- #11771981October 18, 2026UNKNOWN OR OTHER, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Description: This is my second NHTSA complaint regarding unprompted automatic emergency braking in this vehicle. While driving at normal speed on [XXX], with no obstacle, pedestrian, or vehicle ahead, the vehicle's forward collision system activated without warning — audible alert sounded followed by the vehicle automatically applying hard braking and coming to an abrupt stop in the roadway. There was no obstruction, stopped traffic, or hazard present to justify the activation. I had 4 passengers in the vehicle at the time, including two children. The vehicle came to a sudden, forceful stop, creating a risk of rear-end collision from following traffic and a jolt risk to occupants. This is a recurring defect — I filed a prior NHTSA complaint for a similar unprompted braking event [reference no-XXX ] that resulted in a shoulder/neck injury to me as well as headache. Both incidents are documented on dash camera video and audio, including the audible system alert preceding each braking event. I dropped off the vehicle at Thousand Oaks Honda to fix the safety recall on Monday September 28, 2026. Recall Number: TN2 XNT XP2. While they are trying to fix the recall at the dealer's place Software crash happened in the middle of the process according to the Hyde who handled my case in the front office. The whole vehicle is not operatable. They tried to fix in on the same day but failed. I was returned back the vehicle on 30th, September, 2026 with the attached certificate for DMV(Case #[XXX]) approval for the new year registration. When my wife & her sister while driving the vehicle in the busy street in Simi Valley yesterday [XXX] evening around [XXX] The car suddenly stopped in middle of the street in the traffic with so many messages on the front screen of the vehicle, they are about to take [XXX] freeway in few minutes. With the help of a people the vehicle is pushed to near by gas station. Then it was towed by Honda USA to Thousand Oaks Honda dealership. By keeping my vehicle nearly 3 days at Thousand Oaks Honda, they were unable to take my vehicle in suitable operational state without safety issue. The driving of this vehicle is safety issue & fatal incidents could have happened if it operates in the middle of the freeway at that time. This vehicle has safety issues in driving California roads INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

I was driving on an interstate in Florida on vacation while using my GPS which was showing on my cars screen. As I was following directions the screen went blank. This caused me to not know which exit to take as it came across exits and while cars following behind me the lost of GpS guidance in the mist of it giving me a direction caused a panic. I use GPS systems all the time and that was the reason the car was purchased because of the gps system and screen. I have been having issues with this screen going out several times and have reported to my dealership and even wrote a letter to Honda’s main office. I was advised by the main office that this is a known issue and there is no fix for it at this time. I have been told by the dealership since I purchased the car in 2023 maybe after 2 months of having the car I experienced issues with this car and screen. It goes blank and volume controls stop working and all I’ve heard from them is it is a known issue when no fix at this time. This issue could have caused me to get into a bad accident due to face the screen going out without any warning while driving on an unknown interstate with vehicles driving between 70-80 miles per hour.

The contact's fiancée owns a 2023 Honda Accord. The contact stated that the rear tires showed severe inner wear and damage due to a misalignment, requiring three sets of rear tires to be replaced within a year. The vehicle was taken to a dealer, who notified the manufacturer of the failure; however, the technical engineer denied assistance. The dealer informed the contact that the lower control arms needed to be replaced. The vehicle was not repaired. Additionally, the contact stated that the tire repair kit bottle cap detached while inflating a tire. The contact received notification of NHTSA Campaign Number: 26V366000 (Tires); which was associated with the tire repair kit failure. The manufacturer was notified of the failure. The failure mileage was approximately 47,000.

- #11753519July 29, 2026STEERING, ELECTRICAL SYSTEM, FORWARD COLLISION AVOIDANCE: WARNINGS
I own a 2023 Honda Accord Touring Hybrid. At approximately 42,000 miles, all of the warning lights illuminated while driving. The warnings included Adaptive Cruise Control, Forward Collision Warning, Lane Departure Warning, Blind Spot Warning, Parking Collision Warning, and multiple other system alerts. At the same time, I experienced a loss of electric power steering assist, increased steering resistance, and clicking/popping noises coming from the steering rack. The Honda dealer diagnosed the vehicle with a failed Electric Power Steering (EPS) steering rack and quoted nearly $5,000 for the repair. A critical steering component should not fail at such low mileage. This is a serious safety concern because the failure affects the driver's ability to safely control the vehicle. Similar steering components have been recalled on other Honda models, but the 2023 Honda Accord has not been included. I believe this defect should be investigated before it results in crashes or injuries.

