My Tesla Model S has a touch-sensitive horn button on the right side of the steering wheel. In urgent situations, I've often had difficulty quickly locating it to get another driver's attention. Tesla quoted me $1800 to replace the steering wheel with a model that has the horn in the center, like most other cars. This is a safety concern because owners shouldn't have to pay to move the horn for safety reasons. Tesla should be required to retrofit any model with a touch button for the horn to one with the horn in the center of the steering wheel.

The contact stated that a Police report was filed; however, the report number was not provided. There were no injuries sustained. The vehicle was towed to an independent mechanic, where it remained undiagnosed and unrepaired. The manufacturer was notified of the failure but provided no assistance. The failure mileage was 150,332.

The driver's seat assembly on the vehicle suffers from a critical mechanical and software tracking defect that causes the seat to permanently jam into a mechanical lock, creating an immediate and severe safety hazard. The electronic calibration soft-stops for the driver's seat allow the mechanism to travel past the safe limits of the track when indexed or operated. When the seat reaches the absolute mechanical limit of the rear rail, the lead screw/worm gear mechanism binds tightly, resulting in a total mechanical lock where the motor lacks the breakaway torque to reverse direction. This causes the driver's seat to become permanently stuck in the fully rearward position. This defect directly compromises vehicle safety by potentially preventing a driver from safely reaching and operating the brake and accelerator pedals. The vehicle has been taken to the manufacturer's service center on three separate occasions for this exact issue. During these three visits, technicians physically replaced the seat motor, checked track alignment, and attempted recalibration. Despite these repeated hardware interventions, the defect persists, and the seat continues to travel past safe limits and bind into a mechanical lock. The manufacturer has now refused to correct the underlying track/assembly defect under warranty, incorrectly dismissing the recurring, hazardous mechanical failure as "customer education." The vehicle lacks a safe physical fail-safe to prevent the seat from self-destructing into a permanently jammed state.

This vehicle has a recurring suspension defect causing a chassis or suspension component to make physical contact with the inner shoulder of the rear tires during normal operation, producing a sharp linear cut progressing to full cord exposure and fraying of internal steel and aramid belts. This has occurred on at least three sets of rear tires between July 2025 and June 2026. Tesla's own service records document the damage pattern on multiple occasions. No root cause has ever been identified or corrected by Tesla's service department. A tire with exposed and fraying cords on a vehicle producing over 1,000 horsepower presents a serious risk of catastrophic blowout at speed.

The vehicle was being driven northbound in the regular (non-HOV) lanes of Route I-95 under the control of Version 12 of FSD software. I believe that the vehicle would have crashed into the lowered gates preventing vehicles from entering the HOV lanes, which operate in only one direction at a time. Beyond the gates, the HOV lanes contained high-speed oncoming traffic. The problems occurred south of the Dale City exit from northbound I-95. The car’s navigation system gave an audio instruction to keep left to bear onto northbound I-95 and the car did exactly that. Twice. I believe that both crashing into the lowered gates or crashing head-on into oncoming vehicles would likely have resulted in multiple deaths, including my own. I saw the developing problem and took control of the vehicle before it could bear left onto the closed ramp to the HOV lanes.

This car does not have reliable horn system. I have the steering wheel version (not the “yoke” version) but I believe it’s a similar problem on both. The steering wheel center (airbag) does NOT operate as a horn. Instead there is only a extremely small horn sensor on one side of the wheel that cannot be reliably accessed in an emergency situation b/c of its tiny size and b/c its position depends on where the steering wheel has been turned. This is EXTREMELY DANGEROUS. On a recent drive a car pulled into our lane from a side street and I was unable to locate the horn in a timely manner. Fortunately the car braked and I was able to pull around but it was very close and could have resulted in a head on crash. I contacted Tesla about this issue. The service rep acknowledged the problem and said it was an “issue” that had gotten numerous complaints but that all he could offer was a Tesla aftermarket replacement that would cost nearly $1,000 to install. I presume NTSB has gotten many complaints about this incredibly dangerous and egregious latent safety defect in the car’s design. Tesla (at least for the 2024 and on Model 3 - which I also own) has corrected this design defect such that the center area does operate as a horn (as I am sure 99% of cars do). The NTSB should investigate and require Tesla to retrofit any of its cars which have this safety defect. I have no doubt that people will (or likely have been) seriously injured (or even killed) in car crashes that could have been avoided had Tesla retrofitted its vehicles built with this dangerous safety design defect. Please, please open an investigation and require Tesla to correct this inherent safety design defect. Tesla already has the fix (and have implicitly recognized the defect by correcting it on its current models). NTSB’s failure to require Tesla to act will result in many avoidable accidents/injuries. Owners should not have to pay nearly $1k to correct Tesla’s latent/non-obvious safety defect.

