On January 18, 2024 I had the following recalls performed on 2019 Honda Odyssey Elite: Auto Idle Stop and the Media Oriented Systems Transport (MOST) communication coaxial cable. Immediately after the recall work was completed, I began receiving an error code "Collision Mitigation System Problem". The error code would flash after this sequence of events: After my car's engine has come to temperature and is warm. I come to a complete stop, my car goes into Idle Stop and when I release the brake to accelerate and the engine re-engages, this error flashes. And, I can continue to drive. Prior to this recall work, I had never received this error. In addition to this error code, on January 24, 2024, I was at a complete stop on a freeway offramp. When I released the brake and tried to accelerate, my car would not engage and very loud clunk, followed by the orange "Brake System" light flashed on. Eventually my car engaged and we were able to drive. The clunk was so loud my teenage children asked me if we were hit by another car. On, Friday January 25, 2024 after dropping the car off at the dealership we were told that "The diagnostics have revealed a 'defective Multipurpose Camera and AIM'. And, the cost to replace is $1793.77 plus tax.

All safety systems which I had come to rely on failed all at once. Honda claims it’s the multi function camera that just randomly went bad. This affects safe driving. Condition Is ongoing

The transmission is unreliable. You push the gas and it hesitates to accelerate or switch to the next gear to speed up. It can shake or rattle the car. Has a trouble at low speeds or shifting to the next gear to increase speed. This makes it dangerous to try to pull out into an intersection because the transmission or gear shifting doesn't always hit and you can get hit or injured. This transmission has been weird since the car was new.

A defective/leaking seal in the rear of the vehicle area caused water damage and corrosion on the tailgate control unit. As a result of the control unit failure the blinkers and lights stopped working while driving. In addition rear tailgate does not open. Dealer confirmed the issue, fixed the seal and replaced control unit. Research shows this is a recurring problem on this model vehicle.

10/4/2023 (55K miles), the vehicle suddenly lost power at interstate speed and dropped to three cylinders. Warning lights came on disabling all automatic safety systems. I took the vehicle to a parts store and OBD indicated an oil pressure sensor in VTEC Bank 1. After restarting the vehicle I once again had all 6 cylinders, but all safety systems failures were still indicated on the dash. I replaced the battery (5.5 years old) and all systems returned to normal. After a the PGM-FI update for idle stop issues (recall) was performed at the dealer on 1/31/2024, two days later (2/2/2024) the previous event recurred resulting in severe loss of power and all safety systems indicating failure. I replaced the 4 month old battery under warranty believing the battery had failed prematurely. The problem did not resolve after replacing the battery and multiple restarts. I returned the car to the dealer service center on 2/3/2024 running on three cylinders and then suddenly 6 again after a restart part way there. Dealer diagnosed issue as intermittent fault P2652 Rocker Arm Oil Pressure Switch B (Bank 1) and replaced it at my cost but did not charge diagnoses. Operation returned to normal. ***I have driven Honda vehicles for 25 years with very few issues. Something is seriously wrong in their ECU program/electrical system that causes vehicle to suddenly lose power and safety systems due to an intermittent oil pressure sensor fault. I am a Mechanical Engineer with more than 18 years working in mechanical and electrical aerospace systems. Honda needs to identify the root cause of this failure mode and issue yet another recall to address the systemic hardware and software issues. Both times this occurred, I had 4 young kids in the car and could not maintain safe interstate speed or accelerate to get on the highway.

The vehicle had low voltage indications and electrical issues that would affect many of the electrical systems in the car (power doors, main entertainment system, etc). More concerning, the vehicle would shift into park while driving and then would shut off. This posed a serious hazard when driving quickly. Honda was unable to remedy the issue and could not come up with a definitive cause. Another recall with these same problem was issued for other VIN numbers of the same make, model, and manufacturing year. My VIN was not included in that recall. The recall needs to be expanded to include more VINs as this is an issue in 2018 Honda Odyssey vans and poses a risk to me and my family while driving in its current state. My transmission is now experiencing issues due to shifting into park while driving and needs to be replaced.

