FORWARD COLLISION AVOIDANCE: ADAPTIVE CRUISE CONTROL, FORWARD COLLISION AVOIDANCE: WARNINGS, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
When driving with the adaptive cruise control on, the cruise will shut off without notice at random times. The forward collision warning light then comes on. After driving for some time the light will go out and the adaptive cruise will work again. This repeats many times on a trip randomly. This also leaves the vehicle without forward collision protection and automatic emergency braking. This is very dangerous at highway speed when you lose power driving with traffic. When reported to the dealer, they said it was a known issue without a solution and to not use the adaptive cruise and drive carefully without the forward collision protection active. This has been an issue since the car was new in November of 2024.

ENGINE, ELECTRICAL SYSTEM, FORWARD COLLISION AVOIDANCE: WARNINGS
ENGINE MALFUNCTION SERVICE NOW APPEARED (ERROR) I took the vehicle to the dealership for evaluation. The mechanic checked, reset, and reprogrammed the module. I was instructed to return if the error occurred. It did after driving 5 miles. The mechanic rechecked for possibly having different errors. I was instructed that it was okay to drive with the errors. Unfortunately, after coming to a stop, I was unable to accelerate! I was on a major road and I was unable to move off to the side of the road. The vehicle in this state is life-threatening. (A tanker truck thankfully was able to stop!) I was able to limp to the dealer where I purchased the vehicle. NOTE: This has been a well-documented issue (Check OUTLANDER FORUM). My 2024 Outlander briefly had the errors, but they cleared themselves. I feel this needs expedited to severe, requiring a mandated software update before a life is lost! This error inhibits frontal collision sensors!

2024 Mitsubishi Outlander PHEV brake-by-wire failure during a downhill U-turn at 100% battery state of charge. Pedal went completely soft to the floor, unable to stop, causing a crash.

SUSPENSION
I’ve had it looked at twice at two different places and they don’t see anything wrong with it but when you hit bumps, especially going high rate of speed, it feels like you’re gonna lose control of the backend. When I looked online, there’s multiple people with the exact same issues and they take it in and they see that everything‘s tight and nothing’s worn out but even my daughter doesn’t like driving our car going down the highways because it feels like you’re gonna lose control. My car doesn’t even have 50,000 miles on it and I just bought it. The back end feels like it kicks out to the side.

WHEELS
I took my outlander in on May 6 2026 for routine oil change , tire rotation, basic multi inspection to bruce walters service (same company i bought it from new 15 months ago). Inspection was flawless with green light on tires, brakes, the works. It had approx 24k miles on it. When I left the dealer that day I noticed A LOT of roaring, slight vibration, worse when I cut wheel left. I called them and they could not inspect the car for 28 days , appt they made was june 2nd 2026. This only started when they rotated tires. At my appt yesterday, June 2nd, 2026 they charged me 150 diagnostic and said I need 4 new tires even though my tread was awesome. 25k miles on it by the time of this appt. I've owned it year and half. The tire walls are bad but green light on tread..all 4 tires. The mitsubishi lot said that shouldn't happen and sane companies service dept should have done something. But no, they are wanting me to pay almost 1k for new tires, a 25k mile brand new car should not need tires. Service dept blames brand of tires. They did not check, to my knowledge, suspension or shocks etc. Something caused a new car to do this or faulty tires , all 4 , not just 1 or 2.

The contact owns a 2022 Mitsubishi Outlander. The contact stated while driving at an undisclosed speed, the accelerator pedal was depressed, however the vehicle failed to accelerate as intended. The RPM's increased. In addition, the vehicle hesitated and jerked. o warning lights were illuminated. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The dealer was made aware of the failure. The contact has an upcoming diagnostic testing appointment scheduled. The contact referenced an unknown recall; however, the VIN was not associated with a recall related to the failure. The manufacturer was not made aware of the failure. The failure mileage was approximately 70,000.

