The contact owns a 2025 Lexus RX 350. The contact stated that while driving into a parking space, the vehicle suddenly experienced unintended acceleration and drove forward at a high rate of speed before crashing into the brick wall of an office building. During the crash, no air bags deployed, and the brake pedal was hard when pressed and did not stop the vehicle. Also, the manufacturer's APP indicated a malfunction in the engine and engine throttle during the incident. The driver sustained injuries to the knees, arms, and hips, which required medical treatment. An incident report was filed by the facility Patrol Officer, and the vehicle was towed to the collision center. The cause of the failures was not determined. The manufacturer and local dealer were notified of the incident. An engineer from the manufacturer was sent to examine the vehicle and reported no trouble found. The contact indicated that upon delivery of the vehicle, several electronic features did not function properly, including the display and navigation screen. The manufacturer and local dealer were notified of the failure. The failure mileage was 106.

HI, I purchased a brand new 2022 Lexus RX350 from Hendrick Lexus of Charleston SC. Since day one I had dangerous issues below: 1. The transmission shifts up and down so hard, I've almost gotten rear ended due to the car and not braking. This I was told by two separate Lexus dealers I'm feeling a hard "transmission gap"-whatever that means. To add fuel to this dangerous defect is my autistic son would need to be specially medicated to ride in the car b/c he panics and tries to jump out. 2. The car also is equipped with a "rear kick sensor" that never worked, The service manager in Charleston kicked my bumper and swore at the car. I had another Service tech on video for minutes unable to open it. 3. The car, scarily, will not start on me or recognize the key. I've been 3 hrs from home and nearly stuck in a grocery store parking lot with my autistic son freaking out again. This is documented on video. 4. As if there could even be more, the car is equipped with rear collision warning. An example of this, is backing up in a grocery store parking lot and it senses someone walking and auto stops the car if I'm not already breaking. Well, last Sept I was moving, had my autistic son's anxiety super high, and hit a POD for moving in my driveway doing maybe 2 miles per hour. But, and the big but is why did the pre-collision not kick in. This little bump cost insurance over $15k and yet Hendricks is saying its not the car! The mysognostic behavior I've been privy beyond demeaning and offensive. 5. The big kicker is the car went to Hendricks Collision as I wanted a certified Lexus dealership to fix it and do my 30k service. They would only do the 30k service, service writer-again another mansplainer didn't capture all my three continuous problems properly, filled the RO out days after I dropped the vehicle last Nov of 2024-yes, a YEAR AGO. Very poor quality front windshield, very fragile. Breaks and chips off very fast even with small rocks which impairs visibility even after repairs leading to replacement of front windshield due to poor visibility especially glare from the sun. Vehicle now has a lots of rock chip damage making to a position to costly repair from $3000 to $5000

Was pulling into parking spot when car went haywire and started accelerating on its own. It would not stop until it has hit 5 cars and crashed into a cement wall. The brakes did NOT stop the acceleration. This occurred at the exact same time there was an incoming call which was not answered. The airbags did NOT deploy. The car appears to be totaled and has been towed to a State Farm affiliated auto yard. There were no indicator lights that something was wrong BUT the Lexus app showed 5 warnings that came in to my text messages same day. These 5 alerts are indicative of exactly what occurred.

Wireless charger caused iPhone 12 to overheat. Picked phone up from charger before it became to hot to handle. Dealership admits issue, "there has been a significant improvement in wireless charging in our cars over the past couple of years. The wireless charging on our earlier models do have connectivity issues and sometimes may even overheat, especially in iPhone. This is a known issue with no resolution currently, unfortunately. I hope this helps." If the dealership recognizes an issue with overheating, I suggest a recall before the charger or phone starts a fire.

I was at the gas station, in line waiting for one of the three gas pumps to be available. I put my car in reverse to back up and pull around another car in order to go to the open pump in front. When my car was in reverse motion, I stepped on the brake pedal, but the car did not stop. I stepped on the brake pedal numerous times, but the brake did not function. My car backed into the front of the car in line behind me damaging both vehicles. The automatic emergency braking system did not activate, and there was no rear collision warning sound or message. I switched my car from Reverse into Park to stop its movement, but it was too late to avoid the collision. I had a passenger in my car who corroborates this description. Fortunately there were no pedestrians behind my car, so the only damage was to the vehicles. My vehicle has not been inspected by anyone since the accident, but will be soon. I have owned this car since April 2021 and previously had a few similar situations when moving in the forward direction. Previously, the brake problem occurred while moving forward at very low speed pulling into parking places. The brake failed to activate, and I had to throw the car into Park to stop it, but this was the first time I had experienced this problem traveling in reverse. When this situation occurs, I am pressing on the brake pedal, but the car is not stopping and the brake pedal seems to go to the floor. On March 5, 2025 I took the car to a Lexus dealer and reported the brake failure problems. They inspected, road tested, and checked for codes, but could not identify the problem. I believe that the collision back up system should have prevented this accident, but it did not function. I suspect that there may be some kind of computer issue that prevents the brake from actuating. The car will be going in for repair soon, and I have requested that a complete diagnostic review be performed of this brake problem.

