I was driving our Tesla Model Y and, when attempting to use the windshield wiper, full self driving was enabled. The feature was never enabled my by husband or I through the touch screen on the car. We purposefully did not want to be helping to test this feature as we have a 2 year old and 2 month old that are often in the car with us (both of whom were in the car when this happened). This happened twice on my drive on the afternoon of [XXX]. I did not have a destination plugged into the map on the tesla and was navigating via google maps on my phone (which does seem to be required for this feature to be enabled according to Tesla's help video on youtube: [XXX] ). Both times, a pop up appeared on the tesla screen asking me to give feedback on the auto driving feature while I was driving. Both times, I was able to disable the feature after about 5 seconds by pressing the brake pedal. Upon returning home, my husband was able to go into the car's settings and return the car to "traffic-aware cruise control." He was able to confirm that "Full Self Driving (supervised)" had been selected despite neither of us enabling it. Autopilot began beeping with messages to pay attention while on highway. Moving the wheel did not remove message to the point car was shaking in lane due to turning the wheel. Warning beeps and messages got louder and more distracting and would not go away until brake petal was used at highway speeds to disengage autopilot.

Car was being driven into a car wash. Driver went to put car into neutral and car lurched forward and crashed into car in front of it without gas pedal being engaged. Vehicle has not been inspected by manufacturer or others. No warning lamps or other messages. May have been related to recently installed full self driving software that was automatically installed but was not requested by vehicle owner. Vehicle owner did not intend to engage any sort of self driving or automatic acceleration systems.

The battery coolant started leaking when we connected the car to a supercharger. After charging a low coolant warning came up on the screen, but the message read “OK to drive”. Started driving home when more alerts came up “Power reduced” and “Speed limited to 68 mph” but still showed “OK to drive”. About 30 miles later the screen abruptly shut down on the freeway. Was able to pull over to a stop on the shoulder and call a tow truck. Screen never came back up even after attempting to restart, car is now at the service center. There is a pump that has continued running for over 2 days without stopping. The interior lights turn on and off every 30 seconds. There has been no damage from road debris or anything else to this car. The vehicle only had ~350 miles on it, and we only had it for 3 days before it malfunctioned. The screen displays vital information such as speed, turn indicators, and so on. Without the screen being on it is not possible to safely operate the vehicle.

While using Autopilot or Beta self-driving - when a Minimum Speed Limit sign is detected on [XXX] in Georgia, the system usually reduces to the speed of the vehicle to posted minimum speed (40 miles an hour) instead of maintaining current speed limit. Tesla has failed to provide a software update to address both recalls, and has made it impossible for me to get support to have this rectified. They cancelled a service appointment I made and are no longer responding to my inquiries.

Tesla recently rolled out an update that gives everyone in the US a trial of FSD. After this update I've had numerous situations where the car would panic and act like I was about to collide with a parked vehicle both while driving down a residential street in daytime and when coming around corners. This never happened in previous version. Also the auto windshield wipers no longer work when its raining but will turn on when its dry and cause dirt to smear on the window, mainly when the sun is facing the driver which makes it very dangerous. The last issue is pretty big, the car has been exhibiting severe phantom braking when on the highway using adaptive cruise control (FSD I guess now?)

While driving to work, using FSD, the autopilot steered me into the path of an overtaking vehicle moving at about 20 mph faster than my vehicle. By quickly taking over steering and speed control, I was able to avoid a collision. A couple miles further down the road, the car could not decide on which exit lane to use, and weaved between two possibilities, probably alarming and angering drivers behind me. Again, I had to take over control. Taking over speed and steering control was necessary two more times in my 25 minute trip, most of it on well-marked multi-lane highways. Of the four incidents, only one would have resulted in a serious high speed collision, had I not taken over very quickly. Having coached robotics competitions, and piloted aircraft with autopilot, it is my opinion that the FSD (SUPERVISED) software is at alpha stage in development (not yet up to beta stage, at which it could be safely released only to select skilled drivers, with suitable disclaimers and warnings) . At delivery, no introduction to the car's controls is provided, so customers are on their own regarding finding out, by trial and error and the occasional potentially lethal collision, how to safely drive the vehicle with FSD engaged. The manufacturer should be required to instruct all customers on the very high level of vigilance required to operate with FSD engaged.

A Tesla Model Y has one screen which is all instruments, warning indicators and reverse camera. On March 3rd, 2024 I had just completed about a 40 mile drive. I parked the vehicle for approximately three minutes and upon returning to the vehicle the screen failed to turn on. The vehicle ran and drove perfectly normally, but I had zero instrumentation or warning indicators. I drove home about two miles this way without incident. Upon exiting the car and closing the door (which is the equivalent to turning off the key in a traditional vehicle), the screen turned back on as normal and has since not repeated this issue. I contacted Tesla service about it and even sent them a video showing the vehicle operating with the screen not functioning and I was told to just reboot the car and they brushed it off and would not see the vehicle over this incident.

