Vehicle Information: •Make: Tesla •Model: Model Y •Year: 2024 Description of the Problem: After a software update pushed by Tesla, the AP4 computer in my 2024 Tesla Model Y shorted out, causing a complete failure of all systems controlled by this computer. The following safety-critical and driver-assist features have become non-functional: 1.Cruise Control (adaptive or otherwise) 2.Forward Collision Warning 3.Lane Departure Warning 4.Lane Keeping Assistance 5.Blind Spot Warning 6.Parking Collision Warning 7.Full Self-Driving and Auto Park features 8.Cameras required for Autopilot functionality Additionally, because the GPS system has failed, the vehicle’s Navigation system is inoperable. The GPS stays in the last position prior to the update. This impacts Tesla’s unique route planning feature that sets up Supercharger stops during long journeys. Without a functioning navigation system, I cannot properly plan my travel or locate charging stations, which severely affects the usability of the vehicle. Safety Concerns: The failure of these systems, especially safety features like forward collision warning, lane keeping assistance, and blind spot monitoring, puts both the driver and others on the road at risk. Losing access to safety-critical functions after an update is unacceptable and poses a serious danger to vehicle occupants and surrounding traffic. Incident Details: •The issue occurred immediately following a software update. •Tesla pushed the update remotely without providing any warning or indication of potential risks. •The failure appears to be related to a hardware short in the AP4 computer, directly caused or exacerbated by the update. Desired Outcome: •A thorough investigation into Tesla’s software update process and its effect on vehicle hardware and safety systems. •A resolution that prevents similar failures in other vehicles and ensures system updates do not compromise vehicle safety or critical functionality.

Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around Thanksgiving. I scheduled the soonest appointment available for 01/03. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 1,000 miles on it and is ~1 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, back camera, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either.

Tesla has told me the AP4 computer has failed in my car which has resulted in them having to replace it. I had this issue start right around Thanksgiving. Bought my Tesla Model Y on [XXX]. Exactly one month later on [XXX] I start my car to find that none of the cameras are working. This means Autopilot, cruise control, rear camera, blind spot monitoring and many more safety features are completely down. In addition the GPS does not function meaning my car is not trackable and Maps don't work, another safety concern and main reason we bought the car. This is a widespread issue that Tesla Service Centers have been informed about from Tesla that it is a bad computer that needs to be replaced - in a 1 MONTH old car. My only option is to wait for a computer to be assigned to me which is at earliest on January 21st of 2025, almost 6 weeks without a fully functioning brand new $46,000 Car with only 1400 miles on it. Tesla are also unwilling to make any kind of restitution such as a loaner car, or a refund for the Full Self Drive and Connectivity packages I just purchased but cannot use as my car is broken. Cameras not working with latest Software update on November 26th. without even inspecting the car in-person, Tesla service agent told need new AP4 computer on 2024 model Y which is 1 month old . I think Tesla knows about this issue and there is lot of wait time for getting the AP4 computer. Tesla service advisor states - car is totally safe to drive with just mirrors till appointment in few weeks. As per NHTSA, if a car is not having a backup camera, that is a safety violation and there are lots of Tesla vehicles driving without backup cameras. No backup camera, navigation, battery life dying faster. It seems like a liability to me to drive this way and very negligent to me.

