While driving my 2024 Ford F-150 Lariat PowerBoost at highway speed, the panoramic sunroof glass suddenly shattered without warning. The truck had been parked inside my garage immediately beforehand. Approximately three minutes after leaving the garage, I entered the highway, and about two minutes later there was an extremely loud bang. There was no indication that any object or road debris struck the vehicle before the glass shattered. The interior sunshade was closed at the time, which prevented broken glass and debris from falling into the passenger compartment. After hearing the bang, I noticed significant wind noise and discovered that the panoramic roof glass had shattered. There was no known impact or other event that would explain the sudden failure. I am reporting this because the unexpected shattering of panoramic roof glass while a vehicle is traveling at highway speed presents a potential safety hazard to occupants.

Received vehicle back from dealer saying prior brake problems had been repaired and confirmed with Ford Engineers. Drove approximately 3 miles before brakes failed again and I was not able to stop at the intersection. Avoided an accident, but had vehicle towed again to be repaired. This is the third complete brake failure in less than 300 miles on the vehicle.

My truck has just under 130k miles. Bought it at 18k miles. At 47k miles I had to have the cam phasers replaced and timing chain fixed. At 76k miles the trans (10r80) blew and had it replaced. At 105k miles I had to have the cam phasers replaced again. Multiple times I was denied through ford warranty. And now ford is turning there back after I had it inspected at Koch 33 ford for a reimburment in just the trans! Please help! When the transmission blew I was doing 65mph and it just dropped to first gear and i had a rear wheel lock up and skid all over. After i stopped i had no foward or backwards.

Despite several transmission services there remains a clunky shift between gears 7 and 8. The problem continues. Other owners of this model have told me that this problem eventually broke their vehicle, and they were left stranded beside the road. Stranded beside the road is a safety risk.

Execution of manufacturer safety recall service resulted in immediate, catastrophic transmission malfunction and sudden loss of motive power upon leaving the franchised dealership, creating an imminent road hazard. Details of the Incident: Recall Intake: On XXX, the vehicle was driven into the authorized Ford dealership Cloninger Ford of Morganton in [XXX] under its own power for an open manufacturer safety recall involving the transmission molded leadframe and PCM software update to prevent sudden, unintended downshifts. Immediate Failure Post-Service: After technicians completed the recall service and returned the keys, the vehicle was driven out of the dealership lot. Upon reaching normal road speed and shifting from 3rd to 4th gear (less than 0.2 miles from the service bay), the transmission violently malfunctioned, lost forward drive engagement, and rendered the truck completely disabled and undrivable in active traffic. No road test was performed by the dealership prior to releasing the vehicle. Safety Risk: The sudden failure and inability to shift gears properly left the vehicle stranded in roadway traffic, presenting an immediate collision risk. The vehicle had to be limped back into the dealership service drive immediately. Remedy Failure & Denial: The dealership acknowledged that the recall-mandated PCM software calibration update triggered the severe transmission failure. However, both the manufacturer and the dealer are now claiming the failure is a "pre-existing condition," refusing to complete the safety remedy without charging the owner $7,000 for a full transmission replacement. The safety defect that prompted the recall was not remedied and was instead exacerbated to total mechanical failure during execution of the recall procedure. Vehicle demonstrates an intermittent lag during acceleration, particularly during throttle position changes. Typical throttle response is appx 0.5s as measured by stopwatch. When the issue is replicated the delay can exceed 1 second. This is most concerning during situations where moderate to brisk acceleration is needed at a specific time such as turning across traffic at an intersection or merging with traffic flow with a large speed disparity from one lane to the other. Vehicle has been taken to multiple Ford dealerships multiple times. The ECU and TCU have been reprogrammed and the situation does not change. Dealership has noted an issue one time but otherwise claims that they have not been able to show the issue to be out of allowable parameters by Ford. Numerous forum posts online discuss the same symptoms and comment similar findings by Ford dealership techs. What they can readily replicate is understood but considered "normal". This behavior has created 2 different situations that nearly culminated in severe accidents with myself driving the vehicle, one time with my child in the seat exposed to oncoming traffic when the accelerator lag caused a severe situation. The first severe situation was crossing traffic outside of my child's school with oncoming traffic having to slam on brakes in order to avoid the collision. The second situation happened while merging lanes on a highway going from ~45mph up to ~60mph and almost caused the neighboring lane to rear end the vehicle. The dealerships have noted the delay exists but to be considered "within allowable limits" when it happens. No warning lamps come on as a regular pattern. This pattern first appeared when the vehicle was less than 500 miles on the odometer. The first dealership ignored the information.

