The contact called on behalf of the owner of a 2013 Ford F-150. The contact stated that while his father-in-law was driving at an undisclosed speed, the vehicle unintendedly downshifted from 6th to 1st gear. The father-in-law pulled over to the side of the road, turned off and restarted the vehicle and the vehicle returned to normal functionality. No warning lights were illuminated. The contact stated that on a separate occasion, after the vehicle was stopped at a traffic light, the vehicle shifted into first gear. The vehicle then went into LIMP Mode. The contact stated that the failure persisted. The vehicle was not diagnosed or repaired by an independent mechanic. The manufacturer was not notified of the failure. The failure mileage was unknown.

Car randomly downshifting at freeway speeds. Mechanic is not sure what is wrong or how to fix. When Google, it seems to be a common issue with this year and I clearly very unsafe with people in the car on the freeway

The transmission failed at only 63,000 miles. Prior to the failure i had no codes or warning lights. It began to shift harshly and jump gears. At a stop light it would remain in 7th gear and then trying to get moving again would require a large amount of acceleration, then it suddenly would jump down to 1st gear and shoot ahead. This is a large pickup and i am concerned for the safety of pedestrians. Also, at faster speeds the transmission would suddenly downshift causing a large jerk and could easily result in a loss of control. After the transmission failed, i did some research online and was shocked to see so many similar postings from owners with this same transmission. Ford Motor Company agreed to pay a portion of the repair as they are fully aware this is a common problem with this 10 speed transmission. I hope you look into this as i feel it is an extreme safety issue for pedestrians as these are large pickups. Thank you.

- #11694636October 20, 2025EXTERIOR LIGHTING
The light bar on the tailgate that is the brake lights and reverse lights is known to get moisture inside and stop working. It is a known problem with Ford as they recalled them 2 years ago and replaced it under warranty. However the replacement part suffers from the same issue. So, my replacement light bar has stopped working, and even though I have Fords Premium Care extended warranty that includes all LED lighting, they will not cover the repair. This part is a safety item (brake lights and reverse lights) that is known to fail, it should be a safety recall item covered out of warranty anyway, and SURELY covered with an extended warranty (Ford Premium Care with Lighting added) but it apparently is not .... It seems that this safety issue should be covered and replaced Thanks

I own a 2022 Ford F-150 Lightning Lariat (VIN [XXX] ). Around June 6, 2025, I brought my truck to Apple Ford Shakopee for a scheduled appointment because I noticed moisture collecting inside the rear light bar. The service representative told me Ford considers that moisture normal, likely from a car wash, and that it would dry out on its own. At that time the truck had fewer than 36,000 miles and was still under the bumper-to-bumper warranty. The moisture never went away, and the light bar is now failing — my backup lights are dimming and flickering. I’ve since learned there was a recall (23S30) on 2022–2023 F-150 Lightning Lariat and Platinum trucks for micro-cracks in the rear light-bar lens that allow moisture in. My VIN currently shows no open recalls, but this failure appears identical to what the recall described. Additionally, owner forums are full of reports from other Lightning owners experiencing the same issue — including those who already had the recall repair completed, whose replacement light bars are now showing moisture again. This suggests the defect may persist even after recall repairs. I’m reporting this because the issue compromises the rear lighting and visibility of the vehicle. I hope Ford and NHTSA will review whether the recall scope or remedy should be expanded to include affected vehicles like mine and ensure owners aren’t left with expensive out-of-pocket repairs for a known manufacturing defect. - #11694608October 20, 2025FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, STEERING, LANE DEPARTURE: ASSIST
On October 8, 2025, my wife and I purchased a 2025 Ford F-150 Lariat, 3.5L Full Hybrid, with Ford's BlueCruise hands-free driving feature. As first-time users, I reviewed the owner's manual and Ford’s videos to understand BlueCruise. After several uses, I noted unpredictable behavior and now report a serious safety issue warranting federal investigation. On October 15, 2025, while driving north on I-95 near Boston with BlueCruise engaged in hands-free mode on an approved route, I pressed the Memory 1 recall button to adjust my seat. A chime sounded, and the instrument panel stated that Memory Recall is not active while the vehicle is moving. Unexpectedly, pressing the button deactivated BlueCruise, causing immediate deceleration and returning steering control to me. During a shallow left turn, the vehicle proceeded straight, almost deviating from the lane. I quickly regained control, preventing a lane departure. The owner’s manual and Ford’s resources do not mention that Memory Recall can deactivate BlueCruise. This undocumented behavior is highly concerning. On October 20, I contacted Ford (Reference Number [XXX] ), but their response was inadequate, offering only a generic process without technical team access or follow-up commitment. BlueCruise’s lane-centering struggles with unclear or conflicting lane markings. On I-95 sections under resurfacing, temporary markings are placed 1-2 feet from fainter original lines. BlueCruise inconsistently tracks either set, causing unpredictable lateral movements, including sudden lane-edge excursions or rapid shifts over bridges where markings misalign. When another vehicle overtakes closely, crossing into our lane, BlueCruise momentarily loses lane-keeping ability due to obstructed markings, resulting in rapid steering movements. While such overtaking reflects poor driving by others, an automated system must handle these common scenarios reliably. The daytime running lights on the Driver's side of my truck went out, which in the case of my vehicle means replacing the whole headlight assembly, which Ford is stating costs $2,000. I did not realize how much of a problem this is until I started reading online forums where it seems to be a problem with many Ford F-150 owners. Having to replace an entire headlight assembly for the running lights is asinine. This is a safety issue, and Ford should be made aware that they are creating undue hardships for their vehicle owners, as safety issues have to be addressed, and for some unforeseen reason, warranties and extended warranties do not seem to cover this issue, which appears to be very widespread, and most likely a manufacturing defect.

