- #11759473August 25, 2026FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Vehicle Information 2021 Ford F-150 XL Supercrew approximately 35,000 miles. While traveling southbound on [XXX] near [XXX], Ok, I was operating my 2021 Ford F-150 XL at approximately 75 mph with the cruise control engaged. As I approached slower traffic ahead, I attempted to disengage the cruise control by pressing the cruise control cancel/off button on the steering wheel. Despite pressing the button multiple times, the cruise control would not disengage. Same thing while tapping the brakes it would not disengage. The vehicle continued to maintain speed, and the cruise control system remained unresponsive to my commands. In response, I applied the brakes to safely reduce speed, I did not perform a panic stop or abrupt braking maneuver. Once I had the vehicle slowed, I moved safely onto the shoulder of the highway, well away from traffic. I then shifted the vehicle into park, shut off the engine, and restarted the truck. After restarting the vehicle, I elected not to use the cruise control system for the remainder of my trip, which lasted approximately eight additional hours to Houston, Texas, I have not used the cruise control since the incident because I am concerned that the condition could recur and create a safety hazard. No collision, property damage, or injuries occurred. However, the apparent failure of the cruise control system to disengage when commanded raises significant safety concerns and warrants investigation. There was no warning light illuminated. The contact pulled over and waited before restarting the vehicle. The contact stated that two days prior to the failure, a message was displayed indicating that an over-the-air software update was performed under NHTSA Campaign Number: 25V455000 (Fuel System, Gasoline). The vehicle was taken to the dealer, who determined that an unknown sensor inside the transmission had damaged the transmission and that the unknown sensor needed to be replaced. The vehicle was not repaired. The manufacturer was made aware of the failure. The failure mileage was approximately 82,449.

Within the last 6 months, the driver's side front door, the driver's side rear passenger door, and the passenger side rear doors actuators stopped working and the doors do not lock. The doors do not lock manually with the key fob or lock automatically when the truck reaches 13 mph. This is a safety issue as passengers could inadvertently open the doors when driving when the doors should be locked. This also prevents the factory installed alarm to work properly. it seemed odd that the 3 actuators seem to stop working around the same time.

I own a 2021 Ford F-150, VIN [XXX]. NHTSA recall 25V455 / Ford recall 25S75 affects my vehicle because of a potential fuel-pump failure that can cause loss of fuel pressure and an engine stall while driving. The recall was issued XXX, and my vehicle still shows an unrepaired recall with the remedy not yet available. I am concerned about the length of time that has passed without a repair being available for a safety-related defect that could cause the engine to stall while driving. I am requesting that NHTSA review the continued delay in providing a remedy. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

My 2021 F-150 has been diagnosed with a failed electric steering gear Code PSCM C1B00 49 2F. I understand there was a recall on some 2021 trucks with similar steering issues, although my VIN is not included. The part is on national backorder and my dealer says there are approximately 150 vehicles ahead of mine. This is a safety-related steering component, part number "ML3Z3504G"

My vehicle was left rendered unable to steer during use. It was taken to a local shop All Japanse Auto, who found the damage was consistent with Ford Saftey Recall 21S28. Upon speaking to AutoFair Ford of Haverhill service they informed me Ford did not include my truck in the recall despite it matching year and model and they would not be addressing the issue for me. Loss of steering mid use is amazingly careless and should be cause for taking more aggressive action

- #11758129August 20, 2026VEHICLE SPEED CONTROL, ELECTRICAL SYSTEM
Due to a dangerous design flaw in the 2021 Ford F150, I was put in a potentially dangerous situation by completely losing all power in the vehicle on a backroad. Design flaw is the DCN module, which floods into the vehicles electrical system/body of vehicle through the cowel drain that can supposedly be blocked by a singular piece of pine straw. Due to the design flaw, the vehicle completely lost all power and all electronics failed while driving down the road at 45 mph, putting both me and my father (riding passenger seat) in immediate danger. Ford knows and is aware of the design flaw and has refused to issue a recall, not only affecting my safety, but now I am going back and forth with ford over the fix to the tune of $2800 that they refuse to fix (vehicle is a gold certified cpo, has extended warranty). When asked how to prevent in the future, ford was and is not aware of how to prevent in the future, and I can not get to the cowel drain clog without professional equipment and ford performing. There is no way to tell the cowel drain is clogged or not, and there is not anything in the ford owners manual related to maintenance of the cowel drain. Again, due to the inappropriately designed drain, this caused water leakage into my truck, posing an immediate threat to my life and others around me. I have all of the videos and pictures needed to show the failure of the system to provide to move this forward to get resolved. I had complete loss of power and electronics on the road. I tried to attach video but it would not let me. I can attach via email when requested.

I believe my vehicle is exhibiting the same transmission issues leading up to the failure of transmission components identified in NHTSA Recall #: 26V402. My vehicle is having hard downshifts at low speeds, usually between 3rd and 1st gear, while comping to a stop. It seems to happen randomly. My VIN number was not found to be in the effected vehicles when searching on the NHTSA Recall page.

I received codes p2888, p288a, p0c2c all within the current recall parameters however my vin is not listed in the recall currently but falls within the same guidelines for those model years, engine, and trim package. It is the parking/pawl codes. I cannot currently drive my vehicle and I am about to face very heavy charges with getting a repair for my vehicle. As of now the Ford service writer at Platinum Ford North in Pilot Point, Texas has told me it is unsafe to drive and repairs will need to be made and that the repairs are currently not within any safety recalls for my specific VIN.

- #11757079August 14, 2026FUEL/PROPULSION SYSTEM
On June 20, 2026, my 2021 Ford F-150 5.0L V8 suddenly lost engine power while I was driving on the freeway. The vehicle became disabled and had to be towed. The vehicle was towed to an authorized Ford dealership on June 23, 2026, where it remains. The dealership inspected the vehicle and confirmed that it is affected by Ford Recall 25S75 / NHTSA Recall 25V455 and advised that there is currently no available repair or remedy for the fuel pump issue. Ford Customer Service has also opened a case regarding the vehicle and provided a loaner vehicle, but no repair or estimated date for a remedy has been provided. The sudden loss of engine power while traveling on the freeway created a significant safety hazard. The vehicle could no longer maintain normal speed or acceleration, creating a risk of being struck by other vehicles traveling at highway speeds and a risk to surrounding motorists. Ford's recall identifies this fuel pump issue as capable of causing an engine stall while driving. The vehicle remains at the Ford dealership and is available for inspection upon request. To my knowledge, it has not been inspected by police or an insurance representative. There were no known warning symptoms prior to the failure; the vehicle was operating normally until it suddenly lost engine power on June 20, 2026. The failure is particularly concerning because the recalled component appears to have failed in the exact manner described in the recall, leaving the vehicle disabled while driving. Ford currently has no available remedy, and the vehicle remains unrepaired at the dealership.
