I am reporting a suspected safety defect involving inconsistent deceleration behavior in my 2026 Hyundai IONIQ 5. I regularly use Hyundai’s i-PEDAL one-pedal-driving mode. In this mode, releasing the accelerator produces strong regenerative deceleration and can bring the vehicle to a stop. However, the vehicle does not retain this selected driving behavior after it is turned off and restarted. It reverts to a mode requiring substantially more use of the brake pedal. The behavior also changes in connection with shifting into reverse. In a Costco parking lot in [XXX], I stopped near the customer pickup area while using i-PEDAL. I briefly turned off the vehicle while a passenger loaded groceries. When I restarted and began moving, I expected the strong accelerator-release deceleration the vehicle had provided immediately before shutdown. Instead, the vehicle continued moving because i-PEDAL had been reset. A crosswalk ahead contained pedestrians, and I had to apply the brake pedal to avoid striking them. No collision or injury occurred, but this was a serious near-miss. The concern is not simply that the vehicle offers different regenerative-braking settings. The safety concern is that a driver-selected setting that materially affects accelerator-release deceleration changes automatically after a brief shutdown or simply backing up. A driver who has adapted to i-PEDAL may reasonably expect the vehicle to decelerate as it did moments earlier. The regeneration level should be set and remain as the driver prefers, not reset on each start or backup of the vehicle. I request that NHTSA evaluate whether this inconsistent default behavior creates an unreasonable risk of unintended vehicle movement or pedestrian collisions and whether the vehicle should retain the selected mode or provide a prominent warning requiring driver acknowledgment whenever the deceleration behavior changes. Active powertrain failure on my 2022 Ioniq 5 SE, which poses an immediate and severe safety risk. On May 18, 2026, the vehicle suffered a catastrophic loss of power and a complete failure to charge. At Murdock Hyundai of Lindon, technicians diagnosed it with trouble code P1AA600 and recommended a total high-voltage battery pack replacement at a cost of over $44,000. Immediate Fire & Safety Risk: The dealership technician explicitly warned me that if I continue to attempt to drive or plug in the vehicle, it will cause an electrical fire. Visible Electrical Arcing: The service technician stated that they physically witnessed an electrical arc occur when connecting the vehicle's high-voltage charging components. The technician's diagnosis strongly points to a catastrophic internal short circuit or a failed Power Relay Assembly (PRA) within the high-voltage housing. The company completely stopped working and Hyundai would not help. This has been an ongoing issue with other cars too. I tried multiple times working with both the deal and Hyudnai.

The vehicle will not turn off. I returned it to the Dealer, Oakland Hyundai, and their service department has had the vehicle since 7/27/26. When I brought the vehicle in to service it had 2,519 miles. Service rep has advised that technicians are trying to find the faulty part and have been unsuccessful to date. A technician from another location is scheduled to see the vehicle during the week of 8/31/26. I have filed a case with Hyundai America and will send a demand for restitution on 8/28/26. I view this situation as a clear safety issue - the car will not turn off.

- #11759157August 24, 2026STEERING, LANE DEPARTURE: ASSIST
While driving on the freeway with adaptive cruise control and lane keeping assistance on heading east into the morning sun my car suddenly attempted to veer strongly to the left as I exited the shadow of an overpass. I was able to stop the lane assist because I had a firm two handed grip on the steering wheel. Even with the firm grip and attentive driving I nearly left my lane as the wheels suddenly and strongly jerked to the left.

My car starting throwing a “check electric vehicle system” warning intermittently beginning on July 7, 2026 which went away when I restarted the car. This happened again on July 27, 2026 three to four times while I was driving. The car did not lose power. As I was going to dealer the warning cleared and no code showed up in my diagnostic history. On August 12, 2026, the car once again showed "check electric vehicle system” and would initially clear when I restarted the car. I then began having intermittent power. Floored, the car would only go 27 mph. Then it would speed up again, then slow again. I was not in “turtle mode.” Luckily, I was able to safely get the car to the dealership without having an accident. DTC P1A8D13 did show in my diagnostics. The technician found that the 12v battery input had melted at the ICCU. This is very dangerous and could have led to a vehicle fire. My 2025 is on its 2nd ICCU (now 3rd) and has not had any recalls, so this is very troubling.

