While driving at steady speeds around 50–65 mph, the vehicle intermittently feels like it shifts into neutral and the engine revs unexpectedly. After a moment, the transmission re-engages. This creates a safety concern, especially at highway speeds. Issue appears consistent with reported problems involving OSS (Output Shaft Speed) sensor or transmission lead frame signal loss in 2015–2017 Ford F-150 vehicles currently under NHTSA investigation.

While driving my 2021 Ford F-150 PowerBoost, the vehicle issued a pre-collision warning followed immediately by a restraint system warning. The airbag then deployed without any collision or impact. There was no crash, obstacle, or reason for deployment. The airbag deployment shattered the windshield and caused interior damage. This appears to be a false activation of the pre-collision system and unintended airbag deployment, creating a serious safety hazard. This could easily result in injury or loss of vehicle control. The vehicle needs to be investigated for a potential defect in the airbag system, sensors, or pre-collision assist system.

I am filing this complaint regarding an unresolved safety recall on my 2021 Ford F-150 (VIN: [XXX] ), Recall 25S75 / NHTSA 25V455, issued July 2025. As of today, April 10, 2026, no remedy is available and Ford has not provided a firm commitment beyond a verbal statement from a customer service representative. I contacted Ford in writing via certified mail (delivered March 27, 2026) and by email. Ford Case Number: [XXX]. I received one email response on March 31, 2026 from Patrice Hargrove, Truck Concern CXS, stating the remedy would not be available until June 2026, that Ford had not advised me to stop driving, and that Ford would not provide a loaner or rental reimbursement. I responded the same day asking for written confirmation of the June 2026 date and confirmation that my correspondence had been documented in my case file. I followed up again on April 6, 2026. I have received no response to either email. My concern is not only the recall itself but the history of this specific vehicle. In October 2022, this truck stalled in the middle of the street while I was driving with my children in the vehicle. It was towed and serviced, with the cause attributed to alternator failure. The failure mode described in Recall 25S75, an engine stall caused by fuel pump failure, is the same experience I have already had with this truck. This vehicle has also undergone a rear axle wiring harness repair in June 2024 and a transmission service under Customer Satisfaction Program 25L02 in December 2025. I have two young children who ride in this vehicle regularly. Ford has acknowledged a safety defect that can cause an engine stall while driving, has provided no interim safety accommodation, and has not responded to my follow-up correspondence in over a week. I am requesting that NHTSA review Ford's handling of this recall and the adequacy of Ford's response to affected owners. See attached document for complaint. To Whom It May Concern: This letter is submitted as a formal pre-packet notification to the National Highway Traffic Safety Administration (NHTSA). It is not a cover letter and it is not the evidentiary submission itself. Its sole purpose is to formally notify NHTSA that a certified, multi-issue dispute packet has been prepared and is being distributed concurrently to multiple governing, regulatory, and internal oversight entities, and to coordinate assignment of a reviewing party for secure evidentiary transmission. Background and Context This matter concerns a 2017 Ford F-150, purchased and serviced through West Motor Company, Preston, Idaho, and financed through Ford Motor Credit Company LLC. Safety- and recall-related concerns affecting the vehicle have arisen and remain unresolved, directly impacting the condition and value of the collateral securing the associated Ford Credit retail installment loan. Safety / Recall Concerns and Dealer Handling The vehicle has exhibited safety-related powertrain and braking symptoms consistent with known recall or safety campaign issues. Despite written requests for a complete VIN-based recall inspection and completion of all applicable recall work, these matters were not fully resolved. Dealer handling and diversion away from identified safety concerns have impaired both vehicle safety and collateral value. Related Loan Servicing Impact Concurrently, a Ford Credit dispute has been initiated regarding loan servicing practices that materially impaired my ability to access, understand, reconcile, and resolve the account. These practices include customer-portal lockouts during delinquency, forced fee-based payment channels, inconsistent fee assessments, and non-transparent interest application. The combination of unresolved safety issues and servicing barriers undermines any reasonable path to account stabilization. Purpose of This Pre-Packet Notification This correspondence serves to formally notify NHTSA that a multi-issue certified

