Sept 20, 2025. 2025 Model X door alarmed for being open when it was actually closed. Door would only open intermittently. Your in car and cannot escape through drivers door. It was locked shut and thought it was open. Parked car requested Tesla service. Mobile unit came out replaced electric opener. Door continued to lock driver in and out of car randomly for the month of October 2025. We parked car and scheduled appointments with Tesla multiple times. They were not concerned this was a safety issue. They reschedule our appointments multiple times. At the end of October after several service appointments and multiple parts replaced the door started working. Their final comments we "THEY WERENT ABLE TO VERIFY THERE WAS AN ISSUE."

While driving my 2022 Tesla Model X at highway speed, I experienced a sudden loss of steering assist shortly after Supercharging. The steering wheel became extremely difficult to turn, and I had to use significant force to maintain control. The vehicle displayed a ‘Steering Assist Reduced’ alert, FSD became unavailable, and the suspension behaved unexpectedly. After the incident, the vehicle was taken to Tesla service. At this time, I have not received a clear explanation of the root cause of the steering‑assist loss. Because this event occurred at highway speed and created a safety concern, I am submitting this report so the incident can be documented and reviewed.

On January 31, 2026, at 10:30 PM, I was driving from Fort Lauderdale to my home in Boca Raton, Florida. I was using FSD supervised mode. Close to my home, a big red image suddenly appeared, the car started accelerating fast, and I had no control over the car or its steering wheel. The car hit several electrical posts and only stopped after the airbags engaged. My wife was in the passenger seat. The front of the car was totally damaged, and I will have to total it.

I am writing to formally document a life-threatening mechanical failure involving my 2022 Model X on January 30, 2026. While my wife was operating the vehicle, it suffered a simultaneous and catastrophic collapse of all primary safety systems. During operation, the vehicle experienced a total loss of steering and braking control. This was accompanied by a rapid cascade of critical system alerts on the primary display, including “ABS Malfunction,” “Stability Control Disabled,” and “Steering Assist Reduced/Disabled.” Most critically, the vehicle exhibited Sudden Unintended Acceleration (SUA), increasing speed in direct opposition to the driver’s physical application of the brake pedal. This resulted in a near-miss where the driver narrowly avoided a construction crew.

The contact owns a 2025 Tesla Model X. The contact stated that after the latest update was pushed out, the speed regulation on the vehicle failed to function as intended. The contact stated that the speed regulation was set to "Standard Mode" and the vehicle accelerated to 70 MPH in a 55 MPH Zone. The contact stated that was not the mode selected. In addition, when the contact set the speed regulator to "Crawl Mode," the vehicle drove 10-15 MPH below the speed limit. No warning light was illuminated. The contact was unable to control the speed of the vehicle. The vehicle was not taken to the dealer. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was unknown.

The turn signal buttons intermittently fail to work. Many presses of the tuen signal buttons do nothing. When it stops working it stops for several minutes and then will start to work again. The problem is happening more and more frequently. The yoke has been replaced 3 times already for the same issue and after about a year just started happening again.

I am reporting a catastrophic suspension failure resulting in complete wheel separation on my 2023 Tesla Model X Plaid shortly after Tesla service. While driving, the front left wheel separated from the hub, rendering the vehicle not drivable and creating an immediate risk of loss of control, collision, and serious injury or death. Tesla towed the vehicle back to its service center and issued written service documentation stating (verbatim): “The front left wheel had separated from the hub. The issue was caused by missing hardware on the upper control arm, leading to further damage.” Tesla did not attribute the failure to impact, road debris, wear, or misuse. The missing hardware was at a critical suspension attachment point. As a result, Tesla replaced multiple major safety-critical components under Goodwill / Internal Service, including the front left active damper/air spring, upper control arm bracket, halfshaft, wheel speed sensor, and multiple structural fasteners. A wheel-off event caused by missing suspension hardware represents a severe safety defect with high risk of catastrophic injury or fatality. This incident raises serious concerns about suspension assembly, post-service quality control, and whether other vehicles may be operating with missing or unsecured hardware. I respectfully request NHTSA review this as a suspension/wheel retention safety defect and evaluate whether additional investigation or corrective action is warranted.

