ELECTRICAL SYSTEM, ENGINE, VEHICLE SPEED CONTROL
I am reporting a severe, recurring safety hazard on my 2020 Toyota Corolla. The vehicle suddenly displays 'Drive-Start Control Malfunction' and completely cuts off engine power in motion, leaving the car stranded in active traffic. This happened after accelerating from a STOP sign on a main avenue. This is an inherent manufacturing defect present since delivery. In total, this chronic electrical surge has burned through 6 batteries and 3 Engine Control Modules (ECMs) at different mileages. Within the first weeks of purchase, it burned its first battery, which was replaced at Toyota and documented under the VIN history. Dealer staff deceptively called it 'normal,' leading me to replace subsequent batteries at AutoZone. [XXX] Toyota received the vehicle 4 times for this exact failure under Service Advisor [XXX]. They replaced the ECM twice, replaced a destroyed battery on a third visit just 400 miles later, and during this fourth visit on XXX, they only diagnosed the current burned ECM. Under Order XXX, I paid for 4.5 hours of specialized diagnostic labor, but the shop negligently just swapped parts and ignored the root cause. On XXX, an Operations Chief from Toyota Corporate called me under active BEC Case #XXX. He issued a final, verbal denial for Goodwill assistance based solely on mileage and age. Crucially, the Operations Chief explicitly admitted during the call that Toyota does not know what is causing the ECMs to burn out and does not know how to perform the proper diagnostic procedure to isolate the electrical root cause. He gave me to understand that my only option was to pay to replace the computer again and just put the car back on the road without a real fix. He actively refused to provide this denial in writing, forcing an unsafe vehicle back onto public roads. I have all four invoices to prove this chronic pattern. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)

POWER TRAIN, SERVICE BRAKES, ENGINE
What component or system failed or malfunctioned: complete slave cylinder failure resulting in loss of braking and ability to shift. Luckily I was traveling at a low rate of speed when this happened, but if this happens to anyone on the freeway or while turning against traffic for example they would have no control of their car and could be seriously injured. I took it to the dealership and they diagnosed it as a slave cylinder failure at only 70k miles. This is incredibly pre-mature. The vehicle has not been inspected by the manufacturer, only the dealership but I am trying to escalate the issue. No warnings at all, I regularly get my car serviced on there were no instances of the clutch acting strange or brakes losing power (because the clutch and brakes share the same fluid reservoir) prior to this incident.

ELECTRICAL SYSTEM, STRUCTURE:BODY
I am unable to lock or unlock the driver’s side door of my 2020 Toyota Corolla using the key fob because the door lock actuator is no longer functioning properly. I must manually use the physical key to lock and unlock the driver’s door. I consider this a significant safety concern because the power locking system is no longer functioning as intended, potentially affecting my ability to quickly secure or access the vehicle, particularly in an emergency. I have contacted Toyota directly to report the issue and request assistance with the repair, but Toyota is currently declining to cover the repair. I am proceeding with having the vehicle repaired independently because I need the door lock system restored to proper working condition. I have also learned that other Toyota owners have reported similar premature failures involving the door lock actuator. I am requesting that NHTSA document and review this complaint to determine whether this represents a recurring defect or potential safety-related problem that warrants further investigation.

SERVICE BRAKES, POWER TRAIN
The concentric slave cylinder in my vehicle is a big issue with these cars. There has been so many reports of issues happening with these cars manual transmission Corolla's. This can cause fluid to spew, brakes to stop working, and no longer able to drive the vehicle safely. This part should be recalled and covered by Toyota.

The CV joint, there is a clicking sound when sharp turns are make to include U-turns. Spoke with a mechanic and was instructed to reach out to the dealership and check for recall on it. I did see any but I see a Technical Service Bulletin ( TSB-0106-20) for 2015- 2021 Corolla models regarding a potential clinking noise during sharp turns and u turns. This may have started three months ago. It is more noticeable now that I am driving more.

Part is the coolant bypass valve. Toyota has been taken to court but dealership claims now that I’m over 100k miles I can’t do it. There was also a second part about 10 years but I wasn’t clear on the details of the outcome. Toyota has acknowledged this issue in court but failed to do so at the customer level in my case. engine light came on at 97k and now I’m at 104k which is over that 100k mark. Issue is I’ve only take my car to Toyota for repairs. They have known about this but failed to repair or notify me of it. I would have immediately brought my car in at 97k if. I had known. This puts not only me but my 3 kids and entire family at risk every time i drive. This is known to the dealership and they been taken to court for failures to notify customers of this known defect. It’s been inspected by Toyota the last time I took it in about a week ago at 104k miles.

