FSD encouraged agresive driving. I don't think the Tesla FSD is ready for prime time yet.

The car turned right ignoring the do not turn sign And headed for the railroad tracks. I had to slam on the brakes because the car was not going to stop. Both B-pillar camera housings on my 2023 Tesla Model Y Performance become extremely hot even when the vehicle is parked in a closed garage and not in use. On the morning of the incident, the garage temperature was approximately 78°F. The car had been parked overnight for over 12 hours, plugged in, and charging to 80%. While the vehicle was preconditioning, no defrost features were active. Despite this, both pillar surfaces measured 156°F with an infrared thermometer—hot enough to cause skin burns within seconds according to standard burn-threshold data for human skin (~155°F). Both sides heating to this temperature simultaneously suggests an electrical or control malfunction keeping the pillar camera assemblies energized. These components sit near the seatbelt anchor and side-curtain airbag area, raising concern of fire, electrical, or occupant-safety risk if the condition persists. Continuous heating while parked could also degrade wiring insulation or camera electronics used by the vehicle’s driver-assistance systems. The vehicle and affected components are available for inspection upon request. The issue has not yet been inspected or confirmed by Tesla, but a service appointment request was submitted immediately after the event. Tesla repeatedly rescheduled the appointment, delaying it by more than six weeks despite the described safety concern. No diagnostic alerts, warning messages, or notifications appeared in the vehicle or Tesla app. This abnormal heating has occurred more than once under similar conditions and does not appear to be caused by ambient temperature or sunlight exposure.

The contact owned a 2026 Tesla Model Y. The contact stated that while depressing the brake pedal to attempt to stop at a stop sign, the vehicle failed to respond as intended. The contact mentioned that when attempting to make a left turn, the steering wheel seized and became difficult to maneuver, and the vehicle crashed into a curb and merged into a landscaping facility. The contact was able to shift the vehicle into neutral, then reverse. The vehicle began to work as intended, and he looked out of his rearview mirror. He could see smoke coming from the rear passenger side, and he heard an abnormal noise coming from the rear passenger tire. No warning lights illuminated. A dealer was contacted. The vehicle was towed to a dealer to be diagnosed; however, the diagnosis was unknown. The contact mentioned the dealer, and insurance deemed the vehicle a total loss. The vehicle was not repaired. The manufacturer was notified of the failure and offered no assistance. The approximate failure mileage was 4,900.

🚨 Immediate Escalation: Safety-Related HV Drive-System Concern / Diagnostic Fee Dispute / No Loaner – Urgent Review Needed VIN [XXX] I am requesting immediate escalation of a safety-related concern involving my 2021 Model Y (approx. 90,000 miles) currently assigned to the Tesla Wilmington Service Center. The advisors (Shelly & Amanda) has stated the diagnostic fee will not be covered until after inspection, but the earliest inspection is October 22, and I’m being asked to drop off the vehicle now with no loaner or transportation support. This is a High-Voltage Drive-System concern that may fall under the Drive Unit Limited Warranty (valid through Feb 2029). The local team refuses to classify the case under warranty review or confirm transportation support, leaving me without safe mobility for weeks. Please escalate this to a Corporate Service Manager or HV Warranty Specialist today and confirm: 1. That the diagnostic will be covered pending warranty review, and 2. That loaner or transportation support will be provided. I’ve been going back and forth with the Wilmington team for a week, and this matter has not been resolved. Tesla has removed the end users ability to control the speed of the vehicle in the latest version 14.n of Full Self Driving (FSD). They are deciding for us what the proper speed is with little to no regard for the posted speed limits (which the car frequently has incorrect). They has gone so far as to say, input from occupants in the car are no longer needed or desired with regard to speed. They feel the car should travel the same speed as other traffic, even if that is 20 mph over the posted speed limit. I urge you to revoke their license to use this software on the roads in the United States, until such time as they reintroduce the ability of the driver to control the speed of the car. I am a big fan of FSD, and I use it almost exclusively, and I believe it has the potential to be safer than many of the people driving today, but speed kills, therefore I insist on having the ability to limit the speed of any car I’m responsible for driving. One has to wonder where the adults are at Tesla, where they suddenly believe input from the responsible party in the car is no longer desired or even allowed. Having worked many years in and around emergency medicine… this is just completely misguided, and given they are doubling down on this, you guys are going to have to explain it to them. :(

My model Y tesla deployed the emergency brake system while I was driving the vehicle. There wasn’t any vehicle or anything in the way for my tesla to do the emergency brake. I did get whiplash from the incident and been experiencing neck and shoulder pain. Lucky I was able to get an appointment to service my Tesla the next morning but at this moment they haven’t given me any diagnostic. My Tesla did have a message related to the camera that showed up like 5-10 minutes prior to the incident but the warning message disappeared after a few seconds.

I would like to formally report several incidents I have experienced with my newly purchased Tesla Model Y. I acquired the vehicle on [XXX], collected it from Washington D.C., and drove it back to Houston between September 20 and 21. Prior to my return journey, I conducted a test drive. During this drive, at dusk, I engaged Full Self-Driving (FSD) mode. The vehicle stopped appropriately at a traffic light; however, once the light turned green, it moved forward but veered into the yellow safety buffer zone located between the lanes of opposing traffic. I intervened by manually steering the car back into the correct lane. A similar occurrence happened in Washington D.C. while attempting a left turn under a highway overpass—the vehicle again crossed into the yellow lines. These incidents suggest that the FSD system did not reliably detect the yellow lane markers. On the weekend of [XXX], during my drive to Houston, I primarily used FSD. While navigating a road construction zone, the system failed to recognize large safety cones (yellow columns) and nearly collided with them. Fortunately, I was attentive and promptly took control, braked, and changed lanes to avoid an accident. This demonstrated the FSD’s inability to identify these safety markers. On Tuesday, [XXX], I drove the vehicle to work and returned home using FSD. As I exited the highway onto a ramp merging with frontage lanes, I observed that FSD did not reduce speed appropriately and nearly made contact with vehicles on the frontage road. This indicates that the system was unable to interpret the ramp as a short section intended for deceleration and safe merging. On [XXX], while returning home in the evening after work, I used FSD due to light traffic. The vehicle navigated turns and stops satisfactorily and paused roadside before my house. I was driving in McKinney, TX on [XXX] taking a friend to his destination. FSD (full self driving) was turned on and enabled for this drive. Normally the car is extremely good at noticing its surroundings, including school zones amidst other things. It is getting better all the time with software updates. However, I turned right into a school zone where the car did not notice that there were other cars going to pick up children. The school zone sign looked like it was abnormally above the ground and so there is a possibility FSD didn’t read it. Unfortunately a police officer noticed me and by the time I did the car was going 16 miles over in a 20 mph school zone. No warnings or anything was shown on the screen to indicate that I had come into a school zone, the speed limit was showing at 40 mph and not 20. Car did not indicate to me that I was not paying attention to the road or doing anything dangerous; it was just going with what it perceived to be flow of traffic.