It was a windy day I was driving to work I was only driving max 14 mph and there were leaves flying due to the wind and without any warning the phantom break came on and my car just randomly stopped. It was in my neighborhood at 1830. It left me traumatized for few minutes because there was no need for the car to randomly stop like it did. The outcome could have been an injury if I was on the freeway or if a car was behind me. Till this day ever time I drive it i wonder if I will have the same incident again.

1. The Horn Button in NOT located in the center of the steering wheel but is located as a separate button off to the side and therefore is difficult to located in times of emergency when it is needed. 2. Where the button is located it is not intuitive in times of panic and emergency to try and located where to press the horn button. If you happen to be turning the wheel, it is almost impossible to find the button. 3. On future Tesla X models. Tesla the Manufacture decide to relocate the horn back to the center of the steering wheel. 4. We have had several close calls and missed accidents partly due to the fact we could not located the horn button in times of emergencies. 5. It seems like it would be a simple fix for the manufacturer to install an updated steering wheel for the safety of all occupants and other drivers.

Vehicle: 2023 Tesla Model X Component: Power-operated doors / door latching (auto-close / “cinch” close) Incident type: Finger pinch injury during powered door closing Summary of incident: While using my 2023 Tesla Model X, a powered door closing sequence resulted in a finger pinch injury. The door appeared to close normally and then performed an additional “cinch”/pull-in motion to fully latch. During this final motion, my thumb was caught in a very narrow gap and was pinched. There was no adequate pinch detection/anti-pinch reversal before contact and compression occurred. The gap was extremely small, leaving little time to react. Injury: Thumb crush injury with immediate blood under the nail (subungual hematoma), swelling, and altered sensation/numbness. I obtained medical evaluation and an X-ray, which showed no fracture. A splint was prescribed to protect the thumb. Safety concern: This is a significant pinch hazard because the powered closing/cinching can occur quickly and unexpectedly, and the door does not reliably detect or reverse when a finger is in the closing path. This presents a risk of more severe injury (fracture, nail-bed injury, nerve damage), especially for children. When it happened / conditions: Normal use, low speed/parked. (Add date/time/location if you want.) The incident occurred during the final powered latch/close step (“close then push/pull for full close”). What I want investigated / remedy requested: Request NHTSA to investigate the Model X powered door closing/cinching behavior and pinch detection effectiveness, including whether there are known blind spots or missing/insufficient anti-pinch protections. Recommend Tesla provide a software update or hardware change to improve pinch detection/reversal and/or limit cinch force/speed, and provide clearer warnings/settings to prevent unexpected powered closure.