10 speed cvt transmission. Truck was struggling with 1-3 shift, reving, hesitating, and then jumping. Hard shifts, and clunking. Took it to transmission shop got valve body replacement... less than a week later transmission could not hold 10th at highway speed kept down shifting to 9 and then going back to 10 again and again. After 15 min the display showed it in NO GEAR then 1st GEAR at highway speed and then through a wrench warning light and went into limp mode. Back to transmission shop... cdf drum failure, bad bushings and an over haul to get it back up and running again. 8,000.00. First repair 7-24 second repair 8-22.

**2025 Ford F-150 – Repeated Wiring Harness Damage** I am reporting two separate incidents involving a 2025 Ford F-150 that resulted in significant damage to vehicle wiring harnesses and required replacement of major wiring components. On March 4, 2026, the vehicle sustained wiring damage during a dog-related incident. The vehicle was inspected by a Ford dealership, which determined that a major rear wiring harness required replacement. The repair cost approximately $2,054. The affected wiring was associated with the rear-axle/rear electrical system. In August 2026, a second, separate dog-related incident again resulted in extensive wiring damage. Ford ordered multiple wiring components to repair the vehicle, including RL3Z-13A409-F, RL3Z-14A411-B, TL3Z-13A576-AFA, and TL3Z-15K857-A. These components include major wiring assemblies and wiring associated with the ABS/wheel-speed sensing system. My concern is not simply the damage caused by the incidents, but the apparent vulnerability and accessibility of critical wiring on the vehicle. Two separate incidents within approximately five months resulted in substantial wiring damage and replacement of multiple harnesses and electrical components. Damage to wiring associated with braking, ABS/wheel-speed sensing, lighting, or other vehicle systems could potentially cause loss or malfunction of safety-related functions. I am requesting that NHTSA document and review these incidents to determine whether the routing, shielding, protection, or accessibility of wiring harnesses on the 2025 Ford F-150 presents a potential safety defect or broader safety-related trend.

I have a 2013 Ford 150 King Ranch 4X4 with the 6R80 transmission and approximately 125k miles on it. This morning, I was stopped at a red light and upon accelerating my truck would not shift out of first gear. The speedometer stopped working, and engine light came on. I pulled over and turned off the truck to inspect it. I restarted the truck and it was shifting appropriately for about 5 miles when it all of a sudden shifted into first gear while I was driving approximately 35-40mph, nearly causing an accident until I could safely pull over. I researched symptoms and found information regarding the molded lead frame on the 2013 model f150s. I understand Ford warranty coverage was limited to 10 years, but I believe Ford should continue to honor this known defect in there vehicle. This is a serious safety concern. If it would have been raining, or icy weather, it could have caused a serious accident and/or death to myself or others. Ford is aware of these issues, and yet owners are still responsible for their defective parts after warranty expires. I believe the manufacturer has an obligation to address this known problem. I am requesting Ford review my vehicle and this failure as a safety-related defect and provide assistance with the repair, as well as covering the cost of repair in full. At minimum, I am requesting Ford provide reimbursement based on the known history of the component and the potential safety consequences of its failure.