I am reporting what I believe may be a safety defect involving my 2023 Honda Hybrid. I owned the vehicle for less than three years and kept up with all recommended maintenance. During my ownership, I repeatedly experienced sudden tire pressure warnings and rapid drops in tire pressure. Because I was concerned, I took my vehicle to a Honda dealership approximately four to five weekends in a row to have the tires inspected. Each time I was told that everything appeared to be normal and no problem could be identified. In March 2026, while driving on the freeway to school, the tire pressure suddenly dropped again. I then heard a very loud popping noise and initially believed that my tire had blown out. Immediately afterward, I lost control of the vehicle and crashed, and my car was declared a total loss. After the crash, I observed that the wheel had turned inward. I was later told by the responding police officers that the wheel had turned inward rather than the tire simply blowing out. I decided to report this because I have learned of other people with the same model who described similar problems involving sudden tire pressure loss or repeated tire warnings despite being told by dealerships that nothing was wrong. While I cannot verify the cause of those incidents, the similarities make me concerned that there could be an underlying safety issue affecting other vehicles. I respectfully request that NHTSA investigate whether there may be a defect involving the tires, tire pressure monitoring system (TPMS), suspension, steering, or another related component. I am willing to provide any additional information requested. Also I don't know if this was the correct channel to go through, I have never reported something like this before. I also send Honda a longer email detailing everything, so I hope they are aware of this issue. Please contact me if you have more questions, I want to help any way I can. While driving my 2023 Honda Accord Hybrid, the vehicle suddenly displayed multiple warning messages, including a Check Engine Light, and immediately entered limp mode. Engine power was significantly reduced, making it difficult to accelerate and safely keep up with surrounding traffic. This unexpected loss of power created a potential safety hazard because the vehicle could not respond normally to throttle input. The vehicle was diagnosed by an authorized Honda dealership. The dealer confirmed Diagnostic Trouble Code (DTC) P2101 and determined that the throttle body assembly had failed. The throttle body assembly and gasket were replaced, and a software update was also performed before the vehicle could be safely driven again. I paid approximately $670.34 for the repair at the Honda dealership. After researching this issue, I found reports from other owners of recent Honda Accord Hybrid vehicles experiencing similar P2101 throttle body failures resulting in limp mode. I am submitting this complaint because an unexpected throttle body failure that causes a vehicle to enter limp mode while driving may present a safety risk, especially if it occurs in traffic or during highway driving. I believe this issue should be reviewed to determine whether it affects additional vehicles.

The car has continually asked to to be updated through the driver control center screen. IT states that this is a dealer update and must be brought in to have this done. Until this update is completed the above systems will not work. I have brought the car in for the lat 25k miles 4 oil changes and have been told each time that it has been fixed. It still has not been fixed, and the dealer is telling me there is nothing they can do. This is definitely a safety issue as these important systems are not working.

Before the collision, while turning left from a complete stop, the steering wheel suddenly became extremely stiff and would not turn normally. There were no dashboard warning lights (no EPS, brake, or hybrid warnings). The steering problem occurred before impact and contributed to the head-on collision.

Component/System: Integrated Control Module (ICM) CPU — software defect identified under NHTSA Recall 25V-785 (Honda Recall TN2). The vehicle is available for inspection upon request. Safety Risk: While driving on a California highway, I attempted to accelerate to pass another vehicle and the car failed to accelerate normally — it did not respond with expected drive power. This is consistent with the recall defect, which states the ICM CPU can reset while the vehicle is in operation, causing the vehicle to lose drive power and increasing the risk of a crash. This occurred at highway speed with other vehicles present, creating an immediate danger. Dealer Confirmation: I have visited an authorized Honda dealership twice regarding this recall — once upon receiving the recall notification in November 2025, and again in January 2026 during a scheduled oil change. On both occasions, the dealer confirmed the recall was on file for my vehicle but stated they did not have the software or equipment available to perform the remedy. The repair has not been completed as of the date of this complaint, June 26, 2026 — more than seven months after the recall was issued. Inspections by Other Parties: The vehicle has not been inspected by police, insurance representatives, or other outside parties in connection with this issue. Warning Lamps or Prior Symptoms: No warning lamps or dashboard messages appeared prior to or during the acceleration failure incident. The recall defect relates to an internal software condition that does not appear to trigger a visible warning before the power loss occurs. The highway incident described above is the only symptom I have personally observed that may be connected to this defect.

There were no warning lamps or messages prior to the failure. My family was out of town, when the fire occurred. However, the neighbors were home and their home also sustained some damage. My home was a total loss. On June 16, 2026 I received a call while out of town that my home was on fire and it was a total loss. Statements from the neighbors and fire marshal were that the garage was on fire and then spread to the rest of the home. The only thing in the garage that could have started the fire was my Hybrid vehicle.
