- #11759784August 26, 2026EXTERIOR LIGHTING
While the daytime running lights are on (this vehicle has front and rear daytime running lights, so when the DRL’s are on, the taillights are illuminated as well as the front daytime running lights). When the DRL’s are on, and you use a turn signal, the rear turn signals are almost invisible to the naked eye during the day time. Reason being, the DRL’s are on, so the rear tail lights are slightly illuminated. This vehicle also has a pulsating (fade on/off) turn signals in the front and rear. It’s not a sharp on/off like a typical LED. So when using the turn signal, the rear turn signals (which also act as the taillight and brake light) are very very dim and hard to see as compared to when the DRL’s are off. The photo I will provide shows the left turn signal at its brightest point, the one on top with the daytime running lights off, are very bright and visible, whereas the bottom photo (which shows the rear daytime running taillights on) also shows the left turn signal at its brightest point, and is almost indistinguishable as to whether the turn signal is flashing. Again, both have the left turn signal on at its brightest point. This is very unsafe, and applies to all 2024-2026 BMW X5’s. J would like this rectified.

The contact owns a 2021 BMW X5. The contact stated that the vehicle failed to immediately start. The vehicle started later. The engine and a yellow warning light flashed on the instrument panel. Upon research, the contact located the German Federal Motor Transport Authority (Kraftfahrt-Bundesamt), for KBA Reference Number: 16131R, which was associated with the starter motor. The contact stated that the failure was associated with NHTSA Campaign Number: 24V576000 (Electrical System, Engine and Engine Cooling) and 25V644000 (Electrical System). The vehicle was taken to an independent mechanic who determined that the failure was associated with the starter motor, which was replaced. The local dealer was not contacted. The vehicle was not diagnosed or repaired. The manufacturer was contacted, and the contact was informed that there was no recall on the VIN associated with the failure. The failure mileage was approximately 49,000.

Description of the Complaint: I own a 2022 BMW X5 (VIN: [XXX], current mileage: approximately 39,500 miles as of 08/24/2026) equipped with the B58TU engine, which uses a plastic oil pump (part number 11418646454) rather than a metal one. I have not yet experienced an actual failure, but I am reporting this as a precautionary safety concern based on the following: 1. On BMW's official parts reference site (RealOEM.com), the oil pump for my specific vehicle is flagged with an "Attention! Observe PuMA measure 65494813" notice, indicating BMW's own internal system has identified a known issue requiring special observation for this part. 2. I have learned that BMW Korea has acknowledged this plastic oil pump issue following a government investigation, and has committed to a full free repair program for affected vehicles in Korea. 3. I am aware of multiple reports and complaints from other B58TU engine owners in the United States regarding this same plastic oil pump issue. 4. Given that the oil pump is a critical component for engine lubrication, a failure of this part while driving — particularly during highway or long-distance travel — could pose a serious safety risk, including sudden loss of engine oil pressure and potential engine seizure at speed. I am filing this report to bring this issue to NHTSA's attention before an actual failure occurs, and to request that this be considered as part of any pattern analysis regarding B58TU engine oil pump defects in the United States. - **Year:** 2022 - **Make:** BMW - **Model:** X5 - **VIN:** [XXX] - **Mileage:** 39,500 - **Component:** Engine and Engine Cooling (or "Engine Oil System" if listed separately) - **Attachments:** RealOEM.com INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

- #11758732August 22, 2026SERVICE BRAKES
I am requesting NHTSA assistance regarding a serious and recurring braking safety concern involving our BMW X5. We initially reported braking problems to BMW and brought the vehicle to the dealership for evaluation. We were advised that the vehicle was operating normally. Subsequently, a brake related recall was issued, and the vehicle underwent recall related service. Despite this, braking concerns continued. We again reported the problem and were advised after evaluation that the vehicle was operating properly. There were subsequent incidents involving abnormal braking behavior. During 1 incident, my wife was slowly parking the vehicle when it did not stop as expected despite repeatedly applying the brake pedal. The vehicle continued moving and collided with a wall before eventually stopping. We have also experienced abnormal brake vibration and other concerning braking behavior. Because of these repeated safety concerns, the vehicle is currently the subject of pending lemon law litigation involving BMW North America. Most recently, we returned the vehicle to the BMW dealership because of the braking and safety concerns. The vehicle remained there for approximately 4 months. We have not received a clear explanation of what diagnostic testing, inspection, repair, or corrective work was performed during this extended period. We were then informed that the vehicle was ready for pickup. When we went to retrieve it, it was covered with substantial dust and had very low tire pressure. We were unable to take it home. Our primary concern is safety. We do not know whether the reported braking malfunction was identified or corrected, what was done during approximately 4 months at the dealership, or the technical basis for releasing the vehicle for operation. We regularly transport our young children in this vehicle and are extremely concerned about driving it without knowing whether the braking problem has been properly identified and addressed. We respectfully request

