I am writing to formally report a serious safety failure involving Tesla’s Full Self-Driving (Supervised) feature, which resulted in property damage and a severe safety hazard.While FSD (Supervised) was fully engaged, the vehicle failed to detect a large block of wood in the roadway. The vehicle struck the object, causing significant damage to the brand-new car's left-side tire and body, t touched two other cars hence I called 911 and reported it to ensure they remove the wood. Because I did not know how to manually trigger the dashcam save at the exact moment of the crash, video footage was not captured locally. However, the vehicle’s internal sensors, and Event Data Recorder (EDR) should have logged the active FSD status and sensor data at the time of impact and potentially save it.Immediately following the incident, I contacted emergency services via 911 to report the roadway hazard. I have since visited a Tesla Service Center, where staff informed me that I must wait for the formal complaint process to proceed.Due to recent publicly reported fatalities involving FSD, I strongly believe this software is not safe for public roads, and I have chosen not to renew the feature. I am requesting a comprehensive corporate review of my case. I expect Tesla to pull the vehicle's internal logs, take accountability for this hardware and software failure, provide a full repair of the vehicle, costs about $2000 as estimated by their collision center and issue a complete reimbursement for the damages.

I purchased a new 2026 Tesla Model Y Standard and took delivery on approximately 30 May 2026. Within the first week of ownership, the vehicle displayed multiple safety-related warnings, including: * Steering Assist Reduced * Power Braking Assist Reduced * Automatic Emergency Braking Unavailable * Lane Departure Avoidance Features Unavailable * Automatic Vehicle Hold Disabled The warnings have occurred more than once. They initially disappeared and then returned on a later drive. Both incidents occurred while the vehicle was operating with Full Self-Driving (FSD) engaged. During the most recent occurrence, while parking at less than 3 mph, the vehicle’s braking assistance appeared reduced and the brake pedal required significantly more effort than normal. As a result, I nearly struck a curb because the vehicle did not slow as expected. The warnings indicated that increased steering and braking effort may be required. Because the vehicle is brand new and these warnings involve steering, braking, and collision-avoidance systems, I believe this creates a significant safety risk. I no longer feel the vehicle is safe to drive until it is inspected and repaired. Tesla has been notified and a service request has been initiated.

While operating a 2026 Tesla Model Y on Full Self-Driving (FSD) Supervised, the vehicle's vision-based lane-keeping system failed on an unmarked residential road. At approximately 26 MPH, the vehicle approached a blind crest of a hill. Due to the lack of painted center lines, the FSD software miscalculated the road geometry and drifted significantly into the center/oncoming lane exactly as it reached the top of the hill. This lane departure created an immediate, severe safety hazard, as it placed the vehicle directly in the path of potential oncoming traffic with zero forward visibility over the crest. Due to the critical lane positioning error, immediate driver intervention was required. The driver took manual control of the steering wheel to force the vehicle back into the correct lane, causing the FSD system to instantly disengage. Simultaneously, the driver applied manual braking to reduce speed drastically down to 10 MPH to safely clear a resident working on the right shoulder and stabilize the vehicle. Prior to the failure, there were no warning lamps, error messages, or symptoms indicating the system would fail to maintain proper lane positioning. The system appeared to be functioning normally up until the crest of the hill. The vehicle and its Autopilot/FSD components have not yet been inspected by Tesla, police, or an independent service center following this specific event. The vehicle remains available for inspection upon request

