The Full Self Driving (Supervised) system (FSD(S)) failed by running a red light. Video is available for inspection upon request. My safety was not at risk - this time - because the system waited until there was no cross traffic to resume motion, even though the light was still red. I have not tried to reproduce the problem for a dealer since it happens infrequently. (This is the third time in perhaps 6 months of subscription to TESLA's FSD(S). TESLA has not informed me of any inspections they have performed, even though they have been notified through the channel they provide. There was no indication from the car that it perceived any failure. It stopped only when I applied the brake. Prior symptoms include two previous incidents of stopping for a red traffic light, waiting until there was no cross traffic, then resuming motion while the traffic light was red. This is the first time I thought to save dash cam video.

Reporting a stress fracture in the front windhsield of my model 3 tesla. Looking online and speaking to attorney its a very common problem. Mine resulted simply from a heat wave it crack down the middle in a c shape emanating from the top edge of the windshield. No contact evidence and no contact or accident occurred. It’s now unsafe to drive my vehicle and tesla will not cover it under warranty. They are refusing to label it as a stress fracture though it clearly is. I’m filing this report so more awareness and pressure can be put on tesla to improve their windshield design. Its very unsafe to drive my car like this and now I’m forced to choose between my safety and affording a $1700 replacement. I have had several friends with the same problem and same response by tesla. My car had not been driven for 3 days and I have very low mileage, 18k. Car is 3 yrs old and in good condition.

Door cannot be opened during power loss / emergency. I am a Tesla Model 3 owner in California. I discovered that the rear doors do not have a mechanical emergency release. Tesla service confirmed that my vehicle does not include any manual rear emergency door release system. This raises a serious safety concern because, in the event of a crash, fire, or 12V power failure, rear passengers—especially children or elderly occupants—may be unable to exit the vehicle. I request NHTSA to investigate whether the absence of a mechanical rear emergency release complies with FMVSS 206 and whether Tesla’s design poses a safety risk during emergency egress.

I purchased a new 2025 Tesla Model 3. During normal driving conditions, the rear bumper detached without any collision or impact. The bumper became loose while the vehicle was moving, creating a potential safety hazard. Tesla Service inspected the vehicle but declined warranty coverage and refused to properly repair the issue. The bumper was only temporarily reattached and remains misaligned. Additionally, shortly after purchasing the vehicle, the seat belt assembly had a failure and had to be repaired by Tesla under warranty. While Tesla addressed that issue, it adds to my concern about the overall quality and reliability of a brand-new vehicle. Because the vehicle is new and the failure occurred during normal driving, I believe this may indicate a defect. I am requesting assistance in reviewing this situation. The vehicle presented an error that the audio system was offline, and that the horn, speakers, and internal safety sounds may not work. After driving on the PA turnpike and almost getting in an accident, this error causes the horn to not functioning at all. I was not able to alert the other driver due to this issue, and had to therefore slam on the breaks due to this issue which caused a safety risk to myself and other drivers on the road. This issue has been consistent since the error originally appeared a week ago. The horn does not function, as well as the reverse pedestrian safety sound that alerts pedestrians of the vehicle reversing. In the interior, no safety chimes, audio, turn signal clicks are audible and therefore are not able to be perceived. After putting in a service request and being told I was to be scheduled 2 weeks out and not provided a loaner, I called the dealership whom confirmed this, and stated they are not able to provide a loaner vehicle or move the appointment closer regardless of their being a potential safety issue. There are no other warning lights on in the vehicle, and the alert popped up out of no where, as well it does not mention the horn *does not* work, only warning that the horn may not work.

- The rear middle seatbelt became locked and will not unwind or extend. The seatbelt remained stuck in the retracted position, making it unusable. The vehicle and seatbelt assembly are available for inspection upon request. However, Tesla has already fixed the issue, but I feel we were wrongly charged because I believe this should be a recall. I searched this issue online, and others have reported the same issue. Because we rarely have the need to use our rear seats, it wasn't until years later we discovered this issue. - Because the rear middle seatbelt cannot be extended, that seating position cannot safely be used. Any passenger sitting in that position would not have a functioning restraint system in the event of a collision, creating a safety risk. - Yes, Tesla service inspected the vehicle and confirmed that the rear middle seatbelt was locked and not functioning properly. - The vehicle has been inspected by Tesla service. It has not been inspected by police, insurance representatives, or other third parties. - No warning lights, alerts, or messages appeared prior to discovering the issue. The seatbelt was found to be locked when we attempted to use it, and it would not extend.

An incident that occurred while Full Self-Driving (FSD) was engaged. During the drive, the vehicle unexpectedly steered toward the curb while making a right turn. I was actively monitoring the vehicle and immediately took control of the steering, but the vehicle still made contact with the curb. After the incident, the vehicle developed steering misalignment, vibration while driving, and abnormal noise, which suggests possible suspension or wheel damage. Since FSD was engaged at the time, I would appreciate it if Tesla could review the vehicle logs and investigate whether there was any system malfunction or unexpected behavior from the FSD system.

