The passenger rear tire (235/60R19 - Michelin Primacy A/S) randomly blew out while driving at high speeds on the highway. No trauma via a pothole or other object was noted. Tire was inflated to the manufacture recommended PSI (33 PSI). No signs came up until the tire blew out. The tire just randomly decided to give out while driving.

Around 11 am on Sat., May 2, 2026, I was sitting in a very long line in Starbuck's drive thru when my 2025 Lexus RX hybrid suddenly and uncontrollably accelerated--like an uncontrollable bucking horse. Brakes didn't work. I couldn't stop my out of control car, and the multiple computer sensors also didn't stop the car. It was like the throttle was engaged because of a computer glitch. In an effort to avoid hitting the car in front of me, I immediately tried to swerve, but still hit the car, then jumped the exceptionally high curb and ended up in the grass below and almost into the busy road ahead of me. It was terrifying. The car in front of me hit the car in front of her. The exact same issue happened to me on June 18, 2025 when my car unintentionally accelerated--also in a very tight space and going at 2 mph. There was no accident at this time because my car's computer stopped me from plowing into the building immediately in front of me after unintentionally accelerating. The next day at the recommendation of the person who sold me the car, I took my new Lexus to the service department. They gave me a loaner and told me to contact Toyota's/Lexus' Brand Engagement department in order to start the review, which I did. The review was conducted and ended around July 9, 2025 . On Mon., May 4, 2026 I took my car back to the Lexus dealer where I purchased, but was not given a loaner, which I requested, and was told I would need to wait several hours to have this inspected. I then took my car to Boyce Body Werks. A State Farm agent examined my car and initially approved almost $24K worth of damages. Boyce Body Werks is saying when they disassembled my car that there was more damage underneath my car. I also notified Toyota/Lexus about the same issue that happened one year prior. They also refused to give me a loaner, and I am now forced to pay my rental fees. Boyce Body Werks--a Lexus certified dealer has multiple pictures of the damage. My car is still under warranty.

- #11736812May 9, 2026AIR BAGS, SEAT BELTS
The car does not alert when a seat belt is not worn in the back seats. This feature is mentioned in the owner's manual, but the car fails to alert when a rear passenger does not buckle their seat belt. I have raised the complaint twice and proved to lexus service center, that car doesn't warn or alert the safety belt. It was also proven and checked by Lexus with their technician. The problem is accuring right from the time car was purchased, When check with the dealer it was designed , but their manual confirms the seat belt not worn warning light is included in the package. It put the any passener riding mainly children at serious risk

I am experiencing ongoing issues with my 2023 Lexus RX500h related to the powertrain/transmission. The vehicle has exhibited abnormal shifting behavior, including hesitation, jerking, and inconsistent acceleration. At times, the vehicle does not respond smoothly when accelerating, which raises safety concerns, especially when merging into traffic or driving at highway speeds. The issue occurs intermittently and has persisted despite multiple attempts to diagnose or address the problem. The condition affects the drivability and reliability of the vehicle and has not been resolved.

- #11731649April 16, 2026SERVICE BRAKES, ENGINE, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
On April 1, 2026, while pulling into a parking space at our church, I was applying the brake when the vehicle suddenly and forcefully surged forward without warning. The unintended acceleration was intense and unexpected, occurring despite continuous braking input. This resulted in the vehicle striking the building and causing significant damage to both the vehicle and the structure. This incident has occurred twice within a 10 day period, both times during low-speed parking/stopping maneuvers. There were no warning lights, alerts, no automatic emergency braking engagement, no air bag deployment, or prior indications of malfunction. I did report this to the police and they filed an Incident Report. Lexus was notified and an independent analysis engineer examined the data from the vehicle's CDR System. This report was sent to me and to Lexus.

- #11731039April 14, 2026LANE DEPARTURE: ASSIST
When in cruise, the car will merge itself into the next lane if you hold the turn signal partially. If you keep holding it or fully engage the turn signal (maybe you forgot to turn off you signal, or held it too long), the car will merge through the solid white line off the road into a ditch. This happened to me once, and I noticed it trying several other times.

