CRITICAL FLUIDITY/ACCESSIBILITY DEFECT The vehicle lacks a standard mechanical horn assembly, instead it is equipped with a small electronic button. This configuration creates a severe safety hazard due to the following critical failure points: High-Stress Unusability: The button is highly difficult to locate during an unfolding emergency, causing a dangerous delay in driver reaction time. Loss of Function During Maneuvers: When the steering wheel is actively turning, the button's position shifts constantly, making it physically impossible to find or press reliably while evasive steering is underway. Failure to Warn: This design directly compromises the driver's ability to sound an immediate audible warning, drastically increasing the risk of an unavoidable collision.

On XXX, at approximately 3:30 PM while using FSD (Supervised) on software 2026.21.6, my Tesla Model X came to a sudden complete stop in the middle of a busy four-lane intersection. I had to intervene. The FSD feedback prompt appeared; I selected Critical. Approximately one mile later, while programming a destination, FSD pulled over unsafely on a busy multi-lane road with barely enough room for 50 mph traffic. I intervened again and selected Critical on the prompt. Neither event appears in the Dashcam Viewer (only Sentry parking clips visible). These continue a pattern. Previously, while driving from [XXX] to [XXX], the car jumped across freeway lanes unsafely. I reported it via the Tesla app and to Cathedral City Service. Technicians lacked diagnostic equipment, said they would escalate to the FSD team for research, and promised to realign the front cameras on a return visit. Months later: no follow-up and cameras still cosmetically misaligned. Camera Calibration menu only shows Clear option (I did not clear it). I have submitted a Service request and emailed Tesla requesting internal logs/telemetry and full escalation. Filing this for an independent record. The power steering fails very often making it hard to turn and happens when I’m on the highway going 60+ mph which is very dangerous and a threat to my safety. The only way to fix it in those moments is to pull over turn the car off which consists of me getting out of my vehicle and waiting two minutes. But usually when it happens once that day it occurs all day even if I reset the car and try all the other troubleshooting options which make it very difficult to rely on the car for safety. It happened only once every few months but now it happens almost every other time I drive my car. This is not a normal wear and tear issue as I’ve had cars before with over 50k miles and not once have I had a power steering failure on those cars which are older and have more mileage.

On 7/29/2026, 2:45 PM PST, I tapped the in-car myQ interface to send an "Open" command. Due to network latency/ connection drops in the Tesla infotainment system, there was a significant delay before the open command executed. After it finally opened, I proceeded to back out in Reverse. However, due to buffering/retry logic during the network reconnect, the system unexpectedly transmitted a follow-up "Close" command without any driver input. Because this automated ghost command executed while the vehicle was actively backing out, the overhead garage door unexpectedly began closing, striking the car and shattering the left Falcon Wing door glass. Safety Concern: The infotainment software suffers from a critical retry/buffering flaw where latent network API payloads execute out of sequence or trigger unintended secondary commands after initial connection drops. Transmitting an unprompted "Close" signal to an overhead garage door while a vehicle is actively moving in Reverse creates a severe collision hazard and safety risk for occupants and bystanders.

