The vacuum pump. This vehicle was inspected by Dunn Tire. Yes, the vehicle is available for inspection. The safety issue that occurs is the loss of power assist to your brake system, and if not repaired, can impact the camshaft or the oiling system. (Please see attachment).

My rear backseat driver’s side door handle broke on March 16th, 2026. I either have to roll down the window and open the door from the outside or I have to let passengers out from exiting from the front seat. I took it in for inspection by the dealer, West Kendall Toyota on March 17th, 2026. They had to order a new part. They tried repairing it on March 31, 2026 but failed to do so and had to reorder the part. It is still broken as of today April 1st, 2026. My front seat driver’s side door handle also broke in August of 2025 and I could only get out through opening the window and unlatching the door from the outside door handle. This is a serious safety issue as passengers could become trapped should the car be hit on the other side and if even more so if the power goes out on the car. I feel Toyota is not doing enough to fix the issue and is not making a quick effort to repair this and is showing a lack of concern for consumer safety.

I got into a head-on collision with a cement center median going roughly 45-50MPH after fishtailing on black ice. My airbags did not deploy at that speed, I contact toyota corporate and they only removed the CDR information and did not assess the airbag sensors themselves. I was able to press the SOS button for EMS to arrive. My safety was put at risk as none of my airbags deployed. It was not inspected by my insurance company and was taken to an automotive shop to be repaired. No prior warning systems were enacted,

What component failed? The rear brake system (rotors and pads) and the components associated with Safety Recall 25TA04 (rear brake hoses). The parts are available for inspection. * How was safety put at risk? The dealer (Island Toyota, St. Croix) labeled my 2024 Tacoma "UNSAFE TO DRIVE" on Invoice #[XXX] but refused to perform the Federal Safety Recall (25TA04) for rear brake hoses unless I paid for an $800 brake job first. This violates federal law by making a safety recall conditional on customer-paid maintenance. I am currently driving a vehicle the dealer documented as "unsafe" because they withheld the free safety remedy. * Has the problem been confirmed? Yes, the dealer confirmed the rear brakes are failed but refused the recall and the 36,000-mile warranty claim. * Has it been inspected by others? Not yet. However, the dealer’s technician failed to record actual millimeter measurements for the pads or rotors on the official invoice. * Warning lamps or symptoms? Metal on metal grinding noise from the rear brakes. No warning lamps appeared prior to the failure. * Additional Note on Records: The dealer falsified my service records on Line C (Recall 25TC07), claiming I was "unwilling to wait" for a repair that was never even mentioned to me. The dealer also claims I denied service on Line B, but what actually happened is I refused to pay $800 for a premature brake failure that should be covered by my 36,000 mile warranty or as consequential damage from the recall. The dealer then used my refusal to pay for pads as an excuse to withhold the Federal Safety Recall (25TA04). I never denied the safety repair I denied a bill for a factory defect. I have uploaded photos of both rear wheels. The left side is healthy, but the right side is completely destroyed with deep scoring. This uneven wear at only 34,613 miles proves this is a manufacturing defect, not my driving style. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

Immediately after the manufacturer recall requiring full frame replacement (completed November 2017 by the same dealership referenced below), the vehicle developed abnormal handling not present before the repair, including improper tracking and rear end instability. No warning lights/diagnostic messages were present. These issues were reported to the servicing dealership within weeks and repeatedly over the following years. Despite multiple short initial inspections and two extended service periods—totaling approximately fourteen months in the dealership’s possession—the dealership repeatedly reported no defect found and continued to advise that the vehicle was safe to operate. Due to ongoing safety concerns, an independent inspection was recently obtained. It confirmed measurable rear axle offset and a thrust angle deviation outside acceptable tolerances. The defect is visually apparent: the rear wheels are not symmetrically aligned, and the vehicle exhibits a noticeable “crab walking” effect when viewed from behind. The root cause remains undetermined and would require further diagnostic work and significant disassembly. All documentation and inspection findings are available upon request. Safety risk: The vehicle does not track correctly and shows instability, especially in low traction conditions such as rain or snow, increasing the risk of loss of control. The manufacturer has been formally notified through repeated outreach, including ongoing contact with the Brand Engagement Center since December 2025 and multiple certified letters to Toyota North America Legal/Consumer Affairs between January and March 2026. The only response received was a generic statement from the Brand Engagement Center indicating that the vehicle is out of warranty and an offer to return it to the same dealership that performed the frame replacement and failed to identify the defect. No other response, investigation, or resolution has been provided by Toyota North America.

