During my trip, traveling at speeds ranging from ~30 to ~75mph my car suddenly and with no advance warning braked to a full stop, four separate times. I was not using cruise control or any driver assisted features. There were no obstacles in the road. I did not get in an accident because there were no other cars near me on the two different roads I traveled on.

I used the Full Self Driving software Tesla provided us in April. On [XXX], the car did not detect a curb therefore the car went right over the curb which popped both right side tires and the front rim. Damage was $1,800. I brought this up to Tesla and they informed me that it was my fault for not taking over and refused to help with the repairs. I understand taking over if there was a 3-foot shoulder, but on roads with no shoulder such as this one I found it impossible to correct in time. Also, at the very last split second, the torque on the wheel was too tight too correct (not enough time anyways). This is a busy state maintained road in the middle of Columbia, Missouri near a college campus and my safety was put at risk while I had to find the next side street to pull over. This is dangerous software that is not ready yet. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

On [XXX] around 9:20 pm we were driving south on [XXX] in Vancouver, WA. The car was on cruise control (not autopilot or autosteer) at 65 mph. The car braked really hard and dropped the speed to nearly 20 mph. We do not know why the car braked, there was no one in front of us. Fortunately there was no one behind us other us we could have been rear-ended. I quickly took control of the car and increased the speed. We have not driven the car on highway since then or taken the car to the dealership yet. There was a message on the screen after the incidence but it disappeared before we could read it. Safety issue with Tesla's auto pilot braking without cause in the middle of freeway. Many times accidents were about to happen when I intervened and press onto the gas pedal. It seems like there are other complaints and not sure why NHTSA is not doing about it? Is Tesla that powerful that government does not want to disturb anything?

I recently saw there was a recall update issued by Tesla regarding the seatbelt alert. My driver seat does not sense I’m in the seat, and allows me to drive with out a belt on. Even if they send an update to my car, I won’t be alerted. Someone could use the lack of sensor to not remain in their seat. Inflatable doll with sunglasses could take my spot. My question is, is this included in the recall regarding the seat belt notification when not buckled since it’s part of the safety restraint system?

WE WERE TRYING TO PARK THE CAR IN OUR GARAGE WHEN THE CAR SUDDENLY SPED UP. AND HIT OUR THE FENCE . i RELEASED THE PEDAL BUT IT KEPT DRIVING UNTIL I HIT THE BRAKE. AIR BAG DID NOT DEPLOY. THE CAR?S FRONT WAS SMASHED. OUR FENCE IS ALSO BADLY DAMAGED. I HAVE ASKED TESLA TO INVESTIGATE IF IT WAS BECAUSE OF THE AUTOPILOT TAKING OVER BUT HAVE NOT RECEIVED A RESPONSE FROM THEM YET AFTER MORE THAN A WEEK.

This is my fifth Tesla, so i.am very familiar with the Self Driving feature which is mostly good. However, recently I got a Model Y and with in 24 hours I was rear-ended on the freeway! I had the car in Full Self Driving mode (FSD)! For some reason it just braked hard in the middle of my driving however, traffic was backing up. I injured my lower back and neck and I'm now having to go in for therapy and schedule a CAT scan. Luckily the person behind me was fine. After the incident I downloaded the zip disk which records incidences. The car had documented pre and post accident! But the actual incident there is no record... almost like it got wiped out!?!

My family and I were on [XXX] heading toward Sacramento when the Tesla Y suddenly braked. We were using autopilot and going around 70 mph. The car was trying to pull a full stop in the middle of the freeway! I quickly disengaged the autopilot and stepped on the pedal to speed back up. I'm guessing the Tesla saw a hazard on the road and put on the brakes. The problem is there were no hazards. Our safety was put at risk because we could've been reared, and it could've caused a pileup. Luckily, it was early Sunday, and there weren't a lot of cars on the road. We stopped using the autopilot immediately, so the problem has not been reproduced. We have an appointment with Tesla in four days. I will be reporting the incident to them. I'll ask for an inspection then. There were no warning lamps or other symptoms prior to the sudden braking. It was a very jarring experience. I have my kids in the back seats. The Tesla accelerated by itself while I am driving in a heavy traffic. To avoid hitting the front car, I braked several times, which didn’t work at all. It still accelerated, instinctively I steered wheels to the right front and hit a small hill. Airbags are out. I struggled to get off the car and shouted for help. 911 was called and emergency ambulance took me to ER. Luckily I am alive, it could be much worse that I would tremble when thinking of that crash scene again. This happened on [XXX]. I am waiting for the report from the police and the next steps from Tesla Insurance. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

The Indicator for the park break is the word "PARK" and not the symbol for the parking break ((P)). This is dangerous because the word "PARK" is ambiguous and could mean the vehicle is shifted into park (without the park break engaged) OR the park break is applied. In fact my spouse was parking the vehicle on a hill and not engaging the park break. He insisted that there was no difference between the two. There is a huge difference. Only after engaging the park break and listening for the rear breaks to electronically engage was I able to convince him that "PARK" was actually ((P)) and that the park break was NOT applied until the button was pressed and the word "PARK" appeared. Please use the correct ((P)) symbol to indicate the parking break is engaged and to clearly differentiate it from the vehicle being shifted into Park.