1. 10R80 (CDF DRUM BUSHING) failure. The vehicle has pressure loss when put into gear, driving or slowing down, taking your foot off the gas, sluggish when put in first gear to drive, performance, and harsh shifting- at any speed when driving, missing gears - it' jolts hard (shifting up or shifting down) when the vehicle shifts, particularly between gears 1–3, 3–4, 6 - 8 it misses 2nd and 7th gear. While driving the vehicle it goes into to neutral despite being in gear. The vehicle has stopped on the road while driving and not able to put the vehicle into gear to drive at all. Wrench Light On: Codes P0732-00, P076F-00, P07F6-00, P07F7-00, U0252-00 Yes the vehicle is available for inspection upon request. 2. Our safety is at risk since the vehicle suddenly changes gears up or down causes major jolt sitting in the seat and abruptly slows down on the rode with traffic, or if you slow down or stop it might not go back into gear. I have had back surgery (lumbar) and it hurts my back. not to mention, the whiplash to our necks every time the vehicle jerks in and out of gear. Vehicle suddenly slows down making it dangerous for vehicle around us as well. 3. We first took the vehicle to Macon Auto Repair 2/23/2026 (no codes at this time unable to determine the problem), 2/24/26 took the vehicle to Rodney's Transmission for a second opinion - they checked the transmission fluids and added one quart of oil (still no codes to determine the problem) . The wrench appeared after the problems started and is whey we took it twice to two different shops not knowing what was the problem. When the wrench appeared we took the vehicle to Rodney's Transmission 4/2/2026 where the vehicle was inspected and the codes are from the diagnostic test done by, Rodney's Transmission, Thompsonville, KY 4. Symptoms yes, described above. Warming light not until after the problems continued to occur.

The component involved is the Electronic Power Assisted Steering (EPAS) system. It intermittently fails and returns unexpectedly, causing sudden changes in steering effort and jerking of the steering wheel. This creates a serious safety risk because vehicle control can change without warning, especially at highway speeds. The steering system (rack and pinion/EPAS assembly) has been replaced twice, but the issue persists. Replaced parts are not available for inspection. No manufacturer inspection has confirmed a permanent fix. This failure puts my safety and others at risk due to unpredictable steering behavior and potential loss of control. Other owners have reported similar EPAS failures, including sudden loss of steering assist, near-collision events, and accidents documented in public complaint records. The vehicle has received repeated repairs for this issue, including two rack and pinion replacements, without resolution. No police or insurance inspection has occurred. Warning symptoms include intermittent steering assist loss, sudden changes in steering resistance, and inconsistent steering feel. These occur without consistent warning, making the failure unpredictable and dangerous. This appears to be a recurring defect in the EPAS system that has not been reliably resolved through repair attempts or existing recall actions.

See attached document for complaint. I am writing with concern that a safety recall on my vehicle has not been corrected by Ford. Please see enclosed recall notice, dated July 2025. Since receiving the notice, I have not been notified to have the vehicle repaired and a recent online check for my VIN number indicated no schedule to complete the recall repair. In my opinion, nine months is too long a delay in correcting a major defect in the vehicle that could cause grave serious injury or death. I would like the repair to be completed immediately for the safety of my family and my well-being.

I just started the truck, put it in gear, and began driving slowly through the parking lot. The gas pedal was unresponsive—even when pressed all the way to the floor, the truck would not accelerate or move. This has happened multiple times, both while rolling slowly and sometimes from a complete stop. It occurs intermittently.

this is the 10 speed transmission. When starting from a stop the truck will start to shift thru gears and then shift into neutral and the engine will rev very high and then slam back into a lower gear.

Multiple front brake line ruptures on 2016 F150 putting safety at risk. 7/26/2023 front passenger side brake line rupture while driving requiring new brake line. 4/6/2026 front driver side brake line rupture while driving requiring new brake line, replaced front passenger side brake line again because it was cracked and about to rupture again. Ford is using defective brake lines, brake lines should not be considered a routine maintenance item like a car battery. This is putting my family at risk and other drivers.

The contact owns a 2019 Ford F-150. The contact stated that after starting the vehicle, the transmission hesitated to shift into gear. Additionally, the vehicle shuddered and stalled while driving. The engine revved and experienced rapid acceleration. The transmission shifted into reverse(R) one time, and the contact reversed into a parking space and turned off the vehicle. The vehicle was able to restart and shift into gear afterwards. There was no warning light illuminated. The vehicle was taken to an independent mechanic, but no diagnostic trouble codes were retrieved. The contact was informed that there may be water in the fuel line and to allow the vehicle to run out of fuel to drain the water. The failure persisted, and the vehicle was taken to a transmission specialist, who determined that the valve body needed to be replaced. The vehicle was not repaired. The manufacturer was not made aware of the failure. The failure mileage was approximately 62,500.

The problem is that the paint in the cargo area is peeling or fading, and I think that problem is a factory defect. That part of the cargo area is galvanized, I need to know if that is in a recall, thank you.