My 2022 Tesla Model 3 experienced a heat pump compressor failure at approximately 89,457 miles. Tesla Service Center diagnosed an internal failure in the heat pump system. The heat pump system controls cabin heating, windshield defrosting, and thermal management for the vehicle. When the system failed, the vehicle lost heating and proper defrost capability. This can impair windshield visibility during cold or foggy conditions, creating a potential safety hazard while driving. Tesla quoted approximately $3,000 to repair the compressor and declined warranty assistance despite the system being critical to safe vehicle operation.

I continue to experience phantom "forward collision avoidance" warnings. Tesla has looked at this several times, even once charging me $60 to clean the cameras (should be under warranty in my opinion). Latest response to problem: known issue, no known hardware cause. In other words live with it. They even suggested buying FSD package at $1200/yr as a solution! One more than one occasion the TACC and Autosteer disengaged with traffic behind me. Very scary.

The Michelin tires used for this particular model have a foam issue. This foam is put by Michelin to reduce road Noice and they are coming apart thus creating a rattle in the car making it a hazard while driving on the highway. Tesla replaced one of my tires on May 27, 2025 free of charge and told me it was a known issue. Today 2/25/26 I have the same issue and they're quoting me almost $400. I told them about the tire issue and why there's no recall and they told me to contact NHTSA. They just want to change them one at a time until warranty expires so they don't have to replace all defective tires. I guess they're waiting for somebody to have an accident and sue them to fix the problem.

While using Tesla's Full Self-Driving (Supervised) software on the Pacific Coast Highway (PCH) and other highways, the vehicle’s automated steering system repeatedly malfunctions by failing to maintain lane centering. Specifically, the car drifts or actively steers across double yellow lines into the lane of oncoming traffic. This issue is a persistent failure of the Advanced Driver Assistance System (ADAS) and Lane Keeping Assistance. The vehicle has attempted this maneuver multiple times at [Approximate Speed, e.g., 40-60 mph], requiring immediate and forceful manual steering intervention to avoid head-on collisions with oncoming vehicles. The system provides no warning chimes, error messages, or "Take Over Immediately" alerts prior to crossing the lines; it appears to perceive the oncoming lane as a valid path or fails to recognize the double yellow boundary entirely. This has occurred during [mention conditions: e.g., bright daylight with high glare / night time]. The vehicle is equipped with Hardware 3.0 (HW3). I was using the vehicle's Autopark feature to parallel park on a public street in a normal/spacious parking spot. I selected the parking spot on the center screen and started Autopark. During the maneuver, the vehicle reversed into the parked car behind me and caused a dent (low-speed crash). I was supervising the maneuver and did not press the accelerator or brake before impact. After the incident, I reviewed dashcam footage and vehicle telemetry. The rear vehicle was clearly visible in the rear camera view. Telemetry shows the vehicle had already shifted to Drive before impact, but it continued moving backward for about 1 second and then contacted the parked car behind. Telemetry also shows accelerator pedal position was 0 and brake was not applied during the impact sequence. This appears to be an automated parking/close-range obstacle stopping failure or delayed stopping response. No warning lamp or prior vehicle fault message was observed before the incident. The issue has not been reproduced since the incident. I have dashcam footage and telemetry logs documenting the event.

The rear camera malfunctions intermittently and causes other errors to happen, like Automatic emergency braking unavailable or forward collision warning unavailable. When the camera error happens, it says the camera is unavailable. I took my car to the Tesla dealership three times. The first time, they did a continuity check and said the harness was degraded, and they replaced the harness and the rear camera. A week later, it happened again. The second time they reset the camera calibration and it worked. The day after, it happened again. The car is at the Tesla dealership for the third time (02/20/2026).

I am filing a safety complaint regarding water intrusion into the rear tail lamp assembly of my 3-year-old Tesla Model 3. The rear tail light has progressed from minor condensation to active water accumulation and visible pooling inside the sealed housing. This is not light fogging — there is standing moisture inside the assembly. The vehicle has not been involved in any collision and there is no visible external damage to the lamp. I have already been pulled over, and drivers behind me have indicated that the affected tail light is not clearly visible. Reduced rear illumination creates a serious safety risk, particularly at night or in poor weather conditions. A malfunctioning tail lamp increases the likelihood of a rear-end collision. Rear lighting is a federally regulated safety component. Water intrusion into a sealed LED lighting assembly creates risk of electrical malfunction, corrosion, and sudden loss of rear illumination. The vehicle is currently out of warranty due to mileage; however, it is only three years old. A sealed LED tail lamp is not a wear-and-tear item and should not fail due to normal mileage accumulation. This appears to be premature sealing failure. I contacted Tesla Service regarding this issue. Their written response stated that because the vehicle is out of warranty due to mileage, any repairs would be at my expense. They did not address the safety concern or the presence of internal water accumulation. There are numerous reports from other Model 3 owners describing similar tail lamp water intrusion issues. I am requesting that NHTSA review whether water intrusion into sealed rear lighting assemblies constitutes a safety defect and whether further investigation is warranted.
