The contact owned a 2024 Tesla Model Y. The contact stated that while the contact's wife was driving at an undisclosed speed, the vehicle was involved in a T-bone crash. The contact stated that the driver's side of the vehicle crashed into it while the vehicle was stopped at a four-way intersection. The contact stated that none of the driver's side air bags deployed. The contact stated that only the passenger's side air bags were deployed. The fire department and Police department arrived on the scene. The fire department had to cut the driver's door off to remove the contact's wife from the vehicle. The contact's wife was transported to the local hospital. The contact's wife was severely injured. The contact's wife sustained injuries to the head and the neck along with other injuries. In addition, the contact's wife has continued treatment with a chiropractor, physical therapy, and continued medical appointments. The vehicle was condemned as a total loss. A Police report was filed. The manufacturer was not made aware of the failure. The failure mileage was approximately 1,000.

In addition, the contact stated after parking the vehicle, the front wheels failed to move while turning the steering wheel to the left or the right. The contact stated that the door failed to open while using the key fob. There was no warning light illuminated. The vehicle was towed to a local dealer where it remained for further diagnosis and repair. The manufacturer was not made aware of the failure. The failure mileage was approximately 21,000.

This has happened several times and I am unsure what to do to prevent it. I have small children who have various things in the back seats. Sometimes during sudden braking a water bottle or a toy will roll under the drivers seat and under the pedals which makes for a very scary situation. I’ve read online that this has happened to other Tesla owners as well. I hope that there can be a fix for this as currently there is no barrier between the drivers seat and the back seats.

Front left suspension came apart without any collision. Over the course of a couple days, noticed a sound in the suspension when turning the steering wheel to make a 90 degree turn or more. Pulling out of a driveway and hear loud sound and then steering wheel would not move the front left tire. After getting out to inspect what happened, noticed there were two large bolts on the ground and a large piece of the suspension was hanging down under the car and sitting on the ground. There were no warnings prior to this occurring to signal that something was loose or malfunctioning. Luckily I was on a quiet residential road when it happened and was able to get out safely but it would have been extremely dangerous if this happened in a different location.

Front right side wheel disengaged from the steering wheel. It was very scary situation to deal with and luckily it happened while I was on alley. I was not sure what was going as suddenly car was not under my control and acting weirdly. I hit a parked car and came out of the car safely. When parked the car then realized right wheel was disengaged. Called the Tesla insurance and they are not responding for over a month now since the incident happened. Tesla is saying I need to facilitate all this with the tesla authorized facility. Bottom-line, car is with tesla insurance and nothing is happened for over a month.

At approximately, 14,300 miles and while still under full manufacturer's warranty coverage, the front end of my vehicle started vibrating excessively at speeds of over 65 MPH while driving on smooth road surface (highway driving). I took the car to the local TESLA dealership to have the problem diagnosed. They determined that the "foam lining" on the inside of the tire separated and caused the wheel to become unbalanced. Their fix was to remove the foam lining and reinstall the tire on the wheel without the foam lining. I now have 1 tire out of 4 that does not meet the original OEM spec and condition. Below are the notes from the invoice for $256.10 that I had to pay to correct the problem. Repair Notes: Verified customer concern. Found RH front tire to have foam separation. Removed foam rebelanced tires. Test drove vehicle to verify vibration no longer present. Verified repairs.Correction: Tire - Front - RH (Remove & Install) - Remove and Install This seems to be a common problem with these foam lined tires manufactured by GOODYEAR according to TESLA owner blogs. It seems that either TESLA and or GOODYEAR are not claiming responsibility for a potential design and or manufacturing problem which is why I am filing a formal complaint. I will also be filing a complaint specific to the GOODYEAR tire.

While using adaptive cruise control the vehicle regularly decelerates for no reason and without warning "phantom braking". I experience this about once a week, but experienced it multiple times on a longer drive this weekend. The unexplained abrupt braking could result in being rear-ended by another vehicle.

I found that two bolts that connect the front right suspension assembly to the body of the car were loose (actually one had completely come off). On 9/12 I heard a rather loud clanging sound when I accelerated. When I arrived home I looked under the car, saw those loose bolts, and took the two pictures that I attached, which were taken from in front of the behind the front right wheel. We bought this car new from Tesla, and received it in late August 2023. We would expect that those bolts were not properly tightened when the car was assembled at the factory, since servicing done afterwards should not have affected them. This is the list of servicing that was done after we received the car, and before 9/12: - Tesla service fixed some rattling sounds that we heard inside the car by adjusting the interior trim, and by fixing something in the brake pedal that was making some rattling sound sometime (replaced the brake pin, insulate the braked pedal and lubricated the brake pin) - A body shop replaced the side mirror on the left side (i.e. opposite side of the bolts that had come off), which had been hit by another car - A tire shop fixed a hole in a tire and rotated the tires

The hood latch failed and caused the fronk to open and smash the windshield. This happened while the car was on a trailer being transported. The front hood had opened multiple times while in garage over the last few months and unfortunately it decided to open while on the trailer. The hood flew open and smashed the windshield and caused $6k worth of damages and tesla will not pay for the fix even though there is an active recall for hood latch on my tesla Extremely scary and dangerous that this can happen on a brand new 2023 car

