On four occasions while operating the vehicle down the roadway the dash screen and infotainment / vehicle information displays all have went dark off and reset causing disruption to vital information. Cruise control stayed active. It is unknown if the advanced safety features failed during the reset.

I slowly put my foot on the brake and suddenly heard a noise under the vehicle. It sounded like loud grinding. As pulled forward I looked in the rear view mirror and didn't see anything in the road that may have fallen out from under my vehicle. When I pushed on the gas I was suddenly accelerating forward very quickly without my control. This was maybe one or two seconds after I heard the noise. I hit the brakes and nothing happened. My only choice was to hit the back end of the other vehicle as there were pedestrians on either side. The vehicle was continuing to accelerate even after I tried to put it in park. I was worried if the car in front of me moved, that I would keep going. I even pressed the emergency brake button twice and nothing worked. Nothing was working at all. I called the police. They told me to try turning off the car. I pressed the button to turn the car off, but it took several pushes to work. I could hear that the rpms lowered extremely slowly. Once the police came they didn't see anything in the road at all. Vehicle was moved to a body shop and when they turned on the car it immediately started revving really loudly again.

There are two issues with the headlights as it relates to the instrument panel and controls. First, the symbol on the instrument panel is the same for both headlights and running lights. This means at dusk, I cannot know whether my auto-headlights have turned or if I need to override the auto-feature to turn them on manually. Second, the headlight controls are buried in a menu, hidden behind the steering wheel. The "quick menu" is inconvenient, but the fact that it is hidden bedding the steering wheel means that I have to lean forward and around to fiddle with it, which has led to me drifting into another lane as I am distracted. From the driver's perspective, the "quick menus" are all hidden behind the wheel, and the control panel is just far enough out of reach that the driver must adjust and lean forward to reach for any controls not on the steering column.

1 March, Onstar failed while utilizing the gps late at night. Stuck on side of the road until a passersby assisted me. 2 March, took truck to dealership, after 4 days said no issue found. Driving home, same issue persisted. Called dealership, told to bring back, 45 minutes away. Declined because was told no issue found. 6 March, morning truck completely dead. Called dealership and was told to have towed back to dealership. I declined because you didn’t find any issues when it was there. Following morning, 7 March, had truck towed to local dealership. After a week, was told Onstar telematics module failed and needs to be replaced. As of today, March 29, the part is on back order.

The Chevrolet Equinox EV emits an unusual and loud sound while in "Drive" or "Reverse" and stopped or at low speed. The sound has widely been described as "angelic" - it is a higher pitched sound which does not sound similar to any other vehicle or even a "white noise" sound. This sound is played both inside and outside the vehicle, but only up to a specific speed threshold. I believe that the unnatural tone and volume of this sound is not reminiscent of an approaching vehicle, limiting its utility as a safety feature for the blind or particularly children who are more likely to be attracted to the unusual sound if they do not recognize it as a vehicle. Moreover, the sound is played separately by a speaker inside this vehicle. This serves no useful safety purpose. It starts and stops abruptly as you move above and below the speed threshold, potentially distracting or disturbing the driver. Moreover, drivers who are seeking to avoid hearing this sound inside the vehicle or disturbing neighbors with the unnatural sound may be subconsciously learning to maintain a speed above the threshold (around 22 mph), even when it is unsafe to do so. I believe this vehicle should be recalled to shift to a more natural white noise sound, reduce the exterior volume, and restore the ability to disable the interior sound.

On February 23, 2026, while driving my 2026 Chevrolet Equinox EV home from Folsom Chevrolet following a prior repair visit, the “Service Park Assist System” warning illuminated, first intermittently, then after a few miles, continuously for the remainder of the 11-mile drive home. During the same drive, while reversing with no objects present behind or around the vehicle, the automatic emergency braking system activated and applied the brakes with significant force without cause. The vehicle was returned to Folsom Chevrolet (Repair Order #559378, opened February 23, 2026). Diagnostic code U1126 was found — indicating the left rear short range radar sensor had lost communication on CAN Bus 8. The sensor module was on national backorder, keeping the vehicle out of service for approximately two weeks. The module was replaced and programmed on March 7, 2026. Safety concern: Unexpected activation of automatic emergency braking with no objects present creates a serious collision risk from following traffic. This failure occurred on public roads during normal driving. Additionally, the U1126 CAN bus communication fault is the same vehicle communication architecture implicated in the battery/electrical failure documented in Repair Orders #558923 and #559167, suggesting these failures share a common root cause in the vehicle’s electrical communication network rather than representing isolated component failures.