When we purchased this car in June 2024 it did not have an engine revving issue. The engine, electrical or transmission maybe all of them have SERIOUS SAFETY ISSUES! When driving and you let off the gas pedal the engine starts revving very loudly, it scares the heck out of you because you don't know if you can stop! I have a serious mechanical background, there is always the possibility of the transmission not disengaging and forcing the car NOT to stop! This issue is VERY DANGEROUS AND SCARY! We get very nervous driving a car we paid almost 70K for! Yes reported to the dealer Mungenast Lexus of St. Louis, yes they have duplicated the issues. After keeping our car now on two different occasions for weeks at a time they went to Lexus engineers who are now trying to say its normal! if so, why didn't it start doing the revving until there were thousands of miles on the car! THIS IS A COVER UP! Not been inspected by the manufacturer yet, they only spoke with them. No reason for police or insurance yet. No, there has never been a check engine light appear. The only other thing the car did, after the first time of it revving like this it died when we were pulling out in traffic, it didn't die it actually shut off! This has only happened the one time, it shutting off, can it do it again you bet! These cars are dangerous, I hope nobody has to die to prove this!

Lexus RX Sunroof exploded on it's own. I just left my driveway and was the only vehicle on the road. There were no projectiles, the sunroof glass simply exploded and glass flew into the air. if I was driving at 60 MPH, the exploded glass would have become a projectile and could seriously hurt a bystander. They were no warnings. I filed a complaint with Lexus Reference # 250820001020 but their response was that this was not a mfg issue. I pointed out that they are two class action suits on Lexus, but they insist that this is not a recalled part. This is a safety issue with glass flying at 40 to 60 MPH in the open air.

While driving on the freeway the sun roof "exploded" shattering glass out onto the highway. The noise was loud and frightening. Upon pulling over I observed most of the sunroof was gone with fragments of glass remaining along the edges of the roof. I believe this to be a major safety issue. If the ceiling cover had been open, the passenger in the rear seat could have been seriously injured. No doubt flying glass is a danger to any cars following. The vehicle was inspected by the Lexus dealer who declined to cover the damage under warranty. My insurance (CSAA) agreed to pay the cost of replacement. The glass shop which did the work indicated this is a manufacturing defect and indicated they replace 5-6 sun roofs a week.

Front wiper motor damaged two times in fourth years. First time on 2023 and was replaced by warranty at the Lexus Dealer and the second time was damaged on August 22, 2025. Now it is damaged but not covered by warranty at this time with 52,000 miles on the car. The wiper motor was damaged and it can not move down to hide inside down the hood, when the front wipers on they moved and the wiper hits the hood and peel the hood's paint.

gas gauge very inaccurate. manual calls for 'low fuel' at 2.6 gallons, actually comes on between 7 -8 gallons left in tank. Dealer accepts it is a problem but has no solution. attempting to get full range may leave car out of gas on highway, since gauge cannot be trusted.

We live on dirt road in Kansas, and must drive 2.5 miles to get to paved road. This RX 500h Sport does NOT have proper gaskets around the doors. We can actually see dust rolling into the front and back doors as we drive. We have had numerousLexus RXs,& GXs over the years and never experienced a dust problem at this level. Again, we can actually see dust clouds entering the car as we drive down our daily 2.5 dirt road trips.

My 2021 Lexus RX 350 (mileage approx. [19,000], has developed a severe mold contamination issue caused by HVAC condensate drain failure. This issue is a known design defect covered by Lexus Technical Service Bulletin L-SB-0020-21, which identifies blockage or poor drainage in the HVAC system that allows water to enter the cabin. As a result, water intrusion under the carpet has led to mold growth, making the vehicle uninhabitable and unsafe to drive. The cabin now contains visible mold and musty odor, raising serious health and air quality concerns. I brought the vehicle to Lexus of Route 10 (Whippqny, NJ), who acknowledged the water intrusion but have refused to repair it under warranty or goodwill. Instead, they are pressuring me to file a personal insurance claim, even though the issue stems from a known factory defect. The vehicle has now been out of service for weeks, and I am driving a loaner vehicle while mold continues to worsen in mine. Lexus Corporate stated that they "no longer goodwill" these repairs, despite the ongoing safety and hygiene risks. There has been no recall or warranty extension, and I believe other owners may be similarly affected by this unresolved defect. This is a serious safety issue due to the potential for mold-related health problems, interior air contamination, and possible electrical damage from long-term moisture. I urge NHTSA to investigate whether this HVAC defect poses a broader consumer risk, and whether Lexus is avoiding its responsibilities to repair or disclose this issue to other drivers.

I experienced a sudden release of toxic fumes from the vehicle between the steering column and instrument panel area on June 29, 2025, while driving. The fumes struck my face, caused immediate bitter chemical-like taste and made me feel physically ill and extremely concerned for my safety. The fumes were strong enough to leave a lingering taste described as “bitter and green” with initial symptoms of headache, lightheadedness, nausea, vomiting green substances, eye irritations, nose and throat irritations. The local auto dealership inspected the vehicle, no mechanical defect was identified, and they noted that they cannot reproduced the event that led to the release of the toxic fumes. However, the vehicle manufacturer took possession of the vehicle for 1 week to conduit further inspections

The driver turned off the vehicle and the vehicle restarted after several attempts. The vehicle was driven to the shoulder of the roadway. The contact stated that the failure had occurred twice. The contact stated that the transmission was previously replaced in January 2025. The vehicle was not diagnosed or repaired by an independent mechanic or the dealer. The manufacturer was made aware of the failure. The failure mileage was approximately 7,500.