Supervised FSD does not have a remedy if it assigns an incorrect speed limit to the current roadway. The only action available is to disengage FSD. This incorrect speed limit happens quite often, especially in construction zones with temporary speed limit changes. Tesla used to have a ‘max speed’ setting that the driver could use to override an incorrect speed limit, but they removed that capability.

I feel Tesla has introduced two safety issues with a recent version of software relating to the "Full Self Driving (Supervised)" feature. Issue #1. When disengaging the Full Self Driving ("FSD") software, Tesla positions a dialog box over the main center display screen. The box takes up a fairly large portion of space. The purpose of the box is to provide feedback on the reason for disengaging FSD. This dialog box does not dismiss by itself. The only way for the user to dismiss it is to tap one of the 4 options presented or leave a voice message. This is an unnecessary distraction that in no way is required for the user to operate the vehicle. Tesla is mandating feedback and introducing distractions when users take over control of the vehicle for their sole benefit. It is also akin to a penalty for disabling FSD, as it a generally negative experience for most people. Issue #2. Tesla has introduced a mandatory "FSD Streak" counter. This number appears on the smaller instrumentation screen for the S & X models. The counter counts the number of miles consecutively traveled by FSD. When a driver disables FSD for any reason, the counter resets to 0. The gamification of keeping FSD enabled is a major safety issue. Some people are highly susceptible to increasing this number as high as possible as if it were a video game. In situations where they should likely disable FSD and make manual control, there are inevitably users that will hesitate or maybe even not take control at all because they do not want to reset the counter. Tesla does not have any options to disable or remove this counter, so even people who know they are susceptible to this cannot remove it. This is akin to placing an open beer in the car and saying people will not drink it because they have self control... only - as we know, addiction is a disease, and games are just as addicting as many substances.

2022 Tesla Model S Plaid purchased CPO from Tesla Florida Inc. Jacksonville on 8/29/2025 for $75,533.19. Multiple unresolved safety defects. (1) Trunk closure obstacle-detection failure 10/15/2025: key fob fell in gap between liftgate and roof; powered liftgate continued upward opening cycle and compressed fob against fixed roof glass until rear glass shattered. $4,100 repair. Same cycle on a child hand or finger would cause crush injury or amputation. Tesla Model X tailgate and falcon doors have obstacle detection; Plaid liftgate lacks it. (2) Passenger-side rear door handle requires extreme pressure to operate, raised 10/25/2025; iced over 1/19/2026 compounding egress hazard. (3) Center console haptic backup gear selector non-functional plus observed main touchscreen reset/blackout events: no manual gear control during foreseeable touchscreen failure event. (4) HV battery umbrella valves and flood port plunger required replacement 10/25/2025. (5) Rear spoiler bilaterally detached as of 5/13/2026 on 200mph-capable vehicle. (6) Front trunk latch unreliable; requires 2-3 commands to open; risk of hood opening at speed. (7) USB charging circuit melted a portable battery bank 11/2/2025; fire-risk overcurrent indicator. (8) Tire sidewall damage Tesla circled "OK" at CPO delivery; refused replacement 5 times; Tesla SMS 1/13/2026: "sidewall damage would need to be replaced." (9) Multiple unauthorized digital-access events: Track Mode created post-service; Charge Current set to 5A; Dog Mode disabled; pre-delivery key card 8/28/2025; phone key 3/14/2026; both front door reflectors found loosened 11/8/2025. I was driving down the local road and got ready to go under a overpass and next thing u know my EV just slammed on brakes like I was about to hit someone! Seatbelt choked me and made me gag! But when I came through this time It caught me off but after that I realized there's a problem,especially going under that overpass! This couldn't been very dangerous to anyone! This is a VERY DANGEROUS and IMPORTANT SAFETY CONCERN! I STRIVE SAFETY AND THIS COULD BE FATAL!! I NEED TO REACH SONEONE IN SAFETY! ASAP

