I was using actual smart summon to get my vehicle out of my garage. While standing behind the vehicle, it indicated that the car was in drive with no reversing lights on. The vehicle then rapidly lurches backwards, striking me and a freezer. After reviewing the dashcam footage the vehicle stated it was in drive and never once activated its reverse lights, even when they was objects and a human behind the car, still reversed at a high rate of speed.

The roof glass is unusually susceptible to failing and cracking under minimal impact. A very small item such as a pebble impacted the roof. The glass cracked. I understand the roof glass is a structural component. I would not expect a roof to require complete replacement due to a minor pebble.

Attached is a photo of the lower driver-side knee bolster/under-dash panel after it detached while I was driving. The panel dropped directly into the pedal area and interfered with normal foot access to the brake and accelerator pedals. Given that no broken attachment points or damaged clips were reportedly identified, I am requesting a formal review of both the safety concern and the handling of this issue.

I was traveling east on I-215 in [XXX] , exiting on [XXX] . I was in the RH lane, signaling to turn Rt. A small commercial van was in the two lanes Left of me with no turn signal on. Suddenly the van began coming into my lane. I turned my steering wheel slightly to the Rt to avoid a collision. Suddenly the autocorrect turned my wheel sharply Rt, then sharply Left. The car was rocking so sharply I feared a rollover or collision. The blind spot camera on the driver’s side did not pick up the van on my Left. The driver-side seat belt buckle/receptacle failed during normal use. There was no collision, impact, or misuse involved. While using the seat belt as intended, the buckle assembly separated/broke unexpectedly, causing the seat belt to no longer function properly. This appears to be a failure of a critical safety component rather than normal wear and tear. The vehicle should not be considered safe to operate until the restraint system is properly inspected and repaired. The failure occurred without warning and under ordinary driving conditions.

Component/System Failure: Total failure of the Autopilot ECU (DAS Computer) and the Front-End Module (FEM) wiring harness. Incident Description: My 2021 Tesla Model 3 (37,000 miles) experienced a catastrophic and "silent" failure of the entire vehicle vision suite and all active safety systems. A Tesla Service technician (Warrington, PA) inspected the vehicle and confirmed the root cause was a factory-defective (crimped) internal coaxial cable in the wiring harness. This manufacturing defect caused an electrical short that destroyed the primary safety-critical computer. Safety Risk: This failure resulted in a direct violation of FMVSS 111 (Rear Visibility), as the backup camera was rendered completely inoperable. Furthermore, all collision-avoidance features—including Automatic Emergency Braking (AEB), Forward Collision Warning, and Blind Spot Monitoring—were disabled. Critical Safety Concern (Silent Failure): The vehicle provided zero driver-facing alerts, dashboard warnings, or lamps to indicate that these safety systems were offline. I unknowingly operated a vehicle that was non-compliant with federal safety standards and lacked all primary crash-avoidance technology. The failure codes (APP_w045 and VCFRONT_a158) were only discoverable by manually entering the vehicle's "Service Mode." Manufacturer Status: Tesla Service has confirmed the hardware failure and the wiring defect but is currently refusing to cover the $2,800 repair cost, despite the failure involving systems currently under federal recall (SB-26-00-016). Tesla is in a protracted battle with me over fees related to damaged property and illegal self-help repossession procedures. They have been formally notified that any trespass onto my property is not permitted because they said they sent armed repossession agents because of “angry customers” like me. I currently have a complaint for those issues filed with the Attorney General of Pennsylvania. Tesla’s recent attempts to repossess the vehicle were initiated approximately two hours after informing the Pennsylvania Attorney General’s office that Tesla had not contacted me in any way, that Tesla was not being factual. Tesla’s response to that was again to not contact me but to send me a form letter stating they intend to disable my vehicle remotely. I am a privacy-minded person, and part of choosing the vehicle was a modicum of privacy guaranteed by Tesla. Tesla publicly says they do not so this, I was told they do not so this in the course of buying the car, and recent news seems to state they say they can’t do it. They have yet to repair a windshield and cabin camera that seem to be damaged. In other words, they intend to disable and lock my car without my permission or knowledge, without verifying the contents of the car. In other words, if there happens to be an occupant at the time they illegally disable the car, they intend to kill them. I’m asking that my contact information be kept private because they have already threatened to use lethal force, make extremely reckless decisions regarding their procedures, and retaliated by initiating this process when I informed a government agency that they lied to them.

I purchased the Full Self-Driving (FSD) package for approximately $10,000 on my 2022 Tesla Model 3 Long Range AWD. Tesla marketed FSD as a feature that would enable autonomous driving capabilities including Navigate on Autopilot, Auto Lane Change, Autopark, Summon, and city street driving - all to be delivered via software updates. Tesla has since confirmed my vehicle's HW3 (Hardware 3.0) computer cannot support FSD as advertised. There is no hardware upgrade path. The FSD Transfer Program that allowed owners to move their purchase to a new vehicle ended March 31, 2026 with no accommodation for affected owners. The system regularly fails to perform as advertised. It requires constant human supervision despite being sold as "Full Self-Driving." I have sent three formal complaints to Tesla's executive team (May 7, 8, and 12, 2026) with zero response. Tesla's own support chatbot confirmed I am not eligible for FSD transfer.

