I recently purchased a 2023 chrysler pacifica hybrid that did not have a recall listed for the loss of power incident. Within 24 hours of buying the car it died on me while driving. I was driving on a two lane busy road when without warning the vehicle beeped loudly and suddenly I lost all ability to use propulsion. The car illuminated that I should brake and it will shut off soon but soon was immediate and I had zero time to safely exit the road. After dying a little turtle symbol showed up on the dash and the check engine light became illuminated. Once I was able to get the vehicle restarted the check engine light remained illuminated and ai arranged for the car to be seen at the dealer right away. I took it to the dealer within a few minutes of this event and they did the update that was supposed to fix something. Within 3 days it repeated the same event. It is now sitting at the dealership to be looked at again. I had no time to stop safely, had I been on the highway the ability to remove myself safely from the road would have been zero percent chance. How long before the government steps in and holds Chrysler accountable for their poor quality and repairs that fix nothing? I lost power completely. I lost the ability to maintain speed in order to safely remove myself from the road. My family could have been killed. These are family vehicles, out yourself in the shoes of someone with children, would you feel safe putting your kids in a car that could die on you at any moment without any real warning signs?

We have experienced continuous issues while driving the vehicle at highway speeds, with the steering exhibiting a gap in responsiveness. The steering acts like there is a dead zone between 11 and 1 o'clock when turning the wheel, where nothing happens. Once the vehicle finally reacts to the driver's inputs, it causes the car to jerk from side to side. It is bad enough that it causes my entire family to become carsick during even short trips. Still, my primary concern is that the steering is not responsive, and if the issue worsens or changes, it could lead to a loss of control over the vehicle. In addition, the jerking caused by this may cause loss of control when traveling in inclement weather or icy conditions. We took the vehicle in for diagnosis, assuming an alignment issue, but the dealership verified the alignment was within the manufacturer's requirements. In addition, the dealership told us, "This is just how the Pacifica drives. All of them have this issue with the steering". Today, I came across an investigation into a similar issue with older model Pacificas, and I believe this is not only the same issue but also warrants investigation of all model year Pacificas. There were multiple comments within the forum post of owners experiencing this issue in newer vehicles than the current investigation. Please consider expanding your investigation to all model years. Thank you.

I was recently on a trip and I was traveling I39 in Wi at 70mph there was a Semi next to me on the right side and another Semi in front of me approximately 5 car lengths when all of a sudden the the brakes applied with the dash lighting up saying Brake in big red letters this braking action happened for about 2 seconds This was a very dangerous situation as there were vehicles behind me

The second-row seat back will not stay in a fixed position. How would that do in a car crash with no fixed back position? Just flop backwards all the way from a rear crash? This part was broken. When adaptive cruise control is enabled and I’m at cruising speed, new vehicles entering the lane in front of my vehicle are not being acknowledged leading to my vehicle getting too close and collision becoming likely. This leads to me having to takeover control frequently to prevent accidents. This happens often, not always, but it is becoming more and more frequent. This happens at all speeds from 30 mph to 70mph

While driving in the carpool lane on the freeway, the vehicle displayed “Stop Safely. Vehicle Will Shut Off Soon” and lost all power. I am pregnant and had four children (ages [XXX] , [XXX] , [XXX] , and [XXX] ) in the vehicle. The outside temperature was over 100°F, and we were two hours from home. The car was completely disabled in heavy traffic, creating an extreme safety risk for my family. After waiting about 15–20 minutes, the vehicle restarted and we were able to continue. A code scan showed P0A78 – Drive Motor “A” Inverter Performance. This is a serious safety concern, and I am requesting immediate inspection and repair under warranty. While driving my vehicle soon after purchase I notice an engine light on along with a huge fan noise from under the hood. Addressed the issue with dealer they diagnosed the car and so called fixed the issue. about a month ago I’m driving the car on a residential street and noticed the car jerking loud fan noise and the whole dashboard light up saying service hybrid service engine and transmission.. Suddenly the engine stops in moving traffic. The vehicle windshield wipers cut on by themselves and the car stayed in park.. tow the vehicle to the dealer using only a dolly tow truck due to vehicle unable to be placed in neutral. Dealer calls to give update and noted no codes were stored and there is currently no issue with vehicle..He suggested to keep the vehicle longer to see if the problem returns. I’m afraid the electrical issue that is going on with this hybrid will cost me and my children life one day. this vehicle is not safe to drive and I am stuck in a purchase contract.