- #11760444August 28, 2026SERVICE BRAKES, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, FORWARD COLLISION AVOIDANCE: WARNINGS
I purchased a brand-new 2026 Ford F-150 Raptor from Ken Grody Ford on August 25, 2026. On August 26, 2026—one day after the purchase—I was traveling in traffic at approximately 45 mph when the truck suddenly lost power-assisted braking and most of its braking effectiveness. The brake pedal required extraordinary pressure, and even with that pressure the truck was slowing only marginally. I activated my hazard lights and had to cross multiple lanes of traffic while attempting to bring the vehicle to a safe stop. This created an immediate and serious risk of a collision with surrounding motorists. Multiple warning lights and messages appeared, including a red “CHECK BRAKE SYSTEM” warning. After I had safely pulled over, I recorded a video showing the warnings displayed by the vehicle. The truck had only 255 miles on it at the time. No collision or injury occurred because I maintained control of the truck, activated the hazard lights, applied extraordinary pressure to the brake pedal and took immediate evasive action to reach a safe location. I immediately reported the incident to Ken Grody Ford. Because this serious braking failure occurred one day after I purchased a brand-new vehicle, the dealership subsequently agreed to unwind the transaction. As of August 28, 2026, I have not received a definitive engineering explanation identifying the cause of the failure. I am reporting this incident because the sudden loss of power-assisted braking and severe reduction in braking effectiveness in a virtually new 2026 Ford F-150 Raptor created a substantial risk of injury or death. I request that NHTSA document and investigate this incident and determine whether similar braking-system failures have occurred in other Ford vehicles.

This lead frame issue is never ending. Well over a decade without a proper remedy. I’m very familiar with it. but this one nearly ended my life. After the near collision I called my mechanic and bought a new ford lead frame from a dealership. It’s doing fine now, it’s been 1 year. the incident caused my truck to go into park or possibly reverse at highway speed just as I passed a semi truck and returned to the right lane. If the trucker had not been paying attention it would have been bad. There was nothing I could do. . On top of this, he was under the impression that it was intentional. So I almost got into a physical altercation until I convinced him to look inside my truck as my gear indicator light was dancing from gear to gear while not touching anything.. To add to the seriousness, my taillamps never came on because I didn’t touch the brake. It was a horrible timing.. . Shortly after this i received a notice from Ford explaining the cause and symptoms of this issue, but then stated there was no fix at the time, but the vehicle was “safe to drive.” That is irresponsible. This was the only time mine did it, no prior issues. I got new ford parts and had it replaced. my mechanic said they deal with this weekly! Then I get a letter from Ford saying there is a fix and it’s a computer programming issue which is totally not true. That’s just a smoke screen, but it also goes on to say it could be a lead frame and if you’ve already fixed it before you got a letter, they will refund your money, but I highly doubt that. But they should stop claiming these vehicles are safe to drive because they absolutely are not if they’re having an issue regarding recall 26S28. So not only did i use new Ford parts, I paid a reputable EX Ford Technician Who owns his own business to perform the services because ford didn’t offer a fix. only to waste my time reprogramming the ecm. I work, I don’t have time. It’s irresponsible to say it’s not a safety concern. It’s just bad policy

The contact owns a 2016 Ford F-150. The contact stated that the vehicle was taken to a dealer to be serviced under NHTSA Campaign Number: 26V237000 (Power Train). The vehicle was not experiencing any failure prior to the recall repair; however, after the recall repair was completed, the transmission started downshifting roughly and would not shift beyond second or third gear, creating a severe road hazard while driving on the highway. The vehicle was driven to the contact's residence. The dealer was notified of the failure but informed the contact that the issue could not be resolved. The vehicle was not repaired. The manufacturer was notified of the failure and advised the contact to reach out to the dealer; however, the dealer was unwilling to assist. The failure mileage was 200,000.