The contact owns a 2024 Lexus RX 350H. The contact stated that while attempting to start the vehicle, several unknown warning lights illuminated. Additionally, the door latches locked independently, and the contact and wife were locked inside the vehicle for several hours. The contact eventually remembered to pull the mechanical door latch and exited the vehicle. The vehicle was towed to the dealer where it was diagnosed with battery failure. The contact was informed that the battery needed to be replaced. The vehicle was repaired. The manufacturer was made aware of the failure, and a case was filed. The failure mileage was approximately 17,500.

I bought a CPO lexus 2025 RX 350 in Jan 9, 2026. This car was lightly used in less than 3 months and right now it's around 7685 mileage. However, just with 3 months I purchased this car, the A/C stops cooling and the dealership said this is due to the condenser was damaged by small road stones where the small road stones can easily pass through the grille. - We have noticed that there are multiple reports of Lexus/Toyota SUVs with condensers easily punctured by normal road debris. - Dealers acknowledge it as a common pattern. - Loss of A/C can affect defogging/visibility and is risky in extremely hot weather, e.g. causing hard breathing, etc for driver and passengers in the car. This issue needs to be fixed by Lexus/Toyota.

Car stopped running during turn. Then started creeping along ,then after a bit ,started working normally. Happened a few times after. Then later car wouldn’t start in garage, had to get jumpstart. Took to Lexus dealer and found out the 12 volt battery was shot.they replaced it. Very frightening experience. Could have had an accident.

2025 Lexus RX350H purchased new on 09/04/2025. The sunroof portion of the panoramic sunroof exploded, sounded like shotgun blast. Then we heard some wind noise but realized none of the windows were open/affected. I started to open the inside electronic sunscreen but noticed glass shards so I immediately closed it back up. We were greatly startled and I could have swerved/loss control of the car, thankfully I did not. Also, if the interior electronic sunscreen would have been open at the time of explosion we could have been cut from falling glass shards and would have probably lost control of the car. We were crossing the Old Tampa Bay on the Howard Frankland Bridge (I-275) heading to Tampa, FL Airport. There are no overpasses on the bridge so no falling debris was possible and we did not see any air-born road debris that impacted the roof of the car. There were no other dents etc. in the roof or car body indicating an impact. When I took the car to Lexus to get the sunroof replaced the service representative stated it was the third replacement recently. When I picked the car up a couple days later I spoke with the Service Manager about my extended warranty coverage and the lack of evidence of an impact from road debris. He stated warranty would not cover the replacement and Lexus would not cover the replacement even on a 4 month old vehicle.

- #11724502March 15, 2026BACK OVER PREVENTION: SENSING SYSTEM: CAMERA
On November 11, 2025, while reversing inside a garage, the rearview camera image— which had initially displayed correctly on the vehicle’s screen—suddenly froze while the vehicle remained in reverse mode. At the same time, the Pre-Collision System (PCS) failed to engage the brakes while the vehicle was traveling at approximately 2–3 miles per hour. As a result, the vehicle struck the garage structure and sustained damage. Toyota investigated this incident (Case #XXX) but based its conclusion on the incorrect assumption that I had reported either a brake system malfunction or a complete absence of an image on the display (a dark or blank screen). Because of this, Toyota determined that it could not provide assistance regarding the resulting vehicle damage. This characterization is inaccurate. The issue was not a blank screen. The rearview camera image appeared normally and then froze while the vehicle was still in reverse. My vehicle is currently subject to Recall Campaign No. 25LA06, which addresses reliability issues involving critical safety-related electronic systems. The rear visibility system is a federally mandated safety feature under 49 CFR §571.111 (Rear Visibility Requirements). When the image freezes during active vehicle operation, the system no longer provides the required real-time visibility necessary for safe reversing. Given the existence of an active recall campaign aimed at addressing reliability issues within the vehicle's electronic safety systems, this malfunction should be considered potentially linked to a defect covered by Recall Campaign No. 25LA06. The failure of the rear-view system during vehicle operation directly contributed to the occurrence of the collision.

