I have had this happen twice, once on city streets going to work the car was not in auto pilot and it went from 40mph to 0mph instantly. If there was a car behind me I would have gotten slammed into. The second time, my mother was in the passenger seat and I was on the freeway going about 75 to 80mph and it was on auto steer and the car went from the speed I was going to almost 0 and I instantly took control of the vehicle. We both could have been killed because of the last incident and I thought the cameras saw something on the road but there was nothing there. No cars were near me when both events occurred.

I was traveling northbound on [XXX} in Sugar Grove, IL with the traffic aware cruise control on. As I passed under [XXX] the car braked heavily for no reason. No cars in the area...thankfully. I could have easily been rear-ended. It has done this on more than one occasion at this location. I filed a "bug-report" via the right hand button on the steering wheel. I was also traveling [XXX] near Lasalle-Peru with the cruise control on at about 80 mph when the vehicle suddenly braked heavily. No cars around at all. The moment was extremely frightening. I went to drop my son to a summer program to a park. After dropping him, as soon as I started, car just accelerated very fast and went on the sides. I was able to turn the steering back into road but it kept speeding and went on the other sides. In the process, cars tires got impacted. Within 5-10 secs, car got from one side walks to the other side walk and then car stopped somehow. Fortunately, it did not impact any other cars parked or other cars on the road. But the whole thing was very scary.

I was traveling at highway speeds on a highway with no vehicles or other obstacles in front of me. I had adaptive cruise control on. Then, the emergency brakes deployed for no reason (so called “phantom braking”), decelerating the car quickly. I turned off adaptive cruise control as fast as I could and nothing bad happened. But the situation was dangerous, as if a car was close behind me it could have rear ended my vehicle.

The contact owns a 2024 Tesla Model 3. The contact stated while driving at approximately 65 MPH, the vehicle shut off. The contact stated that the vehicle lost electrical power and the main computer screen was black. The contact stated that the steering wheel was difficult to turn left or right, and the brake pedal was soft while depressing the brake pedal. The contact was able to coast to a stop. The contact attempted to restart the vehicle but was unable. The contact was able to get the main computer screen back online. The contact stated that the computer had several unknown warning lights displayed and a timer was activated and counted down and the vehicle shut off. The contact had the vehicle towed to a dealer. The dealer diagnosed the vehicle and determined that there was blown fuse, and that the vehicle went into "Fail Safe Mode” and shut down all systems. The vehicle was not repaired. The manufacturer was informed of the failure. The failure mileage was approximately 20,000.

Curvature Assist feature agressively brakes while driving at highway speeds. This is supposed to slow down for curves, but frequently intervenes for straight, clearly marked lanes. Feature cannot be turned off. Drivers behind me have gotten agitated several times for the sudden, harsh braking, and I do not feel comfortable driving this car.

Incident Summary: On [XXX], I was in an accident and dropped my Tesla Model 3 at Tesla’s North Phoenix Collision Center in Nov 2024. I received the car back in Dec/Jan. Immediately, I noticed dangerous swaying, bouncing, and veering when hitting dips—especially on the freeway. The car was unstable and unsafe to drive at high speeds. In Feb/Mar, Tanner (assistant manager at the collision center) drove the vehicle with me in the passenger seat. After hitting a dip on the freeway, he panicked, exited, and agreed something was wrong. I was later told they did a wheel alignment, but the issue remained. This was the second time they returned the vehicle in an unsafe condition. I nearly crashed due to how unstable it felt. In May 2025, I brought the car back again. Tanner suspected the steering column but said no diagnosis was done. He gave me a loaner vehicle, which had a flat tire. I had to drive my own unsafe car for nearly a month while waiting for another loaner and parts—no resolution during that time. On 6/13/2025, I contacted the Tesla Glendale Service Center. The advisor expressed frustration, saying this collision center often offloads unresolved issues. I also contacted Tesla Support (877-798-3752), who advised me to file this NHTSA complaint. As of June 2025, the issue is still unresolved. My car continues to sway and veer dangerously on the freeway, and Tesla North Phoenix Collision has failed to properly diagnose or fix it despite multiple returns. The contact owns a 2022 Tesla Model 3. The contact stated that while attempting to reverse into a parking spot, the cameras failed to activate, along with the sensors, which included the backup sensors failed to alert that there was an object. As a result, the contact crashed in reverse into a cement pole on the front driver's side door and mirror. No property damage was reported, and the vehicle came to a stop upon the contact depressing the brake pedal. The air bags were not deployed. The vehicle was going 5 MPH at the time of the crash. The contact attempted to retrieve the recording of the crash, and there was no record. No warning lights were illuminated. A police report was not filed. The vehicle was not destroyed. No injuries were reported. The local dealer was contacted, however the vehicle was not diagnosed. The vehicle was not repaired. The manufacturer was not notified of the failure. The approximate failure mileage was 17,290.

