2021 Tesla Model Y suffered a complete failure of the front lower lateral links, compliance links, and upper control arms at only 29,103 miles. The vehicle has been inspected by the manufacturer. It is currently being repaired at my cost. There was no warning lights or messages. I had noticed a creaking sound upon turning, especially while in reverse. I believe this mirrors the exact mechanical failures seen in NHTSA Campaign 21V835000. However, my VIN number was not listed in this recall.

Tesla issued Service Bulletin SB-25-31-004 on April 17, 2026 covering this exact condition. Tesla declined to perform the repair under the bulletin citing out-of-warranty status. A touchscreen alert directing me to service was active at the time of denial.

While driving in FSD the car is ping ponging between the lane. It didn't used to do this but now does it on any road. Highway, city, turns, etc. The oscillation causes me and my passengers to be sick and it looks like a drunk driver on the road.

The vehicle activated in Autopilot mode without my intentional input, causing me to lose control and resulting in a serious crash. Prior to the collision, the vehicle's Autopilot system, engaged without my input and abruptly accelerated. I was seriously injured as a result. The vehicle, a brand-new 2024 Tesla Model Y, was in normal operating condition with no prior mechanical issues. I had not modified the vehicle in any way. Following the crash, Tesla provided data to attorneys regarding the incident which I believe to be inaccurate and inconsistent with my experience. I am requesting that NHTSA: 1. This is not okay and incredibly unexceptional for vehicle standards. This vehicle spends 90% of its time in a garage. I have contacted my insurance company.

The Tesla Autosteer (equivalent of adaptive cruise control) failed to brake on seeing the car ahead braking. In the 5 seconds before impact, the car ahead had started braking. The autosteer did not brake for the first 3 seconds and then It asked me to take over 2 seconds before collision. It did not apply automatic emergency braking causing me to rear end the car ahead of me on the freeway at around 30mph This is supposed to be a safety driver assist feature which instead was the primary cause of the accident. The take control was too late to avoid collision. This is a serious safety issue.

We were trying to drive home and the middle seat belt, where my youngest child was sitting, would not latch. it would 'click' in, but if you pulled on it, it would pop out. This was terrifying because we had to drive home on a 2-lane highway with oncoming traffic. I checked online and found that many others have reported this issue because the seat belt latch sits rigidly upright in a divot in the seat, which acts as a funnel to direct any crumbs or dropped items to go into the seat belt latch, which then apparently causes the issue. This is a death trap design for children, because of course children drop things and make messes all the time, and applies to all three of the seat belt latches in the back. What is even worse about this issue is that as a parent I listen for the Click to know they have buckled, and I also check the seat belt indicator on the dash screen. Even when this model Y seatbelt latch isn't going to hold the seatbelt in place, it makes the Click sound and also shows as latched on the dash screen, giving parents a false sense of security. It looks like no children have been injured or killed yet because of this design flaw, but please do not wait for that to happen. Please make them fix this issue immediately before tragedy strikes a family. Please watch the video I linked to in the PDF to see how the seatbelt clicks into place, but then can be pulled free by a firm tug on the seatbelt.

Model Y (VIN: [XXX]). On XXX, I brought my vehicle to Tesla Service for replacement of the front wheel hub assembly and paid approximately $1,000 for the repair. Shortly after picking up the vehicle, I noticed abnormal knocking and clicking noises while turning the steering wheel. Because I was concerned about the safety of the vehicle, I sought a second opinion from an independent repair facility. On XXX, the independent shop inspected the vehicle and found that two wheel hub mounting bolts had not been properly tightened and that one mounting bolt was completely missing. The repair invoice states that the vehicle was being operated in an unsafe condition and that continued operation could have resulted in wheel hub failure and a serious accident. The shop corrected the condition by replacing the missing bolt and properly tightening the wheel hub assembly. I incurred an additional expense of $284.47, which would not have been necessary had the original repair been completed properly. A 2023 Tesla Model Y experienced a sudden electrical failure while parked/being prepared for use. Multiple warnings appeared on May 26, 2026, including high-voltage system service required, low-voltage electrical system power reduced/vehicle shutting down, cabin climate control limited, and steering/braking/parking brake functions degraded. The vehicle could not be driven normally and required flatbed recovery. The failure created a safety concern because the vehicle showed high-voltage and low-voltage faults together with steering/braking/parking brake warnings and could have left the vehicle disabled without reliable restart or parking brake function. The manufacturer service center inspected and repaired the vehicle after several days. The component should be available through manufacturer service records, and screenshots of the warnings, tow/recovery, and service records are available.