On [XXX] as I was driving on [XXX] in Fl and got in to some traffic I noticed the car was shuddering a bit when I braked and I could here metal against metal coming from the brakes. I called SIXT 800 number and spoke to a Gentleman at their call center in Asia. He asked me why I needed brakes . I was shocked at his response , needless to say. He told me he reported it and I needed to return the car in Miami at the airport . I spent the night in Jupiter as it was around 5 pm. The next day [XXX] I drove the car to the MIA Airport where it was exchanged for a new car. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

We live in [XXX]. We own a 2021 BMW X5-45e. It has about 85,000 miles and has always been serviced by the BMW dealer here in [XXX] – Pierce BMW. On the evening of XXX I drove the vehicle home at 6:30PM after a 30 mile drive and parked the car outside in the shade of our house. The temperature at the time was approx. 95 degrees F. At 6:45PM we left the house and drove on the freeway and city streets of [XXX] about 8-10 miles to see a concert in downtown [XXX]. The temperature at that time was about 90-92 F. There was no wind. We then parked outside the venue for about an hour and watched the concert. We came out of the concert at about 8:30 and started the car. I drove about 200 feet to the corner and stepped on the brake. As the car slowly came to a stop the emergency/parking brake suddenly engaged at about 2-3 miles per hour. The car jerked to a very sudden and violent stop. To get the car going, I had to disengage the parking brake which was on with the indicated brake light. The engine was still on and there were no warning lights flashing. I turned the corner and accelerated again to about 15 miles per hour and touched the brakes to bring the car to a stop. The same thing happened. The car jerked to a sudden stop. To get the car going, I had to disengage the parking brake. The engine was still on and there were no warning lights flashing. It also happened a third time. That time, my elderly wife complained of pain in her back. After the third time, I turned the engine off and waited about one minute. I then restarted the car and drove it home with no problems. We have had no further issues, but this is a big concern as we no longer trust the car to perform as expected. This is a very worrisome issue and would like some answers, but apparently none are forthcoming. I took the car to Pierce BMW and they could find no fault code or problem. While driving eastbound on [XXX] on XXX between approximately 9:29 and 10:21 AM, the vehicle suddenly displayed a red warning instructing the driver to “Take over braking function immediately.” At that moment, adaptive cruise control braking and automated braking assistance disengaged without warning, requiring the driver to manually brake while in traffic. This created an unexpected safety hazard for the driver, passengers, and surrounding motorists. Prior to this event, the vehicle had exhibited repeated Driver Assistance instability across multiple service visits, including inconsistent adaptive cruise control behavior, braking‑related restrictions, and GPS mislocation affecting ADAS functions. The vehicle has been inspected by the manufacturer’s authorized dealer across five service visits, including replacement of the head unit and multiple software resets. The underlying malfunction has occurred repeatedly, but the specific braking alert has not been reproduced by the dealer. No crash, fire, or injury occurred. My 2022 BMW X5 is having the transfer case star to fail. Where the symptoms currently are at is the car has a shudder in certain situations. I can feel it when maneuvering at slow speeds and I can feel it at times at speed or when on a steep hill. I bought the extended warranty for the car and the maintenance package for the vehicle. I'm at 68k miles and the shudder started before that but I am still covered under extended warranty. I have had all maintenance done on the vehicle at the local BMW dealership including all tire needs and have always replaced tires as necessary and number of tires at a time they suggest. When I took it in as the vehicle is reporting needing brakes 2 weeks ago, I mentioned the shudder and asked about tire tread on the 2 front tires. They came back with that the tires were fine in spec with the back 2 but that shudder is the transfer case. The dealer also then mentioned a couple things: - They now recommend changing the fluid in the transfer case by 60k miles due to amount of failures they are seeing (my X5 was in the dealership multiple times before an after 60k and it was never recommended). - that since it is not in the recommended maintenance for the xdrive4.0i that the car would not alert to have it changed and therefore BMW would not cover changing the fluid under the UltimateCare (service maintenance plan they sold me). That the transfer case was meant to be a for life fluid. - I asked what happens next if the fluid is not changed, that it will continue to degrade and once it fully gives out the vehicle will become inoperable - I asked does the fluid change save the transfercase and they said it seems to help many vehicles and the shudder disappears but for some it does not. - I then asked would the BMW Extended Warranty purchased cover it if it fully failed or the fluid change did not solve the shudder and I was told maybe fully, partially or not at all. Ext Warranty brochure says it is covered at platinum level I hav