A 2021 Tesla Model Y experienced steering/suspension warnings followed shortly by a steering-assist reduction event. On or around May 21, 2026, the vehicle displayed: “Steering alignment service recommended — OK to drive, schedule service soon.” The vehicle was brought to Tesla because the owner was concerned the car had detected a steering, alignment, suspension, or safety-related condition. The Tesla invoice from that visit appears inconsistent. It states that “a wheel alignment check was completed,” but also states “No Labor Performed — Customer Did Not Authorize Repairs or Inspection,” with 0.00 labor hours and no charge. A documented steering/suspension diagnostic inspection, alignment report, alert-log review, or safety assessment was not performed by Tesla or inspected. The owner was advised to replace tires and return later. Approximately 90 miles later, during a low-speed three-point turn, the vehicle made a loud pop/clunk from the front area and displayed: “Steering assist reduced — Steering may require increased effort.” A related alert also stated Lane Departure Avoidance features were unavailable. This created a safety concern because steering effort may unexpectedly increase during low-speed turns or parking maneuvers. Tesla later inspected the vehicle and documented torn front lower lateral/compliance link bushings and vehicle log data showing steering gear alerts. Tesla replaced the front lower lateral links and front lower compliance links and performed alignment work. After those repairs, Tesla stated a steering gear alert was still present and recommended steering gear/steering rack replacement with additional alignment work. The concern is that a steering/alignment warning occurred shortly before the physical front-end failure and reduced steering-assist event, but the earlier Tesla visit does not clearly document that the warning was meaningfully inspected or safety-assessed. On XXX, at approximately 8:01:49 AM Eastern Time, my 2024 Tesla Model Y (VIN: [XXX]) was operating under Full Self-Driving (FSD) software version 14.3.3 when the system directed the vehicle into the outer wall of my residential garage without driver input. FSD was actively engaged at the time of the incident. The vehicle sustained significant damage to the front bumper fascia and wheel well trim. Additional damage includes a cracked front fog light/corner lens assembly with surrounding fascia stress, also caused by the same incident. The incident also caused paint transfer damage to the garage wall trim. I have preserved dashcam footage documenting the incident, which is consistent with FSD-directed vehicle behavior. I am referencing NHTSA Preliminary Evaluation PE25012 as this incident is consistent with the pattern of uninstructed FSD-directed maneuvers currently under federal investigation. The vehicle’s secondary Autopilot Processor / car computer is internally faulted and stuck in a continuous wake/shutdown loop. This causes severe standby battery drain (~20 miles of range lost per day while parked) and completely prevents all over-the-air software updates - including federally mandated safety recall remedies. On April 4, 2024, Tesla Service diagnosed the fault and linked it to the drain and inability to update. On the same visit, Tesla added two safety recalls (23V-838 Autosteer controls and 24V-051 Brake/Park/ABS indicators) to the work order. They then demanded ~$3,000 customer payment for computer replacement before the recalls could be performed. After I declined the paid repair, Tesla marked both recalls as “completed” via software update despite the same invoice stating the vehicle could not receive updates and remained on software version 2023.38.9 (which predates both remedy firmwares they claimed was installed). Tesla is effectively paywalling federally required safety recall remedies by requiring the customer to first pay for computer replacement first. Two additional recalls remain open (21V-389 seat belt fasteners and 24V-554 hood latch). The problem has been confirmed and documented multiple times by Tesla Service.

The contact was able to pull off to the side of the road while the vehicle made a loud squealing noise and swerved simultaneously. The contact stated that her passenger side wheel had came off and a bolt. The vehicle was towed three separate times: first to a nearby parking lot, then to the contact's residence, and finally to the dealership. The vehicle was not officially diagnosed, but the contact noted that the lower suspension component was completely separated from the front passenger side wheel and touching the ground. The vehicle was not repaired. The manufacturer was not notified of the failure. The failure mileage was 29,844.

On the evening of June 3, 2026, while driving on the I-575 Express Lanes in Cobb County, Georgia, the vehicle’s autopilot/driver assistance system malfunctioned and steered the vehicle into the express lane boom barrier. The boom bar struck the windshield, causing catastrophic windshield failure with glass imploding into the cabin. Glass shards covered the driver’s seat, floor, and dashboard. The driver sustained glass puncture wounds to both forearms and forehead, as well as neck, back, and right arm soft tissue injuries. Component failed: Autopilot/ADAS system. Vehicle is available for inspection. Tesla was notified directly at time of incident. Insurance company has been notified. SR-13 filed with Cobb County. No warning lamps or messages appeared prior to the autopilot malfunction. The root cause of the windshield damage was the autopilot system failure, not driver error or external road debris. Photographic documentation of vehicle damage and driver injuries exists.

