My Tesla Model X was in Autopilot mode (v13.2.9) at the time of the incident. The vehicle’s advanced driver-assistance system was actively controlling steering and speed, while I remained attentive with my hands on the wheel I was closely monitoring the car’s operation but was not manually steering, braking, or accelerating during Autopilot engagement The vehicle collided with stationary flexible delineator posts. The impact occurred at 55MPH. The flexible delineator posts were the only object struck. No other vehicles were involved in the accident, and no persons were injured.No passengers or bystanders affected. The collision with the flexible delineator posts caused damage to my vehicle’s front bumper area and cracked right headlight. There was no damage to any property besides my own vehicle. The incident was captured on the Tesla’s built-in camera system (dashcam). I have saved the video footage, which clearly shows the car’s approach and the impact with the flexible delineator posts. This video is available and can be provided for review.

Steering got stuck on the road while driving. I couldnt move or change lanes. It was really scary. Then I slowed down and then I had tp pull out of the road to the shoulder. Then I waited for 20-30 mins and restarted the car. Then it worked. I was able to go home. I contacted Tesla and they said they need to look into the issue. At the service center, now they are saying they need to replace entire steering system and the cost is 5000. This is not fair and unprofessional. And it is very scary experience.

I own a Tesla Model X 6-seater. The second-row seatbelt repeatedly becomes twisted and fails to retract or lay flat after only a few days of normal use. I have brought the vehicle to the Tesla Service Center in Austin, TX (2323 Ridgepoint Dr) multiple times — five separate repair visits so far — for the same issue. Each time, the technicians temporarily correct the twist or adjust the belt path, but the issue returns within days. I have attached screenshots of all five repair records, including Tesla’s technician notes describing the repeated attempts to resolve the problem. The problem appears to be caused by misalignment of the seatbelt retractor or seat design, not by user misuse. When the belt twists, it cannot retract properly or lay flat across the passenger, which may reduce its effectiveness during a collision. Tesla Service has not offered a permanent fix or design correction, and the defect continues to recur. This appears to be a systemic issue in the second-row seatbelt system of the Model X six-seat configuration. Because the defect involves a federally required safety restraint that may not function as intended, I am reporting this as a potential safety defect for further investigation.

In an emergency when the vehicle loses battery power, the rear falcon wing doors are very difficult to unlatch and open. A person has to be able to pull the speaker off the door, which is located at the bottom of the door panel (in good condition I had a difficult time doing it ). Once the speaker cover is pulled off then you have to pull on a thin wire to unlatch the car door. I can do it, but my wife can’t. Children wouldn’t be able to do it and elderly people won’t be able to do it. This is truly a safety hazard that can be easily corrected. I tried reporting this to Tesla, but was not able to report the information. Their customer service line doesn’t have a callback or tell you what the wait time is. I waited 45 minutes one day and 25 minutes the second day. On the first day, I was told that it would be a 20 minute wait.

The wing-style door of the car closed while my 1-year-old daughter's leg was sticking out. It dragged her leg sideways as it closed, and he femur broke. None of the sensors worked, the door should have detected motion or pressure, but they did not and it resulted in my daughter's leg breaking.

This is the third Tesla vehicle we’ve owned, and while our previous two vehicles performed excellently, this one has presented serious and recurring problems with the Full Self-Driving (FSD) system. Since the second service visit, we have experienced four separate incidents in which the vehicle has either driven off the road or abruptly slammed on the brakes without warning. Despite two prior service appointments at the Tesla Service Center in Sarasota, FL, these issues remain unresolved. We also scheduled a third appointment, which Tesla later cancelled.

Tesla remotely blocks a car powertrain without warning if a service invoice wasn’t paid before leaving a service center. This poses a safety risk because the car might unexpectedly become non-driving, such as after returning from a hike with young kids in a remote area, without water, food, or medications. To reproduce this issue, follow these steps: 1. Leave a Tesla service before a service or part is charged to the card on file (this can happen for various reasons). 2. Park the car in an underground parking garage or a remote area with no cellular connectivity. 3. Attempt to return home. The result is that the car will be blocked by an unknown PIN, even if the invoice was automatically charged to the card on file while driving. Without Internet connectivity, it’s impossible to work around the PIN. The car has be towed to an area with cellular connectivity to resolve the issue.

The vehicle has start throwing a safety restraint error. We got into the service mode and it shows there is electrical problems with the driver side airbag system and that it may not deploy in the event of an accident. Tesla is delaying the appointment to review this for 12 days. Their app advised to not drive the car due to a safety issue as all safety measures may not work appropriately. Seems to be a safety level recall.

