The center high-mounted stop lamp (CHMSL / third brake light) fails on 2021 and newer Tesla Model S vehicles (including Plaid). This is a common issue reported by many owners, particularly as vehicles exit the warranty period. No redesigned or corrected replacement part has been made available by Tesla. Pre-2021 CHMSL units are incompatible and will not fit. Repair requires removal and replacement of the entire rear liftgate glass assembly, which is extremely expensive and labor-intensive for owners. Prior model years did not experience this problem. A non-functional CHMSL reduces the vehicle's visibility to following traffic and increases the risk of rear-end collisions, especially at night or in poor weather. My car had a software update last night. This morning, there is now a forced "feedback" prompt that comes on the information screen whenever I disengage the autopilot. This prompt grays out the rest of the screen and does not disappear until I manually select a feedback option. This forced prompt is very distracting, requiring manual user input to get rid of, which makes driving distracting and dangerous.

2024 Tesla Model S. Repeated rear tire and rear wheel failures with abnormal wear and possible rear alignment or suspension-related defect. Tesla service history shows repeated rear tire replacements beginning at about 5,165 miles, a later four-wheel alignment, a right rear tire replacement for uneven tire wear under the basic vehicle warranty at about 21,594 miles, a documented bent left rear wheel at about 27,047 miles, a left rear wheel replacement due to crack after a low-pressure/PULL OVER SAFELY alert at about 30,489 miles, and then a right rear tire blowout at about 38,358 miles. In the same March 12, 2026 service record, Tesla also noted inner sidewall damage on the opposite rear tire. Concern is an unresolved recurring rear tire, wheel, alignment, or suspension defect creating blowout risk and unsafe operation. Tesla has replaced tires and wheels but has not identified or corrected the root cause.

The vehicle developed a leak in the AC condensate drain. The leak is directly above the safety restraint control module. The module got wet deactivating the vehicle and causing havoc with the airbag system. Many electrical systems turned off such as the power to the steering wheel buttons( the left blinker, the right blinker, and the horn). Luckily this happened while my vehicle was parked but if this happened while someone was accelerating or driving on the highway they would lose control of the vehicle. I spoke to a Tesla Service Tech about the concern and he informed me that this AC leak happens very often in the 2021 and newer Tesla Model S vehicles and that Tesla is aware that the AC drain leaks onto the Restraint Contol Module but has not created a technical service bulletin or a recall for the safety issue.

The power train of my 2025 Model S failed similar to failure of 7-6-25 which I previously reported to NHTSA (complaint #11718413) and Tesla. Upon getting into my car on 2-7-26 and putting it in reverse, several alert messages displayed including "stability control disabled, traction control disabled, and emergency braking unavailable". I drove 1/4 mile to a safe area to call Tesla roadside assist, since my steering was noticeably degraded in real time and the car was not safe to drive. The car was towed to the nearest Tesla service center. Tesla made statements on their service report attached here, that this is a "known characteristic of the vehicle" and that if it happens again I should, "exit the car, let the car go to sleep and then once the vehicle is awakened the alert will not be present and it will operate normally". That is not a valid solution, and even though they told me they found nothing wrong when they looked at the car two days later on 2-9-26, I had walked away from the car for more than an hour on 2-7-26 when this happened and the alerts were still there. The tow truck driver who arrived three hours later also saw them and noticed the degraded steering when he drove it. Tesla's solution stated on their service report is not valid or safe. This issue has happened twice in seven months on this car of mine and it needs serious engineering attention.

The power train of my 2025 Tesla Model S failed. As a result, the car had several systems alert on my console that they were "unavailable" or "degraded", and in addition to the alerts I experienced their loss or degradation in real time. This included braking, steering, stability control, and traction control. Normally you don't have to hold the brake pedal down at a stop light on this electric vehicle, but when the failure occurred my car was rolling backwards at the stop light and so I had to hold down the brake. Also, steering was noticeably degraded and not stable. The car was also exhibiting behavior similar to a stuck accelerator whereby even with my foot off the accelerator the car would not slow up. Tesla had to tow my car to their nearest Tesla service center, and they replaced the front body controller and the battery controller which took three days. There were no warnings whatsoever or anything relating to the power train that were peculiar prior to this incident. It occurred all of a sudden after getting into my car and putting it in reverse to back out of a parking spot. I was then bombarded with several alert messages as mentioned above and drove only about half a mile until I pulled into a parking lot and called Tesla for roadside assistance as the car was obviously not safe to drive.

