On 5/27/26, when the car was parked in parking lot, received an alert on dashboard, "check electric vehicle system", when the car was turned on. The car would not shift in to drive or reverse so had to get towed to nearby Hyundai dealership. The EV system was recalibrated and test driven 122 miles to verify proper operation. Then on 6/23/26, when driving on road received an alert on dashboard "power limited". The car would not accelerate and had to get it towed again to same dealership. The dealership performed tests on ICCU and 12V battery but came back negative.

During a family road trip, my 2025 Hyundai Ioniq 5 (~9,000 miles, less than one year old) suddenly showed red "Check Electric Vehicle System" and 12V battery warning lights and then completely lost power. The 12V system failed entirely - the vehicle could not be driven or shifted, and the doors and the rear hatch could not be locked or unlocked. It had to be flat-bedded to a Hyundai dealer because it was completely inoperable. The dealer diagnosed a failed ICCU (Integrated Charging Control Unit) and a blown ICCU safety fuse (fault code P1A9096) and is replacing both under warranty; the component is available for inspection. The ICCU converts high-voltage battery power to charge the 12V system; when it fails, the vehicle loses all electrical power and drive capability. The warning lamps appeared immediately before total power loss, with no prior symptoms at the last scheduled service. This is a known, safety-critical failure on this platform: a sudden, complete loss of power can strand occupants and, if it occurs while driving, cause loss of motive power - a serious crash risk. My vehicle was stationary when it failed, but the same failure at speed would be extremely dangerous. I have video evidence of the car completely bricked out.

The contact owns a 2023 Hyundai Ioniq 5. The contact stated that while driving at higher speeds occasionally, the vehicle suddenly lost motive power. The "Smart Smooth Cruise Control Conditions Not Met" warning message was displayed on the instrument panel. The contact restarted the vehicle as a temporary fix; however, the failure recurred. The vehicle was taken to a local dealer where it was diagnosed with an unknown electrical failure. The vehicle was repaired; however, the failure recurred. The vehicle was taken back to the same local dealer; however, the cause of the failure could not be determined. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 47,000.

Complaint Summary: Mandated federal safety recall remedies performed on my vehicle have directly induced a cascading, critical safety failure of the vehicle's high-voltage propulsion management system. My 2022 Hyundai IONIQ 5 (VIN: [XXX]) was brought to an authorized dealer on XXX at 200,224 miles to complete safety recall Campaign 272 / NHTSA Recall 24V-204 (ICCU Software Update and Fuse Inspection). After receiving this software flash, the vehicle's electrical charging system failed (DTC P0D5C00), requiring a full hardware replacement of the Integrated Charging Control Unit (ICCU) on XXX. 142 miles after the physical replacement of the high-voltage ICCU, the vehicle displayed an emergency battery malfunction message on the dashboard. Diagnostic scans reveal active fault code P1BC700 in the Battery Management System. The manufacturer's dealership is now charging $1,909.77 out-of-pocket to replace the Power Relay Assembly (PRA).The current recall modifications for the E-GMP platform are failing to safely mitigate high-voltage current spikes. The electrical strain or servicing procedure required by the recall has directly compromised the adjacent power relays. This creates a severe safety hazard, leaving drivers exposed to subsequent high-voltage component failure immediately after completing mandated federal safety recalls. NHTSA should investigate cascading high-voltage component failures occurring in vehicles directly following the completion of Recall 24V-204. 1. Sudden Unintended Acceleration On two (2) separate occasions, after coming to a complete stop at a red light with foot on the brake pedal, the vehicle suddenly lurched forward on its own. 2. Brake Pedal Malfunction On more than ten (10) occasions, when attempting to stop at a light, the brake pedal briefly behaves like the accelerator for approximately 1–2 seconds. Driver has to release the brake and press it again to restore normal braking function. 3. Doors Unlocking/Opening on Their Own On more than one hundred (100) occasions, approximately 10 minutes after parking — with no key fob or phone nearby — a door opens on its own. Confirmed via mobile app notification and visual confirmation from a distance each time. 4. Apple CarPlay Connectivity Issues CarPlay frequently fails to connect. During phone calls specifically, the connection drops in and out intermittently. 5. Sticky Residue on Door Handles A sticky, adhesive-like residue is present on the interior passenger door grab handle and rear door handles. Residue does not clean off and transfers to hands on every use.