ENGINE
What component or system failed or malfunctioned, and is it available for inspection upon request? The vehicle experienced an apparent engine failure involving the engine cooling system and possible head gasket and/or engine damage. Prior to failure, the vehicle had a strong sweet coolant smell and was later diagnosed by the servicing dealership as having a coolant leak and possible engine-related damage requiring engine replacement. The vehicle is currently located and available for inspection at Don Herring Mitsubishi. How was your safety or the safety of others put at risk? While driving after leaving the dealership, the vehicle began jerking, hesitating during acceleration, and losing power unexpectedly. A flashing check engine light briefly appeared and disappeared. The sudden loss of normal engine performance created a serious safety hazard because the vehicle could have stalled in traffic, increasing the risk of an accident or collision. I also have eight children, including [XXX] twins, and the loss of transportation has prevented me from reliably getting my children to school, medical appointments, and obtaining necessities. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Don Herring Mitsubishi inspected the vehicle and identified a coolant leak and possible major engine damage. The dealership advised that the engine may need replacement. The warranty company, CNA Extended Warranty, stated the dealership’s submitted notes referenced a blown head gasket and requested further teardown and inspection before approving repairs. The vehicle remains unrepaired while awaiting further action and/or engine replacement. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? The vehicle has been inspected by Don Herring Mitsubishi and reviewed through CNA Extended Warranty as part of the repair claim process. SEAT BELTS, STRUCTURE:BODY
The seat belts are much slower to retract than with other cars, including our 2019 Mitsubishi Outlander we also own. My wife has had the front passenger seat belt get stuck in the door at least 3 times (likely more) which has required us to make multiple stops at the dealership to fix it. This is certainly a safety risk since the door remains partially open this certainly is something that should be recalled and fixed. Most recent day was today 5/7/26 but has happened many times.

Automatic Emergency Breaking sometimes occurs while driving in rainy weather even when there are no obstructions in the front of the vehicle. This causes spinouts and exccessive wear and tear on break pads and rotors. Also, many shops and dealerships are having issues with putting 2022 year model outlanders in "maintenance mode" to be able to change rear ebs brakes. Due to this you cannot drive your vehicle and/or rish not having proper break functions. THIS IS URGENT!!!

STEERING
I was parked in a parking spot and when returned to my car and turned my car on a loud bang came from the front of the vehicle. Both of my front tires were facing inward. The drivers side front tie rod snapped with a clean break for no reason. The vehicle was towed to the dealer where after inspection there was no obvious reason for this to happen, no damage to the tire, aluminum wheel, or any other component under the car. There was no warning of any kind, the vehicle was driving perfectly smooth and not pulling to one side just before this happened. The vehicle and damaged tie rod are available for inspection. Needless to say If i were driving at highway speeds there could have been catastrophic injury. As of right now Mitsubishi only viewed pictures, I asked them to inspect the vehicle but they did not. I would prefer NHTSA inspect the tie rods because if this happened while parked the tie rod could be defective and dangerous.

Following sustained highway operation and varied elevations in tropical conditions, the vehicle’s Auto Start-Stop system may activate during traffic congestion. In certain instances, the battery warning light illuminates upon engine automatic restart. If the auto start stop is activated once more on the engine restart the battery warning light will turn off. I went to the dealership and they wanted me to leave the vehicle for diagnostics or kept using it which will change the Mild Hybrid battery and avoid recurrence. But the issue keeps happening.

The contact owns a 2024 Mitsubishi Outlander Equipped with Nexen Roadian GTX in size 255/45R20. The contact stated while driving the vehicle at 30 MPH she noted her tire pressure was very low, she pulled into a gas station to fill it with AIR, she drove the vehicle home, and next day the tire was flat again and was filled back up. The vehicle was taken to a independent mechanic where it was diagnosed to have pieces of tires coming off with no actual puncture of the tire. The tires were replaced at owners cost. The manufacturer was not notified of the failure. The failure mileage was 27,000.