On XXX, at approximately 5:00 PM, my daughter was operating our 2024 Tesla Model Y on [XXX] near the [XXX] exit in [XXX]. The vehicle suddenly became uncontrollable. According to the driver, the steering wheel appeared to move on its own, the vehicle accelerated unexpectedly, braking was ineffective, and the vehicle zig-zagged across multiple lanes of traffic from the far left lane to the far right lane. The vehicle reportedly performed approximately half of a 360-degree spin before control was regained. Several motorists reportedly pulled over after observing the vehicle’s erratic behavior. Tesla Service later inspected the vehicle and advised that no fault was found and that the vehicle was operating within factory specifications. However, no explanation was provided for the loss-of-control event. Due to the seriousness of the incident and the risk of severe injury or death, I am requesting that NHTSA investigate this matter as a potential vehicle safety defect. Vehicle: 2024 Tesla Model Y Date of Incident: XXX Location: Southbound XXX Express Lanes near the [XXX]/[XXX] exit split in [XXX]. At the time of the incident, Tesla Full Self-Driving (FSD) Supervised was engaged. I was traveling in the Express Lanes and intended to remain in the Express Lanes. I was not attempting to take any exit. As the roadway approached a lane split/gore area, the vehicle began steering toward the striped boom barriers that prevent drivers from going into oncoming traffic, rather than remaining in the travel lane. There were not impending impact alerts or lane deviation alerts. There were no unusual weather conditions, and I did not observe any road conditions that would have made the lane path unclear to a human driver. As soon as I saw the vehicle veering into the boom barriers, I intervened by taking control of the steering wheel. The system disengagement alert/chime occurred only after I took over. Despite my intervention, the vehicle made contact with the boom barrier assembly. The impact caused significant damage to the windshield and driver's side door frame. Glass entered the passenger compartment. My infant child was in the vehicle at the time. Fortunately, no injuries were sustained. Tesla Dashcam footage of the incident was preserved and is available. A claim was opened with Tesla Insurance, and I was advised to submit a vehicle malfunction report for review of vehicle telemetry and FSD operation during the incident. After the collision I maneuvered the vehicle back into its lane, exited the toll highway and parked at a nearby shopping center to assess our injuries the vehicle. While there, another Tesla parked near me, and the driver had experience the same exact situation while his Tesla was in autodrive. I am requesting investigation into whether the vehicle's driver-assistance system improperly tracked toward the boom barriers rather than remaining within the intended travel lane. I was driving my Tesla in self drive mode, and ibecause it started raining. I reduce the speed to standard. I had a notification on the screen, an alert in red. That the tesla reached the maximum speed in that mode “standard”. I was surprised and decided ti canceled the FSD feature and took control of the vehicle. Then when I was driving in the expressway very close to my exit on the right lane, the car accelerated by itself and turn to the left unexpectedly. I tried to returned the car to my lane and I never had control again of the car. Not the steering wheel, not the breaks responded. I was spinning on the expressway until I hit the concrete barrier with the rear bumper. Hopefully I did not hit any car nor they didn’t hit me. The car shutdown after the impact. Tesla sent me the footage of the impact, ignoring the moment that caused that incident that was some seconds before. The brakes were automatically turned onz and the wheels did not move. I know something wrong happened. Specially because the incident some minutes before when I was driving in standard mode and accelerated with no reasons. I have safety concerns. Also I noticed that after the last upgrades the car was havung navigation issues that I reported immediately when the autopilot was removed and we have to select the options.