On August 21, 2026, my 2016 Ford F-150 was serviced at Five Star Ford to perform safety recall 26S28 (Powertrain Control Module software update). Immediately following this software flash, the transmission's shifting logic became severely corrupted and unsafe to drive. The vehicle currently acts as if it has completely lost 2nd gear, experiences a severe lag when shifting up into 6th gear, and repeatedly fails to lock itself into 3rd, 4th, or 5th gears. The dealership is currently using a temporary lack of hard Diagnostic Trouble Codes (DTCs) to deny coverage for an internal physical lead frame component replacement under the 26S28 extended warranty guidelines, claiming instead that a complete, out-of-pocket transmission replacement assembly is required. The safety update has directly induced a severe driving hazard, leaving the vehicle unable to properly find or maintain standard highway gears.

I am reporting a safety-related transmission problem I experienced with my 2020 Ford F-150 equipped with the 10R80 automatic transmission. While driving normally, the transmission shifts hard (ie: jerks, clunks) between 4-5 and 5-6 gears, and has on occasion failed to grab 1st gear when starting from a stop (revs, doesn't go). This occurred while turning across traffic, creating a potentially dangerous situation because the truck did not respond as expected when I needed predictable acceleration or propulsion. I believe this incident may be related to the known 10R80 CDF clutch-cylinder/sleeve problem. Ford technical information identifies axial movement of the CDF sleeve that can cause hydraulic-circuit leaks and harsh or delayed engagement and shifting. This condition can result in slipping, RPM flares, delayed engagement, unexpected gear changes, harsh downshifts, lurching, or loss of propulsion. This is a safety concern, not merely a transmission quality or reliability issue. Unexpected loss of propulsion or unpredictable shifting while merging, accelerating into traffic, crossing an intersection, turning, or traveling at highway speed can create a collision hazard. Because I personally experienced this unexpected transmission event, I request that NHTSA investigate the 10R80 CDF clutch-cylinder/sleeve condition in Ford F-150s. Please review Ford warranty claims, dealer repair records, transmission replacements, consumer complaints, and field-failure data to determine whether this is a safety defect requiring a recall. I believe owners should not have to wait for a potentially dangerous transmission failure before this known condition is addressed. Furthermore, I have already had two transmission rebuilds under warranty, and it's going back to the shop for a third time with the same issues 9/8/26. THE SAME ISSUES. I bought this truck in 2024 at 70K miles and it has been nothing but problems.

Short in the rear window defroster. Within 5 minutes of turning off the defroster after only having it on for 2 minutes at 2:30am leaving work, the back window exploded outward and shattering. No rocks, no other folks on the road. Upon research, and after contacting insurance company and the glass repair company, this is a very common issue in 2015-2019 F-150s. But there are no safety recalls. Praise God my children were not in the backseat, they would have been riddled with glass. How is this not a safety issue?

Not sure if I can file an issue for this, but I wanted to let you know that a Ford Safety recall caused other issues after it was applied. In late July 2026, my son received a Ford safety recall notice (Recall #26S28) for his 2017 F 150 requiring a PCM software update. I brought the truck to Crown Motors in Redding on August 11th for the recall service. Before this appointment, the truck had no performance issues. Immediately after picking up the truck, a noticeable pinging sound occurred under acceleration. I returned the vehicle on August 12th, where Crown Motors confirmed the issue and advised spark plug replacement. This did not resolve the problem. I was also told that continued driving could damage the engine, so the truck remained at the dealership. Over the following days, Crown Motors worked with Ford engineering and repeatedly attempted software-related fixes. Crown staff informed me they believed the recall update caused the issue, and the first internal Ford team reportedly agreed. However, on August 25th, Ford’s second internal review determined the software update was not responsible and stated that all repair costs would fall on me—despite the clear timeline showing the problem began immediately after the recall service. Customer Service provided no assistance and refused to escalate my concern. As a result, my truck is currently non-operational, Crown Motors does not have a fix, and Ford will not cover repairs triggered only after Ford’s own safety recall. I believe the Spark Knock issue is an additional safety issue caused by the PCM recall update that was I was told could cause engine failure.
