I was driving the van on interstate 85 in NC on my way to Charlotte from Raleigh. I was traveling at 70 mph when the van suddenly lost power. I could not accelerate and had to immediately pull over to the shoulder coasting on momentum only. I barely missed being hit by a semi truck as I attempted to get out of the way of traffic. The van remained stalled on the shoulder as I attempted to restart the van. It took 5 tries to get the van off the shoulder and onto an exit where I was able to park the van at a Waffle House parking lot to await a tow truck. Each attempt to restart the van would give me about 30 seconds of power before the van would stall out again. I was traveling with my wife and 4 small children and this was a very harrowing experience and could’ve have ended in disaster with that semi barreling down on us. The error code on the dash was “service charging system” Chrysler needs to fix the is issue before someone dies in a horrible accident.

My 2021 Pacifica PHEV has 16,200 miles on it at time of incident on 10/31/2022. I had just pulled off the highway to get gas when the issue began. I received a “service electrical system” on the dash and a check engine light and I had to turn off and back on to get it to restart. The check engine light stayed on and upon continuing my drive home another warning light came on telling me that the emergency braking was no longer operating. The warning light stayed on for a few miles and then went off. I took snapshots I will include. I also noticed that the vehicle was recording erroneous data such as suddenly having an electrical charge of a large percent when I had zero electrical charge. I got the vehicle to the dealership for repair. On 11/1. No loaner or rental reimbursement was available from Chrysler so I was without a Vehicle during repair. I received the vehicle back on 11/3/22 and I was informed that the vehicle had error codes POE 15-50 Hybrid/EV battery pack coolant heater A control and P1649-00 (? Not sure I wrote the number correctly) HCP Int… code. The paper also showed POE 15-00 HVAC Blocker Power but he only mentioned the first 2. The service technician indicated that the vehicle had a Z76 recall (I was not aware of a recall) and that they had done a software update to fix it and the vehicle should note be fixed. Upon reading other online complaints I see multiple reports of the same thing happening repeatedly to other 2021 phev Pacifica’s with low mileage while driving at high speeds including within 10 miles of having the same repair done. I am now scared to drive this vehicle on the highway after reading of multiple other 2021 PHEVs no longer operating on multiple vehicles while at highway speeds, despite multiple attempts at repair. I am afraid for my safety and for anyone else in my vehicle and I I want Chrysler to pay off my note immediately and take back possession of the vehicle. I no longer feel safe driving it.

22' Pacifica has advanced front brake assist . It does not work. I have tried and it only sets off a audible warning noise and dash says BRAKE. NO braking assist like it was stated it would have. I took it back to dealership 3 times now and have been told every time it has no codes. This safety braking does not work. The settings are all on . But there is no assist braking at all.

We drove at normal interstate speed when the “accelerator stopped responding.” The digital dash cluster then showed a message stating “Service Charging System” and “Shift to ‘P’ to Start.” The vehicle was turned off and turned back on after waiting roughly one minute.

Driving down the highway at 80 mph, the engine suddenly quit. The check engine light and battery light illuminated after the engine had quit. I was able to coast to the shoulder. Once stopped the ignition could not be turned off to try to restart the van. It would only move from Run to ACC. After sitting without touching anything for 4-5 minutes, the Ignition indicator moved to Off and then was able to be restarted. This is the third occurrence, the first happening while stopped at a red light and the second in a parking lot when shifting from reverse to drive. The dealer agreed to pick up the van from my work and drive it to their service center. The van quit on that driver once on the return trip. The van had just been in service for 36 days and was returned as operational with no defects on October 19, 2022. The parking lot event occurred 5 days later on October 24 and the highway shutdown on October 26. This is obviously a major safety risk. If I had not been driving at 0500 with minimal traffic the outcome could have been worse.

Same issue as the existing complaint PE22008 on 10/24/2022, South-bound I-95 in Maryland, driving around 65-70 MPH. The van completely stopped excelling. Luckily I had enough speed/inertia to cross the 2-3 lanes to pull-off onto the shoulder. I put the car in PARK and then turned the car in off and then on again, was able to continue driving afterwards. Dashboard screen read "Service charging system" which seems odd because if it is an issue with Hybrid system, the gas engine should kick-in. This happened again 3 hours later. In both situations, I had my wife and children in the van.

While traveling in the #1 lane at 65mph on the freeway, the engine suddenly lost power and I had to coast to the far right lane amid traffic to enter the emergency stopping area. This has happened 4 times in the last few weeks. The dashboard display says there is a charging system problem. I am able to restart the car after stopping completely and opening the driver's door. This creates a very unsafe situation. I took it to the dealership today and they performed the Z76 recall campaign software update, however, upon driving 10 miles on the freeway, it happened again.