Turn signals turn off prematurely. Disengaging the cruise control or entering a turn lane after the turn signal is activated appear to turn off the turn signal before reaching the intersection or turn. This occurs whether I use the half or full press on the turn signal stalk. To be honest I'm not sure the difference between the half and full press, since they appear to function the same. This is dangerous for 2 reasons. 1.) Vehicles waiting at the intersection may believe I have changed my mind on turn and take actions on the belief that I am not going to turn. 2.) It creates a distraction as I need to react and take further action to re-activate the turn signal rather than focusing on traffic and the turn.

Phantom braking occurred when the adaptive cruise was engaged when a truck entered the road from the right side. The car abruptly slowed when the truck crossed my lane although there was no eminent danger. The car behind us nearly collided with us due to the sudden braking incident. This is the third sudden braking incident; however, the others were related to a bridge or overpass.

Two incidents: 1) Non-accident: My wife was slowing to make a right turn at a stop light and the steering locked up for about 3 seconds, where she wasn't able to regain manual steering control. We did not have full self-driving mode at the time. Luckily, she was only going about 10 mph, and was able to brake before running into oncoming traffic. 2) Accident: Our son was driving on a country road, along a section with a slight curve. We had a free trial for full self-driving mode during the previous month, but it ended two weeks before the accident, so he could not have had it in self-driving mode. It appears that he didn't make the curve, but as evidenced by a single long tire skid mark, continued almost perfectly straight, across the oncoming lane, and ran into a redwood tree. The car rolled and ended up upside down, against the bank back on the right side of the road, with the lithium batteries on fire. Our son and his passenger sustained severe injuries and neither has a memory of what caused the accident. Naturally we suspect the possibility that the steering locked up again. But it also seems likely that the AEB and ABS were not functioning correctly, as if they were, there should have been several short skid marks for the tires on both sides of the car, rather than only a single long skid mark. Additionally, there's no shoulder on that road, and there is a clump of bushes that come very close to the white line, so it's possible that a steering auto-correct initiated the incident. We have not yet been able to recover EDR or log data from the vehicle.

On June,05,2024 there was a storm and there was flood areas. I drove the vehicle while passing these parts came out because water underneath removed them. The manufacturer did not tight the screws correctly and water caused detachment underneath behind the electric battery where the Rear electric motor is located. I reported this and schedule a service appointment but Tesla the manufacturer said they will not cover this under warranty. This is not fair. This should be secure and safe and not detached on its own when water hits it. This should be cover under the manufacturer warranty.

I reported this issue previously (11589255) and am providing an update: I took my car to the nearest dealer with my less-than-1-year-old non-functioning key card that TESLA told me I had to replace at a cost of 2/$45. I mentioned to the dealer, who is kind an helpful, about the automatic braking in cruise control. I had already disabled "Apply Brakes when Regenerative Braking is Limited" under Dynamics on the screen. (see photo) He showed on the screen, under Autopilot, that I could disable Automatic Emergency Braking, which I did. I was concerned that this warning pops up: "The vehicle will not automatically apply the brakes even when a collision is detected" (see photo) but he suggested I try it to see if it would solve the cruise-control braking. I was hopeful, and disabled it, knowing that it might take a while to know if that solved the problem. I drove the 30 miles home on the frontage road instead of the interstate, and when there were no other cars around, I set cruise control at 50 mph, the speed limit. I came to a series of small hills in the road, and was reminded of the first time the automatic braking happened, on a 60 mph road with longer amplitude hills of greater height. Amazingly, it happened again! Pretty significant braking and slowed me to 25. This time I waited it out to see if it would resume the cruise control at 50. It did not. I have since restored the Automatic Emergency Braking as that seems important for a crash situation. I have restored the "Apply Brakes When Regenerative Braking is Limited" since that obviously was not the problem either.

On two occasions, the Tesla hijacked 911 calls during emergencies. One time I was inside someone's house talking to 911. Suddenly I was cut off. The 911 call was hijacked by the Tesla and the person in the car did not understand how to respond to the emergency. On the second occasion, I was outside the vehicle, but the windows were open. I called 911 and was talking to them, but then the Tesla hijacked the call. I tried to continue the conversation through the open windows of the car, but this route of communication suddenly stopped. I changed to speakerphone and was able to complete the call through my phone. Tesla states that all car systems that use Bluetooth (Tesla, Apple Carplay) have this feature and that all is operating normally. (I guess if all employees steal from their employer, all is normal when someone steals from their employer.) Tesla says that before making a call, someone would have to disconnect Bluetooth or unpair the phone from the car. But that is not the solution. Tesla should use, for example, the in vehicle camera to determine if someone is in the vehicle before hijacking the phone call through Bluetooth and there should be an active prompt for Tesla to take over the call. Since Bluetooth is needed in order to unlock the vehicle, it is not a good solution to force users to turn Bluetooth on and off on the phone. Tesla indicated that they would log the complaint, but kept saying that since every other car did this they would probably not take corrective action.