Widespread issues acknowledged privately by Tesla service managers, advisors, and engineers. Affects all models built from July 2024 to present with most recent “AP4” computer and hardware, primarily out of Fremont factory. Suddenly, computer is short circuiting. This results in MULTIPLE systems failing given effectively ALL systems are dependent upon this “AP4” computer and hardware, and initially causes GPS and Camera sensors to fail also. This system failure includes: -federally required rear view vision system -adaptive cruise control -automatic emergency braking -blind spot warning -forward collision warning -lane departure warning -lane keeping assistance -parking collision warning -rear cross traffic warning -visibility and wiper systems -all external lighting systems -speed control Internally, Tesla service advisors and managers are being told to advise customers that vehicles can be safely driven until fixed - which is currently diagnosed to be a removal and replacement of this “AP4”, which is heavily supply chain restricted. Customers are being told to wait “weeks” - with estimations in January and February - for this to be fixed. Service Advisors and managers are being told to not acknowledge clear safety concerns given vehicles are no longer compliant with all applicable federal laws and regulations, such as those requiring rear view vision field of view expansion, etc. Further, internally Sales advisors and Managers are being told to continue heavily pushing sales to meet record sales targets to impact stock price. Several private conversations regarding ethical concerns of pushing noncompliant and known defective vehicles for stock value and potential criminality. Service advisors and managers also privately concerned regarding legal liability of impacted and defective vehicles in accident, given Tesla is communicating to continue driving. My own personal vehicle has been impacted, and information above has been gathered privately.

All the cameras on the car failed. There is no rear view backup camera. There are no side cameras. GPS doesn't work. The headlights go crazy during the day or night. Flashing brights and blinding people on the road. I need to manually adjust the lights every time. The screen will stay in dark mode during the day. And day mode during night. There is no cruise control, warning systems, etc.

My navigation and all cameras abruptly stopped working when I was headed home from work on 12/2. When I entered the information for Tesla service it was automatically switched to AP4 computer short. I told the service center this Tesla is not safe to drive and is lacking all safety feature and asked for a loaner, but was repeatedly denied. My service appointment continued to be pushed by from 12/6 to 12/16 to 1/6 now. When I asked again for a loaner I was told no and that the issue was widespread causing the computers to be in back order.

The lack of cameras impact the ability to see the rear when going into reverse since the rear camera is not functioning. I scheduled the soonest appointment available for 12/13 however Tesla contacted me and moved my appointment because they stated that the part was unavailable at this time and should be delivered by 01/20/25. Not offering me a rental or anything on my brand new '24 Tesla. It has just over 2500 miles on it and is ~2 months old now. This issue impacts: emergency braking, blind spot detection, lane keep/centering, assisting with parking, automatic high beams and wipers, cruise control/ adaptive cruise control, Vehicles built in GPS to be stuck at one location every time, has made my touchscreen not automatically brighten or dim, my auto dimming mirrors be too dim in most driving conditions, constantly fail to install system improvements and safety updates, and because my vehicle keeps trying to do this, it has caused increased battery drain as well. Additionally, the vehicle came to a stop on the highway when my wife and I were in the car on the night of [XXX], forcing us to perform an emergency pullover and abandon the car 22 miles north of Rochester, MN overnight. This has increased the safety risk of my vehicle tremendously... Tesla has not seemed to publicly acknowledge the issue yet either. All camera and navigation systems stopped working on 11/30. With no cameras working, there are significant blind spots when driving. I cannot see anything when the car is in reverse due to no backup camera and the mirrors are stuck in a dark "tinted mode" further decreasing visibility when driving. The local Tesla service center says they do not have parts in stock to fix the issue. They have repeatedly delayed any scheduled service appointments, without offering any solutions. The car feels very unsafe to drive. There were no prior warnings of the issue, it just stopped working.

After several days in the shop, TESLA informed me THE MAIN computer was broken and needed to be replaced, but it was back ordered. They asked me to reopen their loaner and drive this defective car, until the computer comes in.

Accident report [XXX] I was driving on [XXX] fort smith ar to Bentonville Car was on autopilot, it was raining on and off and with all other cars we were driving around 70-80 mph All the sudden it started a short slip on the right then second after that it started spinning and took us out of the road car totaled car. I believe problem is autopilot did not disengage when car started to slip but instead it was still on until the car got out of the road and hit a large tree. This is 3rd time for me to have this type of accident with same situation slip then spin then out of the road. I believe autopilot is the problem here 100%. This autopilot system is very dangerous.
