My 2025 Honda Accord Hybrid Touring has an unresolved tire repair kit safety recall, NHTSA 26V-366, Honda campaign DOV. I contacted Bay Ridge Honda in [XXX], on XXX, to request a recall repair appointment. I was told the necessary parts were unavailable. I contacted the dealership again on XXX, and received the same response. As of XXX, 62 days have passed since my first request, and the recall remains incomplete. NHTSA’s recall information states that repair parts are available in limited supply, but I have been unable to obtain the repair. I have since relocated to [XXX], and need American Honda to arrange the free recall repair through an authorized Honda dealership near my current residence. I am concerned about the safety of the unrepaired tire repair kit and the continued delay. I request that NHTSA review the delay and that American Honda provide a written repair timeline, help locate the correct replacement parts, and confirm any complimentary tire-related roadside assistance available while I await repair. I was at a stoplight waiting to turn left. I got the green arrow light so I proceeded to follow the vehicle in front of me who also turned left, I was not following close to the vehicle and the vehicle in front of me did not brake. Then my cars automatic emergency brake engaged in the middle of the intersection while tuening left and casued the cars behind to crash and my back to be injured. I am very upset as this has happened on 2 other occasions where I did not track the mileage or insections. I called the dealer where I purchased the vehicle, Mungenast Honda in St Louis MO. They have yet to schedule me a service appointment, but I did ask for one.

"While driving, the front driver's side driveshaft snapped due to an active safety recall. The vehicle was towed to a dealership for the recall repair. Immediately after the new driveshaft was installed, the car began shaking violently under braking and driving. The dealership inspected it and diagnosed a broken tie rod caused by the snapping driveshaft, but the dealer is refusing to cover the consequential damage or towing reimbursement, leaving an unsafe vehicle undriveable on their lot the incident happened "While stopped at a traffic light at [XXX] and [XXX] taking a left turn, I pressed the gas to go through the intersection. The vehicle made a loud clunk sound and completely lost all drive power (driveshaft snapped). I had to manually push the car ('Flintstone it') out of the intersection to safety.". After losing all power during a left turn at [XXX] and [XXX], I coasted down and up a hill. Because the broken axle severed the transmission connection, the vehicle would not hold in Park, causing it to roll and end up facing the wrong direction on the wrong side of the street while waiting for a tow. The car was subsequently towed to the dealership for the recall repair." INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11771981October 18, 2026UNKNOWN OR OTHER, FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING
Description: This is my second NHTSA complaint regarding unprompted automatic emergency braking in this vehicle. While driving at normal speed on [XXX], with no obstacle, pedestrian, or vehicle ahead, the vehicle's forward collision system activated without warning — audible alert sounded followed by the vehicle automatically applying hard braking and coming to an abrupt stop in the roadway. There was no obstruction, stopped traffic, or hazard present to justify the activation. I had 4 passengers in the vehicle at the time, including two children. The vehicle came to a sudden, forceful stop, creating a risk of rear-end collision from following traffic and a jolt risk to occupants. This is a recurring defect — I filed a prior NHTSA complaint for a similar unprompted braking event [reference no-XXX ] that resulted in a shoulder/neck injury to me as well as headache. Both incidents are documented on dash camera video and audio, including the audible system alert preceding each braking event. My 2018 Honda Accord EX-L with the 2.0L turbocharged engine (K20C4) and 10-speed automatic has had a cylinder head gasket failure at approximately 111,000 miles. The vehicle has been maintained consistently. Starting in early October 2026, the vehicle began shaking violently under acceleration while the engine was cold, including when accelerating at highway speed and climbing hills. The shaking lasted for roughly the first 15 minutes of every drive. No check engine light or diagnostic trouble codes appeared. In mid-October 2026, a Honda dealership diagnosed coolant leaking past the cylinder head gasket into the combustion chamber and quoted approximately $5,000 for the repair. Replacement parts are on backorder with no estimated arrival date. Violent shaking during highway acceleration is a safety concern, and continued coolant leakage into the cylinders can cause further engine damage. In August 2026, American Honda extended head gasket warranty coverage to 10 years or 150,000 miles on 2018–2022 Accord 1.5T and hybrid models for the same condition: coolant entering the combustion chamber and causing misfires. The 2.0L turbo engine was excluded. A pending class action (Alcantara et al. v. American Honda, consolidated with Bissell v. American Honda, S.D. Cal.) alleges head gasket failure in Honda's 1.5L and 2.0L turbocharged engines. I ask that NHTSA investigate head gasket failures in 2018–2022 Accords with the 2.0L turbo engine, and whether this engine should be included in Honda's remedy.