POWER TRAIN
FAILED COMPONENT & AVAILABILITY: The manual transmission Concentric Slave Cylinder (CSC) failed internally. Due to caustic hydraulic fluid contamination and severe system pressure loss, I had to replace the internal CSC unit, the clutch disc, the pressure plate, the flywheel, the master clutch cylinder, and the hydraulic clutch lines. The failed components have been retained and are available for inspection upon request. SAFETY RISK: The plastic housing of the OEM slave cylinder cracked internally, spraying caustic hydraulic brake fluid inside the transmission bell housing and completely coating the flywheel, pressure plate, and clutch friction disc. This caused an immediate, sudden loss of hydraulic pressure, forcing the clutch pedal to drop and stick flat to the floorboard. This put my safety and the safety of others at extreme risk, as it suddenly prevented me from disengaging the clutch, shifting gears, or safely dropping the vehicle into neutral while driving in active traffic.CONFIRMATION & REPAIR: A certified Toyota dealership diagnosed the issue and issued an unaffordable repair estimate of $4,800 to fix this recurring manufacturer defect. Because of this extreme cost, I was forced to drop the transmission and perform the entire rebuild myself. The total out-of-pocket cost just for replacement parts was $3,200. The vehicle has not been inspected by police or insurance representatives as no crash resulted. WARNINGS & MILEAGE SYMPTOMS: There were no prior dashboard warning lamps, error messages, or clear physical indicators before either failure occurred. The loss of clutch pedal pressure happened instantaneously. This vehicle has suffered this catastrophic internal failure twice: the original factory-installed unit (part number 31400-19025) failed prematurely at 59,000 miles. The updated replacement part (superseded part number 31400-19027), which suffers from the exact same defective plastic composition, failed again just 40,000 miles later.

ELECTRICAL SYSTEM
DESIGN FLAW & SAFETY RISK: Toyota critically mismatched the standard battery to the vehicle's electrical charging profile; an AGM battery should be specified from the factory. Engine programming drops alternator voltage during cruising, deeply discharging the battery, and forces high-voltage surges (15V+) into it during deceleration. Because standard batteries cannot handle these rapid recharge loads during deceleration, the internal thermal load routinely overheats and boils the liquid electrolyte, even when brand new. This continuous boiling vents volatile hydrogen gas and discharges corrosive battery acid inside the engine bay. This structural flaw culminated in an immediate safety hazard when the battery physically exploded under the hood, creating a severe risk of engine compartment fire, chemical acid burns, and complete vehicle electrical blackout in active traffic. COLLATERAL DAMAGE & HISTORY: The chronic discharge and boiling of highly corrosive battery electrolyte severely damaged and corroded surrounding critical vehicle components. The acid completely corroded the transmission mount, the metal battery holder restraint, and the body paint. Furthermore, the corrosive boil-over burned off and destroyed several electrical connectors, plastic shrouds, plumbing hoses, and sections of the primary engine wiring cable harness. Since owning this 2020 Toyota Corolla, the vehicle has suffered systematic electrical degradation, requiring a full battery replacement at least once every single year. The exploded battery could not be safely retained due to its hazardous, ruptured state. WARNINGS & INSPECTIONS: No dashboard warning lamps, battery alerts, or diagnostic codes were triggered on the display prior to the physical battery failure. The rupture and electrical blackout occurred instantaneously. The vehicle has not been inspected by police or insurance representatives as no crash resulted directly from the blast.

Design issue: The 12th-generation (E210) Corolla manual transmission utilizes a CSC unit containing plastic/ABS components that owners and forum communities frequently cite as a weak point prone to premature leakage and mechanical failure. This occured to my corolla on a roadway and presented a severe safety hazard.

"On 04/09/26, I took my vehicle for regular maintenance and an oil change; no potential issues were reported. I operated the vehicle in good faith, trusting its safety. However, on Sunday, 05/03/2026, while driving at highway speeds, the clutch pedal failed and [stuck to the floor / slipped severely]. This caused an immediate loss of power to the wheels in high-speed traffic, creating a life-threatening situation and a high risk of a rear-end collision. No warning lights appeared prior to this failure."

VISIBILITY/WIPER
Windshield Wiper Intermittent setting stopped working, and wipers no long return to their home position. This appears to be a common issue with Toyota. When driving in the rain, the wipers will freeze in the up setting, which is very dangerous for it to occur during heavy rain on the highway causing visibility issues. No warnings of any kind given, the wiper switch appears to just go bad, or wear out over time.