- #11694491October 20, 2025SERVICE BRAKES, HYDRAULIC
The contact owned a 2025 Ford F-150. The contact stated that while driving at 60 MPH, and while depressing the brake to stop the vehicle from hitting a stopped vehicle, the rear end locked up and did not slow down as intended, and the front of the vehicle collided with the rear of the stopped vehicle. The contact sustained a broken sternum and had a concussion, and needed medical attention at a later time. A police report was taken. The vehicle was towed to two tow lots, where it was deemed a total loss. The contact stated that one day she received notification of NHTSA Campaign Number: 25V488000 (Service Brakes, Hydraulic), four days later in the mai,l and believes the failure to be associated with the unrepaired recall. The manufacturer was notified of the failure. The failure mileage was approximately 10,200.

There was a sudden downshift at 50mph and a wrench icon showed up on my truck. I used on OBDII scanner and the codes p0706 and p0707 showed up. My safety was put at risk because I could have been hit in the rear and my rear tire locked up. I got the repairs done for a leadframe install. Is there any way I can get reimbursed for the repairs made, because this was once a recalled issue.

Truck has roughly 185,000 miles & is equipped with the 10r80 transmission. Transmission recently began experiencing harsh & hard shifts, skipping or slipping gears, gear searching and randomly shifts to neutral. Had transmission fluid/filter changed with little to no change in erratic transmission. Issues have been intermittent and I am unable to reproduce manually. 4 transmission shops and a ford dealership have all advised the most likely diagnosis is a failed CDF drum that has been a common issue with these transmissions but they cannot confirm without a teardown of the transmission. Vehicle is still drivable, if issues occur I park, turn the truck off for at least 1 minute. After starting the truck again the issue resolves itself. This issue leaves the vehicle susceptible to stalling/neutral on the roadway and at highway and city speeds.

When in reverse the backup Camera Goes to a clear image then randomly goes from a Black screen to a Green Screen and then back to normal. During that whole process and chime goes off in the truck. A message is displayed on the dash but it disappears too fast. The dealership has been able to recreate the problem but unable to repair my truck because they can’t catch the truck doing that error while hooked up to their computer system. Happens about twice a week.

The transmission would randomly shift into first regardless of speed. several times this occurred at highway speeds causing a sudden decrease on speed nearly leading to a rear end collision. The problem worsened to the point it would suddenly hunt for a gear. Fortunately this did not occur on the highway as it probably would have led to a serious accident. The problem was confirmed and repaired by an independent shop by replacing the molded lead frame in the transmission. There was no warning when the dangerous condition would occur.

I have experienced a reoccurring issue with an hvac drain leak that creates black mold in the carpet this happened in many 2019 & 2020 f150 models. Some early on & some 6 years later (like my vehicle). This was covered in a prior TSB but did not always fix the problem nor did they replace the carpet (to fix the mold issue). Also, the TSB does not cover this issue out of the 36 month warranty. This is a health hazard due to the black mold, especially since their fix does not always cure the issue.

We are having trouble with our rear-view backup camera not working properly like the F-250, F-350, and F-450 Super Duty trucks that are being recalled because their rearview camera images may not display correctly in certain lighting conditions. Our issues with the F-150 is the same as the others however ours has not been recalled and I think it should be.