Vehicle: 2021 Honda Accord Sport Subject: Intermittent Electrical Malfunctions, Multiple Safety Warnings, and Loss of Acceleration I am reporting ongoing safety concerns involving my 2021 Honda Accord Sport. On multiple occasions, my vehicle has displayed several dashboard warnings, including brake system, electric parking brake, vehicle stability assist, hill-start assist, adaptive cruise control, and electric power steering warnings. These warnings have appeared intermittently in the past and sometimes disappeared on their own. On October 18, 2026, while traveling on the highway at approximately 65 mph, these warnings had already been displayed for approximately 30 minutes to an hour when my vehicle suddenly began struggling to accelerate. Despite pressing the accelerator, the vehicle continued losing speed, and the RPM gauge remained around 2,000 RPM. It felt as though something was holding the vehicle back. After several moments, the vehicle suddenly regained power, accelerated, and the RPM increased. This was particularly concerning because I was traveling at highway speed. I also experienced electrical problems with the interior lights. While the vehicle was running, neither the automatic interior lights nor the overhead lights near the rearview mirror would turn on. They remained inoperative after I turned off the vehicle. Later, the lights began working again, but I noticed the right overhead light fading while the left remained illuminated. The battery was replaced earlier this year, in 2026. I am concerned that these intermittent malfunctions could result in a sudden loss of power or failure of important vehicle safety systems while driving. I am requesting that these problems be documented and investigated for a potential safety defect or recall. I have photographs of the dashboard warnings available as supporting documentation.

The steering rack is clicking and clunking at low speed turns or even when the car is parked or still and you turn the wheel, any alignment that’s done to the car goes bad right away because the rack box is messed up, it has play and it vibrates the steering wheel when driving, I called the dealership and they won’t help me or fix the issue because there is no recall on the part but many other Honda owners are experiencing that same issue, the safety issue is that my steering wheel has play and it will turn at anytime, it’s been inspected and the rack is the issue, no warning lamps or messages.

I am having an issue with the ABS/VSA module for my vehicle. The ABS, VSA, Brake and EPS lights illuminate on the dashboard randomly. I took my car to a Honda service center and I was informed that the module needed to be replaced at the cost of over $3000 for the module and another $500 for the labor. I was told that this is a common issue with Honda Accord from 2013 - 2017 model years.

Infotainment System glitching: "ghost touch"; the screen is not registering my touches and randomly selecting/pushing buttons all on its own. Took it to Honda dealership and they have no answer or fix. This happens from time to time while driving, causing a huge safety concern. Extremely distracting to me, the driver. Everywhere I go to get answers, I only get "issue is known to manufacturer but no fix available; they are currently working on a fix". This has happened multiple times intermittently since I've purchased the vehicle in 2021.

- #11771616October 16, 2026ENGINE, FUEL/PROPULSION SYSTEM, ELECTRICAL SYSTEM
On October 12, 2026, my 2024 Honda Accord Sport-L Hybrid experienced a serious and potentially dangerous loss of propulsion while driving. Without warning, multiple warning lights and system malfunction messages appeared simultaneously on the dashboard. The vehicle then switched to electric-only (EV) operation, and the gasoline engine was no longer providing propulsion. I continued operating the vehicle using the remaining hybrid battery charge. As the battery charge decreased from approximately 1–2 bars to empty, the vehicle lost its ability to propel itself and eventually came to a rolling stop. After stopping, I turned the vehicle off and restarted it. The vehicle subsequently resumed normal operation, and most of the warning lights disappeared. However, the emissions system warning remained illuminated. After researching this incident, I discovered NHTSA Safety Recall 25V785000 (Honda Recall TN2), which affects certain 2023–2025 Honda Accord Hybrid vehicles. According to the recall, a software error involving the Integrated Control Module (ICM) may cause the system to reset while driving, resulting in a loss of drive power. The symptoms described in this recall closely resemble what I experienced. However, when I contacted Honda, I was informed that this recall had already been completed on my vehicle. I am extremely concerned that my vehicle experienced a loss of propulsion despite Honda reporting that the applicable safety recall remedy had already been performed. This incident presents a significant safety risk, particularly if a similar failure occurs while traveling at highway speeds, navigating an intersection, or driving in heavy traffic. I am requesting that NHTSA investigate whether the existing recall remedy adequately addresses this potential safety defect, whether the software update was properly implemented, and whether other Honda Accord Hybrid owners have experienced similar failures after recall completion.