My 2020 Honda Odyssey started showing SRS problem error on its 4th year of ownership. Since it’s not under warranty after 3 years I spent close to 1k to replace SRS computer module by dealership from where I purchased my car. Same error came back a month later and again I took it to dealer and they said it has loose connection on cable and fix the loose connection. About a month later SRS light came back again and then I took it to dealer and he said connection is good but one of the connector is not sending the signal to SRS module. They asked me to replace entire SRS module wiring which will be around 4k which might resolve this problem. I reported this problem to Honda Corporate and waiting for their response. Since it is SRS problem, I am more concern to drive the my car with Kids. If I meet an accident and airbag won’t deploy then it will be risk for me and my family.

Windshield wipers and also the key fob and the sliding doors as well as the blinkers do not work all at the same time first time it’s ever happened. It was December 8, 2023 and now it happened again in January 29 of 2024 and the issue still persisted when I look this issue up this has been an ongoing issue for many years and yet Honda has done nothing about this and it’s not just the 2009 it’s other years as well as other Hondas from other years as well, it is a defect within the company where they’re doing something that causes these things to go! It is a safety issue as this just happens out of nowhere without any warning while I’m driving. All of a sudden it just happened the first time and I didn’t even realize it until I didn’t hear the clicking and someone said hey your blinkers are not working. Also, the second time it happened happened all over again while I was driving out of nowhere would not work and it was raining as well and I couldn’t see anything and my lights wouldn’t work so it’s definitely a safety issue .

Recall for driver side view mirror completed. However the replacement mirror is distorted and causes blurred, wavy, double images. This replacement mirror needs to be addressed as soon as possible since proper representation of objects is distorted and poses a high safety concern. 1st incident Fall 2023 Approaching a stop sign, decelerating from 30mph I pressed on the brake to complete the stop but the engine roared, RPMs revved, & the vehicle accelerated, better it lurched, forward. I mashed down the brake pedal as hard as I could but it would not stop the vehicle. All I could do was put it into park. Happens almost weekly, at least 7 times, but not predictably. Once in December, I was approaching a four way stop sign around 3:30pm, I had been driving the vehicle for approximately 30 minutes & the weather was good (no ice, partly cloudy, roads clear & dry). As I came to the stop sign, pressing the brake the final time to complete the stop, it roared loudly, the RPMs jumped high, and the vehicle surged forward through the intersection. I let off the brake and tried to smash it down again, but it wouldn’t go to the floor (only a mild decompression) so I powered through the intersection, thank God no one was coming. I didn’t pull over or turn the vehicle off, I continued going my way home. 15 minutes later, turning into my driveway the brakes worked normally, like nothing had happened. [XXX] I took it to a Honda dealership. They upgraded the transmission & flushed the transmission lines three times. They were unable to replicate the issue & could not detect any codes or communication of an issue in the system. I took it home with no answers. [XXX] I was approaching a stoplight to turn left. I was driving, my brother was in the passenger seat. Weather conditions were good: clear skies, no ice/rain/snow, roads dry & clear. I had been driving the vehicle for less than 10 minutes & I had given it ~ 5 minutes to “warm up” since it’s Minnesota. I met a stop sign w/o issue, but then as I approached a nearby stoplight it surged forward just like before. I pressed the brake to complete the stop, the engine roaring so loudly my brother said “it’s gonna blow up!” Back to Honda dealership on [XXX]. Still no resolution. What component or system failed or malfunctioned, and is it available for inspection upon request? Rod bearing failure lead to engine seizing How was your safety or the safety of others put at risk? We started hearing the ticking in October, I visited the dealership and contacted Honda multiple times and either told to take no action or wait for the recall. Case #[XXX] We expressed our concern to both the dealership and Honda about not addressing the issue because we had a big trip for Christmas coming up. My mother-in-law has terminal cancer and the entire family was getting together for Christmas with her. To get there we drive 20 hours from Arkansas to Utah. We followed the advice of the dealership and Honda and waited. Unfortunately about 50 miles out of Amarillo the van started to make loud engine noise and stalled. While going 75 miles an hour down the interstate I lost power steering and my brakes were very unresponsive. Luckily, we were in the farthest right lane and I was able to get us safely to the shoulder. We feel blessed to have made it off the road, it was a very dangerous situation. Now my van with 70K mile has a seized engine and is stuck 12 hours away in Amarillo Texas. I am requesting that Honda reimburse my rental, ship my van back home to Bentonville, and send a new engine for the dealership to fix the now seized engine. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes the issue has been diagnosed by Brown Honda in Amarillo TX by Todd Burkham the Fixed Operations Director Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Only by Brown Honda in Amarillo TX Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Only the ticking sound that we took the vehicle in for starting in late October. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