I am reporting a safety defect in Tesla’s Full Self-Driving (Supervised) system. On XXX, while using FSD, my 2024 Tesla Model Y (VIN: [XXX]) failed to recognize that the road terminated into the desert after a roundabout on the [XXX] road near [XXX]. The vehicle accelerated straight instead of making the sharp 90-degree left turn, left the roadway, traveled down an embankment, and crashed. Airbags deployed and the vehicle was declared a total loss. Vehicle Data Recorder (VDR) data confirms that Autosteer was active and available during the critical braking sequence. I applied very strong sustained braking (brake master cylinder pressure exceeded 129 BAR with ABS activation). The tow truck driver, [XXX], stated at the scene that this was the fifth Tesla he had recovered from this exact same location due to similar crashes. This documented cluster of at least five identical Tesla crashes at one hazardous spot indicates that Tesla had actual notice of a dangerous defect in its FSD mapping and road geometry recognition but failed to correct it. I purchased a 2021 Tesla Model Y (VIN: [XXX] ) used directly from Tesla. I am the first retail owner outside of Tesla. I am currently experiencing a knocking noise from the driver side suspension. I have requested that Tesla inspect and complete the following federally mandated safety recalls and service bulletins, none of which appear in my vehicle’s service history: • NHTSA Recall 21V-835 / SB-21-31-003 — Front suspension lateral link fasteners may loosen and separate from the sub-frame, causing instability and crash risk • NHTSA Recall 21V-912 — Suspension knuckles may fracture • SB-20-31-006 — Front upper control arm ball joint reseal Tesla service has refused to perform these repairs free of charge, citing expired warranty (warranty expired April 5, 2025). Tesla’s response does not address that these are federal safety recalls, not warranty repairs. Federal law requires recall repairs to be performed free of charge regardless of warranty status, age, or mileage. Tesla has also failed to provide written documentation confirming whether these recalls were completed prior to selling me the vehicle. Instead, I have been redirected to a self-service VIN lookup tool and a general data privacy request form — neither of which confirms when, where, or whether the recall work was actually performed. As a result of this failure, the vehicle is no longer drivable and has been effectively rendered unusable. The High-Voltage (HV) Battery Management System and/or HV battery pack failed, triggering Tesla diagnostic code BMS_079. The vehicle is currently unable to operate due to this fault. The vehicle and battery system are available for inspection upon request. The sudden failure of the HV battery system created a significant safety concern because the vehicle lost its ability to operate as intended. A battery failure can leave drivers stranded unexpectedly, potentially in unsafe locations such as highways, intersections, remote areas, or during adverse weather conditions. The inability to reliably operate the vehicle creates a safety risk for both occupants and other motorists. The fault code BMS_079 was diagnosed and confirmed through Tesla's service system. Tesla service personnel indicated that the HV battery requires replacement and that the vehicle cannot be driven safely until the repair is completed. Prior to the vehicle becoming inoperable, warning messages appeared on the vehicle display indicating a problem with the battery system. The BMS_079 fault code was displayed, and the vehicle subsequently restricted operation.no warning signs prior. I am concerned that a major HV battery failure requiring a repair exceeding $10,000 occurred on a 2021 Tesla Model Y. During the trip, the steering reportedly stopped responding properly, and the driver struggled to control the vehicle. Witnesses at the scene reported that the driver appeared to be fighting with the steering and trying to stop the vehicle or prevent it from drifting. The vehicle then lost control, accelerated or continued moving uncontrollably, flipped/rolled over, and sustained major damage. I am reporting a safety incident involving my 2025 Tesla Model Y and Full Self-Driving. While driving with Full Self-Driving engaged… latest update, the vehicle steered into an express-lane barrier/bar structure. The impact caused severe windshield damage and apparent body damage near the passenger-side A-pillar / windshield frame / roof rail area. The windshield is heavily cracked across the driver’s side and center area, and the vehicle is not safe to drive. I was able to get home safely and filed an insurance claim. I preserved TeslaCam footage and photos of the damage. The in-vehicle Tesla playback shows Full Self-Driving engaged during the incident. I also copied the TeslaCam files from the vehicle’s USB drive. The exported video file does not appear to show the same Full Self-Driving overlay that appears in the in-vehicle playback. I am reporting this as a potential safety issue involving Tesla Full Self-Driving steering behavior and impact with a fixed roadway/express-lane structure. No injuries occurred, but the vehicle sustained significant windshield damage and possible body/frame-area damage.

Incident Report – 2026 Tesla Model Y Date of Incident:XXX Time of Incident: 12:30 am Location: [XXX] I was operating my 2026 Tesla Model Y and was driving manually. Full Self-Driving (FSD), Autosteer, and other self-driving functions were not engaged at the time of the incident. While traveling on Highway 16 at approximately 60 mph, the vehicle approached the right-side white lane line. At that point, I experienced a sudden steering intervention that turned the vehicle sharply to the left. The steering input was unexpected and occurred while I was manually controlling the vehicle. Following the steering intervention, the vehicle crossed the centerline into the opposing lane of travel and continued off the roadway into the ditch on the opposite side of the road. The incident resulted in damage to the vehicle. Road conditions were dry. Weather conditions were safe. Lane markings were clear. I am requesting that Tesla preserve and review all vehicle telemetry, event logs, steering-assistance records, and any other relevant data associated with this incident. Specifically, I request an investigation into whether Lane Departure Avoidance, Emergency Lane Departure Avoidance, or any other driver-assistance feature activated and contributed to the steering maneuver. I am also requesting a determination regarding responsibility for the resulting vehicle damage and whether Tesla will reimburse repair costs if a system malfunction or defect is identified. The contact stated that while driving at 65 MPH, with the autopilot feature engaged, the vehicle approached a lane closure as big, metal arrow road signs where on the road indicating the road closure. The contact, who was distracted at the time, expected the vehicle to acknowledge the road closure and switch lanes. The vehicle failed to recognize the Chevron barricade lights on the highway and the vehicle ran through the road signs without warning. The contact was forced to take over and stop the vehicle due to the failure. Due to the failure, the front, driver side mirror detached from the vehicle. The contact exited the highway and inspected the damage to the vehicle. Despite the failure, the contact managed to drive the vehicle home. The service center was notified of the failure and an appointment was made to have the damages repaired. The cause of the failure had yet to be determined. The vehicle was not repaired. The failure mileage was approximately 21,000.