BACK OVER PREVENTION: SENSING SYSTEM: CAMERA
Vehicle: 2025 Mitsubishi Outlander PHEV I am filing this safety complaint regarding a recurring electrical/software defect in my 2025 Mitsubishi Outlander PHEV affecting the backup camera display and infotainment screen. The backup camera screen repeatedly freezes and goes completely black while the vehicle is in reverse gear. This eliminates all rear visibility during backing maneuvers, posing a serious risk of striking pedestrians, cyclists, or objects — especially children in blind spots who cannot otherwise be seen. In addition, the infotainment screen has shut off entirely at least once while the vehicle was being driven, creating a dangerous distraction and loss of vehicle controls. I want to be clear that my dealership has acted in complete good faith and has done everything within their power to diagnose and resolve this issue. They have worked diligently on my behalf and are not the source of the problem. The failure to provide a fix lies squarely with Mitsubishi Motors North America, which has been unable or unwilling to supply an adequate repair, software update, or resolution despite being made fully aware of this ongoing safety defect. I have contacted Mitsubishi Motors North America directly on multiple occasions. They have failed to provide a satisfactory explanation, a committed timeline for a fix, or any path toward resolution. This is not a dealer-level issue — this is a manufacturer-level failure to address a known defect. This is not a convenience issue — the failure of a federally mandated backup camera system is a safety defect. Federal law (49 CFR Part 571, FMVSS 111) requires rear visibility systems to function properly at all times. I respectfully request that NHTSA open a formal investigation into this defect and determine whether a recall or technical service bulletin is warranted for the 2025 Mitsubishi Outlander PHEV.

POWER TRAIN:DRIVELINE:DIFFERENTIAL UNIT, SUSPENSION:REAR
The contact owns a 2023 Mitsubishi Outlander. The contact stated that while driving at an undisclosed speed, the rear of the vehicle would move independently. The contact stated that it felt as if the vehicle were going to slide and eventually would lose control. No warning lights illuminated. The vehicle was taken to the dealer, where it was diagnosed that an unknown rear chip had failed. The shock absorbers had water, the rear brake pads were worn down to zero, and the rear differential needed to be replaced. The vehicle was repaired. The contact stated that the failure persisted and was worsening. In addition, the contact stated an abnormal grinding sound could be heard coming from the rear differential. The vehicle was taken back to the dealer, where it was diagnosed that the differential gauge had failed to function as intended. The warning light for the 4-wheel drive was illuminated. The brakes and calipers were replaced. The rear differential was not repaired. The dealer informed the contact that the pole where the breather pipe is inserted on the rear differential carrier is off spec, allowing water intrusion. And that the new rear tires were badly worn on the inside and had high spots on the center. The manufacturer was not made aware of the failure. The failure mileage was approximately 46,000.

Good flutter at speeds above 45 mph or high winds. NTSB issued but not notified, recall should be issued as this is a safety concern, called dealer who stated a tsb was issued but I was not notified. Due to being out of manufacture warranty I will have to pay cost of hood replacement. Mitsubishi is not responding to my outreach. I first notified dealer of concern after purchase and was not informed of tsb, I was told it was safe and a design flaw.

The EV portion of the PHEV is often not providing power to the motors, even though, there are still plenty of battery power available. The vehicle sometimes gave "Power Reduced" warning that usually went away after 30 minutes, and the vehicle cannot go into EV mode. When in EV mode and just cruising on the highway, it barely has power to keep up with traffic and sometimes it is losing speed just on a slight incline on the road causing me to move over or take it out of EV mode just to get power from the gas engine. In one occasion, I driving in EV mode and merging onto the highway. I stepped on the gas and the car was barely, if at all, accelerating. Fortunately, there wasn't vehicle coming from the highway.

The contact owned a 2022 Mitsubishi Outlander. The contact stated that while driving at various speeds and depressing the brake pedal, the vehicle wobbled and failed to respond. The contact stated that upon forcefully depressing the brake pedal, the vehicle responded as intended. There was no warning light illuminated. The failure mostly occurred while driving at highway speeds. The vehicle was taken to the dealer where it was diagnosed, and determined that the braking system needed to be replaced. The dealer informed the contact that the repair was not covered under the two-year warranty. The vehicle was not repaired. Additionally, the contact stated that the vehicle failed to respond intermittently while attempting to drive from a complete stop on the roadway. There was no warning light illuminated. The vehicle was not repaired. The manufacturer was notified of the failure and referred the contact to the dealer for assistance. The failure mileage was approximately 25,000.