Incident Date: February 10, 2026 Mileage: 1,260 On the morning of February 10, 2026, my 2026 Chevrolet Equinox EV (1,260 miles) would not start. The instrument cluster simultaneously displayed three warning messages: 1)“Service Battery Fault — Vehicle Won’t Restart,” 2)“Charging is unavailable,” and 3)“Charge Vehicle Soon.” The vehicle could not be placed in neutral and required dolly tow from my garage, as the front wheels were locked and could not roll freely. AAA and I measured the 12V battery at 11.8 volts. AAA technician made attempt to jump 12 volt battery with no success. He made the observation that it was very rare for an EV not to start once the 12 volt battery was jumped. The vehicle was towed to Folsom Chevrolet (Repair Order #558923, opened February 10, 2026). The technician performed a global reset, BECM reprogramming, standalone radio programming, and a manual reset procedure. The repair order was closed with “confirmed no further concerns,” however the root cause — identified in the repair notes as a possible HV module communication fault — was never confirmed diagnosed or repaired. Seven days later (February 17, 2026, RO #559167), the vehicle returned to the dealer with overnight 12V voltage loss, starting at 12.12V and continuing to drop. A parasitic draw test found an 11mA draw. After a second overnight test the dealer closed the repair order as “no concerns found” without identifying or repairing a root cause. Safety concern: A vehicle that cannot be started, cannot be charged, and cannot be placed in neutral poses a serious safety risk — particularly if such a failure occurs on a roadway rather than in a driveway. The inability to shift to neutral prevents the driver from clearing the vehicle from traffic. The root cause of this dual electrical failure has never been confirmed repaired despite two repair attempts.

Recall/Loss of Power I am filing a safety complaint regarding a 2023 Chevy Equinox. The vehicle began losing engine power and will not go over 40 mph. This creates a dangerous driving situation because the vehicle cannot maintain highway speed. The vehicle has a fuel system recall, and the issue appears to be related to the fuel system. The vehicle is currently at a Chevrolet dealership and has been there for almost a month. The vehicle is currently inoperable. This is a safety concern because loss of engine power while driving could cause an accident. I am requesting that this issue be investigated as a safety defect. The vehicle has experienced repeated failures of the integrated telematics system, including loss of GPS location accuracy, loss of data connectivity, navigation malfunction, and intermittent failure of voice command and steering wheel controls. On one occasion, the vehicle experienced a complete display blackout while driving, during which both the infotainment screen and digital instrument cluster went entirely black. The condition has occurred repeatedly and persisted despite multiple repair attempts. The vehicle has been out of service for approximately 82 cumulative days for diagnosis and repair of these issues. An authorized Chevrolet dealership ultimately diagnosed a failure of the telematics control module, which was replaced under warranty. Following this repair, the same data connectivity failure recurred and was documented with photo and video evidence. The vehicle was returned to the dealership for further diagnosis. The failure is intermittent and may not be present during every inspection. The telematics system supports navigation, driver information displays, and emergency communication features, including transmission of vehicle location and system data. Loss of GPS and connectivity may impair the vehicle’s ability to provide accurate location information during an emergency. The prior display blackout resulted in loss of driver information while the vehicle was in motion. The condition has been reported multiple times to authorized Chevrolet dealerships. Diagnostic procedures and programming were performed, and the telematics module replacement confirms a system-level failure. Recurrence after component replacement indicates the condition has not been permanently resolved. Prior symptoms included loss of GPS signal, incorrect location, loss of connectivity, navigation failure, and intermittent driver assistance alerts.

On Friday March 20, 2026, we traded in a Chevy Equinox to get a newer model which is the VIN number we listed in this complaint. Driving home from the dealership the vehicle went from drive to park on its own (driving at 65 miles/hr) and my husband had to immediately put the car physically back into drive. Once we finally got the vehicle to our house, I went out to check it, and the vehicle would not even start. Every single light came on yet it would not start up. We called On-Star and they couldn't do anything and told us we'd have to take it to the dealership. We finally got it to start up again, but the check engine light never went off. The next day my husband was able to get it started and immediately took it back to the dealership it was bought from, which it was actually transferred to from another dealership within the state, but on the way to the dealership we bought it from the car went into park on its own 5 more times, very dangerous and unacceptable. Once the car made it to the dealership, they could not even get the vehicle to shut off once they pulled in into the service area. My husband luckily is a professional driver, but is this was someone not as familiar or calm, this could have been a horrible catastrophe.

My Equinox completely stopped functioning in the middle of a major intersection as I was making a left turn during 5pm traffic. I was able to push the car out of the intersection with the help of other drivers (they had to exit their vehicles to assist) and the car restarted - I drove back roads to chevy dealership with the car cutting out unexpectedly twice more while the vehicle was moving and then restarting. No warning light came on. Dealership said my fuel pump "exploded" and pieces were also caught in the fuel injectors and all had to be replaced.

The first failure occurred in September 2022, then in July 2023, and most recently in July 2025. The fuel pump module was replaced; however, the failure persisted. There was no warning light illuminated before the vehicle stalled and failed to restart. The vehicle was towed to a local dealer, where it was diagnosed with a fuel module failure. The vehicle was repaired, but the failure persisted. The manufacturer was notified of the failure. The failure mileage was 36,492.