In early December 2023, while driving, I experienced a sudden malfunction in my LKAS system, receiving a notification that it had stopped working without any prior indications. In response, I promptly pulled over, turned off the vehicle, and restarted it, hoping to clear any error messages. Unfortunately, the issue persisted. Subsequently, I took my vehicle to the Honda service center, where I was informed that the front camera was malfunctioning, triggering multiple error messages such as collision system problems, auto high beam issues, and adaptive cruise control malfunctions—all of which rendered these safety features inoperable. The service center clarified that, while it was deemed safe to drive the vehicle, the automatic braking system and front camera features were unavailable, and any necessary repairs would be at my expense. The paperwork provided indicated the discovery of DTC U3000-49, citing a multipurpose camera unit/millimeter wave radar internal electronic failure. Additionally, beyond the LKAS system issue, I have repeatedly encountered difficulties with my vehicle's acceleration. Notably, there is a struggle to accelerate from a complete stop, and a significant delay in acceleration occurs when removing my foot from the gas pedal, especially in the middle of intersections. This delay has resulted in near collisions, with other vehicles honking and, at times, coming dangerously close to colliding with my vehicle. Despite reporting this ongoing acceleration problem to the dealer multiple times, the issue persists and has worsened over time. This comprehensive incident report aims to document and address the technical malfunctions in my vehicle, emphasizing the urgent need for a thorough examination and resolution of these safety concerns.

On June 2023, the driver side rear sliding door of my Honda Odyssey abruptly ceased functioning. Despite the infrequent use of this door—since I typically rely on the passenger side for dropping off my three children at school and activities—it is perplexing that the underused door has encountered such a critical malfunction. Given that our family of five heavily depends on this vehicle for transportation, the inability to utilize both doors has not only caused significant inconvenience but also raised substantial safety concerns. The driver side rear sliding door is currently sealed shut and attempts to open it manually have proven futile. It appears that the latching mechanism has completely failed, as the motor makes an audible attempt to open the door, yet no actual movement occurs. This issue has persisted for over six months, prompting ongoing communications with Norm Reeves Honda in West Covina and Honda's customer service team. According to the Honda report, it is suggested that the door may have been forced open. The estimated cost for potential repairs is $1,679.19, pending a proper assessment of the specific problem once access to the affected area is granted. The technician mentioned the necessity of breaking the paneling to reach the latches for replacement. Notably, this is my second Honda Odyssey, and two other family members also own Odysseys. Honda's van doors have a documented history of malfunctions, and it is disheartening to observe the lack of attention to this recurring issue. Moreover, the burden of the associated costs is unfairly placed on the customer. The prolonged delay in resolving this matter and the financial implications underscore a pressing need for Honda to address and rectify these door malfunctions promptly and responsibly.

Warning messages for the following appeared: brake system lane keeping/assist system collision mitigation system problem road departure mitigation system problem auto-high beam problem adaptive cruise control problem None of these safety features now work. Our local dealership told us it was a failure of the multipurpose camera unit. The fix we are told is a $1400 part. The rear camera was just recalled by Honda and repaired within the last month. This failure should not have happened and greatly impacts the safety of the car and it is a common problem when searching Honda message boards.

Over the years of owning this vehicle, we've encountered sporadic electrical problems that have led to the car either stalling or needing a jump start. Despite these issues, inspections of the battery have revealed no obvious problems. Additionally, we've had intermittent system warning messages for features such as Adaptive Cruise Control, Forward Collision Warning, and Lane Departure. These warnings have surfaced and vanished on multiple occasions. The vehicle has never been in an accident or experienced anything beyond normal operating conditions.