The contact owned a 2024 Lexus RX 350H. The contact stated that while the vehicle was parked and running, it unexpectedly turned off with the electronic malfunction error message displayed. The vehicle could not able to be restarted, and the contact was unable exit the vehicle due to the failure of the power locks. Roadside Assistance assisted with removing the contact from the vehicle. The vehicle was taken to the dealer, where it was diagnosed with battery failure. The contact was informed that the battery needed to be replaced. The vehicle was repaired; however, the failure persisted. The vehicle was taken back to the dealer and remained with the dealer. The dealer was not contacted. The vehicle was not diagnosed or repaired. The dealer attempted to return the vehicle back to the residence. The contact was concerned about the issues with the vehicle. The manufacturer was made aware of the failure. The failure mileage was approximately 1,624.

- #11723281March 10, 2026VEHICLE SPEED CONTROL, SERVICE BRAKES
The contact owned a 2023 Lexus RX 350. The contact stated that while the vehicle was stopped at a traffic light, it suddenly accelerated without warning. While driving in a parking lot, the vehicle again surged forward approximately 20 feet. The contact was able to swerve to avoid striking a pedestrian. The vehicle became uncontrollable, and the brakes were inoperable. The vehicle ultimately crashed into a dumpster fence, which fell onto the vehicle. The contact sustained injuries to her mouth, including cuts to the inside and outside of her mouth, and medical attention was provided by a plastic surgeon. The fire department responded to the scene. The air bags did not deploy during the crash. There was no fire, and a police report was not filed. The fire department was on the scene. The vehicle was towed to a local dealer; however, the vehicle was not diagnosed or repaired. The insurance company later deemed the vehicle a total loss. The manufacturer has not been notified. The failure occurred at approximately 18,000 miles.

[XXX] ’s my wife we’re driving in Sacramento. The vehicle was stopped at a traffic signal waiting for the light to change. Suddenly, without warning the vehicle started violently shuddering/shaking, scaring us completely. The shaking stopped when my wife put into park. But for those few seconds it was extremely scary. This incident should be investigated by the NHTSA as a repeat of the incident in moving traffic may cause severe property damage, serious injury and possible loss of life. - #11722677March 7, 2026ELECTRICAL SYSTEM, ELECTRICAL SYSTEM:12V/24V/48V BATTERY
I am reporting a recurring electrical and mechanical system failure on a 2024 Lexus RX 350h Luxury. On [XXX], the vehicle suffered a total power loss that rendered the electronic door latches unresponsive to all inputs. Because the occupants were a [XXX] and a [XXX] strapped into car seats, they were physically unable to reach the interior manual overrides. Critically, the mechanical backup key also failed to unlock the vehicle from the exterior, preventing immediate rescue. This created a life-threatening entrapment scenario. The doors only responded after multiple minutes of repeated manual attempts. Once entry was gained, the vehicle would not start and the headlights began blinking rapidly, indicating a total system communication failure. I had to use jumper cables to restore power. This is the second occurrence of this catastrophic failure, documented on Repair Order [XXX] at Lexus of Westminster. The failure of both electronic and mechanical failsafes constitutes an extreme occupant safety hazard. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

While driving westbound on Route 70 in Ohio at highway speed, we suddenly heard what sounded like an explosion and immediately felt exposure to outside air within the vehicle. The event was abrupt and very startling while in motion. After safely stopping and inspecting the vehicle, we observed that the driver-side rear door window had shattered. The damage included a pronounced irregular opening located approximately in the middle of the window toward the rear edge, with complete fragmentation of the surrounding tempered glass. We inspected the interior of the vehicle and surrounding area for evidence of a rock, debris, or other projectile but did not locate any object that could have caused the damage. This unexpected window failure created driver distraction, potential risk of injury from glass fragments, and sudden loss of window integrity while driving at highway speed. This report is submitted so that any potential pattern of unexpected automotive glass failure in this vehicle model can be evaluated.
