This is a PHEV and upon returning to my vehicle after being plugged into a Rivian public EV charge station I found that the charger handle overheated and smoking. When I left the vehicle 45 minutes earlier it was charging with no issues whatsoever. When I removed that handle, a large piece of the charge terminal came out of the handle a remained attached to my charge port. It was one of the connecting sleeves that melted out of the charger and somehow arc welded itself to the male end of my charge port. I still cannot remove it and so now I am unable to charge my vehicle. I notified the property officials and also notified Rivian, but neither is claiming any responsibility to the damage to my charge port. I took my car my local Chrysler dealer and explained what happened but they denied any fault as well. My vehicle has less than 13k miles and still covered by new car warranty so I thought they might know why that happened. In my judgment what had happened is the charge cable itself had a weak wire or a bad, loose connection to the copper terminal inside the handle that caused high resistance and heat to build, then broke away and bound itself to my charge port once I removed the charge handle. When I try to followup with Rivian, best I can do is communicate with their front line phone customer service (they never allow me to speak with anyone authorized to make decisions about the matter), however Rivian did fix their public charger after the incident, but they're seem uninterested in assisting a fix to my vehicle, which I feel they are at fault So have claimed Chrysler had had charge port issues but they are denying assistance. I have an estimate of around $980.00 to repair my charge port, but neither Rivian nor Chrysler warranty are willing to make repair. I feel victim here, used equipment as advertised but being denied product liability claims by both parties, Rivian and Chrysler.

The contact stated that while driving approximately 65 MPH with the adaptive cruise control mode activated, the message "Adaptive Cruise Control Off" was displayed, and the vehicle suddenly experienced unintended acceleration. The contact stated that the vehicle exceeded 100 MPH and failed to decelerate upon depressing the brake pedal. The contact stated that the vehicle speed continued to increase, nearly causing the vehicle to crash into a semi-truck. The contact was able to drive onto the shoulder of the road, where the contact shifted to neutral(N), and the vehicle stopped. The vehicle was later towed to the rental car facility. The manufacturer and the local dealer were not contacted. The vehicle was not diagnosed or repaired. The failure mileage was 23,000.

On [XXX] while traveling on the interstate the car issued a warning engine light and turtle image to pull off the road as the car would shut off, immediately it had no power, maybe could only go 30 mph. Took the car in on 5/10 which was the earliest appt. Said they did a software update, gave the car back. Took the car again on [XXX] when the same warning appeared again on the interstate, same situation disabled vehicle at high speeds on the interstate and no power. Said they changed a module related to the error and gave me the car back. Yesterday as I was driving on [XXX] 70 mph the car gave the same warning and shut off again, immediately I had no power, 30 mph. The dealer acknowledged my car is not the only one in his shop with this issue. They had my vehicle for nearly a month the time before. This vehicle is not safe to drive on the highways as it can shut off at any time. It's a known problem by Chrysler and their idea of fixing it, is issuing a warning which gives you almost no time to get off the highway before your speed is greatly reduced and even then you're stuck on the side of the highway with cars speeding by in an area with no shoulder or construction zone you'd be stuck in the middle of the road. Someone will wind up dead on the highway, I am lucky that in the 3 times this has happened I have not been hit and have been able to safely get to the shoulder, but I don't see Chrysler being able to fix this issue, the dealership did reach out to the manufacturer each time. I know my car is not the only one that has this issue and I'm concerned for the safety of anyone who owns one of these. A simple google search of this vehicle will show the problem is wide spread. The contact owned a 2022 Chrysler Pacifica. The contact stated that while driving approximately 55 MPH, a very loud snapping noise had suddenly occurred, and the message "Stability System Activated" displayed before the steering wheel suddenly turned to the right, causing the driver to lose control of the vehicle to crash and roll over multiple times. The police and first responders were called to the scene, and the occupants were transported to the hospital. During the crash, the driver sustained multiple neck and back fractures and a right leg injury from the seat belt strap. The front passenger sustained multiple rib fractures. The vehicle was destroyed and later towed away. The cause of the failure was not determined. The manufacturer and local dealer were not notified of the failure. The contact also indicated that due to the injuries sustained in the crash, his right hand is unusable. The VIN was not available. The failure mileage was 5,000.