I would like to bring up a serious safety issue with this vehicle (2024 Chrysler Pacifica Select PHEV) WITH 6997 miles on the odometer. As I was driving, at approximately 11:15AM, I received a message on the dashboard that read, “Stop Safely!! Vehicle will shut off in < 2 minutes”. For my own safety as well as other drivers on the road, I tried maneuvering the vehicle to the side of the road, however the vehicle shut off within 15-30 seconds. The vehicle did not move at all. The serious safety concern I have is “What if the vehicle had stopped running on the major highways full of traffic”? Please note, this is also a well-known issue starting from the 2021 Chrysler Pacifica models. Additionally, a similar incident occurred to another similar make model in Sept. 2024 with VIN # [XXX] . It seemed like, when I received the message, the vehicle's computer system interlocks fuel supply and chokes up the brakes to the wheels so the vehicle is completed locked up in one position and it does not do rolling stop to safely pull over to the side of the road. This is very dangerous because if a semi-truck with 80,000 lbs. load is coming behind you, you will NOT be able to move your vehicle to the side. This can potentially kill all the occupants in the vehicle. This vehicle has been towed to the dealer to troubleshoot/resolve the issue. Vehicle loses power and goes into limp mode. This has happened three times same fault code. The last two times we were going 80 on the expressway. Very dangerous luckily we haven't caused a accident. The manufacturer hasn't fixed the problem. Even though we keep taking it to the dealer. They clear the code and try to recreate the problem with no success. Tell us there is nothing they can do. There are recalls snd repairs for previous years of this vehicle with the same issue. They haven't solved the problem in the 2024s. The car was bought new in November 2024 and has 18,000 miles. It is unreliable and unsafe. See attached photos showing he dash and warning.

Vehicle is new and delivered on 08/27/21. Complaint: The front axle nuts on vehicle loosen over time. The first incident was reported to I-10 Chrysler Dodge Jeep Ram on 09/29/23 (see Invoice), with 24,832 miles as a noise coming from the front left suspension. This dealer replaced both front hubs at that time which resolved the noise. The second incident was reported to Chrystal Chrysler Jeep Dodge Center on 02/13/24 (see Invoice) with 27,952 miles as a noise coming from front left axle. Dealer mechanic went on test drive with me and found noise coming from front left axle shaft nut. RO states, "Found axle nut not torqued to specs. Tight down left front axle nut. Test drove vehicle. No noise found after repair.". The third incident was reported to Hemet Chrysler Dodge Jeep Ram on 08/26/25, and I asked them to check the torque on the front axle nuts. They told me that the odds that they would be loose again was very small and they would need 2 days to diagnose the issue. Because I was leaving for a vacation road trip with the vehicle within a few days, I decided to take it to an independent automotive shop (Cam Stones Automotive Inc.) on 08/26/25 and reported that the vehicle has a history of the axle nuts loosening over time. I asked that they check the torque on the front axle nuts (see Invoice). The invoice states, "Retorque axle nuts and apply lock tight. Suggest contact dealer or NHTSA as to permanent fix. 120 Ft. Lbs.". As I'm sure you are aware of the danger in axle nuts that loosen over time. Is there a Recall on this issue? If not, are you investigating complaints of the axle nuts loosening time and time again?

There are clearly many design flaws on my 2021 Chrysler Pacifica Hybrid. One issue I am having with this car is the Daytime Running Lights. These are expensive LED lights that should function for the life of the vehicle. Unfortunately, I believe there is a problem with the control module on the LED lights, and part of the driver's side light does not function. This is a clear safety issue, and seems to be very common problem people are having with the 2021 Chrysler Pacifica Hybrid. I notified Chrysler about the issue, and because the car was just over 36 months old, they would not repair the issue. They told me I would have to pay for a complete replacement of the headlight which would cost over $1000. Just today, I noticed Ford posted a recall for the same issue they are having with their Daytime Running Lights. I believe it is only fair for Chrylser to provide a voluntary recall on this same issue because of the clear safety hazard it can creates. What is the purpose of Daytime Running Lights if they don't function properly. This is a minivan I rely on to safely transport my family, and I do not want to risk a hazardous collision because Chrysler refuses to repair a faulty part in their vehicle.

On 8/12/2025, he vehicle displayed a warning that said "Stop Safely, Vehicle will shut off soon" shortly after it was parked. It also displayed a red turtle and the check engine light. There was no warning or check engine light prior to the message that the vehicle was going to shut off. Fortunately, we had already parked safely, but were on the freeway traveling and about an hour away from home. It was 100 degrees outside, and we were stranded with no help from Chrysler customer care other than to tow the vehicle, leaving our kids and dog in a parking lot with no car. We declined this option and had it checked by a diagnostic computer at Jiffy Lube, which returned no diagnostic code. Eventually, all of the warnings and check engine lights went off, and we were able to take it home. We took it to the dealership at the earliest opportunity on 08/14, and they confirmed that this is a known issue and would require a transmission replacement. There is no timeline on the repair or replacement, only that it will take "a while". They do not advise that we take the van back because the issue could come nback at any moment and the vehicle could shut off without warning.

After not driving the car for a full day or so, when I drive down my street about 1/4 of a mile to a stop sign and stop the vehicle will not accelerate from the stop. I press on the gas but the car goes around 5 MPH for the next 50-100 feet or so and will not accelerate before finally kicking in and accelerating normally. This has happened over 20 times since purchasing the vehicle as new in late March of 2025.