Rear hatch window spontaneously shattered for no apparent reason. Car was parked in a Florida parking lot, no use of rear hatch and no use of rear defroster. I understand there is a potential class action lawsuit for the Ioniq 5 model years 2022-2025 for this problem.

- #11748381July 5, 2026LANE DEPARTURE: ASSIST
The ioniq 5 highway driving assist, which steers the car and keeps it centered in the lane, will disengage without clear warning - the small green steering wheel icon (which is at the top of the dash cluster) simply becomes grey and blinks, but there's no audible chime to indicate that the car has disengaged the steering assist. I have had multiple times on the highway where it disengaged and I don't notice (because I am looking at the road, not the dash) and one time on the highway it disengaged during a turn and hit the rumble strip, causing me to swerve back onto the road. My hands were on the steering wheel and I was paying full attention, but I was expecting the car to handle the turn, and it just disengaged without me noticing. Had there been another car in the middle lane or I reacted slower, it's likely a crash would have occurred. This has been documented as well on Reddit and confirmed by multiple owners, who have mentioned they have disclosed to the manufacturer: [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Description of Safety Defect:The integrated Forward Attention Warning (FAW) driver-monitoring camera located on the steering column continuously fails to track facial structures or pupils during routine daytime driving. This occurs despite maintaining a proper driving posture, adjusting the steering column layout, and removing any facial obstructions.Safety Consequences:When the system falsely registers a "distracted driver" state due to its faulty sensor calibration, it routinely triggers erratic high-pitched chimes and immediately disables active highway driver assistance systems, including Highway Driving Assist (HDA), Lane Keeping Assist (LKA), and Smart Cruise Control. This abrupt loss of active steering guidance and vehicle speed maintenance creates sudden, hazardous driving situations on open highways. Furthermore, to get the faulty alert chimes to clear, the driver is forced to take their eyes off the roadway to stare directly into the steering column sensor camera, actively causing driver distraction.

I have a 2023 Hyundai Ioniq 5 Limited and had the ICCU "popped" (failed) which took about two weeks to replace. This time we were lucky that in happened at our home but had we been driving the car would it would have completed died. We regularly drive from from [XXX] to [XXX] were there are large section of [XXX] with no cell service. Meaning potentially walking for miles in extreme heat or cold. Hyundai right now seems only interested in continuously replacing the part rather than actually fixing the route cause, making it so the car is not trustworthy or safe to drive long distances. During routine driving, the vehicle suddenly displayed an electrical system. Error code followed several minutes later by a 12-volt battery error. The vehicle was stopped and turned off. When turned back on all warnings disappeared and vehicle was returned to service for approximately 3 MI before warnings appeared again. Vehicle was then parked as it entered limp mode with a maximum speed of 11 mph. manufacturers roadside assistance was called and arrived to jumpstart the 12-volt battery which resulted in the vehicle turning on but not charging despite sufficient charge in the high voltage battery. Vehicle was stranded in a secure parking lot after hours with Windows unable to be closed and doors unable to lock. The vehicle is immobilized due to being unable to put the vehicle into neutral to move it to a suitable location for a tow. Vehicle was certified by dealership as a certified pre-owned vehicle approximately 45 days ago. All listed recalls have been checked and no undue stress has been placed on the vehicle since purchase. Vehicle stranded to adults and one toddler in 80 plus degree heat without any ability to remove the car to a safe location or secure transportation to an appropriate business.

3/31/2025: New Vehicle leased. 11/24/2025: 12V battery failure. Battery replaced by dealership. 4/22/2026: 12v battery failure. Battery replaced by dealership. 6/7/2026: ICCU failure. Car cannot charge 12v battery. Car in “limp” mode - cannot drive faster than 40 mph while on highway. Fortunately was able get car to safe location to be towed. All three instances appear to be due to the same engineering flaw in the vehicle.