Toyota issued this recall about six months ago, and they have been dragging their feet for something that should be rather simple. They have not handled this recall in a timely manner we were originally told it was gonna be corrected in November that obviously didn’t happen and now we were told first quarter of twenty six and obviously that hasn’t happened

Due to my husband driving the vehicle, ISP states it looks like he crossed into an oncoming semi-chip truck trailer, causing a fatal accident and killing him instantly. The 2025 Toyota Tacoma's Safety Sense obviously failed since my husband is now dead. We had the 2025 Tacoma in the prior week due to my complaints that the lane assist was not working properly and the screen did not respond to my touch to turn it on. After the fatal accident, ISP removed the black box to download information for the accident report. I am now waiting for isp report. No jaws of life was used to extract husband they didn't need it Tacoma metal already peeled back like a tuna can.

My 2025 Toyota Tacoma TRD Pro (VIN [XXX] , purchased from Delaney Toyota, DuBois PA) exhibited a recurring Smart Key and door lock malfunction beginning in late October/early November 2025 and continuing through the date the vehicle was traded in February 2026 with the defect unresolved. Symptoms observed: Doors locking autonomously on exit, sometimes before occupants could exit the vehicle System falsely detecting the key fob as being inside the vehicle when it was in the owner's pocket outside, causing the truck to refuse to lock Touch-lock refusal at the door handle Random unprompted beeps while parked and locked with no corresponding lock movement Key card unable to lock the vehicle Dealership witness: During a January 2026 service visit, Delaney Toyota Service Advisor Ricky Carlson was personally locked inside the vehicle by this malfunction before he could exit. This was witnessed firsthand by a trained Toyota service employee and is part of the service history, though it was not captured in the official repair order. My 2026 Toyota Tacoma TRD Pro (VIN [XXX] , delivered 02/28/2026, purchased from Delaney Toyota, DuBois PA) has exhibited a recurring Smart Key and door lock malfunction beginning at approximately 250 miles and continuing through at least 1,814 miles as of March 18, 2026. Symptoms observed: Doors locking autonomously without any input while occupants are entering or exiting the vehicle System falsely detecting the key fob as being inside the vehicle when it is in the owner's pocket outside, causing the truck to refuse to lock Touch-lock refusal at the door handle Random single beeps while the vehicle is parked and locked with no corresponding lock movement Dashboard warning "Key Not Detected" with audible alert while vehicle was in motion (March 18, 2026, approximately 11:06 AM) Dashboard warning "Key Not Detected" with audible alert while attempting to start the vehicle (March 18, 2026, approximately 2:30 PM) Safety concern: On March 18, 2026, a Toyota Brand Engagement Center case manager (case #XXX) confirmed on a recorded call that if the vehicle loses key fob detection while driving and is subsequently shut off, there is a possibility it will not restart even if the owner holds the fob directly to the start button. This creates a real stranding risk on public roads. Additional facts: This defect has never generated a diagnostic trouble code across multiple service visits A full-day diagnostic on March 9, 2026 (RO #XXX) produced no fault codes Immediately after that diagnostic, the vehicle autonomously locked out two Delaney Toyota employees on dealership property — witnessed by the dealership's General Sales Manager and a Product Specialist The identical defect appeared on a prior vehicle, a 2025 Toyota Tacoma TRD Pro (VIN [XXX] ), which was traded in February 2026 with the defect unresolved after three service visits Toyota Corporate case #XXX is currently open INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11725081March 17, 2026FUEL/PROPULSION SYSTEM, VEHICLE SPEED CONTROL, UNKNOWN OR OTHER
On Sunday, March 15,2026, I was driving from Houston, TX to Pharr, TX at the speed of 75mph on the left lane to pass an 18 wheeler, with another 18 wheeler behind me, and my truck started idling erratically. I lost speed and my service engine light came on. My truck slowed down and I almost got in a car accident with the 18 wheelers because the truck did this. The truck behind me slowed down on time and it went immediately to the right lane to avoid hitting my truck. I feared for my life. All the vehicles behind me and the 18 wheelers also went to the right lane to avoid me. By this time, the TRAC light had already come on also. I managed a space of no vehicles and steered towards the right safety lane. I pulled over at the local truck stop and stoped my truck to rest. After about 5 minutes I tried starting my truck and it wasn’t starting. I am a little mechanically inclined, so I started troubleshooting the truck. My code reader gave these codes: P1605-00, P0300-27, P0300-85, and P0302-00. Unscathed to start the truck after a while and got back on the road. The drive was brutal, as the truck kept burping erratically, but I made it home safely. I took my truck to the mechanic and he did some repairs and told me the issue is the fuel pump. I have checked the recalls for my truck but it shows zero recalls for my truck. Luckily I am alive to report this life threatening safety issue that Toyota has on this model. How do I get Toyota responsible to make the repairs on my truck? The part alone is over $1200 at O’Reily auto parts.

