- #11614533September 13, 2024VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
The noted vehicle is one of 7 Teslas (6 Model 3s and 1 Model Y) that I have rented from Hertz for a round trip from Orlando Fl to Naples Fl. Over the last 7 months. Each of the Model 3 vehicles was different, I never got the same Model 3. Each of the vehicles exhibited the same or similar glitch when traveling on [XXX] at [XXX] when either the navigation and/or the adaptive speed control was engaged and traveling at or near the posted speed limit when nearing this specific exit. If only the navigation system is on it will direct the driver to take the exit and then return to the interstate using the on ramp. If the adaptive speed control is engaged, either with or without the navigation feature on, when the vehicle is on [XXX] near or under the overpass it will abruptly try to slow the vehicle to 35 MPH, the speed limit for traffic on the overpass. None of the rental vehicles has the full autopilot activated to my knowledge. This event will occur when traveling in either direction and has only occurred at [XXX] on my 200+ mile trip. Having this occur once or twice would be strange, but 7 times at the same location over 7 months in 7 different vehicles is more likely a glitch of some kind, and one that could result in a crash if a driver is unprepared for the unexpected rapid reduction in speed. [XXX] is heavily travelled with both local and out of town drivers in both personal and rental vehicles. I have no idea if this is occurring at other locations but it seems to be consistent here. Since the adaptive speed control must be using information from the navigation system to display and utilize the speed limit to regulate maximum speed, it is likely the problem is in the navigation system. Purchased 2023 Tesla Model 3 Performance in January 2024. On [XXX] at approximately 2 pm, I got a flat tire drivers side front on a remote highway outside of Reno Nv. At that time, I found out that I didn't have a spare tire or jack supplied by Tesla. I called Tesla Roadside Service and was told before they would send out assistance, I had to agree to purchase a new tire at 540.00 per tire, the towing company couldn't be to my location till after 6pm. I declined Tesla service. The Tesla had less than 18000 miles on it. My son came and assisted me in removing the flat and we took it to Les Schwab Tire center where I purchased a new tire for 375.00 dollars. Les Schwab stated that the wear on the tire was caused by alignment. I contacted Tesla made a service appointment for alignment for August 26, 2024. I got another flat on the drivers rear on [XXX], which cost 283.00 from Les Schwab with the same disposition as caused by alignment. I took the car in the next morning and had the remaining two tires changed as it was no longer safe to drive 766.00 cost, and alignment was to blame mileage was documented by Les Schwab at 18792. When I went for the service at Tesla, I was told they do not warranty tire or alignment and when I told them that I would let small claims decide if that the tire wear was excessive and caused by improper alignment, they threw me out of the dealership and have refused to work on my car. The main reason for filing the complaint is none of this was disclosed when I purchased the car, every car I have ever bought has come with a spare tire, jack, and tools needed to change it. There are no other tire repair shops that can do Tesla alignments, Tesla offers no warranty on their tires and Les Schwab stated even their lowest cost tires come with a 20k mile warranty. Tesla is price gouging for tires 540.00 each and alignment 275.00. Any of the Tesla forums state that this is happening thru all models and tire sizes to most owners. Front passenger safety restraint system fault alert popping up every few minutes while driving. It is the most irritating thing and there is no way for me to fix it without spending over $500 dollars. This is seemingly a common problem for this make and model through reddit posts and car forums. I was also told by a employee at the tesla service center that they see this very often. It is shocking that Tesla hasn’t taken responsibility in fixing this, when there is nothing any of us could’ve done to have this happen. I’ve seen people get quoted anywhere from $500-2,000 for this exact problem. It is so beyond distracting while driving.

My wife and I recently purchased a new 2024 model 3 Tesla with the Autopilot and Full Self-Driving feature. We chose this vehicle because my wife has a long daily commute and we wanted to make the drive easier for her. We test drove a similar model in early August. When we were testing the Full Self-Driving feature, the car failed to safely exit the freeway three times in our short test drive, causing my wife to have to take over. Both times, the car seemed to become confused by traffic cones as it prepared to exit the freeway, causing it to either miss the exit or to change lanes rapidly and unexpectedly, nearly colliding with other vehicles. When we reported this to the employees at the Tesla dealership, we were told that the car needed a software update to address the issue. It is now a month later and we received our vehicle a couple of weeks ago. I have not heard of the release of any software update addressing the issue we experienced. Given that this is the exact feature that prompted us to purchase the car and our intention was to make things safer for my wife, not the other way around, I would very much like to find out when this will be addressed. There are many drivers who currently use the Full Self-Driving feature and I am concerned that it is extremely unsafe.

Ever since receiving this vehicle, the turn signals will intermittently not work. This is the new 'stalkless' turn signal design, using push buttons on the steering wheel. When going to signal a lane change, either direction, the button will not register me pressing it, no matter how hard I try to mash on it. It will not signal, resulting in a dangerous, illegal situation for myself and other drivers either missing my turn, causing a distraction while trying to rectify the situation, or forcing me to make a lane change or navigate a turn without signalling. I have found a slight workaround that works some times, which is to press the button to signal the opposite direction of my intention, then switch it back. Again, this is dangerous. When I raised a service request to Tesla, the soonest available service date given was over 2 months out from when I raised the request. That means I am forced to drive a dangerous, unreliable vehicle without being able to reliably properly signal my intention to other drivers.