Date of incident 9/9/24 11:15 AM 2023 Tesla Model Y traveling north bound I-15 using Tesla Autopilot intending soon to exit Pioneer Crossing / American Fork UT in the far right lane — vehicle made sudden move to right glancing off wall causing scraping damage to right front quarter panel, and hub cap. Although I stepped on the brake in attempt to take control of the vehicle I immediately experienced uncontrolled acceleration and difficulting in steering. I chose not to exit and fought to keep the vehicle in varying freeway lanes attempting to brake to which the vehicle continued to attempt to accelerate against the braking, I also simultaneously and frantically continued to push the right stalk upwards in attempt to disengage Autopilot assuming that was causing the unintended acceleration. I was fighting to keep the car in a lane with faulty steering at time awakwardly steering to avoid slower moving vehicles. I eventually ended up coming to a complete stop in the middle lane of I-15 north bound 9/10ths (nine tenths) of mile north of my intended exit (mile post #278 Pioneer Crossing). Because of faulty controls I was afraid to try to move the vehicle to the side — I immediately put on emergency flashing lights and called 911 on my cell phone. While nervously waiting for emergency services approaching vehicles slowed and made their way around me — per instruction I stayed in my vehicle with seatbelt on. One large pickup truck struck my vehicle driverside rear as he passed by and failed to stop. Utah Highway Patrol and Fire Department Ambulance arrived within approximatley ten minutes. Two highway patrol officers stopped traffic and assisted in me slowing driving my vehicle to the center emergency lane. One office stayed with me until tow. He commented the right rear wheel appeared not aligned correctly suggested the rear axle was possibly broken. I was not injured beyond being extremely frightened. No passengers.

My vehicle started wearing the tire on the left front so I contacted Tesla set an appointment up to have the suspension inspected. They advised they would not cover suspension because it’s a wear item and required me to pay for a suspension alignment which they did and I question the print out on the alignment showing the left front was not in factory specifications, they said it’s within their parameters and they can’t adjust it so I drove the vehicle and came back to have them put new tires on because of the worn inside edge again I question the suspension they said parameters of not affecting the vehicle even though based on specs of the alignment sheet was not meeting The numbers specified by the system again would not do anything to dress. It said it’s not adjustable and it will not affect my tires. I drove the vehicle. Had to replace a new set of tires a year later for the same issue requested them to fix the alignment and they provided me a receipt showing that the suspension is not adjustable so there is an issue with the suspension which causes the tire wear out which can cause it to blow out if I continue to drive it without constantly buying tires .

Multiple, random occurrences of hard braking at highway speeds when there was no object or obstruction ahead. In the 2-1/2 years we’ve owned the car this has happened on two lane and multiple lane highways, generally at speeds over 50 mph. Also, the windshield wipers randomly deploy even when turned completely off. They generally work when it’s raining but the delay function is also randomly not in sync with the settings or actual rainfall.

What component or system failed or malfunctioned, and is it available for inspection upon request? System computer. After a Over the Air (OTA) update, most driver assistive functions are inoperative. System feedback display sometime goes black (speedometer, controls, backup camera, etc). After manufacturer serviced car to verify and resolve issue, issue was not corrected. Screen continues to error, asking for a software update. Per the manufacturer this is a computer issue with the cache, and being told to replace computer. How was your safety or the safety of others put at risk? Since the driver feedback display and cameras are sometimes non-functional. Back up camera can be unusable, lane keeping, and cruise control are inoperative. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? By an manufacturer repair center. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? No, they appeared after an OTA update 9-AUG-2024

The contact owns a 2024 Tesla Model Y. The contact stated that while driving approximately 15 MPH the vehicle suddenly experienced unintended acceleration causing the driver to lose control of the steering and crash into a second vehicle. During the crash, the frontend of the vehicle was severely damaged but the air bag did not deploy. The contact sustained a right-hand injury that did not require medical treatment. Three occupants from the second vehicle sustained undisclosed injuries and were transported to the hospital. A police report was taken at the scene and the vehicle was towed away. The cause of the failure was not yet determined. The manufacturer and local dealer were notified of the failure. The failure mileage was 7,608.

The control arm of my 2023 Model Y front wheel just came completely off without any incident. On August 31st at 9 pm, I was driving home from a park when I just pulled the car into my driveway, I heard a strange squeaky noise from the front wheel area, followed by a huge "BAM ''. I started to turn my steering wheel, and by then, the vehicle felt "stuck'' as if something was preventing the car from moving. I came out of the vehicle and noticed the control arm had just fallen off and was dragging on the ground, I also saw two bolts on the ground so I scheduled a Tesla roadside assistance. The Tesla service told me to call a lift-wheel tow truck to tow my vehicle to a collision for repairs. The control arm defect is a known issue for Tesla.

First time when it happened, i was able to control and It went to back to normal. I did not pay attention after. Second time I took right turn and the car drove in left lane but this time I felt steering was rigid and was unable to control the Steering and hit a tree. Driver seat air bag was deployed and all doors were locked. Gel from air bag burned my right hand wrist.

On multiple ocassions when using adaptive cruise control the vehicles slams on the brakes without warning and for no reason and brings car to a near standstill in the middle of traffic unless I slam the accelerator down and fight the car. Engineers from Tesla inspected the vehicle and said everything was working normally, but it continues to malfunction and almost causes accidents with cars behind in traffic having to slam on their brakes as well.

My husband was driving the used Tesla we recently purchased from the Tesla dealership. We were driving on the highway with regular traffic and no issues around us. Autopilot was not on (we have never employed this feature). All the sudden, the car quickly jerked to the right on its own and then swerved back on its own. We were driving towards the sun so maybe the camera saw a shadow that wasn't actually there. We think either there is a steering issue or that the car might be reacting erroneously to something that isn't there. Just wanted to report this safety concern because I had my baby in the car and this could have caused a car accident.