After car has been parked, when trying to start car , car turns on briefly saying 12 v battery low come to safe stop. Then car dies. Car isn’t in motion since it’s been parked. First time it happened I needed a tow truck and the tow truck needed to jump my battery. Since then battery keeps dying when trying to start this has happened every time I start the car since July 11 2026

ICCU failure. Failure happened while merging onto the road from driveway. Upon failure, 12 V battery was drained and car was immobilized. Prior to car mobilization dash read “Check electrical vehicle system” then many different failures appeared on the dash, then the car shut down. This event left us stranded on the road, unable to move to a safer location. Issue was confirmed with dealer and part had to be replaced. Part was original to the car and issue is known by the manufacturer but not being dealt with publicly.

While driving on normal paved roads, a small vibration was heard from the rear of the vehicle. A person near the car when leaving the origin that morning heard a minor clicking sound at low speed from the rear of the vehicle. The vibration at that point seemed to be related to the rural mountain road that had some rutting. Continuing to drive, but onto better roads, the vibration became louder and louder, and appeared related to speed. Nothing appeared loose when stopping to charge. Vibration became loud (above conversation), stopped to inspect. Removed aerodynamic cover from lug nuts on right rear, and found two lugnuts completely sheared off (fell out when cover removed), with a third hanging on at less-than-finger tight. Vehicle driven slowly to safe location where it was towed to dealer. Dealer reported that the rim was bent in addition to sheared lug nuts as well as a damaged wheel bearing.

In June 2025, I started a lease on a new Hyundai Ioniq 5 SEL. On August 16, 2026, I attempted to start the car, but it immediately beeped and flashed a warning about the 12V battery (I didn't manage to take a picture). It then showed signs of an electrical issue, like flickering interior and exterior lights. It was undrivable. A short time later, there was no power at all. I took out the old 12V battery and measured the voltage with a multimeter, and observed 3.7V. After putting in a new battery, the car operated normally.

Two tires failed dramatically, (on two separate days, but separated by only 121 miles of driving), causing extensive damage the first time and causing a total loss the second time. The first tire has been disposed of, but the second is available still. Each was on the highway and luckily there was not much traffic so I was able to pull safely onto the shoulder each time. However, if I had not been able to get over, our safety and that of others was at risk if we had been involved in a collision. My safety and my that of my elderly, handicapped brother was at risk the first time especially, because this is an EV and at one point on July 4, 100 degree temperatures in the early afternoon in South Carolina, the electrical system had been damaged and we could not, for a few minutes, get doors or windows open. The components (tires) have only been inspected by repair shops. There was no warning either time. The tires are 235/55 R 19.

I started the car and got the warning "check electric vehicle system." The car then lost power. It would only go 27mph with the accelerator pedal pushed all the way to the floor. My safety was at risk because my car was not responding appropriately to driver input. The car is at the dealership as it is undriveable in this condition. This is the second incident with this vehicle within 6 months.

- #11756527August 12, 2026LANE DEPARTURE: ASSIST, LANE DEPARTURE: WARNING, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
When I use the adaptive cruise control and use the lane keeping assistance the system keeps giving me an error message that I'm not looking at the road. I believe this is a known problem and Hyundai pushed an software fix TSB 25-G1-16H in November 2025. My car appears to have the update but still repeatedly gives me the same error message. It's very distracting and since the error message flashes on dash where you usually see the camera monitoring right side of car that feature is impaired. I have had at dealership twice and while the technician knows its an issue there's nothing they can do without Hyundai's decision to correctly fix with software update. I am over 6' tall and where glasses, two things I can't change.