The rear motor failed on my car. It has less than 4,000 miles on it. It is already being worked on. The car barely gave me 10 seconds before it lost power, locked up and wouldn’t move. Yes. They told me the rear motor failed and needed to be replaced. Tesla inspected it themselves and confirmed the issue. Yes, they appeared 10 seconds before it died, I had no other warnings prior to that. This was extremely dangerous as I barely had time to get to a safe place.

- #11612862September 5, 2024FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS, BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING
I started receiving messages that the cameras were occluded and that there was an error and ALL of my system safety features were disabled. No front forward collision warnings, no emergency braking available, nothing available and the system computer that showed all of the traffic and everything completely disappeared from the screen, the car has literally gone blind. All of this started happening on with the camera issues in May, but I thought that it was a software issue and would be addressed in the next update, although the critical disengagements were supposed to be sent to Tesla. I took the car to the service center on 9/4/24 at 9am, and they stated that the did a system test and saw that the triple camera was showing a fault and so they replaced the camera and gave it back to me the same day. Everything worked for the rest of that day for a service that took at least 3hours to figure out. Today after I get off of work at 1pm and I go to initiate FSD and I get 3miles from my residence( I live <8mi from my job) and the system disengages again asking me to take over and all of my critical systems have yet again been disabled. My whole safety is compromised at this point yet again. I immediately bring the car back and they tell me that it's going to take them an additional 30-45 mins to "diagnose" the issue although it's the same thing that's been happening that was supposedly fixed yesterday. After that wait time the service consultant comes back and tell me that the car is throwing all kinds of codes and that, "It's still safe to drive." When I told that it cannot be safe to drive if all of the critical systems were disabled he suggested that I take it and they can do a remote log pull so they can look more into the issue or I can leave it as the part that's needed needs to be ordered anyway. I don't know how they can order a part if they're claiming to not know what's wrong with it. Something else is going on that they don't want to take accountability for!

On several occasions, the turn signal indicator buttons on the steering wheel failed to activate the signal. The button appeared to be jammed, it was not possible to feel the "detent" when pressed. This puts myself and others at risk due to the inability to properly signal when preparing to make a turn which has to then be aborted. I've found the button can only be "unstuck" by clicking the opposite turn signal indicator one or more times (this act can also be confusing to surrounding drivers). This happens randomly. Vehicle was brought to service center however they were not able to replicate the concern.

- #11612676September 4, 2024FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
When traveling on an interstate highway with a speed limit of 65 mph, with navigation system operating and with TACC engaged, the system sometimes resets the maximum speed to that of the speed limit of the exit ramp (usually 40 mph) when I am NOT exiting the highway. This results in rapid deceleration significantly below the prevailing highway speed with the potential of causing a rear end collision from following traffic. This is consistent some certain exit ramps along I-93 in NH, and is prevalent when traveling in the right traffic lane.

With the TACC system activated when traveling on a 2-lane road with a right-hand curve, if there is a semi-truck in the opposing lane, the emergency braking engages with a hard brake application. It can be overridden with accelerator pedal, but it endangers the car from collision with a following vehicle. There have been multiple incidents. I attempted to report the condition to Tesla customer support.

I am writing to express my concern regarding recent design changes implemented by Tesla Motors in their Model 3, Model S, and Model X vehicles. Tesla has removed traditional turn signal and shifting stalks from the steering wheel and replaced them with buttons. While innovation in automotive design is crucial for progress, I believe these changes have significant implications for vehicle safety and warrant thorough investigation. The traditional design of turn signal and shifting stalks has been in place for decades and has become a standardized control method universally understood by drivers. This familiarity contributes greatly to safe driving practices and allows drivers to operate their vehicles instinctively. Tesla's decision to replace these tried-and-true controls with buttons on the steering wheel is a substantial departure from this standard and may impact the muscle memory and reflexes that many drivers rely on in critical moments. Moreover, the ergonomic and cognitive effects of this new control layout have not been publicly detailed by Tesla, raising questions about whether sufficient research and testing were conducted to ensure that these changes do not negatively impact the safe operation of the vehicle. The consequences of such changes, especially in situations that require rapid response, could potentially lead to confusion, delayed reaction times, or even accidents. Given these concerns, I respectfully request that the National Highway Traffic Safety Administration conducts an investigation into the safety implications of these design changes. Such an investigation would determine whether the removal of traditional turn signal and shifting stalks meets the safety standards necessary to prevent accidents and whether adequate testing and validation were performed prior to these changes being implemented.

- #11611442August 28, 2024VEHICLE SPEED CONTROL, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
There is a a very dangerous software issue with new Tesla models where there is a 1-2 second lag between when cruise control is disengaged and when it actually disengages. Especially, at high speeds this is dangerous because the car can continue maintaining acceleration for several hundred feet against driver controls. I’ve had to slam on the breaks several times because of this. This is not a glitch, and this is not unique to my tesla. It’s a known issue reported by the community that needs Tesla’s attention.
