I noticed some alignment issues becoming progressively worse with my car. Suddenly in a parking garage my front right tire over rotated and became stuck in the wheel well. I was able to free the wheel and the car said "alighnment issues but car is safe to drive." I drove home with my sister and her baby on the highway. Then I got off the highway and the car did the same thing becoming stuck in the middle of an intersection. I was able to free the car and proceed carefully home. The next morning my front control arm completely broke free and was dragging on the ground leaving the car completely inoperable. I never had any accident or hit any curb with the car. In fact we brought it in 1 year ago to check alignment issues on the same front right suspension. In the shop now the service technician is saying this is a known issue on the MY resulting from not torqueing the bolts enough. I am shocked that the car has not been recalled for this known issues that is a major safety impact. I don't trust this car at all and frankly makes me question the oversight of NHTSA. When I look online apparently many people have been talking about this issue for over a year.

I was in a parking lot on [XXX] in Mamaroneck, New York waiting in line to get out at approximately [XXX]. Many cars were ahead of me due to the school drop-off rush, with parents and kids lined up in the parking lot waiting to leave. While I was waiting, the Tesla unexpectedly accelerated on its own, but I managed to hit the brakes just in time to avoid colliding with the car in front of me. A few seconds later, my car suddenly accelerated again without any input from me. I tried to steer away from the cars in front while repeatedly pressing the brakes, but they weren’t working. All I could think about was avoiding hitting another car or person, so I swerved the car toward the building in the parking lot. The impact was hard, and the car finally stopped, but the airbags didn’t deploy. I’m deeply shaken by the incident; I could have seriously injured someone or even worse. When I got home, I discovered that this issue is a known problem with Tesla, yet they’re placing the blame on drivers. Having never been in a car accident before, this experience has been overwhelming. As a mother and attorney, I believe it’s crucial to examine this situation more closely. Tesla controls the data, and it’s clear that responsible drivers wouldn’t suddenly veer into a building in a crowded parking lot without cause. The auto wipers come on multiple times a drive without water being on the windshield. This has been dangerous because the wipers will smear material across the windshield at the wrong times creating a visibility hazard. It almost caused a wreck where it activated smearing the windshield while the sun was directly ahead. Tesla’s response was the auto wipers are in Beta mode and I should turn them off to keep them from randomly coming on. This is a challenge since you have to use the center screen to find the wipers and activate them causing one to look away from the roadway.

I was fully stopped at red light of intersection of [XXX] . Suddenly my vehicle accelerate it self and start auto drive at high speed and malfunction, hitting multiple parked vehicles. My vehicle was on auto holding brake feature. I was not able to control my vehicle, even when I tried pressing the brake. No warnings or alarms from my vehicle when crashes into multiple parked cars. The contact owns a 2023 Tesla Model Y. The contact stated while driving at various speeds, there was an abnormal scraping sound from the vehicle. There was no warning light illuminated. Upon inspection, the contact discovered that a bolt was hanging off the front passenger’s side of the suspension. Upon further inspection, the contact discovered that two additional bolts on the bracket were also loosened. The contact was able to tighten the bolt to secure the suspension. The contact called a Tesla Service Center but received no assistance. The manufacturer was not notified of the failure. The vehicle was not diagnosed or repaired. The failure mileage was approximately 49,000.

The issue appears to be an acceleration malfunction and brake failure in my 2021 Tesla Model Y. The vehicle is available for inspection upon request. On [XXX] at [XXX], while waiting for the garage door to open, the car experienced an unknown unintended acceleration malfunction, surging forward uncontrollably. It didn't wait for the garage to open, crashed through the garage into the family room, causing significant structural damage to the framing and door. The City of Eastvale declared the house unsafe, sealing it off. I am now homeless and staying with friends temporarily. During the crash, the airbags did not deploy, and the ABS system failed, preventing an emergency stop. The car stopped 4-5 feet away from my wife, who was on the sofa, posing a critical life-threatening situation. Thankfully, [XXX] and I survived, but this incident caused severe mental trauma for my family. We have not reported the incident to Tesla. The dashcam footage appears deleted during the crash and the car seemed to have only recorded footage before the crash and after. This issue has not been reproduced or confirmed by any dealer or service center, but further investigation needs to be done. The vehicle has not been inspected by Tesla or any service center. However, the police and insurance representatives have been involved. We are coordinating further inspections to find the cause of the acceleration and brake failure. We aim to invoke the Tesla Event Data Recorder (EDR) tool to investigate what exactly went wrong with the car. There were no warning lamps, messages, or symptoms before the incident. I was driving on [XXX] across Kansas and my car suddenly braked hard. I was in cruise control when it occurred. I was not in automated driving mode as I don’t have that feature. This happen once before earlier this year on a two lane highway when a truck was coming toward me in the opposite lane. Emergency braking and alerting engaged. There were no vehicles or objects in front of my vehicle at the time. The braking was hard enough to engage the ABS and bring the car from 40mph to 5-10 mph, risking a rear end collision if someone was behind me. I have owned the car just over 1 year, this is the first time this has happened. No warnings or other errors appeared before the incident and no errors are present after the incident. Neither the dealer or anyone else has inspected the vehicle. Conditions: Dry, Night (9:50pm) no cruise control or lane keeping functions enabled, no cars or objects were seen in front of the vehicle.