On [XXX], I was driving uphill on the interstate at around 70 mph on gas power (hybrid battery was discharged) and warning came up "Stop Safely Vehicle Will Shut Down Soon" and lost power (Limp mode). I was able to move safely to the shoulder, but with 5 passengers (3 young kids) and a dog and a full load of camping gear it was a bit nervewracking. Could not get the car started again (stayed in limp mode) and used an OBDII scanner I had which popped up error code (P0A43 - "intermittent issue with the drive motor "A" position sensor"). The vehicle was towed to the dealer ( 4 hours from home) and after a few days they said the vehicle was fixed and I pushed for them to test drive it on the interstate. Was returned with this comment: " FOUND CODES FOR DRIVE MOTOR "A" POSITION SENSOR CIRCUIT INTERMITTENT AND HYBRID BATTERY PACK STATE OF CHARGE LOW. CHECKED TRANS PLUG AND WIRING VERIFIED OPERATING CORRECTLY. FOUND THAT RESOLVER NEEDS TO HAVE RELEARN PREFORMED. PREFORMED RELEARN ON RESOLVER AND CHARGED HYBRID BATTERY. TEST DROVE VEHICLE ON INTERSTATE AND VERIFIED THAT ISSUE DID NOT RETURN. VEHICLE OPERATING AS DESIGNED." I received a notice of an unlimited mile/year warranty on the transmission for this car, and from what I read on the internet, this is a recurrent issue with a known cause. We were able to get off the road safely in this case, but it is nerve-wracking to drive, especially with my family, in the current state. I had the recall done on my van for the backup camera. After the update was completed my van continues to not work. The screen does not come on and the backup camera does not come on when in reverse. It does not happen all the time. I took it back to the dealership and all they could say was that I needed a new camera. That does not make sense to me because the entire screen would not come on including the radio or camera.

The passenger headrest on the seat has stopped functioning and is it a downward position where the seat cannot be used safely. The headrest will not position for a safe position. Repair center has verified. Manufacturer says this will be an $1100 plus repair. Have found others with this problem on YouTube. As I understand, this is not repairable and must be replaced. I’m asking for a safety recall to be issued with the manufacturer will repair the safely for all customers who are involved as I understand there are many. Thank you.

The side back doors do not work all of a sudden when you push the button on the vehicle frame inside of the van. This is on both sides of the van. They will only shut by pushing buttons on the outside door handles. Also, the windshield wipers will only work on high. There is no intermittent, low, or medium speed working.

6/21/25 , a Saturday, I stopped at a traffic light when gas saver function kicked in and shut car off. The car went into park mode by itself and parking brake engaged. The car would not start and had to be towed. It sat until Monday when I was able to start it and drive it to my mechanic's business a mile from my location and shut it off. Immediately after shutting it off it would not start again. My mechanic was able to start it a few days later and when he drove it about 5 miles it just shut off and wouldn't start again. Today he is replacing the starter and believes the gas saver function may have contributed to the increased wear on the starter leading to failure, as well as the heat from the catalytic converter located near the starter. I am very nervous about driving this car again and worry it may happen again when I am in traffic. This is a major safety problem if not resolved.

The 2nd row passenger side headrest failed to latch in the upright position and prevents anyone from sitting in the seat since the headrest is almost horizontal. I reported it to the dealer and they will replace the headrest due to a plastic piece in the latch broke. While waiting for the part to come in the driver’s side 2nd row headrest also failed to latch. Upon inspection the same plastic piece had broken. Since the van is 38 months old and less than 36,000 miles it is out of the manufacture’s warranty period

When returning the driver's-side, 2nd row seat into a riding position after being placed into a stow 'n go position; the headrest of the driver's-side, 2nd row seat will not return to a locked upright position - causing it to stay flipped forward. The concern is that in a crash, the passenger seated in the driver's-side, 2nd row seat will not have adequate protection from a neck injury due to the protruding headrest.

On June 3, 2025, my 2023 Chrysler Pacific Hybrid gave me a warning message that it was about to shut down. This included a "red turtle" icon that I later learned is called "limp home mode." I was traveling on the interstate at highway speed at the time. I was able to pull over to the shoulder and turn the vehicle off. The risk of a car shutting down on the highway is a huge a safety risk to myself and other vehicles. Stopping on the shoulder of a highway, particularly given that it was early morning and not yet fully light out also felt very risky with other cars passing by. After turning the car back on, the limp home mode turned off, but the engine warning light was still on. I was able to re-enter traffic, which was also a safety issue having to deal with. When the dealership opened, I contacted them. They were familiar with the problem and were able to state over the phone that it was likely a transmission issue. After inspecting it that afternoon, they determined it would need a transmission rebuild. As of June 27, 2025, they still have the vehicle and have not yet made the repairs.

Part 68324912AC Latch Release Cable failed on both middle seats within one week of one another and approximately three weeks after the manufacturers warranty expired on the vehicle. The result is that both seat backs do not latch into place vertically and the entire seat freely slides forward and backward, slamming backward on acceleration and forward on braking. We have read on Internet forums that this is a very common problem. The left side seat failed while my daughter was riding in it and the seat back released. If this had happened during a collision, it could’ve been catastrophic. Upon stopping the vehicle and exiting, we found that it slid freely forward and back as well. The other side failed while the vehicle is parked, and we released the seat back lever. The seats are unusable as a result. If the failure had happened while driving or impact, it could have been devastating as the seat would have slid independent of the seatbelt and the back slammed forward driving the passenger’s head into the back of the front seat. A Chrysler dealer diagnosed the problem charging us $250 for the diagnostic fee and quoted another $750 for the repair. Parts cost about $30 each. There were no warning indicators prior to the failure.