The touchscreen infotainment display in my 2022 Honda Accord is malfunctioning and is available for inspection on request. For many months it has registered touches I never made ("phantom touch"). It changes screens on its own, cancels active GPS navigation, closes or switches other features, and presses buttons, including the voice command button, while I'm using other functions. Safety risk: Because the screen acts on its own while the car is moving, navigation drops out mid-route and the voice assistant activates unexpectedly. I have to look at the screen and fix it instead of watching the road, which is a serious distraction, or I am forced to look at my phone for continuing the GPS navigation because it is no longer registering on the display screen. Dealer confirmation: I have not yet taken it to a dealer, as the dealership that I purchased the car at said it would cost $228.00 just for the diagnostic. Other inspections: Not yet inspected by police or insurance. Warning signs: No warning lamps or messages appear. The only symptom is the erratic touch behavior, which first appeared around 2023 and has become more frequent. I have photos/video of the behavior available. This matches many other owner complaints about 2021-2022 Accords, and I believe it should be investigated as a defect and the repair should be covered for free as a recall.

The vehicles speakers plays a loud glitchy static like noise. It has to do with the Premium Amp and the Audio Unit. Can become too loud to hear emergency service vehicles coming by.

The infotainment touchscreen in my 2022 Honda Accord Sport experiences phantom or ghost touches. The screen changes menus on its own without driver input. It switches audio sources, changes settings, interrupts Apple CarPlay, and becomes unresponsive to touch commands. The problem occurs while driving and causes significant driver distraction because I must look away from the road to determine what the system is doing. The issue affects navigation, phone functions, and audio controls. This is a known problem affecting many 2018-2022 Honda Accords. Honda has acknowledged similar ghost-touch symptoms in technical communications, but no recall has been issued. The defect creates a safety risk by diverting driver attention from the roadway.

The contact owns a 2021 Honda Accord. The contact stated that while his wife was driving at 65 MPH, the accelerator pedal was depressed, but the vehicle failed to accelerate as intended. The vehicle hesitated and then accelerated; however, the vehicle then failed to accelerate above 20 MPH. Several unknown warning lights illuminated. The contact's wife drove the vehicle to a safe location and parked. While attempting to start the vehicle, the engine started and then shut off. The dealer was notified of the failure, and the vehicle was towed to the dealer, where it was diagnosed with a fuel pump failure requiring fuel pump replacement. The contact referenced a recall with a similar failure description; however, the contact's VIN was not included. The manufacturer was notified of the failure, and a case was filed. The failure mileage was approximately 152,100.

1. The service center diagnosed the BCM as faulty. I do not have the faulty part available as it was replaced. Service center work notes: Vehicle no start confirmed, battery voltage is at 12.4V. Vehicle has message on dash to touch key to ignition start stop button, with no success. Proceeded to inspect all main fuses to under hood fuse box, found all fuses are good main 175 amp fuse is ok, along with all four multi fuses. Proceeded to follow troubleshooting for message displayed on dash, tested key fob for unlock and lock functions, tested start stop button functions with no issues. Checked fuses that power is at each supply pin, pin 1 on connector a has 12.2v and pin 18 12.3v. Used the ground from connector pin 51 while testing voltage on previous pins to confirm ground is good. It is suspected that bcm has a internal failure and will need to be checked with known good unit, reprogrammed and retest to confirm vehicle will start. 2. The car in the faulty condition had no noticeable difference from one with a low battery so I tried to jump start the vehicle per the owners manual. This put me at a significant fire risk of over charging the battery. 3. Unknown if the manufacturer, insurance or police inspected it. 4. No warning lamps prior to failure. Dash was entirely blank other than aforementioned instruction about the key fob.

While driving at high speed, the vehicle falsely initiated severe emergency braking and aggressively shook the steering wheel, actively fighting my input to keep the car straight. My safety was at risk as well as others around me who had to brake and swerve to avoid collision while I struggled to get to the highway shoulder. The vehicle remains at the dealership as they are suggesting a steering column replacement. I have filed a case with Honda requesting a master technician perform a formal ADAS camera alignment and an EPS circuit inspection, but they have not yet responded. Honda did approve the dealer's pre-authorization request to replace the steering column. There were no warnings prior to the incident, but as it was occurring, all of the warnings were cycling (brake system failure, steering system failure, road departure warning, battery, etc.) until I made it to a local dealership who read the codes which showed internal steering module errors (U3000-49) and tracking failures occurred.

This has happened several times, usually on interstates, about once every few months. It usually takes place where there is a transition in the road, especially if there is a large shadow combined with a car in front of me. The two places that come to mind include merging on, especially during sunrise or sunset (if the highway is set into the ground like in [XXX]), or when passing under a bridge on the highway, esp with a car in front of me that is slowing down. The latter happened earlier this week, I believe on XXX eastbound in [XXX], passing under the XXX or one of the other streets similar to it.