My 2026 BMW X5 xDrive50e experienced a serious freeway safety event involving loss of braking/brake power, loss of vehicle power/functionality, and multiple vehicle warnings/errors. The incident occurred while I was driving on the freeway and substantially affected my ability to safely operate the vehicle. BMW Emergency Services subsequently towed the vehicle to Irvine BMW on 07/09/2026. Irvine BMW later attributed the event to Emergency Stop Assist (ESA) and the Driver Monitoring System (DMS), rather than identifying a mechanical brake failure. Its written explanation states that ESA can take over vehicle guidance and decelerate the vehicle using multiple short braking jolts. The dealership reported a stored DMS fault but stated that it could not duplicate my reported condition. I have not been provided evidence that the original freeway event was reproduced or that its cause was conclusively established. Irvine BMW initially represented that material covering the display was an aftermarket screen protector and stated that BMW Technical required its removal because it could interfere with the DMS camera. On 07/27/2026, the General Manager apologized and confirmed that the material was actually original BMW transportation protection film. The dealership also proposed reducing ESA sensitivity as an alternative. I remain concerned that the combination of loss of braking/brake power, loss of vehicle power/functionality, and the reported ESA/DMS behavior may represent a serious safety defect or malfunction affecting braking, vehicle control, or driver-monitoring systems. I kindly request that NHTSA review this reported safety event, including ESA, DMS, related vehicle-control systems, and the interaction with the display/driver-monitoring system, and determine whether further investigation is warranted. I can provide dealership records, BMW App data, communications, and diagnostic documents.

- #11756589August 12, 2026FORWARD COLLISION AVOIDANCE: AUTOMATIC EMERGENCY BRAKING, BACK OVER PREVENTION: WARNINGS, FORWARD COLLISION AVOIDANCE: WARNINGS
On [XXX], my 2026 BMW X5 (VIN [XXX], 22,314 mi) was damaged during an automated reverse parking maneuver by BMW Parking Assistant, with automatic emergency braking enabled. Three failures occurred. I have used Parking Assistant nightly for ~6 months to back into my home garage: ~180 identical maneuvers, zero prior incidents or warnings. Nothing about the garage or environment changed. Failure 1: The system calculated and executed a trajectory that steered the vehicle into the garage structure. First contact was at the driver side rear fender flare. Failure 2: No graduated proximity warnings sounded during the approach. The first and only sensor alert occurred simultaneously with impact. The alert at contact proves the sensors were functional, yet no advance warning was given. Failure 3: After contact was detected and the alert sounded, the vehicle kept moving. Automatic braking did not stop it. The scrape traveled forward along the entire driver side door and sheared off the driver side mirror before the vehicle stopped. At parking speed, stopping distance is inches; the continuous scrape from fender flare to mirror is physical evidence of sustained post-contact travel. The system requested driver intervention only after impact. I was actively supervising. The window from failure onset to impact was under one second, below human reaction time. No supervising driver could have intervened. Damage: driver side rear fender flare, door, and mirror. No injuries. No software changes have been made since the incident. A formal case and engineering investigation have been requested from BMW of North America, including written preservation of EDR, parking maneuver logs, sensor data, brake commands, and telematics. Servicing dealer: Crevier BMW, Santa Ana, CA. I believe this indicates a software defect in path planning, sensor logic, or post-contact braking that may affect other vehicles. The front driveshaft universal joint failed causing the drive shaft to bust a hole in the engine and crack the transmission. There was no knocking noise prior to failure. It suddenly failed, disengaged and caused damage. I was told by service manager the u-bolt must have been defective to have failed without warning.

2023 BMW X5 had a defective water pump replaced under warranty at 27,585 miles on 9/12/2024. Today 8/10/2026 at 52,907 miles the water pump again needs to be replaced. BMW has had water pump recalls in the past for similar years of x5 SUV, but our VIN is not included in the current recalls. Photographs of the recalled water pumps look exactly like our pictures. I believe the recall should be expanded to include additional BMW models/VIN ranges.

The high-voltage battery system (this is a plug-in hybrid) began intermittently displaying a "battery not properly charging" warning approximately 6 months ago; the issue would resolve after plugging in and charging. On July 14, 2026, while driving, the vehicle displayed a "critical battery failure - pull over immediately and drive slowly" warning. The driver pulled over and shut off the vehicle; the warning cleared and the car appeared to drive normally afterward. This warning recurred several times over the following weeks under the same pattern. On August 6, 2026, the same critical battery failure warning appeared while driving; this time steering feel and drivetrain response were abnormal, and the air conditioning and several electronic features stopped working. Our safety, and the safety of other drivers on the road, was put at risk by the sudden loss of full vehicle control while in motion. The vehicle has been at an authorized BMW dealership for inspection since August 7, 2026; a full diagnosis is pending. Dealer service staff independently remarked that two other BMW X-series plug-in hybrids came in that same morning with the identical warning and symptoms, suggesting this may be a widespread component defect rather than an isolated issue.

The drivetrain warning light came on shortly after I started the vehicle. I noticed acceleration issues both while driving on side streets and on the freeway. I took the X5 45e into the repair center the day after the notice came on especially given the experience on the freeway, which happened during rush hour traffic. Upon inspection, I was told the starter was beginning to fail and that while there had been a recall on starters on many other BMW models, it did not include the X5 45e. BMW advised I replace the starter immediately due to safety, which cost $2,600. Two weeks later, the same drivetrain light has come on and the X5 is back at the service center for diagnosis.