The vehicle is over reporting it's speed by 5% this is a large discrepancy which effects safety by causing people drivers around you to change lanes because you are traveling under the speed limit. Tesla is unable to calibrate the speedometers are the service center. They have documented the failure. It also adds miles to the odometer prematurely. This problem started after Tesla upgraded the car with a Track Package which is a $24,000 upgrade

I have started receiving airbag & seat safety issue alert but recieved appointment after a month even considering major safety concern. Some of 2021 vehicles are serviced under the recall Tesla sent. But told me mine doesn’t qualify for same issue for same year, same model cars. It seems like issue is with all Tesla Model S 2021 but Tesla charging big dollar amount to repair the safety concern. They have also mentioned there is on going concern with air suspension and all Tesla starts giving problem between 60,000 miles to 90,000 miles. It costs around $6000 to fix that issue and told me these are not safety concerns.

Complaint Category: Tires / Suspension Summary of Issue: I am reporting a recurring and deceptive safety defect involving extreme inner-edge tire wear on my Tesla Model S. On three separate occasions, my rear tires have worn down to the metal structural cords exclusively on the innermost shoulder, while the remaining 90% of the tread appeared in excellent, near-new condition. Safety Hazard: This defect creates a high risk of catastrophic blowout at highway speeds. Because the wear is hidden on the far interior edge of the tire, it is invisible to the driver during standard daily visual inspections. The vehicle’s non-adjustable rear suspension geometry (aggressive negative camber) places extreme stress on the inner shoulder, causing it to fail structurally while the rest of the tire looks safe. Tesla’s Response: Tesla Service has been unable to provide a permanent remedy, stating the vehicle is "within spec," despite the recurring nature of the failure. I am filing this to urge the NHTSA to investigate whether this suspension geometry constitutes a design defect that requires a mandatory recall or corrective hardware.

The horn is a little button on the steering wheel. Tesla has since switched to a center horn. I assume this is due to safety concern that accessing the horn during a panicked situation is difficult. I have almost been in a few accidents as it is impossible to find the horn while evading a potential accident. Tesla will not modify my vehicle as a safety related issue and instead wants to charge almost 2,000. This should be a recall. Tesla switched back to a center horn, a clear admission that there is a design flaw. How they can't modify a vehicle under a recall is ludicrous.

Incident Description: While driving through a parking lot at 20 mi/hr, my vehicle's AEB system activated in response to another vehicle approaching from a perpendicular direction. However, the system's response was dangerously delayed and inappropriate stopping the car right in front of the other car: The system failed to provide any advance warning or alert before applying the brakes The braking occurred too late to create adequate separation from the approaching vehicle The vehicle stopped directly in the path of the oncoming car, increasing rather than decreasing collision risk The abrupt stop left my vehicle in a position where I was at greater risk of being T-boned Only the other driver's quick reaction prevented a collision. Had that driver not stopped in time, the AEB system would have contributed to, rather than prevented, a serious accident.

While operating my 2023 Tesla Model S Long range using Full Self Driving Supervised and software version 14.2.1.25, the speed control no longer is available and has been replaced by speed profiles. When operating the vehicle on Interstate 77 Southbound around Statesville, NC and on or about December 22, 2025, I set the speed profiles to standard in a 55 mph zone in which the Tesla recognized and displayed that it was operating in a 55 mph zone in Standard profile mode, the car varied in speed (with no traffic ahead) from about 57 mph to over 76 mph. This is highly unsafe and I had to repeatedly move back to Chill or Sloth mode at which time the vehicle strongly and unsafely braked causing cars behind me to close on me quickly and have to make strong and unplanned braking maneuvers to avoid hitting me. This was not the case when I bought the car and FSD when operating under 13.2.9. The removal of max speed control is extremely unpredictable and unsafe for me and those around me.