Issue Description GPS freeze: The navigation system remains stuck at one location even when the vehicle is moving. Park Assist not available. FSD not available. Rear camera shows black screen, completely disabling reverse visibility. These issues occur intermittently, often lasting for days, and then resolve themselves unexpectedly. Unfortunately, when scheduling service, the problem often disappears by the appointment date, making diagnosis inconsistent. However, the failures have already occurred more than three times, and each time they severely impact both safety and usability. Safety Concerns Lack of rear camera visibility creates a direct safety hazard during reversing. Disabled Park Assist and FSD compromise Tesla’s advertised safety features. GPS malfunction affects navigation and system reliability.

The vehicle experienced serious drive control failures on three separate dates: September 21, 2024; October 2, 2024; and July 26, 2025. On each occasion, acceleration was either severely delayed or completely unresponsive, and the vehicle rolled backward when attempting to move forward on an incline. The following warnings appeared on the dashboard: “Stability Control Disabled,” ESC (Electronic Stability Control) warning light, and Air Suspension System warning. These symptoms occurred both while stationary and while driving. The vehicle was brought to Tesla service centers twice—on October 10, 2024, and September 2, 2025. On both occasions, Tesla stated that the issue could not be reproduced and returned the vehicle without performing any repairs. During the second visit, Tesla performed a $275 wheel alignment adjustment. The service report noted that the warning lights “may be restored on next drive.” I was verbally told that adjusting the alignment might help identify the root cause, but Tesla never explained how this adjustment was related to the actual symptoms. After performing the alignment, I was simply notified that the vehicle was ready for pickup. This kind of propulsion failure and rollback on inclines presents a serious safety risk to the driver, passengers, and other vehicles on the road. The issue remains unresolved and could occur again at any time without warning.

All trailer hitches installed on Tesla Model X and Model Y vehicles do not meet the standards for towing in the US. These hitches are from Bosal, a European manufacturer, and there is no way to safely tow most trailers in the US including Home Depot, UHAUL, Airstream, boat trailers, etc. and meet the manufacturer safety specifications. Model X and Model Y owners on the road today are not following Tesla's manual and are using a tow ball adapter way outside the safety spec for the Tesla hitch. This poses a serious safety concern which could lead to failure of the hitch and the trailer detaching from the car while in motion. This poses as serious safety concern for the drivers, passengers, and everyone around the vehicle in tow. Please investigate this. I quick look at the Tesla manual specifications and the standard 18" height of trailers in the US makes it clear there is a serious issue here. Please investigate asap. I have contacted Tesla at least 10 times and nobody there is taking action on this issue.

While driving our screen shut down, autopilot and GPS stopped working and the doors would not open. My kids were stuck on multiple occasions and had to climb through the front seats. Tesla has already confirmed that the Driver Assistance System ECU (Turbo A and Turbo B processors) is a known issue. Over 1600 cases reported. This defect disables safety-critical driver-assist functions without warning, which meets the federal definition of a safety defect under 49 U.S.C. § 30102. Provide a free remedy to owners — including replacement at no cost — as required by 49 U.S.C. § 30120. Telsa denied coverage because we are out of warranty. I asked whether the known issue has been reported and why it is not being covered as a known safety defect. Because the problem involves loss of a safety system, it should not be treated as routine maintenance or wear and tear. It is a known safety defect and therefore must be reported and remedied at Tesla’s expense. Tesla refuse to address my concern and would only let me talk to store manager. They told me I had to email business resolutions to get more information. I have not heard back. I was forced to spend $2k to get my car fixed

The contact owned a 2025 Tesla Model X. The contact stated that while driving at approximately 65 MPH with Self-driving Assist activated, while the vehicle in front deaccelerated, the vehicle had aggressively deaccelerated. The contact stated that the self-driving assist had deactivated, causing him to aggressively depress the brake pedal, stopping before it struck the vehicle in front. Several unknown warning lights illuminated. In addition, the contact stated that the vehicle was struck in the rear by another vehicle, causing the vehicle to rear-end another vehicle. The contact sought medical attention for head and back injuries. A police report was filed. The vehicle was towed to a local dealer, where it was deemed a loss. The manufacturer was made aware of the failure. The failure mileage was approximately 6,150.