1. What component failed, and is it available for inspection? The Integrated Charging Control Unit (ICCU) failed. The vehicle is currently at the dealer's service center, so the failed component is available for inspection upon request. 2. How was your safety or others' safety put at risk? After parking and re-entering the vehicle following a grocery stop, the vehicle displayed "Check Vehicle Electrical System" followed by "Check Vehicle Power Supply" warnings. This occurred without warning while the vehicle was in normal use, and the vehicle had to be towed since it was no longer driveable. The sudden, unannounced failure of the electrical/power system created a risk of being stranded in traffic or in an unsafe location had the failure occurred while driving rather than while parked. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. The vehicle was towed to the dealer's service center, where the Hyundai-certified service technician confirmed the ICCU failed and informed us it needs replacement. 4. Has the vehicle been inspected by the manufacturer, police, insurance representatives, or others? The vehicle has been inspected by the dealer's Hyundai-certified service technician, who confirmed the ICCU failure. No additional inspection by Hyundai corporate, police, or insurance representatives has occurred at this time. 5. Were there any warning lamps, messages, or other symptoms prior to the failure, and when did they first appear? No warning lamps or messages appeared prior to the incident. The first symptoms were the "Check Vehicle Electrical System" and "Check Vehicle Power Supply" messages, which appeared suddenly upon re-entering the vehicle after a grocery stop, approximately one week after purchase (used) at roughly 8,000 miles.

This is an EV . We use the regenerative braking to slow down. This car has both a front and rear dash cam. The car is about two and a half years old and has about 20,800 miles. We were approaching a red traffic light with several vehicles stopped in front of us. My partner was driving and had slowed to 8 mph, with her foot off the accelerator, advancing slowly towards the car in front of us when the vehicle experienced unanticipated, rapid acceleration. With quick reflexes, she was able brake hard and avoided a collision. Ten minutes later she was parking at our house, going 4 mph and it did it again, coming within four feet of the building. Neither time was she touching the accelerator, but the car accelerated like the accelerator was pressed to the floor. This is very dangerous.

- #11744555June 16, 2026FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, LANE DEPARTURE: WARNING, LANE DEPARTURE: ASSIST
The driver attention warning feature is defective and often requires you to take your eyes off the road and stare at the DAW sensor in order to eliminate the car's constant "attention" warnings. Even while looking straight ahead while driving, the DAW is often erroneous and will disengage safety features (e.g., lane keeping assist, adaptive cruise control, etc.) because the DAW erroneously thinks that the driver is not paying attention. As another example, if you are driving uphill/downhill or on a curved road and are looking at the road ahead of you at a slight angle, the DAW will erroneously report that the driver isn't paying attention. The system will then disable the cruise control and lane keeping assist, creating a hazardous situation. Alternatively, the driver can look DOWN (not looking at the road) at the DAW sensor and stare at it until the car reacts. This takes your eyes off of the road and creates hazardous situations. As someone who wears prescription glasses, the problem is even worse. I cannot set a comfortable seat/steering wheel configuration where the DAW can "see" through my prescription glasses. If I want to drive in my normal position, I either need to take my glasses off or turn off the safety features (e.g. lane keeping assist and adaptive cruise control) in order to drive. Hyundai is well aware of this problem and it exists across much of the Hyundai and Kia platforms. Dealerships say that the car is working as intended. In reality, the defective DAW system creates more risk and contributes to a significantly less safe ride (as I'm constantly taking my eyes off the road to stare at the darn sensor).

On June 13, 2026, my 2024 Hyundai Ioniq 5 experienced a sudden and dangerous propulsion failure while in active motion. The vehicle had approximately 9,447 miles on it and was at 58% battery charge at the time of the incident. The vehicle was leased in January 2025 and has been serviced exclusively at the Hyundai dealership. While driving with two occupants, the vehicle abruptly lost all ability to accelerate. The dashboard displayed a critical warning: “Stop vehicle and check power supply” along with a red battery icon. This failure occurred without warning and immediately placed us in danger of being struck by surrounding traffic, as the vehicle could no longer maintain speed or respond to throttle input. I was able to coast the vehicle off the roadway. Shortly after stopping, the vehicle issued an additional warning stating that auxiliary functions were draining the battery. Within moments, the vehicle shut itself down completely. This left both occupants stranded in extreme heat (over 100°F) with no air conditioning, no ventilation, and no ability to restart or power the vehicle while waiting for rescue. This created a serious risk of heat-related medical distress in addition to the initial roadway hazard. The vehicle’s complete loss of propulsion followed by a full electrical shutdown represents a severe safety defect. At no point should an electric vehicle with more than half a charge and fewer than 10,000 miles lose propulsion during active driving or shut down all systems while occupants are inside. This incident posed a significant risk of collision, injury, or heat-related harm. I am requesting that NHTSA investigate this failure mode, as it may indicate a broader defect affecting the propulsion or battery management systems of the Hyundai Ioniq 5. The vehicle is currently at the Hyundai dealership undergoing diagnostics and they confirmed they had difficulty jumping it to move it from where the tow truck had parked it on arrival.