Our 2021 Pacifica Hybrid with less than 21k miles on it lost all propulsion/motive power; this happened while I was driving on a freeway just seconds before going onto an overpass at freeway speeds to change freeways. Driving uphill on an overpass at freeway speeds with our kids in the car was very dangerous (especially during weekday traffic in Orange County) but, fortunately, we coasted to a stop safely. The vehicle's other systems seemed to still be working. For example, the vehicle's dashboard was working. As soon as the vehicle's power ceased, an error light on the dash appeared reading "Service Charging System Error," and instructed the driver to pull over and put the car in park. The check engine light has been on since the incident. The car was dropped off at the dealership on October 14th. They said that the electrical guy will be available on October 17th to look at the vehicle.

On Oct. 9, 2022, at approximately 6:30 p.m. EDT, I was braking as I approached a stop sign. For the first second or two of braking, the van slowed normally. Then, as the van was nearly stopped at the stop sign (probably 10 feet ahead of it, and approx. 5 mph or less), while my foot was still gently depressing the brake pedal, the van began to accelerate on its own. In response, I stepped down hard on the brake, lurching the van to a stop. After the hard stop (and explaining it to my family), I gently accelerated through the intersection and parked the van in my driveway. (The stop sign was 2 houses from home.) This was on a residential street, 25 mph speed limit, clear day, dry pavement. Before applying the brakes I had been coasting down the street between 20 and 25 mph. There was no other traffic and no pedestrians. When the van accelerated on its own, we had nearly but not completely come to a stop yet. My foot was still gently depressing the brake to come to a smooth stop. Neither the accelerator nor the cruise control was engaged. With unwanted acceleration while the brake is depressed, I do not think the vehicle is safe to drive. Today (the next morning) I am in the process of calling to have it flatbed-transported to the dealer for inspection. The van is brand new; we've had it 5 weeks, it has 698 miles. No problem or warning prior to this.

OEM equipment wheel locks, incorrectly installed either at factory or dealer, were McGard high security wheel locks, and either incorrectly tightened/torqued and/or the equipment used was warned by the mfg of the wheel locks NOT to be used on wheels where the lug studs were exposed to salt/winter conditions, thus causing the OEM equipment to fail and seize on the van. At the first DEALER performed tire rotation, they could not remove the OEM equipment, on a WARRANTED auto just 18 months into the mfg 3yr/36k mile warranty, and had to resort to brute force to remove seized wheel locks, thus causing documented damage to 2 wheels. Neither Dealer nor Chrysler will admit to liability, tuning this back on me, disregarding their warranty. Full documentation available for this claim. Their denial is not based on any contractual issue in warranty nor in any real fact. Had I had a flat tire on the side of the road, these seized wheel lock nuts may have caused a real & present danger to myself and family in the van. This is the Manufacture's issue to replace damaged equipment caused during warranty work on this auto.

- #11487390October 1, 2022FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, BACK OVER PREVENTION: REARVIEW SYSTEM BRAKING
On more than one occasion when I was on a four-lane highway with no vehicle in front of me in my lane, while I was passing a slower moving vehicle in the outer lane, my vehicle slammed on the aes brakes and would have absolutely caused a crash if there would have been a vehicle behind me. That person could have no possibly reacted fast enough to prevent his vehicle from rear-ending my vehicle. Again, there was no one in front of me, only the vehicle in the outer-lane. As far as the rear-cross-traffic problem, my vehicle is one that is missing Chrysler's Sphere and rear-parking-assist because of the chip shortage. There aren't any sensors in the back bumper at all and I'm not sure what triggers the rear-cross-traffic alert, but it does not alert until I've backed up so far from my parking space that if there was a car coming quickly it would surely hit me. This system does not work any where as quickly as any other car I've had it on in the past.

On 3 separate occasions while driving the dashboard has flashed the error message "Shift to P, Service Charging System". As soon as this occurs, the car begins to lose power and is no longer driveable. Once the car is shifted to "Park" it is temporarily stuck in that position with the car "On" but no power to the engine. For somewhere between 30-90s the car can't be turned off or shifted out of "Park". At some point after a short period - sometimes opening the door seems to help - the car will turn off and can then be re-started. The check engine light is on, but the car can be driven. Within a couple of hours the check engine light goes off and the car seems to behave normally. This has happened to us 3 times over ~2 months now. The car has actually stopped in the middle of the road forcing us to turn on hazard lights while trying to restart. It is extremely dangerous and seemingly common problem. Our local Chrysler service center was only able to book an appointment a month out due to limited technicians qualified to work on the hybrid vehicles. We described the problem in detail but they are not aware of the problem or solutions. We are trying to contact Chrysler Corporate directly to find out what their solution is.