When using cruise control, there has been 4 times that the car has engaged in “phantom braking”. We were driving on highways going the posted speed limit of 70 mph, when the car suddenly slammed on the brakes and decelerated to less than 30 mph – all four times we were thankfully not being closely followed by another vehicle or we would’ve had a devastating crash. One time when we looked at the screen, it showed the outline of a person in the median but there was no person/ other obstacle at all. We have reported this to the Tesla service center and they said it is a known issue, and what we have to do is just click the scroll bar and report it so that the engineers can mark the timestamp and collect data so that some patches can be sent over the air/ uploaded at a future date. Nothing has been done to fix the issue and we feel dismissed about our concerns about our family’s safety, and the safety of drivers around us.

While driving my Tesla, Autopilot was engaged and navigating with the destination entered. At the time of the incident, my hand was on the steering wheel when the vehicle suddenly attempted to make a hard right turn toward a divider. This was not an exit or intended route, which raises serious concerns about what caused the vehicle to react that way. In addition, I have experienced multiple battery-related issues with this vehicle. The battery has already been replaced once, and on approximately 2–3 occasions the vehicle entered “Turtle Mode,” limiting the car to about 35–50 mph. This created a serious safety hazard because it occurred while I was driving on the highway alongside tractor-trailers and fast-moving traffic.

2021 M3 138,000 miles Alerted error code BMS_a074 & BMS_a079 After reading forums, this seems to be a known issue with 2021 in particular. I am reporting in hopes something can be done about it. Yesterday both codes alerted within hours of each other. First that the charging was limited to 50% and would only charge once <50%; then shortly thereafter it alerted with Unable to Charge. I attempted to charge at home with 32A charger when at 28% and charge limit set to 50%— car would not charge. Today I put in Tesla service request with first appointment available in 10 days (still awaiting review). Attempted to try charging again while at home and this time, coincidentally after my service request, it is now charging. I lowered to 20A to trickle in for now…and currently up to 32%. March/April 2025 my ECU went out after 3 12V battery replacements within 2 months. Car has been driving well but without any camera systems (traffic awareness) or entertainment system.

My RWD motor completely went out and lost power on the highway off ramp. Could have turned into a dangerous situation. Not sure why these motors seem to be very unreliable. Tesla has not given me an explanation as to what is exactly causing this. Happens to thousands of owners. And this car is only 5 years old. I had the motor replaced in November 2022 and 3 years later it’s out for good. Now I have to deal with teslas insane costs. I feel like they sold me a defective car.

I am filing a complaint regarding my inability to receive a safety recall remedy without first paying for a separate component replacement. Vehicle: Tesla Model 3 VIN: [XXX] My vehicle has an active safety recall related to hood latch functionality, which Tesla stated requires a firmware (OTA) update. Tesla informed me this update could not be installed unless the vehicle’s car computer (MCU/CPU) was replaced. Tesla classified this component as out of warranty and required me to pay $1,687.66 out of pocket (after a partial discount). Separately, my vehicle developed a charging failure requiring HV battery repair. Tesla confirmed in writing that they could not proceed with the HV battery repair or install required firmware unless the same computer was replaced. Tesla also confirmed that if I did not approve and pay for the computer replacement, they would not proceed with either the battery repair or the recall-related update. At the time, my vehicle was not drivable, and Tesla indicated the loaner vehicle would need to be returned if I did not approve the repair. This would have left me without a functional vehicle, leaving no practical alternative but to proceed. As a result, I approved and paid for the repair in order to restore vehicle functionality and receive the recall remedy. This payment was made under protest. Concern: This situation conditions access to a safety recall remedy on a customer-paid repair, creating a barrier to receiving a mandated safety fix. I request review of whether a manufacturer can require payment for a separate component in order to enable a safety recall remedy and essential repairs. Third-party tire shop inspection on April 25, 2026 identified visible tearing of front lower control arm bushings on BOTH sides (bilateral failure). Rubber bushing material has separated from inner metal sleeve on both left and right front lower control arms in identical pattern. No impact event or curb strike. Symmetric bilateral failure indicates manufacturing/material defect, not impact damage. Failure occurred at mileage well below the 40,000-70,000 mile range commonly cited for this known Tesla Model 3 lower control arm bushing wear pattern. Suggests accelerated failure mode that warrants investigation. Photos from third-party inspection attached. Vehicle scheduled for Tesla service center evaluation under remaining factory warranty.

The steering wheel turn signal buttons on my Tesla Model 3 intermittently becoming unresponsive or getting stuck signaling left or right. This prevents me from reliably signaling lane changes or turns. This is a safety risk because I may be unable to properly signal maneuvers as required under Maryland Vehicle Law §21-604, potentially confusing nearby drivers and increasing the risk of an accident. As this is a moving violation, I can be fined and or points on my driving record The issue has occurred multiple times during normal driving but has not yet been formally confirmed by a service center.