Hazard flasher button in my 2-weeks old new Tesla Model X is not working. On August 23, 2025, I was in a parking lot and needed to turn on the hazard flashers to signal to the car behind me that I was waiting for another vehicle to leave a parking space. I discovered that the hazard flasher button on my 2025 Tesla Model X did not work at all. Since I could not activate the hazard lights, I had to continue driving to find another parking spot. This was the first time I tried to use the hazard flashers since purchasing the vehicle two weeks earlier, and they have never worked. I attempted both a power cycle and vehicle reset, but the hazard button remains completely unresponsive. On this model, the hazard flasher is integrated into a touch-sensitive bar in the center console, and there is no alternative physical or touchscreen backup control. The absence of functioning hazard lights poses a significant safety risk. If I only discovered this defect during a true emergency situation (e.g., breakdown or accident), I would not have been able to warn surrounding traffic. In my opinion, relying on a touch bar for such a critical safety function is unreliable; a physical pressable button should be required. Tesla has scheduled a service appointment, but it is two weeks away, and in the meantime, I am driving a vehicle without working hazard lights. This is a safety-critical defect that should be investigated.

i was in full self driving mode. the car wanted to turn left but i thought it was going in the wrong direction so i tried to continue straight by turning the wheel to the right forcefully but the car would not let me take over and i crashed into a guard rail. when i tried to brake it was not able to stop soon enough, i have a dash cam footage. i have taken over the this way numerous times and it usually does not require much force to do so, this time it simply would not allow me to control the wheel. no one was hurt other that a guard rail. the car was totaled by insurance company. the police assisted me but i never received a police report. no pictures to share, just a dashcam video but i dont know if that would be helpful since there isn't a way to upload it that i can see. I was driving home from work within city limits at less than 30 miles per hour. The roads were waterlogged due to a thunderstorm and the next thing I knew i heard other vehicles near me honking and telling me that something large had fallen off the underside of my car. I stopped the car immediately and was shocked to see that the guard that covers the bottom of my car had fallen off by itself. Apparently the batteries powering the vehicle are situated in the undercarriage and the guard is supposed to protect that area. I called Tesla Insurance Agency and despite me telling them that it would be a possible fire hazard if I continue driving the vehicle and the car was making some noises from the area, they want me to continue driving the car, which I have refused. I am attaching the picture of the part that fell off and also the body shop details of the repairs to be done. My car has only 22,000 miles on it in the last 2 1/2 years that I have driven it and it is shocking that an expensive vehicle such as the Model X should have parts falling off endangering lives of those traveling in it. I would certainly recommmend Tesla inspect all Model Xs as this could be a major problem.

There is an error alert on my car that says front left safety restraint issue service is required. So seems to me the airbags or seat belt may not be working properly and its clearly labeled as a safety issue. We have to drive this vehicle nearly daily and tesla is refusing to fix for a minimum of 10 days even though this fault could threaten our lives. Make them fix ASAP

The primary issues are: The vehicle consistently chooses the wrong lane and drifts across yellow lines. The FSD visualization fails to correctly distinguish road markings, lanes, and turning arrows, causing them to move erratically on the display. The car attempts to merge into lanes that appear in the visualization but do not exist on the road. The vehicle attempts to merge into lanes that are ending, without recognizing the lane-ending markers. My service history for this single, unresolved issue is as follows: June 12, 2025 (Invoice #3000S0013397552): I brought the vehicle in because Autopilot was incorrectly detecting lanes. The service team diagnosed "severe hazing over forward facing cameras," cleaned them, and returned the car to me. The problem was not fixed. July 1, 2025 (Invoice #3000S0013588350): I returned for the same issue, stating that the FSD was driving over yellow lines and not recognizing traffic patterns. The diagnosis was changed to a bad alignment; service notes indicate "multiple angles out of specification". A four-wheel alignment was performed, and I was assured the issue was resolved. July 10, 2025 (Invoice #3000S0013675264): On my third visit for this issue, the service team verified that the Autopilot visualization showed "random turn lane markings". However, I was told this behavior was "considered normal at this time" and would improve with future firmware updates. The "correction" was listed as "Remote - Education". July 28, 2025 (Invoice #3000S0013834479): I returned for a fourth time, explaining that the problems persisted and were a safety concern. I noted that the car had recently tried to merge past a solid yellow line into the shoulder. The service team acknowledged the issue but concluded it is an "issue with V13 due its design" and that "No repairs are recommended for this concern". The assertion that this is a "known characteristic" is unacceptable. The FSD and Autopilot features are now unusable and unpredictable