Vehicle had a software update. After software update the car can see all speed limit signs while driving except for 65 mph sign. Traveling into a town car will register 45 mph, 35 mph, 25 mph signs and update the max speed to reflect the speed limit. However 65 mph signs are completely ignored. Resulting in the max speed being the speed of the last sign passed. Ie leaving a town where speed limit goes from 35 to 45 and then finally to 65 the speed on the car will only register the 45 sign and thus driver is unable to set the cruise or use autopilot at the correct speed. Contact was made with the service department but was told it was there was inconsistency between what the camera is seeing versus what is posted in the navigation maps package. It was submitted for review and will be fixed in the future but until then the only way to use cruise control at 65 is to get off onto a side road where no speed limit sign is posted turn around and get back onto the highway. I have since had 2 more software updates to the car and it is still broken.

- #11582096April 10, 2024LANE DEPARTURE: WARNING, LANE DEPARTURE: ASSIST, FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL
March 31 I received an email from Tesla that I had been selected to participate in a free Full Self Driving (FSD) trial. I have no interest in FSD, and there was no way to opt out of this trial. At some point FSD was downloaded on to my Model Y as part of a regular software update. I was not given the opportunity to accept or refuse this particular feature. While both of these events feel wrong to me, it did not rise to the level of reporting to NHTSA until I realized Tesla had also ENABLED FSD on my car. In other words they switched my configuration from Auto Pilot to Full Self Drive, presumably as part of the software update that downloaded the software on to my vehicle. One could argue that because two features on my screen were different I was able to figure out my configuration had changed, and manually change it back. One could also argue that not everyone would sort this out, and the next time they pulled the stalk down to engage cruise control they would unknowingly engage FSD. And that is not OK. I find the timing of all of this ironic given Tesla just settled out of court on a fatality related to their vastly overhyped FSD.

- #11581597April 7, 2024SUSPENSION
My model Y lower control arm bolt has become loose and disconnected. I noticed a rubbing sound on the front axle in a parking lot and fortunately discovered the source of the issue (disconnected suspension bolt) at home. I believe my vehicle should’ve been included in the recall below as the timeline lines up. My car was produced in the Austin factory and picked up in late November 2022. Can see in the attached photo the loose bolt and the suspension fork hitting the axel. [XXX] [XXX] As of today the car is sitting in the Tesla service center and they are arguing that repair is not covered because of the handwritten pen marks indicate third-party suspension work, but no suspension work has been done. The article citing the manual torque explains the factory markings. This bolt issue has resulted in fatalities at high speed. I’m lucky to have found it at home vs the highway. The contact owns a 2021 Tesla Model Y. The contact stated that while driving at low speeds when making a J-turn beside the contact's steep driveway, the vehicle crashed into the curb then began to roll down the steep hill, and crashed into a tree. The contact stated that the brake pedal was depressed, however, the vehicle continued to roll. No air bags were deployed and no injuries were sustained. The vehicle was taken to the repair shop where the headlight assembly, the hood, and the full front assembly of the vehicle were replaced. The manufacturer was made aware of the issue. The failure mileage was approximately 400.

- #11581268April 5, 2024VEHICLE SPEED CONTROL, ENGINE, FORWARD COLLISION AVOIDANCE: WARNINGS
Vehicle: Tesla Model Y Blue Purchased in Oct’ 23 Tesla setting: Regenerative braking (single pedal driving) Issue: Brake Pedal/Acceleration Pedal Malfunction I was pulling my car out from the parking lot to pick up my mom, from the hospital entrance. I just turned right and had to take an immediate left to leave out of the parking lot. The Tesla was driving at approximately 5 miles an hour at that point. I lifted my foot from the gas pedal for the car to stop with its regenerative braking. However, instead of coming to a stop, the Tesla started rapidly accelerating, with my foot not even on the pedal. When I slammed the brakes, the car was unresponsive and continued accelerating. I started honking madly to warn people that I had no control over the car I was driving. The vehicle continued accelerating over 100-150 meters in 4-5 seconds and banged a parked car (Lexus SUV) at the end of the parking lot. It continued accelerating while pushing Lexus over the curb, destroying road sign poles and landscaping shrubs; crossing the street, and the middle curb. At the end of the street was a tree and a ditch. Lexus couldn’t move any further and Tesla stopped. All airbags deployed. One of the emergency response detection systems called 911 immediately. The first thing the police asked me was if I pressed the emergency brake. I said the car doesn’t have one. I can’t even turn off the vehicle while it is moving. The parking button takes care of it. I used to love my Tesla but I can’t trust it anymore. What I want from them at this time is: Investigate all the incidents thoroughly instead of blaming these as a user error. a) Think about adding the redundancy system. Something mechanical where the driver reacts within a couple of seconds. Software bugs are hard and sometimes it is impossible to find the root cause. Logs may not contain everything that you need to debug the problem. Consider it as a P0 issue. b) Compensate for the damage and property loss.
