During an AWS outage on 10/20/2025, the connect app which can turn on or off the vehicle, unlock/lock doors, etc had issues where it performed an unlock on our vehicle 5 times in 24hrs without us doing anything. Key fob was in sleep mode / low power mode (cannot signal vehicle). Reports of this happening to others found online, but Toyota dealership states no reports on any of this happening.

The vehicle on board information screen will either fail to connect to phone for gps service, or will disconnect mid drive causing the gps service to stop. When the phone fail to connect with the screen it forces me to use the map on my phone causing me to look away from the road and at times have to hope my phone in my hand or risk missing my stop/exit.

When driving, I occasionally hear a sloshing sound. After inspecting the vehicle, I realized it was coming from the rear passenger door of the RAV4. After putting down large bowls to catch the water, I pulled up on the rubber weather strip at the bottom of the rear passenger door and water gushed out. I collected over 13 cups of water out of the door. This is not an isolated incident, as it has happened before. At first I thought it was debris/a fluke that caused this water to accumulate, but after reading forums, there are multiple people out there running into the same issue. It's only ever been the rear passenger door too; none of the other doors are holding water.

The passenger rear tire experienced a catastrophic tread separation failure on the highway at speed limit speed. Since this was on the freeway, loss of control could have been experienced and caused a severe incident injury to drivers in the vicinity of the incident. The problem just happened and will be inspected by an authorized dealer. There were no warnings until the tire of the incident. These tires have been rotated according to schedule and have less than 21,000 miles.

The contact stated while driving 70 MPH, the rear passenger’s tire detached from the rim. The contact stated that the failure had occurred while driving on a three-lane highway. The vehicle was driven over to the shoulder of the roadway. The contact inspected the vehicle and noticed that the passenger’s side rear tire had detached from the rim. Roadside Assistance was contacted and replaced the wheel with the spare tire. The manufacturer was made aware of the failure but provided no assistance. The contact was advised that without a part of the tire, no assistance would be provided. The failure mileage was approximately 7,000.

The contact stated that while driving at approximately 10 MPH, the vehicle came from an intersection with a stop sign and turned left, but the contact was unable to see oncoming traffic. The contact then noticed another vehicle speeding towards her and she depressed the brake pedal; however, the other vehicle crashed into the front of the contact’s vehicle. Several unknown warning lights were illuminated after the crash. The air bags did not deploy. The contact stated that her head was jarred backwards, and she received medical attention at the hospital. A Police report was filed but the contact had not yet received a physical copy. The vehicle was towed to a tow yard but was not yet deemed totaled by the insurance company. The manufacturer was not made aware of the failure. The failure mileage was approximately 15,300.

I was driving on CA Highway 20 on Thursday afternoon, Oct. 2, when the sunroof spontaneously shattered. I was not driving under a bridge or overpass or tree. There were no vehicles directly in front of me. I heard a loud pop, like an exploding bottle, but could see no damage inside the car. Within a few miles, glass was flying off the roof and striking cars behind me. That's when I pulled off the road and saw the roof was shattered. The shape of the break mounded up in the middle, implying the roof had broken upward. There were no rocks inside on the sunroof cover, nor any other debris than shattered glass.

Recently, I have been noticing that my defroster has not been working properly. Only defrosting a portion of the windshield in the front. I have also noticed that my GPS screen will sometimes go completely black while driving. I am also having to reconnect my phone every time I am in the car after it has already been connected for some time throughout my drive. I am not sure completely what’s going on, but I have noticed that the recall that is currently out for Toyota. My car is having the same issues. There is no warning lights or anything that have come up. I did contact Toyota customer care about the issues.

On May 1, 2025, my 2021 Toyota RAV4 Hybrid displayed an SRS Airbag malfunction warning (Recall 23TA15). The passenger airbag was disabled, making the vehicle unsafe to drive. Toyota had announced this recall in December 2023, but I never received any notification. When I contacted Toyota on May 9, 2025, I was told there were no parts available and was even offered a $200 paid inspection instead of a free recall repair. My vehicle remained at the dealership from May until September 29, 2025, without any available remedy. During this period, I continued paying my auto loan and insurance for a car I could not use. Toyota also failed to disclose my right to a free rental vehicle; I only received temporary rentals (Camry, later Tacoma) after repeated demands, and they did not meet my transportation needs. While the airbag recall was still unresolved, a second safety recall (25TD01 – Rear Traction Motor Cable Corrosion) was announced in mid-September 2025. This meant my vehicle had two active safety recalls at the same time while in Toyota’s custody. Both defects were only repaired at the end of September and early October 2025. Toyota failed to notify me in time, delayed the remedy for nearly two years after the recall announcement, concealed rental rights, and allowed my vehicle to remain unsafe under multiple recalls for months. An independent arbitrator has already issued a repurchase decision, confirming Toyota’s responsibility. This demonstrates a systemic failure by Toyota to comply with recall and safety obligations.”

The hood flutters, and this shimmering is distracting, especially on the highway. The dealership adjusted the rubber stoppers, and this did not fix the problem. On the highway, I have had sunlight flashing in my eyes which poses a safety concern, due to this hood flexing. The problem was noted by the sales rep at the dealership when test driving, and even after a service visit two days later. I bought this car, gold certified, a few days ago and it has persisted since. This seems to be a common problem for which there is a TSB for a Lexus vehicle already.

My 2021 Toyota RAV4 has been under airbag recall for over a year (starting around August 2023), and I have been unable to safely use my vehicle since then. Toyota has no estimated timeline for parts availability or repair, and the recall remains unresolved, they originally estimated October 2024, but that was nearly 1 year ago. Their provided timelines are now unreliable. I was provided a rental car, but it is not comparable to my RAV4 (smaller, less safe, lacks features like AWD and collision protection). Toyota has told me that if I accept the inferior rental, I will not be allowed to drive my own vehicle until the airbag is replaced — an indefinite period. I have contacted the dealer and Toyota Corporate repeatedly, but have been told there is no fix in the foreseeable future. Toyota has effectively deprived me of my own vehicle for more than 12 months, with no compensation, no buyback, and no acceptable substitute. This indefinite delay in addressing a critical safety defect (airbags) puts me and other owners at risk. I am submitting this complaint in hopes that NHTSA can hold Toyota accountable and accelerate a fix or require them to offer alternatives such as a buyback, permanent replacement, or compensation. The prolonged unavailability and loss of use of a vehicle due to a safety defect, especially with no viable end in sight, is unacceptable.