On [XXX] at approximately [XXX] on [XXX] in Oakland, CA, I was intending to park the car on the street at the curb. As I started to pull to the side of the road to park, the brake suddenly fully engaged, bringing the car to an immediate complete stop. My foot was not on the brake at the time. There were no obstructions of any kind in the road or in front of the vehicle. No warning sounds, messages or lights were activated before, during or after the incident. The incident caused no injuries, because of the low speed at the time, just a sudden jerk forward. However, if the event had occurred at a higher speed, some kind of injury would have been likely. I am also reporting the incident directly to Tesla. There has been no testing or inspection of the vehicle. The vehicle will be made available for inspection or testing if requested. My vehicle is equipped with Supervised FSD and has had couple of issued on the intersection of [XXX] and [XXX] in Portsmouth, Virginia. It seems every time I stop at the light, the system recognizes the red light and stops, but shortly after it wants to keep advancing forward although the light is still red and has not changed. The vehicle has also had a problem staying on lane at light on the intersection of [XXX] and [XXX] in Suffolk as it is getting ready to turn left prior to entering the entrance road to [XXX] . The image in the screen is correct and the lines are in place, but the vehicle has trouble staying with in the line and needed to be corrected. The contact stated that while the vehicle was in self-driving mode, the vehicle was supposed to merge into the right lane, however the vehicle accelerated into the left lane, and another vehicle collided into the front driver side of the vehicle. The contact stated that he did not sustain any injuries, however the passenger stated he sustained injuries, but the injuries were unknown. No medical attention was required. The contact stated that a police report was filed. The vehicle was taken to dealer where it was diagnosed that the software needs to be updated. The vehicle was not repaired. The vehicle was still drivable after the incident. The manufacturer was not notified of the failure. The approximate failure mileage was 104,547.

Documenting a sudden unintended acceleration experience. Today while driving and trying to stop for a red light, I lightly applied pressure on my brake pedal to brake for the red light. Midway through the braking, my car lunged forward for about half a second. Went through the dash cam footage, and I noticed that the footage skipped half a second as well. Seems like something electrical that caused the car to lunge. This is a huge safety issue.

FSD Malfunction, inspection available. Yellow double lines were not recognized around a turn, causing the car to steer towards oncoming traffic. UNKNOWN. Vehicle was declared fully functional by Tesla Dealership. No warnings, vehicle had many symptoms prior to the last event in which I decided to service the vehicle. First critical malfunction May 21, 2025.

The horn is extremely quiet especially after driving in puddles or in the rain. The horn being a speaker on the front of the vehicle causes it to become clogged and practically muted when used in the rain or after driving through water. The speaker horn is extremely quiet and unsafe.

Attempting to park. Slowly inching forward while turning left. Vehicle accelerated. (Sudden Unintended Acceleration: SUA) Vehicle jumped a curb, bounced off a tree, dropped off the curb before comping to a stop. Internet search shows other similar reports; attributed, by Tesla, as pedal misapplication. This was not "pedal misapplication". I am not looking for anything. But NHTSA should increment occurance counter by 1. Tesla has a problem. Electrical or Mechanical...do not know. They have also lost me as a customer. The car will be sold. I have video...mp4

My pant leg got caught on the plastic seat trim under the seat. Kept my leg trapped so I fell out of the car on the ground in the parking lot. Happened again the same day, but I caught myself before I fell. The incident caused a hole and rip in my clothing, but the rest of my body went to the ground leaving my knee and other leg caught until the end. Had a different outfit on today and it caught again! I did not fall this time either. The time I did fall I hyperextended my wrist and landed on my hip and leg, creating bruising and a sore pelvis, knee and back. Went to massage therapist soon after. Currently, following up with doctor
