The contact owns a 2022 Tesla Model Y. The contact stated that the rear driver’s side seat belt failed to latch. The contact stated no warning light was illuminated. The contact stated he was able to reach out to the manufacturer on the App and made them aware of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 3,500. THE CONSUMER STATED THERE WAS A FOREIGN LODGED IN THE BELT LATCH.

SERVICE BRAKES, VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
While in cruise control, our 2021 Tesla Model Y (WE DO NOT HAVE AUTO PILOT!) applied the brakes HARD dropping between 10-40 mph with no warning. Cruise control stays active. There were no cars anywhere near us, either in front or in the rear. Over thirty (30) Tesla bug reports were recorded on April 25 and April 26, 2022. While we were not SERIOUSLY hurt, had a vehicle actually been behind us, we would have ended up with a vehicle in our back seat and certainly serious injuries. This same thing happened six months ago on our last road trip. Tesla has blown us off saying that this simply cannot occur. Their response is that the computer cannot slow the car without seeing something. Therefore it must have seen something. This is circular reasoning and is meant to blow off the consumer. This is a serious safety issue. The braking on occasion has been violent. This is similar to the reports in other models. Tesla has twice now, within six months, addressed this issue simply by saying "it cannot occur." Seriously?!

VISIBILITY/WIPER, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
Tesla adaptive cruise control requires user to put the windshield wipers to automatic functioning. However, the auto wipers have a high false positive detection rate, which leads to the wipers unnecessarily turning on while I'm driving. Even after exiting adaptive cruise control, the wipers stay on auto. This is extremely problematic because it makes me have my eyes + one one hand off of the wheel to turn the wipers off. Please put pressure on Tesla to remove faulty auto-wipers software from adaptive cruise control. It's dangerous and distracting.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
When the adaptive cruise control is engaged, it will brake excessively for no known reason. It can reduce its speed very quickly sometimes by 10 to 15 mph causing the vehicle behind me to take drastic actions to avoid a collision. I have informed the manufacturer of this issue and told them it happens every day I drive the car, but they have done nothing except saying they are looking into the issue.

My 2021 Tesla Model Y, when using adaptive cruise control engages in phantom braking on a perfect clear road with no obstacles. On a recent road trip the vehicle braked suddenly at least 10 times while driving at highway speeds on both divided and single lane roads. This is cleariy unsafe especially if there is a car following. Tesla is aware of this problem and has done nothing to resolve it.

The contact stated while driving 60 MPH with the cruise control activated, the radar detected a hazard ahead on the roadway and deactivated the cruise control, causing the vehicle to experience phantom braking. The vehicle was not diagnosed nor repaired by an independent mechanic or dealer. The manufacturer was not made aware of the failure. The failure mileage was approximately 3,000.

Autopilot Phantom Braking while using cruise control, adaptive speed control. Occurred 9 times on a single drive. Often with no other cars, or anything in sight - in Death Valley to be specific. Instant breaking would occur, sometimes drastic, at times might have brought car to a complete stop if I'd not taken over control. We are now very reluctant to even use cruise control. This occurs with either adaptive speed control or Enhanced Auto-pilot. FSD is not enabled.

FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
I was driving West bound an the 91 Freeway, just about 1 miles East of the 241 Fasttrack at about 5:30 PM on 30 April 2022. The 241 Fasttrack was shut down for road work and the traffic was averaging about 4 miles and hour, but very stop and go traffic, not a smooth flow. I put the can into dynamic cruise control -- this was done by press down twice on the drive shifter on the right side of the steering wheel. I then took the car out of dynamic cruise control by pressing the drive shifter up once. But I much have forgot I did so as I press the drive shifetr up once again, which puts the can into reverse. I pressed the gas pedal and the car started going backward and I crashed into a vehicle -- I believed the car was in Drive, not Reverse. No one was hurt. My car was undamaged; his 2004 Toyota Corolla had some cracked paint and the left driver side bumper was push back about 3/8th on an inch. I paid the man $450 and he will contact me if he needs more. My complaint is this: the drive selector is being used for drive selection AND engaging and disengaging dynamic cruise control -- it may me to easy to make a mistake where the drive thinks he took it out of dynamic cruise control, but in fact put it into reverse. I do not think the same motion for turning off dynamic cruise control and putting the car in reverse should be on the some stick with the same motion. The only indication is a audible 'tone' when disengaging the dynamic cruise control, but road noise, cabin noise, radio music can be that tone less noticeable. I had the car on 'joe' mode which decreases the tone sound. I don't think 'joe' mode is a safe function as it makes critical sounds less noticeable. The dynamic cruise control sounds should not have to compete with the radio -- the radio should be silenced when the dynamic cruise control tones occur.

Recently while pulling into my children's school, my car has started deciding that the entrance to the parking garage is a large truck stopped diagonally in front of me and slams on the brakes scaring me and causing everything in the car to go flying. Luckily no one has been behind me while this has happened. I am always entering very slowly, maybe 5 mph at the very fastest.

I was about to park the car in my garage and stopped midway down my driveway to take a phone call. After the phone call, I changed it from park to to drive by stepping on the brake and lightly stepped on the accelerator to ease it into the garage. It quickly accelerated. I stepped on the brake to stop it but it failed to stop. It crashed into my refrigerator in the garage. A video is available for you to view

SERVICE BRAKES, VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
On 3/7/22- 3/15/22 I and my wife took our Tesla Model Y 2021 for an extensive trip from San Diego, CA to Sedona, AZ. Our Tesla does not equipped with Autopilot. On 3/7 while on I-8 E Arizona, our Tesla was on cruise control at HWY speed, it suddendly braked from 75 miles/hr to 60 miles/hr and slowly going back up without any appearance reasons! There was No cars near and around. Afterward, it happened several more times so we stopped using cruise control on the second day of our trips until return home due to safety concerns of getting hit from behind. I took the Tesla to Tesla service for a check of the issue about a week after return San Diego, and the Tesla technician took the car for a test drive and found nothing wrong!

We were on the highway the power went out. The car did not have emergency brake . We could have gotten hit. We finally was able to stop on the shoulder with the driver not letting go off his foot pedal to keep the car from moving. We called Tesla and he wasn’t able to restart the car and yet he said the car has lots of power. The back door doesn’t open in an emergency as well

Car set at 75 MPH cruise control, brakes unnecessary with no obstructions or danger, and reduces speed to 45 MPH while having adaptive cruise control active. Tesla response = this is a Beta feature. Read your manual. This is incredibly dangerous and a rear end collision waiting to happen. I feel like this is reckless and to just blame it on being Beta is borderline criminal.

While on cruise control, the car will, out of nowhere, apply the brakes very aggressively ("phantom breaking"). This despite there being no vehicles nearby or any potential threats to safety. This phantom braking is obviously very dangerous and is a reason why I don't really use it anymore. It has happened 3-4 times since I bought my car, and that's despite not really using cruise control very often (for this reason). Clearly there's a software fault of some kind