My 2022 Tesla Model S is equipped with 21" Arachnid cast alloy wheels. The wheel on the front driver side developed a structural crack under normal driving conditions with no road hazard impact or unusual driving events. The crack has caused a significant air leak, with tire pressure dropping from 44 PSI to 34 PSI over approximately 10 miles of normal driving. At 34 PSI on a 265/35ZR21 low-profile tire, there is meaningful risk of rim-to-road contact. The vehicle shakes noticeably during normal driving as a direct result of the pressure loss. The tires have approximately 13,000 miles on them. We are aware of multiple other Tesla Model S Arachnid wheel owners reporting identical cracking under normal use on [XXX] , suggesting a pattern consistent with a manufacturing defect in the cast alloy wheel design. This is a safety concern as the leak rate is significant and the condition could deteriorate without warning. Tesla just sold me a car with defective front half shafts axles. The car makes a reproducible and very concerning accelerator pedal shudder that makes the entire vehicle front vibrate, including the steering wheel and brake pedals while the car is accelerating between 30-60mph. This interferes with safety features like, lane departure assistance and blind spot steering wheel feedback, Confusing the driver and creating a distracting scenario where an accident can happen. Upon bringing this to the attention of the service center in Fort Myers Florida, the service center manager took a drive test with me, and was able to replicate the issue, but immediately started dismissing it as "normal" and within Tesla specs. This is becoming a regular practice at that service center, where they deem warrantable issues as normal and within (their tolerance). I insisted that this issue is not to be taken likely by Tesla, because it is something that involves a safety hazard. Upon further research, I was able to find that this issue is happening to many Teslas model S and X, where the front axles bend due to the weight and high torque of the vehicles. The issue falls completely under warranty and the half shafts should be replaced, but apparently Tesla has instructed their service centers to dismiss and disregard the symptoms, and just tell the customers that no warranty replacement will occur, because it is not necessary, we just have to drive the cars like this, until the car completely fails, or an accident happens because one of the bent axle shafts breaks on a highway during normal drive. I recording this report as evidence that the issue has been reported to the highest vehicle safety authorities, and I expect a corrective action so Tesla can fix my car under the existing unexpired warranty.

I was in full self driving mode and was travelling North bound 110 freeway transitioning to 405 freeway Northbound. In the right lane. Transition curved left. looked at my speed for a split second and as i looked up we were a split second from hitting freeway barrier head-on. We crashed through the barrier and ended on the embankment of the freeway. My car front end was totally demolished, front airbags went off. I was ok, but my wife suffered a L1 vertebrae fracture and was taken to Harbor UCLA Hospital for treatment. She was released 9 hours later with a back brace and pain meds. The accident occured at 8:34 am Sunday April 5th, Easter sunday while on our way to church. Its been over a month and my wife is still in a back brace, but healing slowly. USB Drive taken out of the glove box showed that all cameras cut off about 10 seconds before the crash. Not sure if the whole system turned off.

I have been consistently at risk since I picked up my car in late December 2023. The Horn icon which is impossible for older people to see, especially at night, is located in a place that is very hard to find on the steering wheel and unintuitive. Just yesterday, a large truck was coming around the corner near my house in my lane in the opposite direction and I could not find the horn to honk. It was another near miss. Interestingly, Tesla changed the design and put the horn in the middle of the steering wheel where it belongs starting in January 2024 just a few days after I picked up my car. I need the Horn in an intuitive place in the middle of the steering wheel for my safety and the safety of the other people on the road I believe Tesla should pay to change out the steering wheel