Possible others. •How was your safety or the safety of others put at risk? oThe seat melted stuck to a gym (drawstring) bag I had placed on it. When removed, the hot, sticky parts of the seat stuck to my bag and hands. I was put at risk for potential burns. oA person could get stuck on the seat potentially putting them at risk when not being able to exit the vehicle if needed in a safety situation. oThe heat controls could be shorted in the seat potentially putting anyone near at risk of a heat or possible fire incident. •Has the problem been reproduced or confirmed by a dealer or independent service center? oNot that I am aware of. I noticed that the car window glass broke without any external impact, and there are no signs of external damage. I suspect this may have been caused by stress fractures due to improper assembly. From what I understand, stress fractures often occur at the edges without any signs of external impact, which matches this situation. But Tesla service refused to admit this is a flaw of installation and refuse to replace it under the warranty coverage. After exploration, I found there are plenty of similar cases happened on the internet. And some of them were treated as flaw, some not. How Tesla identify this kind of case as flaw or not? They didn't even check my out in person. I cannot accept that!

The contact called and informed the manufacturer that the current operating system software was out of date. The contact stated that the operating system software version was 2023.44.30.8 and the newest version was 2024.20.3. The manufacturer informed the contact that the operating system was adequate. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The contact had not experienced a failure. Parts distribution disconnect.

When driving on highways the car will jerk the steering wheel and cause the car to turn when the car is in the center of the lane. This causes the driver to counter the force from the steering wheel to keep the car on the road. This only happens when autopilot and full self driving are not being used. I have taken it to the service center twice. The first time they said it was an issue with the car alignment so they realigned the tires and the second time they said it was a problem with the car’s computer so they replaced the computer. This continues to happen and is dangerous. Sometimes it will give a lane departure warning while other times there are no warnings.

On [XXX] , south of Austin, north of [XXX] , on both directions, there are many turn lanes on either side. These turn lanes fork from either of the two highway lanes and do not have clear lane markings (by design, not due to fading) until the lane width is full. Tesla Autopilot with lane assist will, at multiple times along this route, center the car between the current lane and the emerging turn. Once the lane markings begin , the car will violently jerk back into a lane. Avoidance of these situations requires a takeover. Along this route, this is particularly dangerous on banks, hills, and in traffic (even with max following distance configured) due to reduced visibility of the start of the lane markings. NHTSA Recall No: 22V-702, in 2022, was where there was supposedly an OTA update to stop the windows from increasing force in some situations when an object was stuck in the gap between the window and the frame. My 2021 Tesla received that update at the time, and I didn't think much of it...until January 2024 when it actually happened to me. I shut the door with my hand on the window, and at the moment it closed, the window went up to close the small gap. My finger was in there and as I tried to pull it out, the window was still trying to close, not allowing me to do so. Component that failed -- the window, supposedly fixed in 2022 Available for inspection upon request -- returned the leased car in September 2024 How was safety put at risk -- could not remove finger from gap between window and frame since window was continuously trying to close Has the problem been reproduced or confirmed by a dealer or independent service center -- I was ignored by Tesla HQ, and by my closest dealer. The Tesla dealer from which I leased the car (a different one) told me to make a service appointment. Has the component been inspected by the manufacturer, police, insurance representatives or others -- no Were there any warnings or symptoms -- no. No reason to think it would ever happen, then it did in January 2024.

Several times in the last few months I have had extremely hard braking on the highway when using adaptive cruise control (called Autopilot by Tesla). It has happened on a few different highways ([XXX] in Arizona, [XXX] in New Mexico, [XXX] in Colorado) all when going between 65-75 miles per hour. Thankfully, in most cases, there was no one directly behind me or there almost certainly would have been an accident. In one case a colleague of mine was following me and had to slam on the brakes, too, but she avoided an accident by reacting quickly. In all cases the road in front of me was empty of traffic and there was no warning that the brakes were about to be strongly applied. In some cases the car seemed to react to a turn in the highway, or maybe an uphill or downhill section, but it is unclear to me. In all cases the brakes were applied with a lot of force, and in all cases it was pretty scary. This seems like a huge safety issue with the adaptive cruise control of my Tesla Model Y.