I am submitting this complaint to request an investigation into a serious safety defect involving my Tesla vehicle. On multiple occasions, the vehicle has experienced a significant and unexpected loss of acceleration when I attempted to enter or cross intersections. Despite pressing the accelerator, the vehicle failed to respond as expected, leaving me unable to accelerate normally while entering active traffic. This is not a minor hesitation or inconvenience. It is a serious roadway safety hazard. Drivers rely on immediate acceleration when crossing intersections, turning across traffic, or merging. A vehicle that unexpectedly limits acceleration can place occupants directly in the path of oncoming vehicles, creating a substantial risk of a severe collision, serious injury, or death. I reported this issue to Tesla Service and provided multiple timestamps documenting the events. Tesla was unable to identify any fault codes or provide a diagnosis and indicated that because no fault was recorded, the issue could not be confirmed. Through independent research, I found numerous reports from other Tesla owners describing substantially similar acceleration limitation events. Many reports specifically reference Obstacle Aware Acceleration as a possible cause, and some owners report that disabling the feature resolved the issue. If a safety system can falsely detect an obstacle and reduce acceleration while a driver is actively entering an intersection, the system itself may create a greater danger than the one it is designed to prevent. I respectfully request that NHTSA investigate whether Tesla vehicles are experiencing unexpected acceleration limitation or torque reduction during intersection entry and traffic merging situations, evaluate the role of Obstacle Aware Acceleration and related software systems, determine whether a broader pattern exists, and assess whether corrective action or a recall is warranted. This could cause serious injury or fatal crash

The heat pump failed after experiencing temperatures below 10 degrees fahrenheit. There was no warning until after the failure, when I received a notification that the climate system needed maintenance. All heating/cooling, including defrosting/defogging the windshield, is inoperable. The service center confirmed that this is a heat pump failure and requires a replacement of the entire system, and a Tesla employee indicated that this is a common issue in the first 6-9 months of this cooling system design. This is well documented across many forums, and there is a class-action lawsuit in Quebec, Canada related to this failure.

On 05/31/2026, while Tesla Full Self-Driving (FSD) was actively engaged, the vehicle was taking the La Brea Avenue entrance ramp onto the I-10 freeway in Los Angeles, CA. FSD suddenly accelerated aggressively and the vehicle drifted hard to the left without any driver steering input. The driver was forced to hit the brakes and manually override steering to prevent a serious accident. The behavior felt like a system glitch. Both driver-side wheels sustained severe impact damage and are confirmed cracked. Tesla Service Center confirmed the damage in writing on 06/08/2026 and quoted $2,325.74 in repairs. This dangerous FSD behavior matches the pattern currently under active NHTSA engineering analysis.

Tesla recently pushed a software update that introduces a major, unavoidable driver distraction right at the most critical moment of driving: the exact second you take manual control back from Full Self-Driving (FSD). Whenever you disengage FSD, a mandatory, multiple-choice survey pops up on the center touchscreen asking why you took over. Unlike previous versions where this would disappear after a few seconds, it now stays on the screen permanently. There is no close button, and it does not time out. This causes two clear safety hazards while driving: 1 It blocks the screen: The popup actively covers up part of the navigation map and basic screen controls. If you are trying to navigate a tricky intersection or see your route right after taking over, you can't see the map clearly because this giant survey is blocking it. 2 It forces you to look away from the road: Because the prompt never goes away on its own, it forces the driver to physically look down, read the small text options, and tap the screen to clear it. Forcing a driver to do a multiple-choice survey while actively managing a manual takeover in traffic is incredibly dangerous. Tesla should not be allowed to use a mandatory, permanent UI popup to collect data while a vehicle is actively moving. It is a severe visual and cognitive distraction at the exact moment a driver needs to be 100% focused on the road. Please investigate this software design and require Tesla to add an automatic timeout or a quick-dismiss button that doesn't require driver interaction.