On XXX at approximately 465 miles, multiple critical safety systems simultaneously failed on my 2024 Tesla Model X (VIN: [XXX]). Dashboard warnings indicated that ABS, stability control, traction control, steering assist, and automatic emergency braking were all simultaneously disabled. The vehicle was taken to Tesla Service immediately. Tesla diagnosed the issue as a faulty wheel speed sensor and performed a replacement (RO# XXX). This simultaneous disabling of five critical safety systems on a vehicle with only 465 miles is a serious safety defect. Additionally, on XXX at approximately 21,180 miles, a passenger airbag warning light illuminated requiring service. This was resolved and addressed by slight moisture on son's shorts triggering the alert but unsure why this would cause such sensitivity. The vehicle has required 12 service visits since delivery in November 2024 for various recurring safety and mechanical issues. I was using FSD any driving on XXX between [XXX] and [XXX]. Suddenly the car put on the 4 way flashers, slowed down and started to pull to the side of the road. There was no shoulder so the car would have stopped in the driving lane. I immediately turned the wheel back into the center of the lane and stepped on the accelerator to avoid being rear ended. Fortunately the traffic was light. There was no indication that the FSD sensed inattentiveness. I was able to continue driving using FSD. This happened twice on my way to Huntersville on XXX and once on the return trip on XXX. I filed a report with Tesla and have an appointment for next week. While operating a 2025 Tesla Model X on Full Self-Driving (FSD Supervised), the vehicle engaged in leader-car following behavior and trailed a slow-moving vehicle onto an active railroad crossing. FSD failed to ensure the exit path was clear, causing the vehicle to suddenly decelerate from 12 mph to 0 mph, stranding the car directly on the tracks just as the crossing's active warning lights and gates triggered. When attempting to take immediate manual control to clear the tracks, the vehicle suffered a severe drivetrain control lockout. The vehicle remained unresponsive and stuck at 0 mph for approximately 3 seconds despite driver intervention to shift into reverse and accelerate. After the 3-second software lag, manual control was recovered, and the vehicle reversed. A dropping railroad gate arm struck the vehicle's panoramic roof and hood during the escape. Telemetry stats and multi-camera dashcam video of this autonomous software failure and control lockout have been retained.

I own a 2023 Tesla Model X and I am reporting a steering-related safety concern. When making hard left turns, the steering wheel/front end shakes or shudders and makes a LOUD sounds. This does not feel or sound normal and appears to be related to the steering or front suspension system. The issue happens during low-speed turning/maneuvering when the wheel is turned sharply to the left. I contacted Tesla Service regarding the issue. Tesla advised that this is a known issue and that there is currently no available fix. Despite acknowledging that it is a known issue with no repair available, Tesla still wants to charge me a diagnostic fee. I am concerned because this involves the steering system, and I do not believe a vehicle should have steering shake or shuddering during normal turning, especially on a 2023 vehicle. Since Tesla has stated this is a known issue but has no fix, I am requesting that NHTSA review this as a potential safety defect affecting the 2023 Tesla Model X. The issue has not been repaired because Tesla stated there is no fix available at this time.

How was your safety or the safety of others put at risk?** The vehicle struck lane delineator posts at approximately 70 mph without any observable warning or corrective action. Immediate manual intervention was required to avoid a potentially more serious collision with surrounding highway traffic. 3. Has the problem been reproduced or confirmed by a dealer or independent service center UNKNOWN. The incident has not been reproduced or independently confirmed. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others?** A Tesla Service Request has been opened. The vehicle has not yet been inspected by Tesla or any independent investigator. No police report was filed. 5. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear?** No. There were no warning lights, warning messages, audible alerts, emergency braking, or diagnostic notifications before, during, or after the incident.

I am reporting several safety concerns based on approximately one year of experience using Tesla Full Self-Driving (FSD). Primary Event: Between February 23 and February 28, 2026, while traveling northbound on the NJ Turnpike with FSD engaged, the vehicle accelerated to approximately 95 mph in a 65 mph. The acceleration appeared unexpected and excessive for the traffic conditions. I disengaged FSD and immediately took control of the vehicle to reduce speed and continued driving manually.No collision occurred, but I considered the event a safety risk because the vehicle exceeded the posted speed limit by a substantial margin. Additional Observations: 1. On multiple occasions, FSD appeared not to recognize or comply with “NO TURN ON RED” traffic signs.2. Situations where overhead or elevated traffic signs appeared not to be recognized by the system.3. On one occasion, the vehicle navigated toward railroad tracks where the roadway and tracks intersected at an approximate 45-degree angle. The vehicle behavior caused concern.4. When driving directly into bright sunlight, particularly when the sun is low and shining directly into the windshield, FSD performance appears degraded or confused. Human drivers typically use sun visors and other visual cues in these conditions. The vehicle’s driving behavior appeared less predictable during such events. 5. Lane changes are attempted across areas marked by continuous lane where lane change would not normally be expected. Additional Concern: A significant regulatory issue may involve situations where drivers intervene immediately before a collision. In real-world driving, drivers often instinctively take control when they perceive an imminent hazard. Therefore, when evaluating crashes involving FSD, I believe investigators should review vehicle data for period immediately preceding a collision, including several seconds before any disengagement, rather than focusing solely on whether FSD was engaged at the exact moment of impact.