Driving around a winding turn incline , tacoma hesitated at 40 mph while going down incline. Tacoma rpms fluctuates at 40 mph as if the engine is having difficulty accelerating. You can hear the engine stalling or shuttering to catch up to speed. This is occurred driving at a steady 40mph. The Tacoma seems to be stalling out at a certain speed limit and rpm at 40 mph. It only has this symptom at 40 mph going down an incline. This is dangerous and there were vehicles behind me traveling at the same speed going around a winding turn incline. This could of caused a pile up because of the tacoma struggling to maintain speed and accelerate to keep speed. Ive taken this tacoma to Nalley dealer and they've replicated the problem but thought if resetting driving mode would clear the problem. That did not clear the problem and its currently causing the same issue. The tech seems clueless in whats going on when ive explained that it happens at a certain mph and while going down incline. The tech know that this has occurred when he taken the tacoma on a test run. Some how the tech think a reset of driver style handling is the issue. The tacoma isn' responding to a driver style handling reset. Ive scheduled another appointment with the dealer to have the tech check the tacoma again. There are other years of Tacoma's that have this problem. I believe this particular tacoma has the same issue as the other tacoma years that are under a torque converter recall. The symptoms are identical compared to other tacoma year models under the torque converter recalls.I don' want this problem to become a hazard where the tacoma cause a massive pile up in traffic because of the unstable mph at 40 mph.The 2015 tacoma doesn't keep steady speed limit at 40 mph going down incline. The rpm fluctuates at 40 mph as if its trying to distinguish what speed limit the engine is driving at. The tacoma is stalling at 40 mph while making a shutter sound while driving down inclines and winding turns.

2017 Toyota Tacoma painted in MGM (MG1). The trucks paint/clearcoat are quickly failing from the factory paint application. This is a garage kept vehicle with 75k miles. Toyota refuses to accept any ownership of the factory paint application. This is a common issue amongst all Tacoma's, regardless of year or color.

It happened on February 17, 2026 around noon. My truck is park on the drive way, I went outside to checked the condition of the weather and I notice the driver side window glass looks like a crumpled Reynolds Aluminum wrap. I called Toyota Service Dept since the truck won't start. The battery was replaced and so is the window glass but, they told me it's not covered by the warranty. I paid the new window glass for $619.99. I called Geico my insurance and the deductible was $1,000.00. I told Geico I will not claim for the damages for now. I was wondering of all the cars and trucks park in our neighborhood, only my truck sustained this kind of damage. Would this fall under defective materials used by Toyota?

On February 16 I started my vehicle to drive home from work and the vehicle threw multiple error codes and shutdown while driving and would not start again. It was towed to the dealership. The dealership advised the problem was remedied. 11 days later the same problem occurred, vehicle threw multiple error codes and shutdown while driving. Vehicle needed to be towed again to the dealership.
