The contact owns a 2021 Tesla Model Y. The contact stated while driving 20–45 MPH on a six-lane highway, the contact attempted a hard acceleration, and there was an abnormal banging sound heard. The battery was disconnected. The pyrotechnic fuse went into failure mode. There were no warning lights illuminated. The vehicle slowed down, pulsated, and then shut down. The hazard lights, steering, and brake system became inoperable. The brake pedal was depressed to stop the vehicle from rolling. Additionally, while closing the driver's side door, the glass window fractured. The local dealer was contacted and retrieved a system failure with the 12-volt battery. The vehicle was not repaired. The manufacturer was not contacted. The failure mileage was approximately 58,044.

I was sideswiped by another vehicle which just joined our direction of traffic coming out of a cutout. As soon as I saw the vehicle moving fast from inside the cutout to join our direction and into my lane, I immediately decided to move to the lane next to me. I was at the leftmost lane at the time. Halfway into the lane I just felt the violent impact of the other vehicle hitting us. My wife and I were shaken so bad, we were shocked and terrified at what just happened and what else might happen to us. I felt like we were travelling on the front wheels with our rear end up in the air. Next thing we knew, the car stopped in front of a tree. Our iphones and the our car's phone called 911. We were pulled out of the car and taken immediately into an ambulance and Orange Global Hospital, where we stayed for about 2 days. X rays, CT Scan and MRI were taken. NOTE: A ROD STICKING OUT FROM THE FLOOR HIT MY WIFE during the accident. This rod was connected to the wheel and was not supposed to be stick out inside the car during an accident! I BELIEVE THAT A DEFAULT IN CAR DESIGN CAUSED THIS ROD TO HIT MY WIFE , WHICH CAUSED SO MUCH PAIN, DOCTORS COMPARED HER BRUISED RIBS TO BROKEN RIBS!!

I bought a 2023 Tesla Long-range Y in June 2023 at Tesla Santa Fe, New Mexico. I have had trouble with the brakes applying on their own during cruise control. It was very scary the first time it happened, but luckily there was very little traffic on the 2-lane highway I was traveling on. The Tesla dealer in Santa Fe told me they don't have a software fix yet and he simply avoids cruise control. After a few months I cautiously tried cruise control again and got away with it...but the third time, months later, it happened again. When the free month trial of Auto Pilot was installed, I did not intend to use it. Toward the end of the trial, I used it on [XXX] coming home to Las Cruces and realized it might actually auto-brake on the freeway, which could be a disaster. After that trip a Tesla friend shared his near-accident-causing experience with the cruise control braking. Another issue is that the key card didn't unlock the car, then wouldn't start the car. I called Tesla and was told I could reset the key card using the Tesla App, but it did not work. The next day I called Tesla support; he ran the diagnostics and said "oh yes, your car needs to be re-booted". He did so, and the key card worked to lock the car, but not to unlock or start it. I asked him to send me a new key card. "We sell a set of 2 for $45" (this is what came with the car). I told him I paid $53,000 for the car less than a year ago, it did not come with a charge cable, and now the key card didn't work? He looked to see if the key card was covered under warranty and found it was not. I have been unable to reach a person to speak with at either the NM dealer where I bought it or the El Paso dealer where I test-drove and attended the seminar, and have waited on the phone for exorbitant amounts of time while the bot tells me I can manage most problems on the Tesla app. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On Wednesday, [XXX], 2024 between 1015-1045 am while traveling on [XXX] , my vehicle unexpectedly accelerated while entering the left turn lane. On ramp to [XXX] northbound, Monroe county, Pennsylvania. At the time I was giving steering input and had also applied the windshield wiper system. This happened a second time while approaching the intersection of [XXX] northbound. While taking my foot off the accelerator pedal to allow for regenerative braking and using the windshield wipers the vehicle accelerated at a quick rate. In both instances I had to apply the brake to slow it down. I noticed Tesla on that date. Tesla related that someone would contact me. No co text has been made as of the submission of this report. Tesla removed the ability to use only cruise control, with adaptive following distance, and have forced the only option to now use full self driving instead (with a recent soft-ware update). I do NOT feel safe using full self driving (even supervised) and believe Tesla is being unsafe forcing full self driving use. We used to be able to just use cruise control-with adaptive following distance-a driving option in all other recent cars!!! , but cannot do that now in Tesla? Why would a company take away what I consider to be a safer option?? This is not an improvement (but a forced help-Tesla-improve-it’s-unsafe-full self-driving-software!)