while driving the car down the high way at normal highway speeds (70mph) the car lost all propulsion. Shortly before the malfunction the electric motor started growling. A message on the screen appeared "service charging system" and " put car in Park". I was in the far left lane when this happened and traffic was low as it was early in the day (7 am ) I managed to move the car from the left lane through all the other lanes and stop on the side of the highway. I was not able to drive the car and called for tow. While waiting for the tow the car became responsive after about 15 min. I recalled the tow and drove to dealer. The dealer scanned for codes and found a code for "Motor A position error". This is the second time this has happened to this car - previous time the dealer "upgraded" firmware and said it should be fine. Hardware issues cannot be solved by software. Driving on US-23 and "Service Charging System" warning displayed on dashboard. Car lost power and I had to coast to the side of the freeway. Turned car off but could not turn back on. Transmission would not go out of Park. Tried about 10 times and after about 15 minutes the car turned on and could drive. Check engine light remained on for remainder of the drive home. Planned on visiting dealer in the morning but car turned on and no check engine light. This is the 3rd time the "service charging system" warning has happened, but the first time the car died on the road. Every time the check engine light disappeared the next day. Had codes scanned and permanant code P1A21.

While driving on the highway, the vehicle abruptly lost propulsion, flashed error lights, and indicated an issue with the EV battery and charger, and would not charge when plugged in. Error codes flashed related to the Power train control module, Hybrid control processor, and Aux hybrid control processor. The CJDR Fairfax dealer would only provide software updates and would not perform other repairs per Chrysler's recommendations.

I was on the highway traveling at 60mph, with the adaptive cruise control on. The semi in front of me began to brake, I tapped off the cruise control and began braking. The semi came to a complete stop and I pushed my brake pedal to the floor to stop my vehicle. It did not stop. My Pinnacle has emergency braking and forward collision warning. My driver's instrument panel did not give a forward collision warning indication in a timely manner and my emergency braking did not stop my vehicle. At the time of the accident vehicle had less than 10,000 miles, dry weather conditions, and vehicle was used for leisure - in town driving. A month previously, I took my Pinnacle into the dealership because I didn't feel my brakes were working properly. They felt mushy, little resistance, and were making a noise. An inspection was performed by the dealership but were unable to detect a noise or visual issue. My safety was put at risk when a system I depended upon, that was advertised as, "Full speed forward collision warning with active braking assists you by providing both visual and audible warnings as well as applying emergency braking to help bring the vehicle to a full stop, if needed" (chrysler.com/pacifica/safey-security.html) failed. The brakes didn't engage properly to stop us before hitting the vehicle in front of us. The forward collision warning indication didn't happen in a timely manner, it was delayed causing us to lose precious seconds. If this had been a passenger vehicle in front of us we could have injured the occupants and we ourselves could have been gravely injured had we been traveling at a higher rate of speed. The problem has not been confirmed by a dealership, but was taken there prior to the incident less than a month before. I reported the problem to Chrysler the day of the accident. The vehicle is pending inspection by insurance and I have asked that the Electronic Data Recorder be removed and the information on it to be evaluated.

On Saturday 9.3.22, while driving at approximately 40mph, my Chrysler Pacifica lost all power and slowed to a stop (I turned on my hazards immediately and pulled over to the shoulder). Based on the light traffic around me I was able to safety move accross two lanes even without the aid of power steering. Within 60 seconds of me coming to a stop, the Check engine light came on the cluster and an error message saying “charging system fault”. After calling for Chrysler roadside assistance, I attempted to restart the vehicle dozens of times unsuccessfully. At approx 60min after the vehicle lost power, it restarted. On Tuesday, 9.5.22 I took the vehicle to the dealer to diagnose & repair. As of today (9.12.22) they have been unable to recreate the event. Even though the error threw a CIL code, the dealer stated the error is now gone and no codes were stored. They have filed a “STAR” case with the manufacturer to determine next steps. I have refused to accept the vehicle back until the problem itself has been diagnosed and a repair is made.

Was driving the vehicle when it completely died. The “service hybrid electric vehicle system” warning came on the dash and could not get it to go into drive again. Completely dead in the water and could not move. I can’t imagine if I was going down the freeway and this happened. Very lucky to have it on a quiet road. This is a known issue and there are many cases of this happening to others if you check YouTube out or google it. It’s very scary and someone is gonna end up hurt or killed.