7/30/2025 The vehicle experienced a confirmed TOTAL loss of function to include steering, restraint, and braking functionality at approximately 60 to 65 miles per hour. This catastrophic failure placed myself and and others at severe risk. Although Tesla’s service notes confirm the event occurred, no corrective repair was performed. The vehicle was completely inoperative with a matter of seconds with no prior warning. At this point in time the car has continued to not operate the same steering and noises has been in the shop 5 times still no answer or repairs in regard to it shutting down or any other issues that continue to return. The last visit I was asked to purchase new tires because maybe that’s why, I did. I continue to be told nothing is wrong with the vehicle making me feel extremely defeated when I know the vehicle is not the same as it was.

Vehicle: 2021 Tesla Model S Long Range VIN: [XXX] I am reporting multiple safety-related defects that existed at delivery or developed shortly thereafter, and Tesla has refused to diagnose or repair them under warranty. These defects create an unsafe driving condition and risk of loss of control or collision. Defect Summary: 1.Alignment Defect at Delivery: On the day I bought the car, the steering wheel pulled to the left and felt unstable. I reported this immediately to Tesla. They dismissed it as “normal.” 2.Premature Tire Wear: Within months, the left tire wore down to the steel belts on the left side while the right side had normal wear. An independent shop confirmed this was caused by suspension/bushing issues, not road damage. 3.Suspension/Bushing Failure: A third-party Tesla-certified shop confirmed that the suspension components are bent/tweaked and require replacement for safe driving. Tesla refused to inspect these components properly. 4.Unlatched Rear Seat at Delivery: The rear seat was delivered unlatched, which is a major safety hazard during braking or collision. 5.Broken Glass Under Rear Seat: Broken glass was found under the rear seat cushion at delivery, raising concerns about vehicle history and delivery inspection standards. 6.New Defect While in Tesla’s Custody: On Nov 17, 2025, moisture accumulated inside the left taillight while the vehicle was parked outdoors at Tesla’s Stockton facility. This can cause electrical shorting or lighting malfunction. Tesla’s Response: Tesla has repeatedly refused to inspect the suspension properly, dismissed my complaints, and produced an inaccurate service invoice that did not reflect what was discussed. They told me we are “at an impasse.” These defects pose a serious safety risk (tire blowout, loss of control, lighting malfunction). I request NHTSA investigate these issues as they may indicate broader problems with Tesla’s suspension, alignment, and delivery inspection processes. Vehicle: 2022 Tesla Model S (HW 3.0) System in Question: Full Self-Driving (FSD) Capability, initially purchased (Level 1 equivalent) for $6,000, and then upgraded (Level 2 equivalent) for an additional $3,000 (Total $9,000). I. Initial FSD Purchase and Unsafe Performance (Level 1) Upon initial purchase of the FSD package, the system (what Tesla later called FSD Beta, or its precursor) consistently demonstrated unsafe and aggressive behavior that necessitated immediate driver intervention to prevent a crash. Specific Safety Concern: The system would often execute turns, both in city and highway off-ramp scenarios, with excessive speed and aggressive lateral acceleration. This erratic behavior felt uncontrolled and risky, frequently causing the driver to feel unsafe and to rapidly override the system by taking manual control of the steering wheel. Resulting Risk: The system was virtually unusable for anything beyond basic highway cruising (a feature standard on many vehicles), yet its aggressive behavior posed a safety risk on the highway when attempting lane changes or taking exits. II. Upgrade and Deterioration of Practical Safety (Level 2) I upgraded to the top-tier FSD package with the hope of receiving a safer, more functional system that matched the "Full Self-Driving" name. Instead, the system introduced new safety and usability defects: Forced Disengagement/Driver Monitoring Defect: The system's driver monitoring is overly aggressive and interferes with safe, necessary driving actions. It delivers constant "nag" warnings and requires disengagement when the driver performs a brief but necessary safety check, such as looking at a side mirror, checking over the shoulder for a lane change, or briefly glancing at the car's screen to adjust climate/radio. Safety Implication: This defect forces the driver to prematurely disengage FSD in complex traffic situations to avoid a warning/lockout, which is counter-productive to safety.