On Sunday [XXX] I was driving to work, there was a moment where I felt the steering wheel was very stiff to the point where it felt weird and hard to control. I felt the two front wheels a bit odd, but then it went back to normal. I paid no attention to it after. As I was driving on [XXX] , my car started to automatically move to the left and I tried steering my wheel to get back in lane, but the steering wheel got so stiff that even using all my might I could not steer it. Eventually the car slammed against the cement wall three times as it spins. At the end, the police came on site and saw that the two front wheels were pulled inward. I've had Enhanced autopilot since June 24 2022. Since that time I've accumulated approximately 15,000 miles, with approximately 80% being auto pilot miles. On May 14th my car was upgraded to "Full Self Driving (Supervised)" without my consent. I was given 30 days of FSD trial, with no way to opt out. I was presented with a software upgrade that looked like a normal upgrade, however after the upgrade I was told I had FSD. I immediately disabled FSD, only enabling autopilot (Highway only), and attempted a drive from Fairfield, CA to Concord CA along [XXX]. During the drive, the car was not able to maintain lane control, and would suddenly apply hard braking randomly. On two occasions the car accelerated towards stopped traffic, and I had to disengage autopilot. I ended up manually driving the car as it was unsafe to use to assistive systems. I contacted Tesla, who said they where unable to remove the FSD trial, and I was stuck with the upgrade, even though it made the car unsafe to drive. I purchased a new 2023 Tesla Model Y long range Dual motor AWD in September 2023. On March 20, 2024, while I was parking my car in parking lot of strip mall, I somehow became confused and panic. I unintentionally pressed the accelerator with some force which resulted in movement of car in speed and it hit a structure. The car failed to apply the emergency breaks before collision and hit the glass wall of a structure (dining room beside footpath) with great force and break the glass wall. Even then it did not stop after passing through the glass wall. I then turned the car on the foot path beside main boulevard road and then applied the break to stop the car. Front of the car, windshield and battery was badly damaged. The car was latter shifted to Tesla collision center. Battery damage was irreparable and needed replacement. Since the estimate sent by Tesla collision center to Insurance was very high, the Insurance decided to totaled the car. Although nobody was hit by the car and I was also safe, It could have severely injured other person or I could have severely injured if I was not able to stop the car before another collision with fast moving cars on the boulevard. This was a "complete failure of application of Emergency Brake" before the collision - important safety feature to save a life. Please report this to Tesla and send me the feed back. I do not know whether I will get compensation from Tesla for this technical fault as it put my life in danger and there was financial loss.

Took Tesla's 30-day free trial offer of Full Self-Driving (supervised) for my Model Y. Encountered several concerning situations: 1. Autopilot failed to detect a parking barrier arm, causing acceleration towards it; fortunately, I intervened in time. 2. Merged onto the highway in the wrong lane, prompting honks from vehicles behind me. 3. Wheel hit the side road curb when making a right turn, damaging the hubcaps and tire on the right rear side. Unsure if Tesla will take responsibility for repairs, but I feel it's important to share my experience to highlight that Autopilot may not be as advanced as expected.

The screws of front left lower control arm my Model Y came off. There was initially a clicking noise and the noise turned into a larger "boom" noise and when control arm came completely detached from my car. There were two large bolts found below the car. The car was relatively new and never been in an kind of accident or had any type of impact. Luckily, I was backing the car out of my driveway when this occurred, or this could have been a more severe accident, if it occurred while driving on the road. The entire hood of the car landed on the front left tire and there was no way for the car to move. It was towed to the Tesla service center, where they fixed the car. It was covered under my warranty as the car only has 16,700 miles on it. The service center was unable to diagnose the root cause of this issue. The service center indicated that they have seen similar issues to this, but in very rare instances. They said the car is now safe to drive, but this was something I have never had happen with any other car in 30 years of driving. However, I felt that I had to notify someone of this issue.

while driving on the NJ Turnpike I heard a large "pop" noise. Shortly after as I was on the Exit 7A exit ramp I lost all power steering. I nearly hit the guardrail before the car came to a complete stop. I was able to maneuver the car off the exit ramp and pulled over. The was an error message appeared on the screen indicating "power steering reduced." It wasn't reduced it was non-existent. The car was flat bedded to the Tesla service center and a week later the car repair was completed. The service center indicated it was the "steering rack" broke. Although I don't know exactly what that means, even the service center admitted that this failure should not have occurred. The car is 2 1/2 years old with 90k miles. It is out of warranty so i was responsible for payment. The reason for my complaint is that a catastrophic failure in the steering rack should not happen on a 2 1/2 year old car.

While driving on the highway at 75mph, with "Traffic Aware Cruise Control" (aka, "Adaptive Cruise Control") enabled, a white napkin had blown into the lane ahead. This caused the car to slam on the brakes, and rapidly reduce speed to 20 mph, before I was able to re-accelerate back up to speed. I consider this a dangerous malfunction of the Adaptive Cruise Control system (and possibly the Automatic Emergency Braking system as well) on a high speed lane on a highway, which easily could have resulted in a high speed rear end collision and injury. (Fortunately, there wasn't any traffic close behind in the the lane.)

While driving on highway at around 60,65 miles speed, I saw a warning that fronk is open. I dont think I pressed open fronk while driving and not sure even its possible while driving. Fronk wasnt open when I started the drive for sure else would have noticed the warning. Warning actually started after driving for 5,6 miles. I was on highway and took me some time to find a safe place to get on the shoulder and when I checked fronk was actually open. Im not sure how much an issue it was or if Tesla fronk can get loose and fly or hit the windscreen and cause visiblity issue or accident involving other vehilces but this was something which was a big concern for a moment while driving at high speed.