In January my parents Tesla in self driving mode drove them straight through a building. The breaks didn't deploy, the airbags didn't deploy and the steering wheel was shaking and unresponsive. Thankfully no one was seriously injured. That said, there was a lot of physical damage to the restaurant/building and the car was completely totaled. We are certain that there were catastrophic failures with the car that caused the accident however the mechanic has informed us that due to her NDA she cannot provide any details about her findings after examining the car. She sent her findings to Tesla who was supposed to issue a report to us and the insurance company, but several months later we still have not received anything. They are well past the 30 days time limit. We cannot get anyone from Tesla to respond to us. There was a police report and the cops, firemen, and mechanics where all absolutely floored that the airbags didn't deploy given the damage to the vehicle. Self driving failed. Breaks failed. Steering failed. Airbags failed. and Tesla has failed to respond.

Every time I use FSD in my Tesla, and disengage it by steering or the brake pedal, I am FORCED to deal with a popup selection window that takes over my control interface and WILL NOT go away unless I divert attention from a critical issue such as a crash or collision that will be avoided by me taking over control. This popup MUST be disabled ASAP. It steals vital time and attention from actually driving the vehicle and can appear many times per trip. This forced feedback popup is extremely dangerous to the safe operation of the vehicle. This popup must be looked into and Tesla forced to find another way.

As I was driving the Tesla Model X, I received a steering assist reduced error on the dash. There was no immediate noticeability, but after about 5 minutes, it felt like there was ZERO power steering, with no additional warnings in the car. With zero power steering, car required extreme forces required to get to the shoulder. If attempting any kind of turn, car would require forces in excess of 80lbs of pressure to execute. Once safely in a parking lot, I put the car into SERVICE mode to check the errors. There was an EPAS error, where it said Torque Sensor 1 not receiving, along with a Torque Sensor 2 not receiving. This has occurred to me 3 times, in the 3 years I've owned the car. Other than "Steering assist reduced", there is no other warning that the car's EPAS steering system reverts to just manual forces required to steer a car. I do not trust my wife to drive this car, as she would not have the strength necessary to get the car to a safe location. If this happened while turning, there's a strong possibility the car would not turn at all, a huge safety factor.

The force of wheels pushing rain water on the fender garnish causes both the left and right fender garnish 4 push clips to come loose. When this fender garnish breaks and flies off it could hit a pedestrian, another car on the road or the car it is installed on causing an accident

On long drives it works pretty fine and does not do this much on highways only once but in hotter weathers it seems to be much much worse. This happened once before the warranty but I did not think much of it. In one instance the steering wheel was so tight that it hurt my wrist while getting off the road

Incident Description: I was driving on the highway using Tesla Full Self-Driving (FSD) / FSD Supervised at approximately 65+ mph. Traffic conditions were moderate, and weather and visibility conditions were clear. While FSD was actively engaged, my phone began ringing. As I briefly attempted to silence the ringing phone, the FSD system unexpectedly disengaged/canceled during highway driving. I did not receive sufficient audible warning, advance notification, or adequate transition time to safely assume full manual control of the vehicle. There was no meaningful cushion time for driver response before vehicle behavior changed. The disengagement occurred abruptly and created an immediate dangerous situation at highway speed. The sudden transfer of control, without sufficient warning or response time, significantly reduced my ability to react safely. At the time of the incident: - My attention had been on the roadway prior to the phone ringing - I was monitoring the vehicle as required under FSD Supervised - My hands were near the steering wheel - There was no construction, lane merge, or unusual road condition nearby Immediately following the disengagement, the vehicle suddenly drifted and braked, resulting in a collision with the curb/barrier area near the highway bridge. Fortunately, no other vehicles were involved and no secondary collision occurred. I am requesting that Tesla investigate: - FSD disengagement behavior - Warning timing and driver alert mechanisms - System logs and telemetry data - Whether the vehicle generated internal safety/event reports at the time of the incident - Whether the disengagement and transition-to-driver behavior complied with Tesla’s intended safety protocols Relevant evidence preserved: - Tesla dashcam footage - Photos of vehicle damage and scene - Time and location information - Vehicle data request pending