The contact owns a 2023 Hyundai Ioniq 5. The contact stated that while driving on the freeway, the vehicle shut off unexpectedly. The vehicle was coasted to the side of the road and parked. Moments later, the vehicle was restarted, and the vehicle drove normally. The contact stated that the failure had reoccurred on several occasions, and the vehicle was taken to the dealer each time, but the cause of the failure was not yet determined. The vehicle was not repaired. The manufacturer was notified of the failure. The failure mileage was 4,000.

Was driving to vegas, it was 103-107 degrees After fast DC charging drivetrain had issues that when pressing acceleration the car would have almost a loss in power that caused a stuttering movement. If forcefully driven, the error message drivetrain electrical system would pop up. Issue can be resolved but only after letting the car stay in park for 10-15 minutes after finished charging. Issue was repeatable but only if weather was hot enough. This shouldn't happen or at least warn the driver to not drive for a certain period instead of guessing the issue.

My 2025 Hyundai Ioniq 5 experienced a complete and sudden loss of drive power at high speed, generating open fault code DTC P1A9096 (Internal ICCU Failure) [24V-204]. This failure completely matches the component and safety hazard profile under active Federal Safety Recalls 24V-204 and 24V-868 [24V-204].The vehicle was repaired at an authorized retail facility, Hyundai of El Cajon. The facility's un-finalized pre-invoice explicitly verified the ICCU failure code but deliberately omitted the federal safety recall context, routing the mechanical intervention under an internal manufacturer 'TSB 24-EV-004H' to hide the defect from consumer safety databases [24V-204].This represents a deceptive administrative effort to under-report active federal safety recall failures on newer model inventories across the E-GMP platform architecture [24V-204]. I am requesting a formal safety defect enforcement review into the manufacturer's regional reporting data.

Going thru a parking lot to a drive thru at a fast food place, rounding a small curve at a very low speed then trying go to 1 st lane out of 2 lanes the car suddenly went out of control and accelerated to center pole. The pole was a barrier protecting the support pole for the canopy. The car front center hit the pole so hard that the front collapsed. The hood buckled in the center and the pole went half way to the windscreen. The air bags did not deploy. The driver got hit with steering or something in the chest and caused lot of pain for about 10 days. No internal injury was detected. The car was totalled. The car did not complain of any incident except the dash reported something binding when tried to back off from the pole. The car repair company declared it as write off including the auto insurance company. Police was called and they did not take any data no other investigation was done. We feel the car computer malfunctioned and caused the accident.

We are reporting a suspected thermal runaway incident involving our recently purchased used 2023 Hyundai Ioniq 5 and have significant concerns regarding the vehicle's safety. **Vehicle Information** * Purchased XXX, 36,662 miles * Clean inspection and no accidents per Carfax **Incident Timeline – XXX** Between 6:00 and 6:30 PM, while traveling northbound on [XXX] just past the [XXX] exit in [XXX], the vehicle struck a white piece of road debris that appeared flat, similar to Styrofoam (video available). There was no immediate loss of vehicle function. [XXX] The vehicle was then moved to the driveway for inspection after a low coolant warning appeared. Damage to the underside near the driver's side and observed blue coolant leaking from the area. The coolant reservoir under the hood was empty. Based on online guidance advising against driving a vehicle with a coolant leak, we decided to leave the vehicle parked in our garage overnight and arrange a flatbed tow to a Hyundai dealership the following day. The garage door was closed, and the vehicle was **not charging**. At approximately 8:10 PM, a burning smell was detected inside the home. Upon opening the door between the house and garage, smoke was observed. The family immediately evacuated, and XXX was called at 8:12 PM. The fire department arrived at approximately 8:18 PM and found the garage filled with smoke. At 8:30 PM, firefighters removed the vehicle from the garage. At 8:35 PM, flames were observed beneath the vehicle. Firefighters applied water directly to the fire until 11:00 PM, after which the vehicle was removed. [XXX] [XXX] [XXX] Manufacturer was contacted; denies any liability due to debris. However, we believe inadequate safety design contributed, including soft protection shield and poor location of coolant.