My 2022 Tesla Model X has two recurring safety-critical defects that remain unresolved despite multiple repair attempts. 1. Airbag / Restraint System Failure (RCM2 / OCS Defect): The vehicle frequently misclassifies the front passenger seat. It shows a passenger when the seat is empty and activates passenger seatbelt warnings incorrectly. Recently the vehicle displayed: “Front passenger safety restraint system issue: Service required (RCM2_a064).” Tesla attempted to repair an electrical harness in the seat, but the problem returned. A Tesla technician documented that the seat may still need a new occupancy sensor. Because this affects the Occupant Classification System and airbag deployment logic, it raises potential FMVSS 208 compliance concerns. 2. FSD / GPS Safety Hazard (5 Unsuccessful Repair Attempts): Across five service visits, Tesla attempted to fix GPS drift and unsafe FSD routing. The vehicle still attempts incorrect turns, performs premature lane changes, and misinterprets intersections. One recent incident involved FSD attempting a left turn one intersection early, nearly causing a collision. Tesla replaced the GPS antenna, recalibrated cameras, and replaced the computer, but the unsafe behavior continues. These two defects—incorrect airbag/OCS behavior and hazardous automated-driving/navigation behavior—create a significant safety risk to occupants and other drivers. ISSUE: Vehicle intermittently displays "Front Passenger Safety Restraint System Fault - Service Required" with persistent warning chimes and airbag warning indicator. Occurs with and without passenger present. Causes significant stress/anxiety and erodes confidence in restraint system reliability during a collision. REPAIR HISTORY (all under warranty): [XXX] (30,638 mi) - Invoice #[XXX] - Connector grease treatment. Recurred after ~3 weeks. [XXX] (32,399 mi) - Invoice #[XXX] - Replaced Restraint Control Module. Recurred after ~3 weeks. [XXX] (36,106 mi) - Invoice #[XXX] - Pin retention repair. Recurred after ~3 weeks. [XXX] (43,204 mi) - Invoice #[XXX] - Harness tension adjustment. Recurred late Nov 2025. CURRENT STATUS: Fault resurfaced 5th time. Service appointment scheduled ~Dec 14, 2025. Vehicle at ~48,000 miles, approaching 50,000-mile warranty limit. PATTERN: Each repair temporarily resolves issue, but fault consistently returns after 2-4 weeks. Tesla unable to identify root cause despite multiple different repair approaches. ADDITIONAL: Original owner. Also had driver-side restraint wiring repaired Feb 2024 (23,310 mi). Issue may relate to known Tesla restraint system concerns. The vehicle presents a compounded safety defect creating a risk of passenger entrapment and high-voltage fire. 1. High Voltage Manufacturing Defect: On September 3, 2024, authorized Tesla Service technicians diagnosed a critical safety fault. The service record states technicians 'found A/C compressor HV harness was incorrectly routed, rubbing against the steering column' and 'found sand inside the connector'. Tesla repaired this defects under the 'Basic Vehicle Limited Warranty,' confirming it was a factory assembly error. This defect created an imminent risk of a high-voltage short to the steering column, which could result in driver electrocution or total vehicle power loss at speed. 2. Passenger Entrapment: The vehicle has a documented history of Rear Falcon Wing Door failure (7 repair attempts from April 2023 to May 2025). The doors frequently fail to latch or open. On July 8, 2025, the Left Rear Door failed to open due to a sensor fault. 3. Combined Hazard: If the incorrectly routed High Voltage harness causes a vehicle power failure, the rear doors' electronic latches will fail. Because the doors have a history of mechanical jamming, passengers in the rear seats would be trapped in a vehicle with a compromised High Voltage system.

I had my two Teslas parked in the garage. The model X was charging at about 1:30 AM. We heard explosions coming out of the garage. Went to check only to see my model X in flames. We evacuated and called 911 eventually a fire investigation report was made Showing the fire started within the vehicle outward. The battery pack was on fire. We had lithium contamination all over the house and this fire endangered five lives. This incident has been two years ago and until now Tesla refuses to acknowledge the faulty battery and is refusing to do anything about it

Tesla has taken the horn away from the traditional center of the steering wheel and put a ridiculously small button on the side of the steering wheel. Most times when you need to use your horn it’s because of a sudden, unanticipated situation. It’s intuitive to hit the center of the wheel. It’s absolutely impossible to find the tiny button in that situation. One has to take their eyes off the road and look for the button. By the time it is located the situation is over. I understand Tesla has corrected this on their current vehicles and gone back to the traditional center of the wheel. But that does not help owners of the affected autos. They should be required to offer a solution, like a stand alone button that can be attached to the center steering wheel. The current configuration is flat out unsafe.

I am writing to report and formally document a very serious and distressing safety incident involving my Tesla Model X that occurred for a second time, on [XXX]. While driving routinely into the parking garage at my workplace, my vehicle suddenly and violently stopped without warning. There were no obstacles, vehicles, or pedestrians in front of me. The abrupt stop caused my body and head to jolt forward with force, resulting in whiplash. I am still feeling dizzy, dazed, and disoriented, and I believe I may need to consult a neurologist as I have a headache from the whiplash. This is not the first time this has occurred. A similar but less severe incident happened previously while I was driving on [XXX] . At that time, I dismissed it as a possible glitch, but after this most recent and violent episode, I am deeply alarmed. I am extremely concerned about the safety of this vehicle. What if this sudden stop had occurred on the freeway, or while my two children were in the car? The potential consequences could have been catastrophic.
